目的探讨去白细胞富血小板血浆(pure platelet-rich plasma,P-PRP)对距骨骨软骨损伤的治疗效果及机制。方法将2014年1月—2017年10月收治的符合选择标准的36例距骨骨软骨损伤患者,按随机数字表法随机分为对照组(A组)、富白细胞PRP(leuko... 目的探讨去白细胞富血小板血浆(pure platelet-rich plasma,P-PRP)对距骨骨软骨损伤的治疗效果及机制。方法将2014年1月—2017年10月收治的符合选择标准的36例距骨骨软骨损伤患者,按随机数字表法随机分为对照组(A组)、富白细胞PRP(leukocyte PRP,L-PRP)组(B组)、P-PRP组(C组),每组12例。3组患者性别、年龄、病程、Hepple分型等一般资料比较差异无统计学意义(P>0.05)。B、C组分别于植骨处注射2.5 mL L-PRP和P-PRP,A组不注射任何药物。术前和术后3、6、12个月采用美国矫形足踝协会(AOFAS)踝-后足评分和疼痛视觉模拟评分(VAS)评价疗效。P-PRP治疗机制研究:将MC3T3-E1细胞随机分为对照组(A组)、L-PRP组(B组)、P-PRP组(C组),B、C组分别用含5%L-PRP或P-PRP的培养液进行培养,A组用相同含量PBS培养。采用MTT法检测细胞增殖情况,ELISA法检测上清液基质金属蛋白酶9(matrix metalloprotein 9,MMP-9)蛋白含量,测定细胞ALP活性,实时荧光定量PCR(real-time fluorescence quantitative PCR,qRT-PCR)检测细胞中骨桥蛋白(osteopontin,OPN)、Ⅰ型胶原和MMP-9 mRNA的表达,Western blot检测上清液MMP-9和细胞中磷脂酰肌醇3激酶(phosphoinositide 3-kinase,PI3K)、磷酸化蛋白激酶B(phosphorylated protein kinase B,pAKT)、磷酸化c-Jun(phosphorylated c-Jun,p-c-Jun)蛋白表达。结果各组患者均获随访,随访时间13~25个月,平均18个月。无伤口感染、内固定失效等并发症发生。MRI检查示,术前各组损伤程度相似,术后6个月各组均获康复,且C组优于A、B组。术后各时间点各组AOFAS评分和VAS评分均较术前显著改善(P<0.05);术后3、6、12个月C组AOFAS评分均显著高于A、B组(P<0.05);VAS评分各组间比较差异无统计学意义(P>0.05)。P-PRP治疗机制研究:C组细胞增殖吸光度(A)值、ALP活性、OPN和Ⅰ型胶原mRNA相对表达量显著大于A、B组,B组大于A组,差异均有统计学意义(P<0.05)。B组MMP-9蛋白和mRNA相对表达量以及EL 展开更多

Platelet concentrates(PC) [platelet-rich plasma(PRP) and platelet-rich fibrin(PRF)] are frequently used for surgical procedures in medical and dental fields,particularly in oral and maxillofacial surgery,plastic surgery and sports medicine.The objective of all these technologies is to extract all the elements from a blood sample that could be used to improve healing and promote tissue regeneration.Although leukocyte rich and leukocyte poor PRP's have their own place in literature,the importance of non-platelet components in a platelet concentrate remains a mystery.PC have come a long way since its first appearance in 1954 to the T-PRF,A-PRF and i-PRF introduced recently.These PC find varied applications successfully in periodontics and implant dentistry as well.However,the technique of preparation,standing time,transfer process,temperature of centrifuge,vibration,etc.,are the various factors for the mixed results reported in the literature.Until the introduction of a proper classification of terminologies,the PC were known by different names in different countries and by different commercial companies which also created a lot of confusion.This review intends to clarify all these confusion by briefing the exact evolution of PC,their preparation techniques,recent advances and their various clinical and technical aspects and applications. 展开更多
