目的评价曲安奈德术中责任节段神经鞘内注射对腰椎管狭窄症(LSS)患者术后疼痛疗效。方法收集2014年1月至2015年1月在该科行传统椎板开窗减压术的LSS患者86例,根据随机数字表法,将所有患者分为2组。试验组(A组,n=43):术中责任节段神经鞘... 目的评价曲安奈德术中责任节段神经鞘内注射对腰椎管狭窄症(LSS)患者术后疼痛疗效。方法收集2014年1月至2015年1月在该科行传统椎板开窗减压术的LSS患者86例,根据随机数字表法,将所有患者分为2组。试验组(A组,n=43):术中责任节段神经鞘内注射和神经根周围喷洒曲安奈德注射液;B组(空白组,n=43):术中不做特殊处理。记录各组患者的年龄、性别、病程、下床活动时间、视觉模拟评分法(VAS)评分、日本骨科协会评估治疗分数(JOA)评分、Oswestry功能障碍指数问卷表(ODI)评分,并进行统计学分析。结果所有患者均成功完成手术和分组,无神经损伤、大出血及死亡病例。两组在年龄、性别、病程对比方面均差异无统计学意义(P>0.05)。两组下床活动时间比较差异有统计学意义(P<0.05)。两组间术前、终末随访VAS评分比较差异无统计学意义(P>0.05)。两组VAS评分在术后1、3、7d比较均差异有统计学意义(P<0.05)。两组JOA评分和ODI评分术后3、7d比较差异有统计学意义(P<0.05)。结论传统椎板开窗减压术在治疗LSS时,术中应用曲安奈德进行责任节段神经根干预,可以有效预防患者术后反跳痛症状的发生。 展开更多

The mortality rate of acute severe intraventricular hematoma is extremely high, and the rate of disability in survivors is high. Intraventricular hematoma has always been a difficult problem for clinical treatment. Although minimally invasive endoscopic hematoma evacuation is widely used to treat this disease, the technique still has room for improvement. Equipment for the intra-neuroendoscopic technique(INET) consists of two of our patented inventions: a transparent sheath(Patent No. ZL 200820046232.0) and a hematoma aspirator(Patent No. ZL 201520248717.8). This study explored the safety and efficacy of INET by comparing it with extraventricular drainage in combination with urokinase thrombolytic therapy. This trial recruited 65 patients with severe intraventricular hemorrhage, including 35(19 men and 16 women, aged 53.2 ± 8.7 years) in the INET group and 30(17 men and 13 women, aged 51.5 ± 7.9 years) in the control group(extraventricular drainage plus urokinase thrombolytic therapy). Our results showed that compared with the control group, the INET group exhibited lower intraventricular hemorrhage volumes, shorter intensive care-unit monitoring and ventricular drainage-tube placement times, and fewer incidences of intracranial infection, secondary bleeding, and mortality. Thus, the prognosis of survivors had improved remarkably. These findings indicate that INET is a safe and efficient new method for treating severe intraventricular hematoma. Human umbilical mesenchymal stem cells from Wharton's jelly of the umbilical cord were induced to differentiate into oligodendrocyte precursor-like cells in vitro. Oligodendrocyte precursor cells were transplanted into contused rat spinal cords. Immunofluorescence double staining indicated that transplanted cells survived in injured spinal cord, and differentiated into mature and immature oligodendrocyte precursor cells. Biotinylated dextran amine tracing results showed that cell transplantation promoted a higher density of the corticospinal tract in the central and caudal parts of the injured spinal cord. Luxol fast blue and toluidine blue staining showed that the volume of residual myelin was significantly increased at 1 and 2 mm rostral and caudal to the lesion epicenter after cell transplantation. Furthermore, immunofluorescence staining verified that the newly regenerated myelin sheath was derived from the central nervous system. Basso, Beattie and Bresnahan testing showed an evident behavioral recovery. These results suggest that human umbilical mesenchymal stem cell-derived oligodendrocyte precursor cells promote the regeneration of spinal axons and myelin sheaths. 展开更多

This study analyzed the clinical characteristics and outcomes of sacral extradural spinal meningeal cysts with spinal nerve root fibers treated by reconstruction of the nerve root sheaths.The relationships between the cysts and spinal nerve root fibers were examined microscopically,the cysts were partially excised,and the defects were oversewn to reconstruct the nerve root sheaths.The Improved Japanese Orthopedic Association(IJOA)scoring system was used to evaluate preoperative and postoperative neurological function.Thirty-eight patients were included in this study,with a mean age of 41.4±15.57 years.The mean IJOA score was 18.8±1.32 preoperatively and 19.6±0.65 postoperatively,which was a significant difference(t=–3.77,P=0.001).These results indicate a significant improvement in neurological function after surgery.The most significant improvement in neurological function was sensation(z=–2.86,P=0.004),followed by bowel/bladder function(z=–2.31,P=0.02). 展开更多

It is hypothesized that L-carnitine exhibits neuro-protective effects on injured sciatic nerve of rats. Rat sciatic nerve was crush injured by a forceps and exhibited degenerative changes. After intragastric administration of 50 and 100 mg/kg L-carnitine for 30 days, axon area, myelin sheath area, axon diameter, myelin sheath diameter, and numerical density of the myelinated axons of injured sciatic nerve were similar to normal, and the function of injured sciatic nerve also improved signiifcantly. These ifndings suggest that L-carnitine exhibits neuroprotective effects on sciatic nerve crush injury in rats. 展开更多

目的比较超声引导下腘窝坐骨神经阻滞时,神经鞘膜下注射(IPS)及鞘外注射(OPS)的阻滞效果及阻滞相关并发症发生情况,为超声引导下腘窝坐骨神经阻滞最佳路径的选择提供理论基础。方法选取2018年1—6月北京积水潭医院择期或急诊行踝关节骨... 目的比较超声引导下腘窝坐骨神经阻滞时,神经鞘膜下注射(IPS)及鞘外注射(OPS)的阻滞效果及阻滞相关并发症发生情况,为超声引导下腘窝坐骨神经阻滞最佳路径的选择提供理论基础。方法选取2018年1—6月北京积水潭医院择期或急诊行踝关节骨折手术患者60例(骨折部位均不累及内踝),采用随机数字法分为IPS组及OPS组,每组30例。所有患者均采用超声引导侧入路腘窝坐骨神经阻滞,使用0.5%罗哌卡因20 mL,所有手术均在蛛网膜下腔阻滞下完成。比较两组患者操作相关情况、阻滞完全时间、感觉运动阻滞持续时间、术后数字评分法(NRS)评分、术后首次觅求阿片类药物时间及各时间段内舒芬太尼消耗量以及两组患者阻滞及镇痛相关并发症发生情况的差异。结果IPS组操作时间及进针次数少于OPS组[(3.6±1.0)min比(4.5±1.1)min,(2.9±1.1)次比(4.3±1.0)次](P<0.01),而两组操作引起的NRS评分及患者对麻醉操作的满意度比较差异无统计学意义(P>0.05)。OPS组有2例患者阻滞后30 min未达到阻滞完全指标,IPS组阻滞完全时间快于OPS组[(19.3±3.9)min比(28.6±3.0)min](P<0.01)。IPS组患者感觉及运动阻滞持续时间长于OPS组[(21.3±5.3)h比(18.3±3.7)h,(16.2±4.2)h比(13.9±2.9)h],术后虚弱感评分高于OPS组[2.0(1.0,3.0)分比1.0(0.8,2.0)分](P<0.01)。两组患者在术后12 h内均未使用阿片类药物,IPS组术后阿片类药物使用总量低于OPS组(P<0.05)。两组患者均未见阻滞相关并发症的发生。结论与神经OPS相比,超声引导下腘窝处坐骨神经阻滞神经IPS起效更快,感觉及运动阻滞维持时间更长,两者可提供相似的术后镇痛效果,避免了术后12 h内阿片类药物的使用。 展开更多

In this study, after propofol was injected into the injured sciatic nerve of mice, nuclear factor kappa B expression in the L 4-6 segments of the spinal cord in the injured side was reduced, apoptosis was decreased, nerve myelin defects were al eviated, and the nerve conduction block was lessened. The experimental findings indicate that propofol inhibits the inflammatory and immune responses, decreases the expression of nuclear factor kappa B, and reduces apoptosis. These effects of propofol promote regeneration fol owing sciatic nerve injury. 展开更多

Optic nerve sheath diameters in nontraumatic brain injury:A scoping review and role in the intensive care unit

BACKGROUND Neuromonitoring in medical intensive care units is challenging as most patients are unfit for invasive intracranial pressure(ICP)modalities or unstable to transport for imaging.Ultrasonography-based optic nerve sheath diameter(ONSD)is an attractive option as it is reliable,repeatable and easily performed at the bedside.It has been sufficiently validated in traumatic brain injury(TBI)to be incorporated into the guidelines.However,currently the data for non-TBI patients is inconsistent for a scientific recommendation to be made.AIM To compile the existing evidence for understanding the scope of ONSD in measuring ICP in adult non-traumatic neuro-critical patients.METHODS PubMed,Google Scholar and research citation analysis databases were searched for studies in adult patients with non-traumatic causes of raised ICP.Studies from 2010 to 2024 in English languages were included.RESULTS We found 37 articles relevant to our search.The cutoff for ONSD in predicting ICP varied from 4.1 to 6.3 mm.Most of the articles used cerebrospinal fluid opening pressure followed by raised ICP on computed tomography/magnetic resonance imaging as the comparator parameter.ONSD was also found to be a reliable outcome measure in cases of acute ischaemic stroke,intracerebral bleeding and intracranial infection.However,ONSD is of doubtful utility in septic metabolic encephalopathy,dysnatremias and aneurysmal subarachnoid haemorrhage.CONCLUSION ONSD is a useful tool for the diagnosis of raised ICP in non-traumatic neuro-critically ill patients and may also have a role in the prognostication of a subset of patients. 展开更多

Previous studies have shown that transplantation of human bone marrow mesenchymal stem cells promotes neural functional recovery after stroke, but the neurorestorative mechanisms remain largely unknown. We hypothesized that functional recovery of myelinated axons may be one of underlying mechanisms. In this study, an ischemia/reperfusion rat model was established using the middle cerebral artery occlusion method. Rats were used to test the hypothesis that intravenous transplantation of human bone marrow mesenchyrnal stem cells through the femoral vein could exert neuroprotective effects against cerebral ischemia via a mechanism associated with the ability to attenuate axonal injury. The results of behavioral tests, infarction volume analysis and immunohistochemistry showed that cerebral ischemia caused severe damage to the myelin sheath and axons. After rats were intravenously transplanted with human bone marrow mesenchymal stem cells, the levels of axon and myelin sheath-related proteins, including microtubule-associated protein 2, myelin basic protein, and growth-associated protein 43, were elevated, infarct volume was decreased and neural function was improved in cerebral ischemic rats. These findings suggest that intravenously transplanted human bone marrow mesenchymal stem cells promote neural function. Possible mechanisms underlying these beneficial effects include resistance to demyelination after cerebral ischemia, prevention of axonal degeneration, and promotion of axonal regeneration. 展开更多

Effect of a cervical collar on optic nerve sheath diameter in trauma patients

BACKGROUND:As advocated in advanced trauma life support and prehospital trauma life support protocols,cervical immobilization is applied until cervical spine injury is excluded.This study aimed to show the difference in optic nerve sheath diameter(ONSD)between patients with and without a cervical collar using computed tomography(CT).METHODS:This was a single-center,retrospective study examining trauma patients who presented to the emergency department between January 1,2021,and December 31,2021.The ONSD on brain CT of the trauma patients was measured and analyzed to determine whether there was a difference between the ONSD with and without the cervical collar.RESULTS:The study population consisted of 169 patients.On CT imaging of patients with(n=66)and without(n=103)cervical collars,the mean ONSD in the axial plane were 5.43±0.50 mm and 5.04±0.46 mm respectively for the right eye and 5.50±0.52 mm and 5.11±0.46 mm respectively for the left eye.The results revealed an association between the presence of a cervical collar and the mean ONSD,which was statistically significant(P<0.001)for both the right and left eyes.CONCLUSION:A cervical collar may be associated with increased ONSD.The effect of this increase in the ONSD on clinical outcomes needs to be investigated,and the actual need for cervical collar in the emergency department should be evaluated on a case-by-case basis. 展开更多

BACKGROUND Epithelioid malignant peripheral nerve sheath tumor(EMPNST)of the bladder is a rare entity with devastating features.These tumors are thought to originate from malignant transformation of pre-existing schwannomas of pelvic autonomic nerve plexuses,and unlike the conventional malignant peripheral nerve sheath tumor(MPNST),are not associated with neurofibromatosis.The tumor has dis-tinctive morphological,immunohistochemical and molecular features.Addi-tionally,it tends to be more aggressive and have a higher mortality.This is the first case that presents with a synchronous urothelial carcinoma of the bladder and the epithelioid variant of MPNST in the literature.It’s also the second re-ported case of EMPNST originating from the bladder wall.CASE SUMMARY In this case report,we present the detailed clinical course of a 71-year-old patient with EMPNST of the bladder alongside a literature review.CONCLUSION During the management of EMPNST cases,offering aggressive treatment moda-lities to the patient,such as radical cystectomy,is appropriate for the best chance to contain the disease,regardless of the tumor stage and the extent of local disease at initial diagnosis. 展开更多
