目的探讨针刺太冲穴治疗抑郁症的机制及肝经与额叶的联系。方法将符合美国精神疾病诊断与统计手册第4版(DSM-Ⅳ)抑郁症诊断标准的42例首发中度抑郁症患者分为药物组(氟西汀)、针药组(太冲穴联合氟西汀)及针刺组(太冲穴),每组14例,疗程均...目的探讨针刺太冲穴治疗抑郁症的机制及肝经与额叶的联系。方法将符合美国精神疾病诊断与统计手册第4版(DSM-Ⅳ)抑郁症诊断标准的42例首发中度抑郁症患者分为药物组(氟西汀)、针药组(太冲穴联合氟西汀)及针刺组(太冲穴),每组14例,疗程均为1个月,运用3.0T磁共振方法,分别对相应脑区治疗前后进行静息态功能磁共振扫描,采用比率低频振幅(fractional amplitude of low frequency fluctuations,fALFF)的方法分析数据,各组治疗前后进行配对t检验比较,并将治疗后的图像与汉密尔顿(HAMD)评分做相关分析。结果与本组治疗前比较,针药组左侧额叶(BA6、BA9、BA48)、右侧额叶(BA4、BA46)、双侧顶下小叶(BA40、BA48)、楔前叶(BA7)、后扣带回(BA31)、左侧枕叶(BA17)及右侧枕叶中部(BA18)的fALFF值降低(P<0.05);药物组右侧顶下小叶(BA40)和右侧枕叶(BA17)的fALFF值降低(P<0.05);针刺组左侧额叶(BA10)、右侧额叶(BA45)、左侧枕叶(BA19)、右侧枕叶中部(BA17)、左侧楔前叶(BA7),后扣带回(BA31)的fALFF值降低(P<0.05)。针药组及针刺组的左侧额叶fALFF值均与HAMD评分呈正相关;药物组左侧额中回、左侧顶叶、左侧枕叶fALFF值与HAMD评分呈正相关。结论针刺联合抗抑郁药的疗效优于单一的针刺或药物治疗;额叶功能的改变与病情程度相关;肝经与额叶之间存在较为广泛的联系,可能是针刺太冲穴治疗抑郁症的机制。展开更多

In this study, we p...Data mining has the potential to provide information for improving clinical acupuncture strategies by uncovering hidden rules between acupuncture manipulation and therapeutic effects in a data set. In this study, we performed acupuncture on 30 patients with hemiplegia due to acute ischemic stroke. All participants were pre-screened to ensure that they exhibited immediate responses to acupuncture. We used a twirling reinforcing acupuncture manipulation at the specific lines between the bilateral Baihui(GV20) and Taiyang(EX-HN5). We collected neurologic deficit score, simplified Fugl-Meyer assessment score, muscle strength of the proximal and distal hemiplegic limbs, ratio of the maximal H-reflex to the maximal M-wave(Hmax/Mmax), muscle tension at baseline and immediately after treatment, and the syndromes of traditional Chinese medicine at baseline. We then conducted data mining using an association algorithm and an artificial neural network backpropagation algorithm. We found that the twirling reinforcing manipulation had no obvious therapeutic difference in traditional Chinese medicine syndromes of "Deficiency and Excess". The change in the muscle strength of the upper distal and lower proximal limbs was one of the main factors affecting the immediate change in Fugl-Meyer scores. Additionally, we found a positive correlation between the muscle tension change of the upper limb and Hmax/Mmax immediate change, and both positive and negative correlations existed between the muscle tension change of the lower limb and immediate Hmax/Mmax change. Additionally, when the difference value of muscle tension for the upper and lower limbs was 〉 0 or 〈 0, the difference value of Hmax/Mmax was correspondingly positive or negative, indicating the scalp acupuncture has a bidirectional effect on muscle tension in hemiplegic limbs. Methods: Fifty eligible patients were randomized i...Objective: To observe the clinical efficacy of warm needling moxibustion plus flash cupping for remission-stage peripheral facial paralysis (FP) due to wind-cold. Methods: Fifty eligible patients were randomized into a warm needling moxibustion group and an acupuncture-cupping group, 25 cases in each group. The warm needling moxibustion group was intervened by acupuncture at Fengchi (GB 20), Yangbai (GB 14) towards Yuyao (EX-HN 4), Xiaguan (ST 7), Dicang (ST 4) towards Jiache (ST 6), Quanliao (SI 18), and Hegu (LI 4), plus warm needling moxibustion at Quanliao (SI 18); the acupuncture-cupping group received flash cupping on the affected side in addition to the intervention given to the warm needling moxibustion group. The two groups were both treated once a day, 10 times as a treatment course, for 3 courses in total. The House-Brackmann (H-B) facial nerve grading system was observed before and after the intervention to evaluate the facial nerve function in the two groups, and the therapeutic efficacies were also compared between the two groups. Results: The two treatment protocols both can promote the recovery of facial nerve function. The total effective rate was 92.0% in the acupuncture-cupping group versus 72.0% in the warm needling moxibustion group, and the between-group difference was statistically significant (P〈O.05). The treatment group was treated by warm needling method, and the control group was treated by simple acupuncture. Their therapeutic effects were compared after 20 sessions of treatments. Results: The effective rate was 94.0% in the treatment group and 86.0% in the control group, with a significant difference between the two groups (P〈0.05). Results:Blood and synovial levels of TNF-αand VEGF were reduced significantly after treatment in both groups.The lowering(absolute value and difference value)of TNF-αas well as the absolute value of VEGF, either in blood or in synovia,were similar in the two groups(P0.05);but the lowering of VEGF after treatment was more significant in the EA group than that in the SN group(P0.05).Conclusion:EA could effectively lower the contents of TNF-αand VEGF in peripheral blood and joint synovia to improve the internal environment for genesis and development of RA,so as to enhance the clinical therapeutic effectiveness.展开更多

Methods:...Objective: To observe the effect of warm joint needling plus rehabilitation techniques on the balance function and quality of life (QOL) of patients with spastic hemiplegia after ischemic cerebral stroke. Methods: Ninety patients with spastic hemiplegia after ischemic cerebral stroke were randomized into a rehabilitation group, a warm joint needling group and an observation group, with 30 cases in each group. The rehabilitation group was intervened by Bobath therapy, the warm joint needling group was treated with joint needling on the affected side plus warm needling, and the observation group was given the same rehabilitation treatment as the rehabilitation group together with the same warm joint needling as the warm joint needling group. The three groups were treated once another day, 1 month as a treatment course for 6 months. Before the treatment, and respectively after 2-week, 1-month, 3-month, and 6-month treatment, the modified Ashworth scale (MAS) was used to measure the anti-spasm ability of the lower limb, the Berg balance scale (BBS) was adopted to evaluate the balance function, and the stroke-specific quality of life scale (SS-QOL) was employed to estimate the QOL. Results: After 3-month and 6-month treatment, the lower-limb MAS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 1-month, B-month and 6-month treatment, the BBS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 2-week, 1-month, 3-month and 6-month treatment, the SS-QOL scores in the observation group were markedly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05).
