Acute compartment syndrome(ACS) of the thigh following primary total hip arthroplasty(THA) is a highly uncommon complication and has not yet been reported before with regards to the anterior approach through the anterior supine interval. We present a case of a 69-year-old male patient with a history of stroke, who developed ACS of the thigh after elective THA while using therapeutic low molecular weight heparin as bridging for regular oral anticoagulation. ACS pathogenesis, diagnostic tools, treatment and relevant literature are discussed. The patient's ACS was recognized in time and treated by operative decompression with fasciotomy of the anterior compartment. Follow-up did not show any neurological deficit or soft-tissue damage. 展开更多

Intermuscular bones(IBs)are ossified from tendons and only occur in lower teleosts.Positive association between the regulation of scleraxis gene(scx)and tendon development gave us reasons to speculate that the scx gene may play a potential role in regulating the development of IBs.A phylogenetic analysis conducted for this study revealed potential functional differentiation between two scx orthologues,scxa and scxb.The scxa^(-/-) and scxb^(-/-) zebrafish were generated through CRISPR-Cas9 technology to study the role of scx in the IB and rib development.The results showed a significant reduction of the number of IBs in adult scxa-1^(-/-) zebrafish,with almost 70%reduction(15-25 IBs)compared to the wild type scxa-1^(+/+)zebrafish(76-80 IBs).In the scxa-1^(+/+)adults,IBs were observed in both dorsal and tail segments;however,in scxa-1^(-/-)fish IBs were observed only in the tail segment(none in the dorsal segment).Although scxa-1^(-/-) zebrafish had rib defects,the mutants were viable and fertile as adult fish.The scxb^(-/-) zebrafish had the same number of IBs and same skeletal phenotype as the wild-type fish.This suggests that only scxa has a crucial role in the IB development,and confirms functional differentiation of scx orthologues.To further clarify the molecular mechanism by which scxa affects the IB development,we conducted comparative transcriptome analysis of dorsal tissue samples of scxa-1^(+/+)(with IBs)and scxa-1^(-/-) (without IBs),and further verified the expression of key genes via qPCR.This is the first study to identify a gene that controls the amount of IBs in fish,and it provides a new sight into the effects of scxa on the molecular mechanism of IB development in fish. 展开更多

Intermuscular bones(IBs)are slender linear bones embedded in muscle,which ossify from tendons through a process of intramembranous ossification,and only exist in basal teleosts.IBs are essential for fish swimming,but they present a choking risk during human consumption,especially in children,which can lead to commercial risks that have a negative impact on the aquaculture of these fish.In this review,we discuss the morphogenesis and functions of IBs,including their underlying molecular mechanisms,as well as the advantages and disadvantages of different methods for IB studies and techniques for breeding and generating IB-free fish lines.This review reveals that the many key genes involved in tendon development,osteoblast differentiation,and bone formation,e.g.,scxa,msxC,sost,twist,bmps,and osterix,also play roles in IB development.Thus,this paper provides useful information for the breeding of new fish strains without IBs via genome editing and artificial selection. 展开更多

Ectopic adiposity has gained considerable attention because of its tight association with metabolic and cardiovascular health in persons with spinal cord injury(SCI). Ectopic adiposity is characterized by the storage of adipose tissue in non-subcutaneous sites. Magnetic resonance imaging(MRI) has proven to be an effective tool in quantifying ectopic adiposity and provides the opportunity to measure different adipose depots including intermuscular adipose tissue(IMAT) and intramuscular adipose tissue(Intra MAT) or intramuscular fat(IMF). It is highly important to distinguish and clearly define these compartments, because controversy still exists on how to accurately quantify these adipose depots. Investigators have relied on separating muscle from fat pixels based on their characteristic signal intensities. A common technique is plotting a threshold histogram that clearly separates between muscle and fat peaks. The cut-offs to separate between muscle and fat peaks are still not clearly defined and different cut-offs have been identified. This review will outline and compare the Midpoint and Otsu techniques, two methods used to determine the threshold between muscle and fat pixels on T1 weighted MRI. The process of water/fat segmentation using the Dixon method will also be outlined. We are hopeful that this review will trigger more research towards accurately quantifying ectopic adiposity due to its high relevance to cardiometabolic health after SCI. 展开更多

We documented the number, morphology, and distribution of intermuscular bones in five fishes of different ploidy: Carassius auratus (Abbr.WCC, 2n=100), Carassius auratus variety PengZe (Abbr.PZCC, 3n=150), improved triploid crucian carp (Abbr.ITCC, 3n=150), improved red crucian carp (Carassius auratus red var., Abbr.IRCC, , 2n=100), and improved allotet- raploids (Abbr.GAT, (, 4n=200). The number of intermuscular bones in WCC, PZCC, and GxAT ranged from 78 to 83 (n =81), 80 to 86 (n =84), and 77 to 84 (n=82), respectively. The numbers in ITCC and IRCC were significantly lower, ranging from 77 to 82 ( n =79) and 58 to 77 ( n =71), respectively. The average number of intermuscular bones in each sarco- mere, ranked in order from highest to lowest, was 0.721 (WCC), 0.673 (PZCC), 0.653 (GAT), 0.633 (ITCC), and 0.608 (IRCC). There was no difference between ITCC and GxAT or between GxAT and PZCC. However, the average number of intermuscular bones in the sarcomeres of ITCC, WCC, and PZCC differed significantly, as did that of IRCC and the four other kinds of fish. The intermuscular bone of these five fishes was divided into seven shape categories, non-forked (I), one-end-unequal-bi-fork (), one-end-equal-bi-fork (Y), one-end-multi-fork, two-end-bi-fork, two-end-multi-fork, and tree- branch types. Generally, the morphological complexity was higher in the anterior intermuscular bones than in the posterior body. The number of intermuscular bones was similar but not equal between the left and right sides of the body. ITCC had sig- nificantly fewer intermuscular bones than either WCC or PZCC, making it of greater commercial value. Additionally, IRCC and ITCC had fewer intermuscular bones than WCC. Our observations are significant in both fish bone developmental biology and genetic breeding. 展开更多

In this study, we aimed to evaluate changes of vertical palpebral fissure height (VPFH) after unilateral lateral rectus muscle recession. Twenty-five and twenty-six patients who were candidates for lateral rectus muscle recession were assigned into “with” and “without” intermuscular septum dissection study arms. The VPFH was measured at one-day before surgery and in two weeks and three months, postoperatively. Three months after surgery, significant increase of VPFH was observed in both groups (Paired t-test; P=0.005). Also, less widening of VPFH was observed in “with intermuscular septum dissection” group (Change in VPFH in “with intermuscular septum dissection” vs “without intermuscular septum dissection” groups: 0.48 mm vs 1.34 mm; ANCOVA test; P〈0.001). However, such results were not observed two weeks post-operatively (Change in VPFH in “with intermuscular septum dissection” vs “without intermuscular septum dissection” groups: -0.28 mm vs 0.28 mm; ANCOVA test; P=0.302). Intermuscular septum dissection is recommended in lateral rectus muscle recession to partially prevent the undesirable increment of VPFH. 展开更多

Endoscopic intermuscular dissection for locally advanced rectal cancer: A case report

BACKGROUND Endoscopic submucosal dissection is considered curative for patients with early rectal cancer when level of submucosal invasion is<1000 microns with favourable histopathological features.Recent data suggests even deeper submucosal invasion can potentially be curative if R0 resection can be achieved and when no high-risk histopathological features are seen in the resected specimen.To achieve R0 resection,deeper dissection is required.CASE SUMMARY A 66 year old New Zealand European male presented with 3 mo history of per rectal bleeding.He was referred for a colonoscopy test to investigate this further.This revealed a malignant appearing lesion in the rectum.Biopsies however showed high grade dysplasia only.Given endoscopic appearances suspicious for deep submucosal invasion,patient was consented for endoscopic intermuscular dissection(EID).The case was successfully performed,and the presence of muscularis propria was confirmed in the resected specimen.There were no complications and total procedure time was 124 min.Lesion was clear of radial margins however deep margins were positive confirming it was at least a pT2 cancer.Patient was recommended to have further treatment but could not have radical surgery due to comorbidities and instead was referred for long course chemoradiotherapy.CONCLUSION EID is a safe and feasible option for management of rectal cancer in highly selected patients. 展开更多

目的观察右美托咪定联合罗哌卡因用于超声引导下肌间沟臂丛神经阻滞的麻醉效果。方法选取2017年4月至2018年6月在复旦大学附属华山医院北院接受上肢手术治疗的124例患者作为研究对象,以随机数表法将患者分为观察组和对照组,每组62例,两... 目的观察右美托咪定联合罗哌卡因用于超声引导下肌间沟臂丛神经阻滞的麻醉效果。方法选取2017年4月至2018年6月在复旦大学附属华山医院北院接受上肢手术治疗的124例患者作为研究对象,以随机数表法将患者分为观察组和对照组,每组62例,两组患者均使用罗哌卡因配合超声波引导下肌间沟臂丛神经阻滞麻醉,观察组患者麻醉加用右美托咪定,而对照组患者加用等剂量医用生理盐水。比较两组患者麻醉阻滞起效时间、麻醉阻滞持续时间,患者麻醉前10 min (T1)、接受麻醉后20 min (T2)和40 min (T3)、手术完成后10 min (T4)的心率和平均动脉压,并对患者的麻醉效果进行评价。结果观察组患者的麻醉阻滞时间为(684.42±28.53) min,明显长于对照组的(583.32±25.53) min,麻醉阻滞起效时间为(11.23±0.97) min,明显短于对照组的(15.26±1.11) min,差异均具有统计学意义(P<0.05);T1时,观察组患者心率和平均动脉压分别为(80.85±4.99)次/min和(87.47±5.74) mmHg,分别与对照组的(80.91±5.28)次/min、(87.02±6.11) mmHg比较差异均无统计学意义(P>0.05);在T2、T3及T4时,观察组患者的心率分别为(76.11±5.26)次/min、(75.23±6.17)次/min、(78.74±5.21)次/min,均高于对照组的(66.29±5.41)次/min、(65.78±6.03)次/min、(72.78±5.26)次/min,平均动脉压分别为(80.55±6.11) mmHg、(79.38±6.04) mmHg、(83.96±6.02) mm Hg,均低于对照组的(85.38±7.08) mmHg、(84.27±6.42) mmHg、(86.24±6.37) mmHg,差异均具有统计学意义(P<0.05);观察组患者麻醉效果优良率为93.55%,明显高于对照组的73.42%,差异具有统计学意义(P<0.05)。结论右美托咪定联合罗哌卡因在超声引导下对肌间沟臂丛神经进行麻醉阻滞效果显著,可为手术顺利进行提供有效保障,值得临床借鉴使用。 展开更多

目的:为腓肠神经-小隐静脉营养血管远端蒂复合瓣设计提出解剖学依据。方法:30侧经动脉灌注红色乳胶成人下肢标本,解剖观测腓动脉肌间隔穿支、腓肠外侧动脉肌皮穿支与腓肠神经-小隐静脉、腓肠肌外侧头、比目鱼肌以及腓骨营养血管的关系;... 目的:为腓肠神经-小隐静脉营养血管远端蒂复合瓣设计提出解剖学依据。方法:30侧经动脉灌注红色乳胶成人下肢标本,解剖观测腓动脉肌间隔穿支、腓肠外侧动脉肌皮穿支与腓肠神经-小隐静脉、腓肠肌外侧头、比目鱼肌以及腓骨营养血管的关系;小隐静脉浅深交通支。结果:腓动脉肌间隔穿支6~10支,外径0.5~1.6mm,最远的动脉穿支距外踝尖上(1.0±1.3)cm,外径(0.6±0.2)mm。穿支分出骨膜动脉、肌支,营养腓骨和比目鱼肌外侧半。穿支穿深筋膜时,发深筋膜支、皮支、皮神经浅静脉血管,构成腓肠神经--小隐静脉营养血管链。腓肠外侧动脉发2~5支外径0.2~1.2mm肌皮穿支,营养腓肠肌外侧头及相应区域皮肤。小隐静脉浅深交通支距外踝尖上(3.4±0.9)cm,外径(1.7±0.5)mm。结论:腓肠神经-小隐静脉营养血管与肌、骨、皮营养血管同源,以腓动脉的肌间隔动脉终末穿支远端蒂复合瓣,旋转点近外踝尖平面,可覆盖前足创面。 展开更多

目的:观察腰椎周围肌间隙的解剖学与影像学特点,为腰椎后路椎旁肌间隙入路的选择和操作提供形态学依据。方法:选取18具成人尸体湿性躯干标本,一侧行局部解剖观察腰椎周围肌肉及其毗邻结构,另一侧行模拟肌间隙手术入路,观察不同肌间隙的... 目的:观察腰椎周围肌间隙的解剖学与影像学特点,为腰椎后路椎旁肌间隙入路的选择和操作提供形态学依据。方法:选取18具成人尸体湿性躯干标本,一侧行局部解剖观察腰椎周围肌肉及其毗邻结构,另一侧行模拟肌间隙手术入路,观察不同肌间隙的解剖构成及其显露腰椎相关结构的特点;同时选取30例健康志愿者的腰部MRI与CT图像,观察肌间隙的影像学显示特点。结果:Wiltse间隙位于多裂肌与最长肌之间,由脂肪及疏松结缔组织填充,无血管、神经穿行;通过该间隙可清晰显露腰椎关节突关节、椎间孔及横突;75.0%的CT片和93.3%的MRI片可显示Wiltse间隙。LIMP间隙位于最长肌与髂肋肌之间,由一层薄而透明的筋膜分隔两肌,69.4%标本可见脊神经后支的肌支穿行间隙内;该间隙可满意显露椎间孔区及椎弓根;41.6%的CT片和56.6%的MRI片可显示LIMP间隙。Watkins间隙位于髂肋肌与腰方肌之间,由腰背筋膜中层及少量脂肪分隔肌肉形成,75.0%标本可见脊神经后支的皮支及伴行血管走行;该间隙可满意显露椎间孔及椎体两侧;63.3%的CT片和86.6%的MRI片可显示Watkins间隙。结论:熟悉腰椎不同肌间隙的形态特点及穿行结构,术前影像学准确观测有助于椎旁肌间隙入路的选择与操作。 展开更多

对骨舌鱼总目、海鲢总目、鲱形总目和骨鳔总目等低等真骨鱼类肌间骨的数量、形态和分布进行了比较分析,结果如下:1.海鳗肌间骨的数目最多(409枚),其次是刀鲚(221枚),黄颡鱼肌间骨数最少(8枚),双须骨舌鱼次之(70枚),鲤科... 对骨舌鱼总目、海鲢总目、鲱形总目和骨鳔总目等低等真骨鱼类肌间骨的数量、形态和分布进行了比较分析,结果如下:1.海鳗肌间骨的数目最多(409枚),其次是刀鲚(221枚),黄颡鱼肌间骨数最少(8枚),双须骨舌鱼次之(70枚),鲤科鱼类的肌间骨数目在99~133之间。2.从骨舌鱼总目到骨鳔总目,各种肌间骨可归纳为7种形态类型,从简单形态向复杂形态演化,然后出现退化现象。双须骨舌鱼只有形态最简单的“I”形髓弓小骨,没有脉弓小骨;海鲢总目的海鳗出现一端两分叉的形态,而鲱形总目的鲥和刀鲚出现一端多分叉的形态,到骨鳔总目的各种鱼类,开始出现两端两分叉,并在此基础上出现各种更复杂的形态;肌间骨形态在鲤科的鲒亚科最复杂,从雅罗鱼亚科开始退化,到鲇形目髓弓小骨完全消失,而脉弓小骨仅留少数简单的I形;脉弓小骨形态的复杂性要比髓弓小骨低。3.从前向后,髓弓小骨形态依照从复杂到简单的顺序排列,而脉弓小骨没有这种明显的排列顺序。根据低等真骨鱼类的系统发育关系,结合肌间骨形态及其在各肌间隔的分布规律,本文认为,目前鲤科鱼类的各种肌间骨是通过两个途径演化而来的:从“I”形-“卜”形-“Y”形-一端多叉形和从“I”形-“卜”形-“Y”形-两端两分叉形-两端多叉形-树枝形。 展开更多

目的探讨经胸锁乳突肌肌间入路(SMIA)行甲状腺切除的可行性及其在颈前区功能的保护作用。方法回顾性分析2019年5—8月浙江大学医学院附属第一医院甲状腺疾病诊治中心收治的采取SMIA手术治疗的60例单侧甲状腺癌病人的临床资料(SMIA组),... 目的探讨经胸锁乳突肌肌间入路(SMIA)行甲状腺切除的可行性及其在颈前区功能的保护作用。方法回顾性分析2019年5—8月浙江大学医学院附属第一医院甲状腺疾病诊治中心收治的采取SMIA手术治疗的60例单侧甲状腺癌病人的临床资料(SMIA组),以同期采取低领弧形切口颈白线入路(LACA)手术治疗的60例单侧甲状腺癌病人作为对照(LACA组)。比较两组间临床病理特征、手术相关结果及术后颈前区功能的差异。采用EQ-5D-5L量表评估颈前区功能的主观感受,采用Hollander伤口评估量表评定切口愈合的临床效果。结果两组病人在年龄、性别、肿瘤大小、手术时间、术中出血、淋巴结清扫数目、术后甲状旁腺激素水平、引流量等方面差异均无统计学意义(P>0.05)。术后随访1个月,两组在保护颈前区功能和减少术后颈部不适感觉方面差异有统计学意义(P<0.001),且SMIA组均优于LACA组;在切口愈合及临床效果方面,SMIA组Hollander伤口评估量表得分优于LACA组,差异有统计学意义(P<0.05)。结论经SMIA行甲状腺切除不增加并发症的发生风险,且对甲状腺手术颈前区功能保护的作用优势明显,是安全可行的手术入路。 展开更多

目的探讨胫后动脉内踝上肌间隙支逆行岛状皮瓣的解剖特点及临床应用效果。方法解剖观测10具成年人体标本(20侧下肢)的胫后动脉内踝上肌间隙支的起源、走行、数目、管径和分布。10例同侧及对侧踝周皮肤缺损行胫后动脉内踝上肌间隙支逆... 目的探讨胫后动脉内踝上肌间隙支逆行岛状皮瓣的解剖特点及临床应用效果。方法解剖观测10具成年人体标本(20侧下肢)的胫后动脉内踝上肌间隙支的起源、走行、数目、管径和分布。10例同侧及对侧踝周皮肤缺损行胫后动脉内踝上肌间隙支逆行岛状皮瓣修复,年龄20~50岁,供区植皮。结果胫后动脉在内踝上发出肌间隙支2~7支,其外径约0.4~1.8mm,长度约0.3~4.5cm。本组患者修复皮瓣大小为7cm×6cm~20cm×8cm,除1例皮瓣远端有约3cm×1cm的坏死外,10例皮瓣完全成活,7例获随访2个月至3年,皮瓣色泽、质地、外形良好。结论该皮瓣血运可靠,修复范围广,不牺牲主要血管,手术简便安全,是修复踝周缺损的理想皮瓣之一。 展开更多

研究利用整体骨骼染色、形态学解剖和X光透射的方法,对团头鲂(Megalobrama amblycephala)仔稚鱼肌间骨的出现时期、形态以及成鱼肌间骨数目、形态、分布和长度变化进行了观察与分析。结果表明:团头鲂的肌间骨在孵出后20d(体长为1.33 cm... 研究利用整体骨骼染色、形态学解剖和X光透射的方法,对团头鲂(Megalobrama amblycephala)仔稚鱼肌间骨的出现时期、形态以及成鱼肌间骨数目、形态、分布和长度变化进行了观察与分析。结果表明:团头鲂的肌间骨在孵出后20d(体长为1.33 cm)骨化出现,首先出现在尾部,然后向头部方向依次出现,到第40天(体长为2.36 cm)基本全部出现;肌间骨出现与分化的时间受生长发育的影响大于日龄的影响。团头鲂肌间骨数目在108—129,平均为119根,其中躯干部轴上肌中的肌间骨数目最多(40—45根),尾部轴上肌与轴下肌中的肌间骨数目相近(32—39根)。肌间骨形态包括"1"形、"卜"形、"y"形、一端多叉形、两端多叉形和"("形6种类型,各种形态的肌间骨均是从"1"形发展而来;肌间骨越靠鱼体前端形态越复杂。团头鲂躯干轴上肌中的肌间骨显著长于尾部肌肉中的肌间骨(P<0.05),躯干轴下肌中的肌间骨最短,并且肌间骨长度与个体体重与体长呈正相关。研究结果为今后揭示团头鲂肌间骨发生与发育的分子机制,抑制团头鲂肌间骨骨化,培育无肌间骨的团头鲂提供了形态学基础。 展开更多
