目的比较3种方法检测鼠粪中微小隐孢子虫卵囊的效果。方法应用改良抗酸染色法(MAFS)、免疫荧光单克隆抗体(IFA-McAb)检测法和PCR检测法分别对含有不同数量隐孢子虫卵囊的鼠粪样品、隐孢子虫病模型小鼠的粪样品和奶牛粪样品进行检测。结... 目的比较3种方法检测鼠粪中微小隐孢子虫卵囊的效果。方法应用改良抗酸染色法(MAFS)、免疫荧光单克隆抗体(IFA-McAb)检测法和PCR检测法分别对含有不同数量隐孢子虫卵囊的鼠粪样品、隐孢子虫病模型小鼠的粪样品和奶牛粪样品进行检测。结果MAFS检测法I、FA-McAb检测法和PCR检测法的检测域值分别为10 g鼠粪中含有隐孢子虫卵囊2.0×104、2.0×104、2.0×102个,操作用时分别为35 min6、0 min3、h,检测鼠粪样品的阳性率分别为65.00%、70.00%、85.00%,检测牛粪样品的阳性率分别为21.21%、18.18%、27.27%。3种方法的阳性率差异无显著性(P>0.05)。结论MAFS检测法简单、快速,费用低,试验条件要求不高,适合对大量样品的检测和流行病学调查;IFA-McAb检测法、PCR检测法更适合用于隐孢子虫的鉴定。 展开更多

目的:研究菊苣多糖(chicory polysaccharide,CP)对免疫抑制小鼠免疫功能的影响。方法:ICR小鼠随机分为空白组、模型组、药物组(脾氨肽)、CP低、中、高剂量组。除空白组外,其余各组用环磷酰胺造成免疫抑制小鼠模型。分别测定各组小鼠体... 目的:研究菊苣多糖(chicory polysaccharide,CP)对免疫抑制小鼠免疫功能的影响。方法:ICR小鼠随机分为空白组、模型组、药物组(脾氨肽)、CP低、中、高剂量组。除空白组外,其余各组用环磷酰胺造成免疫抑制小鼠模型。分别测定各组小鼠体质量、免疫器官指数、外周血白细胞和红细胞数量、迟发型超敏反应致小鼠足趾肿胀度、腹腔巨噬细胞吞噬能力、血清血溶素水平及血清中白细胞介素2(IL-2)、白细胞介素6(IL-6)以及肿瘤坏死因子α(TNF-α)的活性水平。结果:与模型组相比,CP高剂量组能显著提高免疫抑制小鼠的净体质量增率、免疫器官指数、巨噬细胞吞噬率和吞噬指数、白细胞和红细胞数量以及降低足趾肿胀度(P<0.05)。同时,CP高剂量组还能明显提高小鼠腹腔巨噬细胞吞噬能力、血清溶血素含量,调节免疫抑制小鼠的免疫细胞因子。结论:CP对免疫抑制小鼠具有增强免疫活性的作用。 展开更多

In the present study, the effects of Pleurotus nebrodensis polysaccharide(PN-S) on the immune functions of immunosuppressed mice were determined. The immunosuppressed mouse model was established by treating the mice with cyclophosphamide(40 mg/kg/2d, CY) through intraperitoneal injection. The results showed that PN-S administration significantly reversed the CY-induced weight loss, increased the thymic and splenic indices, and promoted proliferation of T lymphocyte, B lymphocyte, and macrophages. PN-S also enhanced the activity of natural killer cells and increased the immunoglobulin M(Ig M) and immunoglobulin G(Ig G) levels in the serum. In addition, PN-S treatment significantly increased the phagocytic activity of mouse peritoneal macrophages. PN-S also increased the levels of interleukin-6(IL-6), tumor necrosis factor-α(TNF-α), interferon-γ(INF-γ), and nitric oxide(NOS) in splenocytes. q RT-PCR results also indicated that PN-S increased the m RNA expression of IL-6, TNF-α, INF-γ, and nitric oxide synthase(i NOS) in the splenocytes. These results suggest that PN-S treatment enhances the immune function of immunosuppressed mice. This study may provide a basis for the application of this fungus in adjacent immunopotentiating therapy against cancer and in the treatment of chemotherapy-induced immunosuppression. 展开更多

研究玉竹多糖和板蓝根多糖对环磷酰胺致雏鸡免疫抑制的调节作用。将120只1日龄雏鸡随机分成6组,分别为空白对照组、玉竹多糖对照组、玉竹多糖调节组、板蓝根多糖对照组、板蓝根多糖调节组、免疫抑制对照组,除空白对照组外,其余各组在7... 研究玉竹多糖和板蓝根多糖对环磷酰胺致雏鸡免疫抑制的调节作用。将120只1日龄雏鸡随机分成6组,分别为空白对照组、玉竹多糖对照组、玉竹多糖调节组、板蓝根多糖对照组、板蓝根多糖调节组、免疫抑制对照组,除空白对照组外,其余各组在7日龄时饮水免疫鸡新城疫、传染性支气管炎二联活疫苗(LaSota株+H120株)。测定免疫后不同阶段雏鸡新城疫疫苗抗体滴度、血清IgA含量和外周淋巴细胞OD570 nm值,以及脾脏指数和法氏囊指数。结果显示,环磷酰胺造模后,可见环磷酰胺抑制对照组抗体滴度持续处于较低的状态,血清IgA含量较低,外周淋巴细胞增殖受抑制,免疫器官指数较低;在免疫后14 d、21 d、28 d和35 d,玉竹多糖调节组和板蓝根多糖调节组血清抗体滴度显著高于免疫抑制对照组(P<0.05);在免疫后21d、35 d,玉竹多糖调节组和板蓝根多糖调节组血清IgA含量、外周淋巴细胞OD570 nm值和免疫器官指数显著高于免疫抑制对照组(P<0.05)。本试验结果表明,玉竹多糖和板蓝根多糖对环磷酰胺造模形成免疫抑制雏鸡免疫系统具有调节功能,可解除免疫抑制,使免疫系统恢复至正常水平,能够提高雏鸡新城疫疫苗抗体滴度、血清IgA含量,增强外周淋巴细胞增殖。 展开更多

With the rise of probiotics fermentation in food industry,fermented foods have attracted worldwide attention.In this study,protective effects of Rosa roxburghii&edible fungus fermentation broth(REFB)on immune function and gut health in Cyclophosphamide induced immunosuppressed mice were investigated.Results showed that REFB could improve the immune organ index,and promote the proliferation and differentiation of splenic T lymphocytes.In addition,it attenuated intestinal mucosal damage and improved intestinal cellular immunity.REFB administration also up-regulated the expression of IL-4,INF-γ,TNF-α,T-bet and GATA-3 mRNA in small intestine.Furthermore,administration of REFB modulated gut microbiota composition and increased the relative abundance of beneficial genus,such as Bacteroides.It also increased the production of fecal short-chain fatty acids.These indicate that REFB has the potential to improve immunity,alleviate intestinal injury and regulate gut microbiota in immunosuppressed mice. 展开更多

To study the effect of itraconazole on the vaginal candidiasis caused by Candida under different immunity conditions, the fungal vaginitis model was established in female ICR mice by intravaginal inoculation of suspension of C. albicans after the animal had been pretreated with estradiol or dexamethasone. Mice were divided at random into different groups and then treated with itraconazole or IFN-y given by gastrogavage. The burden of the fungus in the vaginal lavage fluids in the mice of the different groups was measured dynamically at different time points after the beginning of the drug treatment. The difference in the effect of itraconazole on the vaginal candidiasis between normal immune system group (group A) and control group (group D) was statistically significant (P〈0.01). The difference in the efficacy of itraconazole among immunosuppressed group (group E), immuno-regulated group (group F) and the control group (group G) was statistically significant (P〈0.01). But on the 5th, 6th, 7th, 9th, 11th day after the inoculation the average level of colony forming unit (CFU) of groups A, E and F showed no statistically significant difference (P〉0.05). It is concluded that the efficacy of itraconazole in the treatment of the vaginal candidiasis under different immunity conditions (groups A, E and F) in mice were all good, but there was no difference in the anti-fungal effect of itraconazole among the three groups. 展开更多

Methods Mice were given RDAEB through stomach perfusion for 10 d (50 mg/d). Then, RBC-C3bR rate,RBC-IC rate (as the index- es of erythrocyte immunity)and E-rosette forming rate,acidic a-naphthyl acetate esterase positive rate, lymphocyte transformation rate (as the indexes of cellular immunity) of mice were tested. Results RBC-C3Br rate, RBC-IC rat- e,E-rosette forming rate, acidic α-naphthyl acetate esterase positive rate and lymphocyte transformation rate in the Cy-RDAEB group were markedly higher than those in the Cy group (P<0.0l),and returned to the levels of normal group. Conclusion RDAEB is effective in recovering and enhancing cellular and erythrocyte immunity of immuno- suppressed mice. 展开更多

目的 研究宏基因组二代测序(metagenomic Next-Generation Sequencing, mNGS)在出现肺部浸润的免疫抑制患者病因诊断中的应用价值。方法 前瞻性收集2019年11月至2022年11月间收治的60例出现肺部浸润的免疫抑制患者的临床资料,采集样本(... 目的 研究宏基因组二代测序(metagenomic Next-Generation Sequencing, mNGS)在出现肺部浸润的免疫抑制患者病因诊断中的应用价值。方法 前瞻性收集2019年11月至2022年11月间收治的60例出现肺部浸润的免疫抑制患者的临床资料,采集样本(包括肺泡灌洗液、血液、深部痰液、肺组织、胸水)进行mNGS和常规实验室检查(培养、涂片等),探究mNGS在该类患者病因诊断中的性能。结果 mNGS检测的总体阳性率为88.3%,显著高于常规实验室检测方法;55.0%(33/60)患者样本的两种检测结果一致,同时mNGS对真菌、病毒和非典型病原微生物的检测优于常规检测;免疫抑制患者肺部浸润的最主要病因是感染,mNGS在病因分析中的检测灵敏度为97.7%,特异度为52.9%,阳性预测值为84.0%,阴性预测值为90.0%。结论 mNGS具有较高的灵敏度、阳性预测值和阴性预测值,可联合多种常规实验室检测方法为临床精准诊断免疫抑制患者肺部浸润的病因提供依据。 展开更多

Clinical experience has suggested the existence of an intermediate form of fungal sinusitis between the categories of non-invasive fungal sinusitis (non-IFS) and invasive fungal sinusitis (IFS).This fungal sinusitis variant demonstrates unhealthy mucosa by endoscopy with fungal invasion,but lacks angioinvasion microscopically,representing what clinically behaves as a ‘pre-invasive’ subtype of fungal sinusitis.Unlike non-IFS disease,patients with preinvasive fungal sinusitis were still felt to require anti-fungal medications due to histologic presence of invasive fungus.While sharing some clinical features of IFS,these ‘intermediate’ patients were successfully spared extended and repeated surgical debridements given the microscopic findings,and have been successfully treated with shorter courses of antifungal therapy.These select patients have had favorable outcomes when managed in a judicious and semi-aggressive manner,in an undefined zone between the treatments for routine fungal ball and aggressive IFS. 展开更多

目的本研究利用小鼠动物模型,探讨烟曲霉感染免疫缺陷小鼠TLR2/TLR3/TLR4对树突状细胞植物血凝素-1(Dectin-1)表达的影响。通过检测TLRs和CLRs受体在不同免疫状态下的表达,揭示介导侵袭性肺曲霉菌病(IPA)肺损伤的关键受体。方法小鼠随... 目的本研究利用小鼠动物模型,探讨烟曲霉感染免疫缺陷小鼠TLR2/TLR3/TLR4对树突状细胞植物血凝素-1(Dectin-1)表达的影响。通过检测TLRs和CLRs受体在不同免疫状态下的表达,揭示介导侵袭性肺曲霉菌病(IPA)肺损伤的关键受体。方法小鼠随机分成四组,A组为正常对照组;B组为环磷酰胺免疫抑制+未接种烟曲霉菌;C组正常状态小鼠+烟曲霉菌接种;D组为IPA感染组,免疫抑制+烟曲霉菌接种。采用real-time PCR方法进行小鼠肺组织各个时相点的TLR2、TLR3、TLR4、Dectin-1和β-actin m RNA的表达,检测受体间的相互表达调控。结果正常状态感染组(C组N+AF),TLR4/TLR2、Dectin-1 m RNA比正常对照组表达上调;IPA模型组(D组CP+AF),Dectin-1和TLR4、TLR2 m RNA比正常状态感染组表达下调;但TLR3 m RNA 48 h、72 h表达没有明显不同(P>0.05)。病理切片模型组小鼠72 h可见较严重的出血和充血;96 h时肺内有菌丝,肺泡间隔增宽,支气管壁破坏。正常状态感染烟曲霉小鼠72 h可见充血,肺泡弹性纤维破坏。结论成功建立了小鼠IPA动物模型,Dectin-1受体表达下调可能是环磷酰胺免疫抑制引起IPA的机制之一。Dectin-1的表达可能是建立在TLR2/TLR4对烟曲霉早期识别的基础上,TLR3可能起协同作用。 展开更多

目的免疫抑制小鼠经气管切开注入烟曲霉孢子悬液建立侵袭性肺曲霉病(IPA)动物模型并动态观察其肺组织病理改变,以期为IPA研究提供在体模型及观察IPA疾病发展过程。方法C57BL/6J小鼠经腹腔注射250mg/kg环磷酰胺×2次构建免疫抑... 目的免疫抑制小鼠经气管切开注入烟曲霉孢子悬液建立侵袭性肺曲霉病(IPA)动物模型并动态观察其肺组织病理改变,以期为IPA研究提供在体模型及观察IPA疾病发展过程。方法C57BL/6J小鼠经腹腔注射250mg/kg环磷酰胺×2次构建免疫抑制状态;麻醉状态下于小鼠颈部切开-0.3cm小口,分离出气管并接种烟曲霉临床株;动态观察小鼠一般情况、生存率、肺部真菌负荷、肺组织大体及病理变化。结果小鼠感染烟曲霉后第4天死亡率为100%;感染后第1天肺部曲霉孢子含量最高,后逐渐降低;肺组织大体表现为出血、水肿及白色菌团结节,呈逐渐加重趋势;病理发现肺泡内炎症细胞渗出,以淋巴细胞浸润为主,肺组织大量出血、坏死,随时间延长而加重,最终正常肺泡结构消失。结论通过环磷酰胺腹腔注射使小鼠免疫抑制,经气管切开成功建立了IPA小鼠模型,过度炎症反应是其死亡的重要原因。 展开更多

non-invasive chronic rhinosinusitis(CRS)following liver or kidney transplant and determine factors associated with disease-related complications,selection of endoscopic sinus surgery(ESS),and disease resolution in thi... non-invasive chronic rhinosinusitis(CRS)following liver or kidney transplant and determine factors associated with disease-related complications,selection of endoscopic sinus surgery(ESS),and disease resolution in this population.Study design:Retrospective chart review.Setting:An academic tertiary care center(Mayo Clinic,Rochester,Minnesota).Subjects and methods:Liver and kidney transplant recipients evaluated by Mayo Clinic otolaryngologists for CRS between 1998 and 2018 were identified.Univariate and multivariate logistic regression analyses were used to determine patient factors and treatment modalities associated with developing complications,selection of ESS,and disease resolution.Results:Fifty-seven patients met inclusion criteria.No patients developed intraorbital or intracranial complications of their CRS.Multivariate modeling demonstrated that the presence of polyps(P=0.036)was associated with undergoing ESS within one year of presentation.A higher Lund-Mackay(LM)computed tomography score(P=0.023)and older age(P=0.018)were significantly associated with decreased disease resolution.No other factors were significantly associated with the use of endoscopic sinus surgery within one year of otolaryngology presentation or resolution of CRS in this cohort.Conclusion:The risk of developing CRS-related intraorbital or intracranial complications in this immunecompromised patient cohort may be lower than originally thought.For liver-and kidney-recipients stable on immunosuppressive medication for many years,prognostic factors for CRS may mirror those for immunocompetent patients. 展开更多

BACKGROUND Severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)causes coronavirus disease 2019(COVID-19).This disease was declared a worldwide health problem with the characteristics of a pandemic.Most patients have mild symptoms and a good prognosis.Information on the evolution and prognosis of COVID-19 in solid organ recipients is scarce.CASE SUMMARY We describe two patients who underwent liver transplantation with a positive test result for detection of the viral sequence for COVID-19,using reversetranscription polymerase chain reaction(RT-PCR),immediately before transplantation.The patients showed good evolution in the postoperative period,without signs of graft dysfunction.The immunosuppressive therapy was not modified.Both patients were discharged for subsequent outpatient follow-up.CONCLUSION In conclusion,it is expected that the experience at this center can be used as an example,aimed at the continuation of transplantations by other services and,thus,the morbidity and mortality of patients with liver disease on the transplantation waiting list can be reduced.Transplant centers must be able to readjust daily to the evolution of the COVID-19 pandemic. 展开更多

The coronavirus disease 2019(COVID-19)has caused one of the worst public health crises in modern history.Even though severe acute respiratory syndrome coronavirus 2 primarily affects the respiratory tract,gastrointestinal manifestations are well described in literature.This review will discuss the epidemiology,virology,manifestations,immunosuppressant states,and lessons learned from COVID-19.Observations:At the time of writing,COVID-19 had infected more than 111 million people and caused over 2.5 million deaths worldwide.Multiple medical comorbidities including obesity,pre-existing liver condition and the use of proton pump inhibitor have been described as risk factor for severe COVID-19.COVID-19 most frequently causes diarrhea(12.4%),nausea/vomiting(9%)and elevation in liver enzymes(15%-20%).The current data does not suggest that patients on immunomodulators have a significantly increased risk of mortality from COVID-19.The current guidelines from American Gastroenterological Association and American Association for the Study of Liver Diseases do not recommend pre-emptive changes in patients on immunosuppression if the patients have not been infected with COVID-19.Conclusions and relevance:The COVID-19 pandemic has prompted a change in structure and shape of gastroenterology departmental activities.Endoscopy should be performed only when necessary and with strict protective measures.Online consultations in the form of telehealth services and home drug deliveries have revolutionized the field. 展开更多

试验旨在研究发酵中草药对环磷酰胺(CTX)致免疫低下小鼠的保护作用。将20±2 g昆明小鼠100只随机分为5组,每组20只,雌雄各两个重复,每个重复5只,各重复小白鼠分笼饲喂基础饲粮。正常组和模型组自来水灌胃,发酵中草药组中草药水溶液... 试验旨在研究发酵中草药对环磷酰胺(CTX)致免疫低下小鼠的保护作用。将20±2 g昆明小鼠100只随机分为5组,每组20只,雌雄各两个重复,每个重复5只,各重复小白鼠分笼饲喂基础饲粮。正常组和模型组自来水灌胃,发酵中草药组中草药水溶液灌胃,灌胃浓度分别为100、200和400 mg·kg-1,灌胃剂量0.1 mL·10 g-1体重,1次·d-1,连续20 d。在灌胃的第5天开始,除正常组外,模型组和发酵中草药组均腹腔注射CTX 40 mg·kg-1,1次·d-1,连续5 d。检测各组老鼠免疫器官指数、血清生化指标及碳粒廓清法检测单核-吞噬细胞吞噬功能。结果表明,与模型组相比,发酵中草药高、中、低剂量(400、200和100 mg·kg-1)均能够极显著提高免疫功能低下小鼠的胸腺指数(P<0.01),显著增加其脾脏指数和单核吞噬细胞的廓清指数和吞噬指数(P<0.05),同时显著降低血清谷丙转氨酶(ALT)、谷草转氨酶(AST)和尿素氮(BUN)含量(P<0.05)。表明发酵中草药对CTX致免疫功能低下小鼠具有保护作用。 展开更多

试验旨在研究4味中药对免疫抑制小鼠免疫功能和肠道菌群的影响,选取70只6周龄雌性昆明小鼠(18~20g),随机分为7组,分别为空白对照组、模型组、玉屏风组、乌梅组、蒲公英组、党参组、石榴皮组。采用环磷酰胺(cyclophosphamide,Cy)构建免... 试验旨在研究4味中药对免疫抑制小鼠免疫功能和肠道菌群的影响,选取70只6周龄雌性昆明小鼠(18~20g),随机分为7组,分别为空白对照组、模型组、玉屏风组、乌梅组、蒲公英组、党参组、石榴皮组。采用环磷酰胺(cyclophosphamide,Cy)构建免疫抑制病理模型,从喂药的第3天起,2~7组以75mg/kg体重腹腔注射Cy,每隔1天注射1次,共注射4次,注射期间正常给药,模型组灌胃0.9%生理盐水,直至停止注射Cy的第2天,共给药7d。检测4味中药对免疫抑制小鼠体重、脾脏和胸腺指数、血液指标、分泌型免疫球蛋白A(sIgA)、IgG及肠道菌群的影响。结果显示,与模型组相比,党参、乌梅、蒲公英、石榴皮可显著提高免疫抑制小鼠体重(P<0.05),其中乌梅组效果最好;显著提高免疫抑制小鼠免疫器官指数(P<0.05),但不能达到正常水平,其中蒲公英组脾脏指数最高;各中药组可提高白细胞数目(WBC)、显著提高红细胞数目(RBC)(P<0.05),但不能达到正常水平;各中药组可显著提高免疫球蛋白数量(P<0.05),党参组sIgA、IgG显著高于空白对照组(P<0.05),且和玉屏风组差异不显著(P>0.05);各中药组可显著降低大肠杆菌数量(P<0.05),显著增加双歧杆菌和乳酸杆菌数量(P<0.05),提高免疫抑制小鼠肠道菌群的丰富性和多样性,其中党参和蒲公英组小鼠肠道菌群的丰富度和多样性较好。综上说述,乌梅、蒲公英、石榴皮、党参均能够颉颃Cy对小鼠体重、免疫功能及肠道菌群的影响,可用于免疫增强类药物的开发。 展开更多
