A total of 164 results had complete data denoting date of cleansing, number of days stored and culture results. All scopes underwent initial cleaning in the endoscopy suite utilizing tap water, and then manually cleaned and flushed. High level disinfection was achieved with a Medivator~? DSD(Medivator Inc., United States) automated endoscope reprocessor following manufacturer instructions, with Glutacide~?(Pharmax Limited, Canada), a 2% glutaraldehyde solution. After disinfection, all scopes were stored in dust free, unfiltered commercial cabinets for up to 7 d. Prior to use, all scopes were sampled and plated on sheep blood agar for 48 h; the colony count was obtained from each plate. The length of endoscope hang time and bacterial load was analyzed utilizing unpaired t-tests. The overall percentage of positive and negative cultures for each type of endoscope was also calculated. RESULTS All culture results were within the acceptable range(less than 200 cfu/mL). One colonoscope cultured 80 cfu/mL after hanging for 1 d, which was the highest count. ERCP scopes cultured at most 10 cfu, this occurred after 2 and 7 d, and gastroscopes cultured 50 cfu/mL at most, at 1 d. Most cultures were negative for growth, irrespective of the length of hang time. Furthermore, all scopes, with the exception of one colonoscope which had two positive cultures(each of 10 cfu/mL), had at most one positive culture. There was no significant difference in the number of bacteria cultured after 1 d compared to 7 d when all scopes were combined(day 2: P = 0.515; day 3: P = identical; day 4: P = 0.071; day 5: P = 0.470; day 6: P = 0.584; day 7: P = 0.575). The velocity distribution in the uppermost crust is symmetrical on two sides of the central line of the sea, and coincides with the structure of crystalline basement. The essential difference in tectonics between the East China Sea and the Yellow Sea mainly lies in that the velocity structures of their lower crust and upper mantle are identical to those of South China and North China respectively. In the upper mantle there exists a high-velocity zone with a nearly EW strike from the Hangzhou Bay, China, to the Tokara Channel, Japan, along about the latitude of 30°N. It is found that between the East China Sea and the Yellow Sea there are systematical differences in geomorphology, geology, seismicity, heat flow, quality factor and gravity and aeromagnetic anomalies, which is related to both left-lateral shear dislocation and right-lateral tear of the Benioff zone from the Hangzhou Bay to the Tokara Channel.It is inferred that the East China Sea was formed by Cenozoic back-arc extension. The boundary between the North China and South China crustal blocks stretches along the southern piedmont of Mts. Daba-Dabie-Hangzhou Bay-Tokara Channel, and the subduction zone at the Okinawa trench is the eastern boundary of the South China crustal block. The movements of the Pacific plate, Indian plate and upper mantle rather than the Philippine plate subduction have played a dominant role for the modern tectonic movements in East Asia.展开更多
AIM: To analyze the surgical results of a slipped medial rectus muscle(MRM) after hang back recession surgery for esotropia.METHODS: Twenty-one patients who underwent reexploration for diagnosed slipped muscle after h...AIM: To analyze the surgical results of a slipped medial rectus muscle(MRM) after hang back recession surgery for esotropia.METHODS: Twenty-one patients who underwent reexploration for diagnosed slipped muscle after hang back recession surgery were included in this retrospective study. Dynamic magnetic resonance imaging was performed to identify the location of the slipped muscle.Ocular motility was evaluated with assessment with prism and cover test in gaze at cardinal positions. The operations were performed by the same consultant.Intraoperative forced duction test was performed under general anesthesia. The empty sheath of the slipped MRM was resected and the muscle was advanced to the original insertion site in all patients.RESULTS: The average age of 21 patients who had consecutive exotropia with a slipped MRM at the time of presentation was 17.4±5.4y(5-50y). The average duration between the first operation and the diagnosis of the slipped muscle was 25mo(12 to 36mo). The mean follow up after the corrective surgery was 28 mo. The mean preoperative adduction limitation in the field of action of the slipped muscle was-2.26(ranging from-1 to-4). All patients had full adduction postoperatively.CONCLUSION: The diagnosis of the slipped muscle should be confirmed during the strabismus surgery. The slipped muscle may be caused due to insufficient suture and excessive rubbing of the eye. When divergent strabismus is observed after the recession of the MRM, a slipped muscle should be considered in the differential diagnosis.展开更多
