一种语音交互智能认知评估小程序的设计与应用

目的研制一款适用于中国老年人的智能语音交互认知功能评估工具,用于早期筛查轻度认知障碍。方法利用微信开发者工具、语音识别技术、机器学习模型,研制一款语音交互的智能认知评估工具(SCS)。招募50~80岁受试者,采用SCS小程序和蒙特利... 目的研制一款适用于中国老年人的智能语音交互认知功能评估工具,用于早期筛查轻度认知障碍。方法利用微信开发者工具、语音识别技术、机器学习模型,研制一款语音交互的智能认知评估工具(SCS)。招募50~80岁受试者,采用SCS小程序和蒙特利尔认知评估基础版(Montreal Cognitive Assessment Basic,MoCA-B)进行评估,对SCS小程序认知评估的准确性作初步验证。结果SCS支持用户通过语音答题自主完成命名、即刻和延迟回忆、符号数字转换等认知任务,结合答题表现和语音的数字生物标志物对记忆、注意和语言等认知功能进行评估,并即刻生成报告,全程仅7 min。207名受试者的平均SCS分数为(86.80±15.53)分,与MoCA-B有良好的相关性(Pearson’s r=0.615,P<0.001)。结论SCS小程序适用于对中国老年人进行认知功能评估,较传统工具高效便利,值得推广用于社区筛查、远程评估和居家自测。 展开更多

Mobile health apps (MHAs) and medical apps (MAs) are becoming increasinglypopular as digital interventions in a wide range of health-related applications inalmost all sectors of healthcare. The surge in demand for digital medical solutionshas been accelerated by the need for new diagnostic and therapeutic methods inthe current coronavirus disease 2019 pandemic. This also applies to clinicalpractice in gastroenterology, which has, in many respects, undergone a recentdigital transformation with numerous consequences that will impact patients andhealth care professionals in the near future. MHAs and MAs are considered tohave great potential, especially for chronic diseases, as they can support the selfmanagementof patients in many ways. Despite the great potential associated withthe application of MHAs and MAs in gastroenterology and health care in general,there are numerous challenges to be met in the future, including both the ethicaland legal aspects of applying this technology. The aim of this article is to providean overview of the current status of MHA and MA use in the field ofgastroenterology, describe the future perspectives in this field and point out someof the challenges that need to be addressed. 展开更多

生物标志物一直是医学领域的研究热点.目前主要以实时荧光定量PCR技术作为分子诊断的检测手段,用于大样本的核酸检测,但该技术具有限制性,会导致假阴性.作为第三代PCR的数字PCR技术,优化并提高了检测灵敏度,无需标准曲线实现绝对定量检... 生物标志物一直是医学领域的研究热点.目前主要以实时荧光定量PCR技术作为分子诊断的检测手段,用于大样本的核酸检测,但该技术具有限制性,会导致假阴性.作为第三代PCR的数字PCR技术,优化并提高了检测灵敏度,无需标准曲线实现绝对定量检测,大大提高了检测的精准度,尤其是对于低表达RNA的检测,显得更为出色.本研究以hsacirc0061276为例,利用微滴式数字PCR EvaGreen染料法对血浆中hsacirc0061276的拷贝数进行绝对定量地检测,成功检测出胃炎和胃癌患者血浆中hsacirc0061276的具体拷贝数.因此,数字PCR作为新型检测技术在临床实际应用中有较大推广价值. 展开更多

BACKGROUND Due to the poor prognosis of gastric cancer(GC),early detection methods are urgently needed.Plasma exosomal circular RNAs(circRNAs)have been suggested as novel biomarkers for GC.AIM To identify a novel biomarker for early detection of GC.METHODS Healthy donors(HDs)and GC patients diagnosed by pathology were recruited.Nine GC patients and three HDs were selected for exosomal whole-transcriptome RNA sequencing.The expression profiles of circRNAs were analyzed by bioinformatics methods and validated by droplet digital polymerase chain reaction.The expression levels and area under receiver operating characteristic curve values of plasma exosomal circRNAs and standard serum biomarkers were used to compare their diagnostic efficiency.RESULTS There were 303 participants,including 240 GC patients and 63 HDs,involved in the study.The expression levels of exosomal hsa_circ_0079439 were significantly higher in GC patients than in HDs(P<0.0001).However,the levels of standard serum biomarkers were similar between the two groups.The area under the curve value of exosomal hsa_circ_0079439 was higher than those of standard biomarkers,including carcinoembryonic antigen,carbohydrate antigen(CA)19-9,CA72-4,alpha-fetoprotein,and CA125(0.8595 vs 0.5862,0.5660,0.5360,0.5082,and 0.5018,respectively).The expression levels of exosomal hsa_circ_0079439 were significantly decreased after treatment(P<0.05).Moreover,the expression levels of exosomal hsa_circ_0079439 were obviously higher in early GC(EGC)patients than in HDs(P<0.0001).CONCLUSION Our results suggest that plasma exosomal hsa_circ_0079439 is upregulated in GC patients.Moreover,the levels of exosomal hsa_circ_0079439 could distinguish EGC and advanced GC patients from HDs.Therefore,plasma exosomal hsa_circ_0079439 might be a potential biomarker for the diagnosis of GC during both the early and late stages. 展开更多

Increasing evidence suggests that the presence and spatial localization and distribution pattern of tumor infiltrating lymphocytes(TILs)is associate with response to immunotherapies.Recent studies have identified TGFβactivity and signaling as a determinant of T cell exclusion in the tumor microenvironment and poor response to PD-1/PD-L1 blockade.Here we coupled the artificial intelligence(AI)-powered digital image analysis and gene expression profiling as an integrative approach to quantify distribution of TILs and characterize the associated TGFβpathway activity.Analysis of T cell spatial distribution in the solid tumor biopsies revealed substantial differences in the distribution patterns.The digital image analysis approach achieves 74%concordance with the pathologist assessment for tumor-immune phenotypes.The transcriptomic profiling suggests that the TIL score was negatively correlated with TGFβpathway activation,together with elevated TGFβsignaling activity observed in excluded and desert tumor phenotypes.The present results demonstrate that the automated digital pathology algorithm for quantitative analysis of CD8 immunohistochemistry image can successfully assign the tumor into one of three infiltration phenotypes:immune desert,immune excluded or immune inflamed.The association between“cold”tumor-immune phenotypes and TGFβsignature further demonstrates their potential as predictive biomarkers to identify appropriate patients that may benefit from TGFβblockade. 展开更多
