目的:探讨应付技能培训对首发抑郁症患者自我接纳和应付方式的影响。方法:将80例首发抑郁症患者随机分为观察组和对照组,各组40例。两组均进行常规治疗和护理,观察组在此基础上实施应付技能培训。采用应付方式问卷、自我接纳量表、治疗... 目的:探讨应付技能培训对首发抑郁症患者自我接纳和应付方式的影响。方法:将80例首发抑郁症患者随机分为观察组和对照组,各组40例。两组均进行常规治疗和护理,观察组在此基础上实施应付技能培训。采用应付方式问卷、自我接纳量表、治疗依从性调查问卷于培训前、培训后1周和3个月对两组患者进行效果评定。结果:应付技能培训后1周、3个月时两组评分比较,观察组的自我接纳水平显著高于对照组(P<0.05,P<0.01);积极应付方式明显优于对照组,且持续时间长(P<0.05,P<0.01);治疗依从性明显优于对照组(P<0.05,P<0.01)。结论:应付技能培训可有效提高首发抑郁症患者的应付技巧和自我接纳水平,提高治疗依从性。 展开更多

①目的探讨应对技能训练(CST)、环境线索暴露(CET)等认知-行为干预对戒断期酒依赖患者焦虑、抑郁、渴求症状的改善以及复饮率的影响。②方法采用整群抽样方式选取某精神病院男性酒依赖住院患者174例,随机分为3组:综合干预组67例,除给予... ①目的探讨应对技能训练(CST)、环境线索暴露(CET)等认知-行为干预对戒断期酒依赖患者焦虑、抑郁、渴求症状的改善以及复饮率的影响。②方法采用整群抽样方式选取某精神病院男性酒依赖住院患者174例,随机分为3组:综合干预组67例,除给予常规治疗护理外,同时给予CST和CET干预措施;应对训练组50例,除给予常规治疗护理外,仅进行CST干预;对照组57例,仅给予常规治疗护理,不给予任何特殊干预。干预前后分别用SAS、SDS和AUQ量表进行心理测量,比较患者的焦虑评分、抑郁评分和渴求评分,并在患者出院6个月时调查分析其复饮情况。③结果干预前3组的评分均无显著性差异(P>0.05);干预6周后,2个干预组的干预前后SAS评分差值、SDS评分差值均明显低于对照组,差异有统计学意义(P<0.01);与对照组相比较,综合干预组的AUQ评分差值低于应对训练组,差异有非常显著的统计学意义(P<0.01);3组患者出院后6个月复饮率分别为:综合干预组31.3%、应对训练组44.0%、对照组54.4%,综合干预组与对照组的复饮率有显著性差异(P<0.01),而应对训练组与对照组复饮率无显著性差异(P>0.05)。④结论CST、CET等认知-行为干预能改善酒依赖患者焦虑、抑郁和渴求症状,尤其是“CST+CET”的综合性干预模式能更显著改善患者对酒的渴求心理,有效降低复饮率。 展开更多

Improving caregiving competence, stress coping, and mental well-being in informal dementia carers 被引量:1

AIM: To study the effectiveness of Reitman Centre "Coaching, Advocacy, Respite, Education, Relationship, and Simulation"(CARERS) program, which uses problem-solving techniques and simulation to train informal dementia carers. METHODS: Seventy-three carers for family members with dementia were included in the pilot study. Preand post-intervention data were collected from carers using validated measures of depression, mastery, role captivity and overload, caregiving competence and burden, and coping styles. To assess program effec-tiveness, mean differences for these measures were calculated. One-way ANOVA was used to determine if change in scores is dependent on the respective baseline scores. Clinical effects for measures were expressed as Cohen's D values. RESULTS: Data from 73 carers were analyzed. The majority of these participants were female(79.5%). A total of 69.9% were spouses and 30.1% were children of the care recipient. Participants had an overall mean age of 68.34 ± 12.01 years. About 31.5% of participating carers had a past history of psychiatric illness(e.g., depression), and 34.2% sustained strained relationships with their respective care recipients. Results from carers demonstrated improvement in carers' selfperception of competence(1.26 ± 1.92, P < 0.0001), and significant reduction in emotion-focused coping(measured by the Coping Inventory of Stressful Situations,-2.37 ± 6.73, P < 0.01), Geriatric Depression scale(-0.67 ± 2.63, P < 0.05) and Pearlin's overload scale(-0.55 ± 2.07, P < 0.05), upon completion of the Program. Secondly, it was found that carers with more compromised baseline scores benefited most from the intervention, as they experienced statistically significant improvement in the following constructs: competence, stress-coping style(less emotion-oriented), sense of mastery, burden, overload. Objective:The objective of this study is to assess the application effect of five elements music therapy introduced in the pain coping skills training of knee osteoarthritis(KOA).Materials and Methods:Totally,80 patients with KOA were selected and randomly divided into the experimental group(39 cases)and the control group(41 cases).The control group was only given routine nursing measures,and the experimental group was additionally treated with five-element music therapy on the basis of the control group,twice a day,28 days in total.The Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)was used to evaluate the functional status of the knee joint of the two groups.The clinical efficacy of the two groups was evaluated by Guiding Principles for Clinical Research of New Chinese Medicine in the Treatment of Osteoarthritis.Results:WOMAC score statistically significantly decreased in the experimental group(35.92±9.48 vs.16.17±5.43,P<0.01)and the control group(36.73±6.42 vs.22.53±7.51,P<0.01)after 28 days of intervention when compared with that before intervention;WOMAC score in the experimental group was lower than that of the control group after 28 days of intervention(16.17±5.43 vs.22.53±7.51,P<0.01).The total effective rate of the experimental group was statistically higher than that of the control group(82.0%vs.51.2%,χ2=11.97,P=0.003).Conclusion:The combination of five-element music therapy and routine nursing measures has better effect in relieving pain and bad emotions of patients with KOA when compared with routine nursing measures alone. 展开更多
