In each case, simple auscultation was used to guide SB tube insertion, with chest radiograph obtained only after complete inflation of the gastric balloon. Two patients died of hemorrhagic shock and one died of mediastinitis.The author suggests that confirmation of SB tube placement by auscultation alone may not be adequate.Routine chest radiographs should be obtained before and after full inflation of the gastric balloon to confirm tube position and to detect tube dislocation.展开更多

Focus was ...AIM To investigate the hemothorax size for which tube thoracostomy is necessary.METHODS Over a 5-year period, we included all patients who were admitted with blunt chest trauma to our level 1 trauma center. Focus was placed on identifying the hemothorax size requiring tube thoracostomy.RESULTS A total number of 274 hemothoraces were studied. All patients with hemothoraces measuring above 3 cm received a chest tube. The 50% predicted probability of tube thoracostomy was 2 cm. Pneumothorax was associated with odds of receiving tube thoracostomy for hemothoraces below 2 cm(Odds Ratio:4.967, 95%CI: 2.225-11.097, P < 0.0001).CONCLUSION All patients with a hemothorax size greater than 3% underwent tube thoracostomy. Prospective studies are warranted to elucidate the clinical outcome of patients with smaller hemothoraces.展开更多
文摘目的比较肺癌上叶切除术后置入单、双胸管的临床效果。方法 2013年3月-2014.12月江苏省昆山市第一人民医院43例肺癌(肿瘤位于上叶)患者,分为双胸腔引流管组[22例,男13例、女9例,年龄(57.2±9.0)岁]和单胸腔引流管组[21例,男12例、女9例,年龄(56.5±9.5)岁]。观察两组的引流管持续时间、引流量、术后住院时间。结果单胸腔引流管组引流量显著少于双胸腔引流管组[(746.4±215.9)m L vs.(522.4±190.1)m L,P<0.05]。两组术后带管时间、住院时间无统计学意义(P>0.05)。结论肺癌(肿瘤位于上叶)上肺切除术后单胸腔引流管引流效果不亚于双胸腔引流管效果,且能减轻患者术后疼痛,带管时间更短,值得临床推广。
