METHODS: A case series study was performed with 16 831 patients who underwent diagnostic esophagogastroduodenoscopy for 2 years at an academic referral center. A hiatus diameter ≥ 4 cm was defined as a large hernia. A sharp fold that surrounded the cardia was designated as an intact gastroesophageal flap valve (GEFV), and a loose fold or disappearance of the fold was classified as an impaired GEFV. We studied the associations between large hiatal hernias and the distinct clinical phenotypes (reflux esophagitis and Cameron lesions), and analyzed factors that distinguished the clinical phenotypes. RESULTS: Large hiatal hernias were found in 49 (0.3%) of 16 831 patients. Cameron lesions and reflux esopha-gitis were observed in 10% and 47% of these patients, and 0% and 8% of the patients without large hiatal hernias, which indicated significant associations between large hiatal hernias and these diseases. However, there was no coincidence of the two distinct disorders. Univariate analysis demonstrated significant associations between Cameron lesions and the clinico-endoscopic factors such as nonsteroidal anti-inflammatory drug (NSAID) intake (80% in Cameron lesion cases vs 18% in non-Cameron lesion cases, P=0.015) and intact GEFV (100% in Cameron lesion cases vs 18% in non-Cameron lesion cases, P=0.0007). In contrast, reflux esophagitis was linked with impaired GEFV (44% in reflux esophagitis cases vs 8% in non-reflux esophagitis cases, P = 0.01). Multivariate regression analysis confirmed these significant associations. CONCLUSION: GEFV status and NSAID intake distinguish clinical phenotypes of large hiatal hernias. In this paper,a detection method combining Cameron decomposition based on polarization scattering characteristics in sea clutter background is proposed.Firstly,the Cameron decomposition is exploited to fuse the radar ...In this paper,a detection method combining Cameron decomposition based on polarization scattering characteristics in sea clutter background is proposed.Firstly,the Cameron decomposition is exploited to fuse the radar echoes of full polarization channels at the data level.Due to the artificial material structure on the surface of the target,it can be shown that the non-reciprocity of the target cell is stronger than that of the clutter cell.Then,based on the analysis of the decomposition results,a new feature with scattering geometry characteristics in polarization domain,denoted as Cameron polarization decomposition scattering weight(CPD-SW),is extracted as the test statistic,which can achieve more detailed descriptions of the clutter scattering characteristics utilizing the difference between their scattering types.Finally,the superiority of the proposed CPD-SW detector over traditional detectors in improving detection performance is verified by the IPIX measured dataset,which has strong stability under short-time observation in threshold detection and can also improve the separability of feature space zin anomaly detection.展开更多

The policy of encouraging agriculture and development for mass tourism has led to environmental problems in Cameron Highlands,Malaysia.Rampant development and land clearing have significantly altered the land cover of...The policy of encouraging agriculture and development for mass tourism has led to environmental problems in Cameron Highlands,Malaysia.Rampant development and land clearing have significantly altered the land cover of Cameron Highlands for the past 30 years.Sensitive highlands areas are prone to landslides and soil erosion which then contributed to the main water pollution issues in the network of river system,sedimentation and siltation.The continuous trend of river water quality deterioration in Cameron Highlands has raised the issues for discussion in this review article.The purpose of this review is to briefly summarize the land use change,agriculture practices,agro-tourism,and agriculture policy and management toward water quality of the river system network in Cameron Highlands specifically in the downtown where most of the development and agriculture activities are concentrated.A rigorous review has been done on the existing literature to determine the relationship between land use change and agriculture practices toward river water quality in Cameron Highlands from 2001 to 2017.The total number of reviewed papers was 68.The outcomes established from previous researchers have highlighted factors such as soil erosion,landslides,agriculture activities,urbanization,and unplanned development associated with land use change have significantly influenced the river water quality in the highland areas.Continuous land use changes without proper development plan and law enforcement may critically threaten the sustainability of river network in the highlands area.展开更多

Dealing with water resources issues requires understanding of the community perception. It is important to create a communicative partnership between community and government towards sustainable water resources manage...Dealing with water resources issues requires understanding of the community perception. It is important to create a communicative partnership between community and government towards sustainable water resources management. Opinion survey is an essential step to gather the point of view from local community. However, it always generates a large and complex dataset that are difficult to be interpreted by decision maker. In order to overcome this difficulty, statistical methods are applied to develop an interpretability model for decision maker. This study demonstrated the application of Descriptive Analysis and Principle Factor Analysis (PFA) to reduce the complexity of opinion survey dataset by revealing underlying information. A total of 106 respondents were interviewed; consisting of 68 male and 38 female respondents respectively. This study first applied descriptive analysis to identify the basic score for each variable, and these variables are soil erosion (68.9%), degradation of water quality (65.1%), degradation of freshwater ecosystem (61.0%), water shortage (50%), agricultural solid waste problem (46.2%), water borne diseases (23.6%), illegal land clearing (21.7%), legal land clearing (15.1%), uncontrolled river water abstraction in upstream (54.7%)), poor solid waste management (34.0%), low awareness of local community (61.3%), haphazard planning and development (74.5%) and administration mistake (37.0%). Based on the PFA result, a total of four rotated factors were extracted, representing different aspects of water related issues in Cameron Highlands. Factor 1, 2, 3 and 4 were summarised to four topics namely: (1) water environment degradation caused by illegal solid waste disposal and low awareness of community, (2) agricultural development leading to negative impacts on water resources such as water shortage and ecosystem deterioration, (3) land clearing activity leading to serious land erosion (4) human health problem due to e展开更多
