目的:比较CT、MRI和18F-PET-CT对鼻咽癌局部肿瘤的检测差异和探讨18F-PET-CT检测鼻咽癌局部肿瘤的应用价值。方法:2003年12月至2005年8月,中山大学肿瘤防治中心连续收治的原发鼻咽癌患者53例进入研究。所有患者在治疗前10天内以同一固... 目的:比较CT、MRI和18F-PET-CT对鼻咽癌局部肿瘤的检测差异和探讨18F-PET-CT检测鼻咽癌局部肿瘤的应用价值。方法:2003年12月至2005年8月,中山大学肿瘤防治中心连续收治的原发鼻咽癌患者53例进入研究。所有患者在治疗前10天内以同一固定体位,完成增强CT、MRI和PET-CT扫描检查,在CT、MRI和PET-CT上分别读片判定鼻咽癌局部侵犯范围。结果:CT、MRI与PET-CT分别检出咽后淋巴结转移31例(58.5%)、36例(67.9%)、20例(37.7%),CT、MRI检出率显著高于PET-CT(P=0.02,P=0.001);CT稍低于MRI,但两者间无显著性差异(P=0.314)。CT和MRI检出的最大横径<1cm的咽后淋巴结转移,PET-CT大部分未能显示;最大横径≤0.5cm或并发液化坏死明显的咽后淋巴结转移,PET-CT均未能检出。对鼻腔、口咽、咽旁间隙、颈动脉鞘区、颞下窝、翼腭窝和颅底骨质侵犯的检出,CT、MRI和PET-CT无明显差异;对海绵窦和鼻窦侵犯的检出,MRI可能优于CT和PET-CT。结论:对鼻咽癌局部肿瘤侵犯的检测,PET-CT较CT和MRI无明显优势;对最大横径<1cm的咽后淋巴结转移,PET-CT的检出率明显低于CT或MRI;对最大横径≤0.5cm或并发液化坏死的咽后淋巴结转移的检测,PET-CT可能存在较大局限。 展开更多

The development of ultrasound contrast agents with excellent tolerance and safety profiles has notably improved liver evaluation with ultrasound(US)for several applications,especially for the detection of metastases.In particular,contrast enhanced ultrasonography(CEUS)allows the display of the parenchymal microvasculature,enabling the study and visualization of the enhancement patterns of liver lesions in real time and in a continuous manner in all vascular phases,which is similar to contrast-enhanced computed tomography(CT)and contrast-enhanced magnetic resonance imaging.Clinical studies have reported that the use of a contrast agent enables the visualization of more metastases with significantly improved sensitivity and specificity compared to baseline-US.Furthermore,studies have shown that CEUS yields sensitivities comparable to CT.In this review,we describe the state of the art of CEUS for detecting colorectal liver metastases,the imaging features,the literature reports of metastases in CEUS as well as its technique,its clinical role and its potential applications.Additionally,the updated international consensus panel guidelines are reported in this review with the inherent limitations of this technique and best practice experiences. 展开更多

METHODS: In the peri... AIM: To evaluate the clinical role of 18F-fluorodeo-xyglucose positron emission and computed tomography (18F-FDG PET/CT) in detection of gastric cancer recurrence after initial surgical resection. METHODS: In the period from January 2007 to May 2008, 23 patients who had previous surgical resection of histopathologically diagnosed gastric cancer underwent a total of 25 18F-FDG PET/CT scans as follow-up visits in our center. The standard of reference for tumor recurrence consisted of histopathologic confirmation or clinical follow-up information for at least 5 mo after PET/CT examinations. RESULTS: PET/CT was positive in 14 patients (61%) and negative in 9 (39%). When correlated with final diagnosis, which was confirmed by histopathologic evidence of tumor recurrence in 8 of the 23 patients (35%) and by clinical follow-up in 15 (65%), PET/CT was true positive in 12 patients, false positive in 2, true negative in 8 and false negative in 2. Overall, the accuracy of PET/CT was 82.6%, the negative predictive value (NPV) was 77.7%, and the positive predictive value (PPV) was 85.7%. The 2 false positive PET/CT findings were actually chronic inflammatory tissue lesions. For the two patients with false negativePET/CT, the f inal diagnosis was recurrence of mucinous adenocarcinoma in the anastomosis in one patient and abdominal wall metastasis in the other. Importantly, PET/CT revealed true-positive findings in 11 (47.8%) patients who had negative or no definite findings by CT. PET/CT revealed extra-abdominal metastases in 7 patients and additional esophageal carcinoma in one patient. Clinical treatment decisions were changed in 7 (30.4%) patients after introducing PET/CT into their conventional post-operative follow-up program. CONCLUSION: Whole body 18F-FDG PET/CT was highly effective in discriminating true recurrence in post-operative patients with gastric cancer and had important impacts on clinical decisions in a considerable portion of patients. 展开更多

目的探讨^(18)F-氟代脱氧葡萄糖(^(18)F-fluorodeoxyglucose,^(18)F-FDG)正电子发射计算机断层成像(positron emissiontomography,PET)/CT与MRI在妇科恶性肿瘤诊断中的应用价值。方法 120例妇科肿瘤患者,经手术切除组织病理或穿刺活检... 目的探讨^(18)F-氟代脱氧葡萄糖(^(18)F-fluorodeoxyglucose,^(18)F-FDG)正电子发射计算机断层成像(positron emissiontomography,PET)/CT与MRI在妇科恶性肿瘤诊断中的应用价值。方法 120例妇科肿瘤患者,经手术切除组织病理或穿刺活检组织病理检查确诊恶性肿瘤46例,良性肿瘤74例,术前均接受^(18) F-FDG PET/CT显像检查、MRI检查。比较良、恶性肿瘤病灶标准摄取(standard uptake value,SUV)最大值(SUVmax值)差异,计算^(18) F-FDG PET/CT及MRI检查诊断妇科恶性肿瘤的灵敏度、特异度和准确度,比较^(18) F-FDG PET/CT、MRI对不同类型妇科恶性肿瘤的诊断符合率。结果恶性肿瘤SUVmax值(9.85±3.97)高于良性肿瘤(2.39±1.28)(P<0.05);18 F-FDG PET/CT诊断妇科恶性肿瘤的灵敏度(95.65%)、特异度(95.95%)、准确度(95.83%)均高于MRI(80.43%、86.49%、84.17%)(P<0.05);^(18)F-FDG PET/CT对宫颈癌、子宫内膜癌的诊断符合率(95.24%、94.44%)均高于MRI(71.43%、66.67%)(P<0.05),对卵巢癌的诊断符合率(85.71%)与MRI(71.43%)比较差异无统计学意义(P>0.05)。结论 ^(18) F-FDG PET/CT对妇科恶性肿瘤具有诊断价值,其诊断准确性优于MRI,可作为妇科恶性肿瘤影像学诊断的主要手段。 展开更多

AIM: To evaluate the capacity of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) for detecting multiple primary cancer of upper gastrointestinal (UGI) tract. METHODS: Fifteen patients (12 without cancer histories and 3 with histories of upper GI tract cancer) were investigated due to the suspicion of primary cancer of UGI tract on X-ray barium meal and CT scan. Subsequent whole body 18F-FDG PET/CT scan was carried out for initial staging or restaging. All the patients were fi nally confi rmed by endoscopic biopsy or surgery. The detection rate of multiple primary malignant cancers was calculated based on 18F-FDG PET/CT and endoscopic examinations.RESULTS: 18F-FDG PET/CT scan was positive in 32 suspicious lesions, 30/32 were true positive primary lesions, and 2/32 were false positive. In 15 suspicious lesions with negative 18F-FDG PET/CT scan, 12/15 were true negative and 3/15 were false negative. Among the 15 patients, 12 patients had 29 primary synchronous tumors confirmed by pathology, including 8 cases of esophageal cancers accompanied with gastric cancer and 4 of hypopharynx cancers with esophageal cancer. The other 3 patients had 4 new primary metachronous tumors, which were multiple primary esophageal cancers. PET/CT imaging detected local lymph node metastases in 11 patients. Both local lymph node metastases and distant metastases were detected in 4 patients. On a per-primary lesion basis, the sensitivity, specificity, accuracy, negative predictive value and positive predictive value of 18F-FDG PET/CT for detecting multiple primary cancer of UGI tract were 90.9%, 85.7%, 89.4%, 80% and 93.7%, respectively. CONCLUSION: The whole body 18F-FDG PET/CT may play an important role in evaluating the multiple primary malignant tumors of UGI tract cancer. 展开更多

目的探讨健康体检PET/CT对恶性肺结节的诊断意义,通过^(18)F-脱氧葡萄糖(^(18)F-FDG)高代谢灶摄取度判定肺部恶性肿瘤的价值。方法回顾性分析981例肺结节患者的病例资料,包括良性肺结节966例,恶性肺结节15例。比较良恶性结节间病灶最大... 目的探讨健康体检PET/CT对恶性肺结节的诊断意义,通过^(18)F-脱氧葡萄糖(^(18)F-FDG)高代谢灶摄取度判定肺部恶性肿瘤的价值。方法回顾性分析981例肺结节患者的病例资料,包括良性肺结节966例,恶性肺结节15例。比较良恶性结节间病灶最大标准摄取值(SUVmax)之间的差异。结果 SUVmax≥2.5为PET/CT诊断恶性肿瘤的重要参考指标,PET/CT诊断灵敏度、特异度、准确度、阳性预测值及阴性预测值分别为86.7%(13/15)、99.4%(960/966)、99.2%(973/981)、68.4%(13/19)、99.8%(960/962)。良性肺结节患者检查结果显示SUVmax为0~6.5,中位数为0.7,其中被误诊为恶性肺结节的患者分别是4例良性GGN经病理证实为坏死性肉芽肿(3例)/机化性肺炎(1例)、2例良性结节经病理证实为肺炎(1例)/纤维组织增生(1例);恶性肺结节患者检查结果显示SUVmax为0.8~16.3,中位数为4.7,其中恶性肺结节患者中2例肺泡癌患者漏诊。结论^(18)F-FDG PET/CT在筛查肺部结节良、恶性诊断中有重要意义,SUVmax对良、恶性程度的判断有指导意义。 展开更多

AIM: To monitor the early responses to irradiation in primary and metastatic colorectal cancer (CRC) with 18 Ffluorothymidine ( 18 F-FLT) and 18 F-fluorodeoxyglucose ( 18 F-FDG) small-animal position emission tomography (micro-PET). METHODS: The primary and metastatic CRC cell lines, SW480 and SW620, were irradiated with 5, 10 and 20, l l l , Gy. After 24 h, the cell cycle phases were analyzed. A dual-tumor-bearing mouse model of primary and metastatic cancer was established by injecting SW480 and SW620 cells into mice. micro-PET with 18 F-FLT and 18 F-FDG was performed before and 24 h after irradiation with 5, 10 and 20 Gy. The region of interest (ROI) was drawn over the tumor and background to calculate the ratio of tumor to non-tumor (T/NT) in tissues. Immunohistochemical assay and Western blotting were used to examine the levels of integrin β3, Ki-67, vascular endothelial growth factor receptor 2 (VEGFR2) and heat shock protein 27 (HSP27). RESULTS: The proportion of SW480 and SW620 cells in the G 2 -M phase was decreased with an increasing radiation dose. The proportion of SW480 cells in the G 0 -G 1 phase was increased from 48.33% ± 4.55% to 87.09% ± 7.43% (P < 0.001) and that of SW620 cells in the S-phase was elevated from 43.57% ± 2.65% to 66.59% ± 7.37% (P = 0.021). In micro-PET study, with increasing dose of radiation, 18 F-FLT uptake was significantly reduced from 3.65 ± 0.51 to 2.87 ± 0.47 (P = 0.008) in SW480 tumors and from 2.22 ± 0.42 to 1.76 ± 0.45 (P = 0.026) in SW620 tumors. 18 F-FDG uptake in SW480 and SW620 tumors was reduced but not significantly (F = 0.582, P = 0.633 vs F = 0.273, P = 0.845). Dose of radiation was negatively correlated with 18 F-FLT uptake in both SW480 and SW620 tumors (r = -0.727, P = 0.004; and r = -0.664, P = 0.009). No significant correlation was found between 18 F-FDG uptake and radiation dose in SW480 or SW620 tumors. HSP27 and integrin β3 expression was higher in SW480 than in SW620 tumors. 目的研究高危分化型甲状腺癌复发和转移中18 F-氟代脱氧葡萄糖(18 F-fluorodeoxyglucose,18 FFDG)正电子发射计算机断层显影术(positron emission tomography/computed tomography,PET/CT)的诊断价值。方法采用回顾性分析方法,研究对象... 目的研究高危分化型甲状腺癌复发和转移中18 F-氟代脱氧葡萄糖(18 F-fluorodeoxyglucose,18 FFDG)正电子发射计算机断层显影术(positron emission tomography/computed tomography,PET/CT)的诊断价值。方法采用回顾性分析方法,研究对象为2018年1月—2020年6月该院的37例甲状腺癌患者,所有患者分别选用18 F-FDG PET/CT与131Ⅰ全身显像检查,比较两种检查方式的复发转移病灶类型、复发转移状况与高危分化型甲状腺癌患者的18 F-FDG PET/CT与甲状腺球蛋白水平的相关性。结果18 F-FDG PET/CT与131Ⅰ全身显像在甲状腺癌患者检查诊断中的效果差异无统计学意义(P>0.05),但两种检查方式在不同转移部位具有不同的优势。18F-FDG PET/CT检查显示高危分化型甲状腺癌患者的甲状腺球蛋白平均水平明显高于18F-FDG PET/CT检查显示阴性患者,差异有统计学意义(P<0.05),18 F-FDG PET/CT阳性结果敏感性与甲状腺球蛋白水平呈正相关(P<0.05)。结论高危分化型甲状腺癌复发和转移诊断中18F-FDG PET/CT的应用效果显著,具备131Ⅰ全身显影功能,且诊断敏感性与患者的甲状腺球蛋白水平呈正相关,可为高危分化型甲状腺癌患者的诊断及治疗提供可靠的参考依据。 展开更多

目的探讨18F-FDG PET/CT对多系统结节病与非霍奇金淋巴瘤的鉴别诊断价值。方法结节病组25例与非霍奇金淋巴瘤20例,均经病理检查证实且未行相关治疗。18F-FDG PET/CT检查后通过目视法确定病变区域并分析代谢区形态特点。结果结节病组淋... 目的探讨18F-FDG PET/CT对多系统结节病与非霍奇金淋巴瘤的鉴别诊断价值。方法结节病组25例与非霍奇金淋巴瘤20例,均经病理检查证实且未行相关治疗。18F-FDG PET/CT检查后通过目视法确定病变区域并分析代谢区形态特点。结果结节病组淋巴结肿大25例,短径(2.07±0.86)cm,SUV max 7.88±3.83,结外器官累及22例。淋巴瘤组淋巴结肿大18例,短径(3.05±1.63)cm,SUV max 7.68±4.58,结外器官累及15例。两组淋巴结大小比较有统计学差异(t=3.174,P=0.003),而SUV max无差异(t=0.110,P=0.913)。结节病常累及胸部淋巴结及肺组织(χ2=9.586,P=0.003;χ2=12.640,P=0.001),淋巴瘤则更常侵犯骨骼(χ2=11.520,P=0.001)。肿大淋巴结的对称性分布结节病较明显(χ2=5.993,P=0.014),而淋巴结融合淋巴瘤更多见(χ2=15.901,P=0.000)。结论肿大淋巴结的SU-Vmax值不能用于鉴别结节病与非霍奇金淋巴瘤。淋巴结的对称性分布、大小、融合趋势,病变好发部位及临床与影像学不匹配现象,有助于两者的鉴别。 展开更多

AIM: To evaluated the value of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) scan in diagnosis of hepatocellular carcinoma (HCC) and extrahepatic metastases. METHODS: A total of 138 patients with HCC who had both conventional imaging modalities and 18F-FDG PET/CT scan done between November 2006 and March 2011 were enrolled. Diagnostic value of each imaging modality for detection of extrahepatic metastases was evaluated. Clinical factors and tumor characteristics including PET imaging were analyzed as indicative factors for metastases by univariate and multivariate methods. RESULTS: The accuracy of chest CT was significantly superior compared with the accuracy of PET imaging for detecting lung metastases. The detection rate of metastatic pulmonary nodule ≥ 1 cm was 12/13 (92.3%), when < 1 cm was 2/10 (20%) in PET imaging. The accuracy of PET imaging was significantly superior compared with the accuracy of bone scan for detecting bone metastases. In multivariate analy- sis, increased tumor size (≥ 5 cm) (P = 0.042) and increased average standardized uptake value (SUV) uptake (P = 0.028) were predictive factors for extrahepatic metastases. Isometabolic HCC in PET imaging was inversely correlated in multivariate analysis (P = 0.035). According to the receiver operating characteristic curve, the optimal cutoff of average SUV to predict extrahepatic metastases was 3.4. CONCLUSION: 18F-FDG PET/CT scan is invaluable for detection of lung metastases larger than 1 cm and bone metastases. Primary HCC having larger than 5 cm and increased average SUV uptake more than 3.4 should be considered for extrahepatic metastases. 展开更多

目的 探讨18F-氟脱氧葡萄糖(FDG)PET-CT显像在Ⅲ期非小细胞肺癌(NSCLC)疗效评价及预后判断中的价值.方法 46例NSCLC患者分别在放疗前1周及放疗剂量达到40~50 Gy时进行18F-FDG PET-CT显像检查,以最大标准摄取值(SUVmax)的变化百... 目的 探讨18F-氟脱氧葡萄糖(FDG)PET-CT显像在Ⅲ期非小细胞肺癌(NSCLC)疗效评价及预后判断中的价值.方法 46例NSCLC患者分别在放疗前1周及放疗剂量达到40~50 Gy时进行18F-FDG PET-CT显像检查,以最大标准摄取值(SUVmax)的变化百分率为50%为分割点,将46例患者分为治疗有反应组(A组)和无反应组(B组),对比分析两组治疗前和治疗中的SUVmax值及其变化率与疗效和预后的关系.结果 A组25例,B组21例.A组和B组治疗前的SUVmax值分别为7.59±3.14和14.72±4.67,治疗中的SUVmax值分别为2.89±1.39和9.82±3.31,两组差异均有统计学意义(均P<0.05).A组和B组SUVmax值变化百分率分别为61.9%±8.7%和33.6%±9.0%,差异有统计学意义(P=0.007).A组和B组患者的1年生存率分别为68.0%和38.1%,2年生存率分别为64.0%和33.3%,两组差异均有统计学意义(均P<0.05).Logistic回归分析显示,治疗前和治疗中的SUVmax以及两者的变化百分率与NSCLC患者的预后有关.结论 治疗前和治疗中的SUVmax以及两者的变化百分率可作为Ⅲ期NSCLC患者早期疗效评价和预后判断的重要参考指标. 展开更多

AIM: To analyze changes in myocardial glucose metabolism using fluorodeoxyglucose (FDG)-positron emission tomography (PET) in patients treated with adriamycin and to investigate the clinical significance of these changes.METHODS: Considering that FDG-PET scanning has the ability to show changes in glucose metabolism in the myocardium, we retrospectively analyzed the FDGPET studies of 18 lymphoma patients treated with adriamycin-based chemotherapy in both the preand posttherapy setting. Cardiac contractile parameters such as left ventricular ejection fraction were not available for correlation in all patients due to the short duration and the level of cumulative dose administered in these patients during the time of the follow-up FDG-PET study. The change in myocardial glucose utilization was estimated by change in standard uptake values (SUV) in the myocardium.RESULTS: We observed a significant change in SUVmean values in the myocardium (defined as more than change in cardiac SUVmean between pre-and post-chemotherapy PET) in 1 patients, whereas 6 patients did not show any significant cardiac FDG uptake in both preand post-therapy PET scans. Patients were divided into three groups based on the changes observed in myocardial tracer uptake on the followup 18 F-FDG-PET study. Group A (n = 8): showed an increase in cardiac 18 F-FDG uptake in the post-therapy scan compared to the baseline scan carried out prior to starting adriamycin-based chemotherapy. Group B (n = 6): showed no significant cardiac 18 F-FDG uptake in post-therapy and baseline PET scans, and group C (n = 4): showed a fall in cardiac 18 F-FDG uptake in the posttherapy scan compared to the baseline scan. Mean cumulative adriamycin dose (in mg/m 2 ) received during the time of the follow-up FDG-PET study was 256. 25, 250 and 137.5, respectively.CONCLUSION: Our study shows three different trends in the change in myocardial glucose metabolism in patients undergoing adriamycin-based chemotherapy. A 41-year old female with metastatic gastrointestinal stromal tumor was referred to 18F-FDG-positron emission tomography and computed tomography (PET/CT) scan before and after one-month treatment with imatinib (Glivec, Gleevec, Novartis, Basel, Switzerland), a tyrosine kinase inhibitor (400 mg/d). Metabolic response was evaluated before and after one month of therapy. The decrease of the maximum standardised uptake value (SUV) was 79% (from 9.8 to 2.1). Positron emission tomography demonstrated complete metabolic response after one-month of imatinib treatment. Additionally, the previous lesion was compared with the coronal computerized tomographic image. There was no difference in the size of the tumor before and after therapy according to CT images. However, metabolic activity was inhibited. 18F-FDG-PEr is a valuable method for the detection of response to one-month imatinib treatment in patients with gastrointestinal stromal tumors. 展开更多

目的探讨原发乳腺淋巴瘤(PBL)的18F-FDG PET/CT显像特征,提高对该病的认识。方法回顾性分析经病理证实的15例PBL的18F-FDG PET/CT显像资料。结果 15例PBL,4例双侧乳腺受侵,11例单侧发病。PET/CT表现主要有三种类型:(1)单发结节或肿块型... 目的探讨原发乳腺淋巴瘤(PBL)的18F-FDG PET/CT显像特征,提高对该病的认识。方法回顾性分析经病理证实的15例PBL的18F-FDG PET/CT显像资料。结果 15例PBL,4例双侧乳腺受侵,11例单侧发病。PET/CT表现主要有三种类型:(1)单发结节或肿块型最常见,占66.7%(10/15),病灶境界清晰,边缘光滑,均为软组织密度;乳头均未见凹陷,邻近皮肤增厚,乳后间隙未见侵犯,乳腺病灶SUV_(max)为16.39(5.92,31.60)。(2)多发结节、肿块型3例,多为数个病灶融合所致;病灶形态、轮廓及密度与单发结节、肿块型相似,而占位效应较后者明显。病灶SUVmax分别为6.95、7.01、33.29。(3)弥漫型2例,范围广,占位效应最明显,乳腺病灶SUVmax分别为8.18、11.52。三种类型间乳腺病灶的SUVmax差异无统计学意义(χ~2=0.737,P=0.692)。临床分期ⅠE期4例,ⅡE期11例;ⅠE期及ⅡE期乳腺病灶的SUV_(max)分别为21.27(7.01,31.60)、11.52(5.92,33.29),差异无统计学意义(Z=-1.044,P=0.296)。结论 PBL的18F-FDG PET/CT表现具有一定特征性:境界清晰、边缘光滑的软组织结节或肿块,18F-FDG摄取较高,有助于PBL的诊断。 展开更多

目的探讨胰腺癌患者术前18 F-脱氧葡萄糖(18-fluorodeoxyglucose,18F-FDG)PET/CT检查与术后复发和生存期的关系。方法选取内蒙古自治区人民医院2010-01-01-2013-12-02行手术治疗的胰腺导管腺癌患者48例,均经病理学确诊。所有患者术前半... 目的探讨胰腺癌患者术前18 F-脱氧葡萄糖(18-fluorodeoxyglucose,18F-FDG)PET/CT检查与术后复发和生存期的关系。方法选取内蒙古自治区人民医院2010-01-01-2013-12-02行手术治疗的胰腺导管腺癌患者48例,均经病理学确诊。所有患者术前半个月内接受过≥1次PET/CT扫描,记录最大标准化摄取值(maximum standardized uptake value,SUVmax)。以无复发生存期≥12个月为结果状态变量,应用受试者工作特征曲线(receiver operating characteristic curve,ROC)评估SUVmax预测术后复发的情况,并分析SUVmax与术后复发及生存期的关系。结果 48例患者随访2~49个月,中位随访时间11个月。48例患者无复发中位生存期为13个月,总生存期中位时间为26个月。SUVmax预测术后复发的ROC曲线下面积为0.809,最佳临界值为5.9,灵敏度和特异度分别为88.0%和73.9%。血清CA19-9水平、淋巴结浸润和SUVmax是胰腺癌患者无复发生存期的独立影响因素,HR(95%CI)分别为2.37(1.23~4.51)、1.98(1.17~3.82)和2.73(2.02~4.24)。血清CA19-9水平、SUVmax、组织分化程度和淋巴结浸润是胰腺癌患者总生存期的独立影响因素,HR(95%CI)分别为2.28(1.55~3.52)、2.25(1.33~3.91)、1.49(1.07~2.15)和1.68(1.14~2.41)。结论 SUVmax可在一定程度上预测胰腺癌患者术后复发和生存情况,术前PET/CT检查对治疗方案的制定具有临床参考价值。 展开更多

应用自行设计的脑解剖功能区自动提取软件研究了正常人脑18F-FDG代谢显像的灰度分布及其与剂量、血糖水平以及年龄等因素的关系。按照标准的Talairach脑坐标图谱,将脑18F-FDG代谢显像经过位移和形变配准到标准空间中去,提取出各解剖功... 应用自行设计的脑解剖功能区自动提取软件研究了正常人脑18F-FDG代谢显像的灰度分布及其与剂量、血糖水平以及年龄等因素的关系。按照标准的Talairach脑坐标图谱,将脑18F-FDG代谢显像经过位移和形变配准到标准空间中去,提取出各解剖功能区象素,计算它们的平均灰度值,同时以灰度值为应变量,年龄、血糖以及剂量为自变量,按多元逐步回归统计方法,找出与灰度值相关的脑解剖区。结果显示:经过图像配准,额叶、顶叶、枕叶、颞叶和小脑以及丘脑、海马、第三脑室、第四脑室等区域可以进行成功提取,各功能区灰度值与小脑灰度值之比分布在1.01±0.15之间,多元逐步回归分析可见除尾状核和海马区域,另外提取的脑功能区的灰度值高低均与年龄大小成负相关,而所有区域的灰度与血糖和剂量均无相关性。表明自行设计的脑解剖功能区自动提取方法能方便地提取脑解剖功能区,并计算其各项参数作进一步的统计分析,但是解剖功能区的提取区域范围尚需更加细致化,才能准确研究认知和精神疾病领域的变化。 展开更多
