The pharmacologic profile of desloratidine offers particular benefits, in terms of histamine H1-receptor binding potency and H1 selectivity. It has a half-life of 21-27 h, permitting a once-daily dose. No specific precautions are required with respect to its administration in renal or hepatic failure. No clinically relevant racial or gender variations in the disposition of desloratidine have been noted.We present here a clinical case of acute reversible idiosyncratic liver toxicity, related to its administration.展开更多

Methods Nineteen guinea pig...Objective To investigate the effects of antihistamine agents, loratadine and ketotifen, on the model of exercise-induced asthma in guinea pigs with lipopolysaccharide (LPS) and metyapone. Methods Nineteen guinea pigs were injected peritoneolly with LPS ( 1mg/kg, i. p. ) and metyapone ( 50mg/kg, i. p. ). Then they were randomized into 3 groups. The control group ( n = 7) did not take any drug, ketotifen group ( n = 6) administered orally ketotifen ( Img·kg^-1· d^-1 ) and loratadine group ( n = 6) administered orally loratadine ( 2mg·kg^-1·d^-1 ) for 4 days. On thefifth day, lung resistance (RL) and dynamic compliance of respiratory system (Cdyn) of 3 groups were measured before and after exercise challenge. The total number of leukocytes in bronchoalveolar lavage fluid (BALF) from 3 groups was counted and differentiated cell type count was measured. Morphometric examination of the animal lungs was performed. Results In control group, RLincreased and Cdyn decreased significantly after exercise challenge. In ketotifen group and loratadine group with no change. There was a significant increase in the number of neutrophils, macrophages and eosinophils in BALF from control group. The infiltration of neutrophils in the bronchial mucosa was shown in control group in the morphometric study. Conclusion Loratadine and ketotifen can inhibit the exercise-induced bronchoconstriction in guinea pigs pretreated with LPS and metyrapone. Inhibiting the formation of inflammation in airway may be the therapeutic mechanism of these H-I receptor antagonists.展开更多

Materials and met hods: Postal questionnaire administered to members of the ...Purpose: To examine the site, severity, and duration, of symptoms in patients with interstitial cystitis. To examine the treatments offered. Materials and met hods: Postal questionnaire administered to members of the Interstitial Cystitis Support Group, based in the United Kingdom. Results: 736 of 1148 (64%) question naires were returned. 696 (94.6%)-patients were women. 471 patients (64%) des cribed daily pain when symptoms were at their worst, and 273 (37%)-described d aily pain at the time of survey. Most patients reported frequency, urgency and n octuria. A wide range of treatments had been offered, but drugs of proven effica cy were offered to less than a third of patients: cimetidine (36%)-; antihista mines (5%); pentosan polysulphate (12%); DMSO (33%); and anticonvulsants (2% ). 495 patients (67%) reported "considerable impact" or more on their lifestyle and 346 (46%)-reported moderate depression or worse. 364 (49%) reported at l east considerable difficulties with sexual intercourse. Conclusions: This survey supports the work of others about the nature of symptoms and their impact upon quality of life. The data highligh t the continuing difficulty in making a prompt diagnosis. We recommend that a Eu ropean survey or registry of interstitial cystitis patients would help to establ ish a minimum standard of care. A suggested treatment algorithm is provided.展开更多
