五阶梯治疗在老年营养不良患者中的应用分析 被引量:5

目的分析营养不良老年患者中五阶梯治疗的具体应用。方法方便选取2017年8月-2018年9月该院所收治需住院治疗65岁以上的105例老年患者作为该次研究对象,将存在着营养风险患者46例随机分成两个研究小组,每个小组为23例。研究组采取五阶梯... 目的分析营养不良老年患者中五阶梯治疗的具体应用。方法方便选取2017年8月-2018年9月该院所收治需住院治疗65岁以上的105例老年患者作为该次研究对象,将存在着营养风险患者46例随机分成两个研究小组,每个小组为23例。研究组采取五阶梯式治疗,基础组则为基础治疗。分析两组效果。结果通过NRS2002筛查可了解到,该106例住院的老年患者当中,≥4分的患者为46例,存在着营养不良风险,男女患者比例为26∶20例。通过MNA的营养评估可了解到:有37例患者可确定为营养不良,男女患者比例为20∶17例。经过膳食营养调查后可了解到:摄入营养素不足的患者为39例,男女患者比例为21、18例;摄入能量不足的患者为30例,男女患者比例为19、11例。研究组患者在入院后第1天生化检查指标评分结果即为:HGB血红蛋白(101.4±2.2)g/L、TRF转铁蛋白(2.3±1.8)g/L、PA前白蛋白(175.4±0.9)mg/L、ALB血清白蛋白(31.3±1.3)g/L、TP总蛋白(54±11)g/L,基础组患者存在着营养不良症状风险,但并非采取五阶梯式治疗,仅给予患者基础的营养宣教指导及家庭饮食调理等治疗方案。患者在入院后第1天生化检查各项临床指标评分结果为:HGB血红蛋白(113±1.5)g/L、TRF转铁蛋白(2.3±1.4)g/L、PA前白蛋白(174.9±1.8)mg/L、ALB血清白蛋白(32.2±1.2)g/L、TP总蛋白(52.4±1.7)g/L。从以上数据对比分析结果中可表明,组间比较数据分析差异无统计学意义(P>0.05);研究组患者在采取五阶梯式营养干预治疗10 d后,生化各项检查指标评分结果即为:HGB血红蛋白(119±1.7)g/L、TRF转铁蛋白(2.3±1.8)g/L、PA前白蛋白(212.5±1.6)mg/L、ALB血清白蛋白(35.2±1.3)g/L、TP总蛋白(58.3±1.7)g/L。基础组患者在住院10 d后,生化各项检查指标评分结果即为:HGB血红蛋白(118±1.7)g/L、TRF转铁蛋白(2.3±1.9)g/L、PA前白蛋白(176.5±1.2)mg/L、ALB血清白蛋白(31.3±2.6)g/L、TP总蛋白(52.8±1.4)g/L。从 展开更多

BACKGROUND Although en bloc dissection of hepatic hilum lymph nodes has many advantages in radical tumor treatment,the feasibility and safety of this approach for laparo-scopic pancreaticoduodenectomy(LPD)require further clinical evaluation and investigation.AIM To explore the application value of the"five steps four quadrants"modularized en bloc dissection technique for accessing hepatic hilum lymph nodes in LPD patients.METHODS A total of 52 patients who underwent LPD via the"five steps four quadrants"modularized en bloc dissection technique for hepatic hilum lymph nodes from April 2021 to July 2023 in our department were analyzed retrospectively.The patients'body mass index(BMI),preoperative laboratory indices,intraoperative variables and postoperative complications were recorded.The relationships between preoperative data and intraoperative lymph node dissection time and blood loss were also analyzed.RESULTS Among the 52 patients,36 were males and 16 were females,and the average age was 62.2±11.0 years.There were 26 patients with pancreatic head cancer,16 patients with periampullary cancer,and 10 patients with distal bile duct cancer.The BMI was 22.3±3.3 kg/m²,and the median total bilirubin(TBIL)concentration was 57.7(16.0-155.7)µmol/L.All patients successfully underwent the"five steps four quadrants"modularized en bloc dissection technique without lymph node clearance-related complications such as postoperative bleeding or lymphatic leakage.Correlation analysis revealed significant associations between preoperative BMI(r=0.3581,P=0.0091),TBIL level(r=0.2988,P=0.0341),prothrombin time(r=0.3018,P=0.0297)and lymph node dissection time.Moreover,dissection time was significantly correlated with intraoperative blood loss(r=0.7744,P<0.0001).Further stratified analysis demonstrated that patients with a preoperative BMI≥21.9 kg/m²and a TIBL concentration≥57.7μmol/L had significantly longer lymph node dissection times(both P<0.05).CONCLUSION The"five steps four quadrants"modularized en bloc dissection techni 展开更多

In order to really understand the biological process within a cell level and provide useful clues to develop antiviral drugs, information of virus protein subcellular localization is vitally important. In view of this, a CNN based virus protein subcellular localization predictor called “pLoc_Deep-mVirus” was developed. The predictor is particularly useful in dealing with the multi-sites systems in which some proteins may simultaneously occur in two or more different organelles that are the current focus of pharmaceutical industry. The global absolute true rate achieved by the new predictor is over 97% and its local accuracy is over 98%. Both are transcending other existing state-of-the-art predictors significantly. It has not escaped our notice that the deep-learning treatment can be used to deal with many other biological systems as well. Recently, the life of human beings around the entire world has been endangering by the spreading of pneumonia-causing virus, such as Coronavirus, COVID-19, and H1N1. To develop effective drugs against Coronavirus, knowledge of protein subcellular localization is indispensable. In 2019, a predictor called “pLoc_bal-mHum” was developed for identifying the subcellular localization of human proteins. Its predicted results are significantly better than its counterparts, particularly for those proteins that may simultaneously occur or move between two or more subcellular location sites. However, more efforts are definitely needed to further improve its power since pLoc_bal-mHum was still not trained by a “deep learning”, a very powerful technique developed recently. The present study was devoted to incorporate the “deep-learning” technique and develop a new predictor called “pLoc_Deep-mHum”. The global absolute true rate achieved by the new predictor is over 81% and its local accuracy is over 90%. Both are overwhelmingly superior to its counterparts.
