Stem cell transplantation for spinal cord injury: a meta-analysis of treatment effectiveness and safety 被引量:6

OBJECTIVE: The aim of this study was to evaluate the effectiveness and safety of stem cell transplantation for spinal cord injury(SCI).DATA SOURCES: PubM ed, EMBASE, Cochrane, China National Knowledge Infrastructure, China Science and Technology Journal, Wanfang, and Sino Med databases were systematically searched by computer to select clinical randomized controlled trials using stem cell transplantation to treat SCI, published between each database initiation and July 2016. DATA SELECTION: Randomized controlled trials comparing stem cell transplantation with rehabilitation treatment for patients with SCI. Inclusion criteria:(1) Patients with SCI diagnosed according to the American Spinal Injury Association(ASIA) International standards for neurological classification of SCI;(2) patients with SCI who received only stem cell transplantation therapy or stem cell transplantation combined with rehabilitation therapy;(3) one or more of the following outcomes reported: outcomes concerning neurological function including sensory function and locomotor function, activities of daily living, urination functions, and severity of SCI or adverse effects. Studies comprising patients with complications, without full-text, and preclinical animal models were excluded. Quality of the included studies was evaluated using the Cochrane risk of bias assessment tool and Rev Man V5.3 software, provided by the Cochrane Collaboration, was used to perform statistical analysis. OUTCOME MEASURES: ASIA motor score, ASIA light touch score, ASIA pinprick score, ASIA impairment scale grading improvement rate, activities of daily living score, residual urine volume, and adverse events.RESULTS: Ten studies comprising 377 patients were included in the analysis and the overall risk of bias was relatively low level. 目的研究白血病抑制因子(leukemia inhibitory factor,LIF)联合bFGF对人BMSCs(human BMSCs,hBMSCs)增殖及分化的影响。方法取第4代hBMSCs根据培养条件不同分为4组:A组为对照组,加入完全培养基(含10%FBS的α-MEM)常规培养;B组加入含10 ng... 目的研究白血病抑制因子(leukemia inhibitory factor,LIF)联合bFGF对人BMSCs(human BMSCs,hBMSCs)增殖及分化的影响。方法取第4代hBMSCs根据培养条件不同分为4组:A组为对照组,加入完全培养基(含10%FBS的α-MEM)常规培养;B组加入含10 ng/mL LIF的完全培养基;C组加入含10 ng/mL bFGF的完全培养基;D组加入含10 ng/mL LIF和10 ng/mL bFGF的完全培养基。采用细胞计数试剂盒8(cell counting kit 8,CCK-8)测定各组第4代hBMSCs生长曲线;倒置相差显微镜观察各组hBMSCs形态变化;流式细胞术检测各组第8代hBMSCs表面标志CD44、CD90、CD19、CD34表达情况。结果 4组细胞生长曲线均近似S形,且细胞增殖速率表现为D组>C组>B组>A组。各组细胞形态观察发现A组细胞较早出现衰老和分化现象;B组细胞早期形态维持较好,但增殖相对缓慢,后期出现部分衰老细胞;C组少量细胞呈神经样细胞分化,但增殖较快;D组细胞增殖迅速,且能长期维持hBMSCs形态特征。流式细胞术检测示,A、C组骨髓基质标志CD44、CD90表达较B、D组低;各组造血细胞标志CD19、CD34均呈阴性,组间差异不明显。结论 LIF联合bFGF不仅可以维持hBMSCs的多向分化潜能,而且能够促进其快速增殖。 展开更多

目的探讨血清外泌体源性circ_0009362对人骨髓间充干细胞(human bone marrow mesenchymalstem cells,hBMSCs)成骨分化的影响及潜在作用机制。方法收集骨质疏松症(osteoporosis,OP)患者的血清并分离外泌体,使用qRT-PCR检测外泌体中circ_0... 目的探讨血清外泌体源性circ_0009362对人骨髓间充干细胞(human bone marrow mesenchymalstem cells,hBMSCs)成骨分化的影响及潜在作用机制。方法收集骨质疏松症(osteoporosis,OP)患者的血清并分离外泌体,使用qRT-PCR检测外泌体中circ_0009362的表达水平。诱导hBMSCs成骨并检测细胞中circ_0009362、miR-29b-3p的表达。双荧光素酶报告实验检测circ_0009362、miR-29b-3p的相互调控关系。碱性磷酸酶(ALP)试剂盒检测ALP活性,茜素红(ARS)染色法检测钙沉积。结果与健康对照组相比,OP患者的血清外泌体中circ_0009362表达升高,诱导hBMSCs成骨后circ_0009362的表达降低(均P<0.05)。外泌体能降低hBMSCs中ALP活性与钙沉积百分比,该作用通过分泌circ_0009362实现。敲减外泌体中circ_0009362表达能部分抵消外泌体的作用(均P<0.05)。诱导hBMSCs成骨后miR-29b-3p的表达提高(P<0.05)。circ_0009362与miR-29b-3p存在靶向关系,外泌体通过分泌circ_0009362抑制miR-29b-3p的表达。过表达miR-29b-3p促进hBMSCs中ALP活性与钙沉积百分比,该作用被外泌体部分抵消(均P<0.05)。结论OP患者的血清外泌体通过分泌circ_0009362下调miR-29b-3p的表达来抑制hBMSCs成骨分化。 展开更多
