VF assessment(VFA) is an efficient, low radiation method for identifying VFs at the time of bone mineral density(BMD) measurement. Prediction models used to indicate the need for VFA may have little predictive power in subspecialty referral populations such as rheumatologic patients or patients who underwent kyphoplasty. Rheumatologic patients are frequently at increased risk for VFs, and VFA should be performed on an individual basis, also taking in account the guidelines for the general population. Kyphoplasty is a new minimal invasive procedure for the treatment of VFs and is being performed with increasing frequency. Following kyphoplasty, there may be a risk of new VFs in adjacent vertebrae. The assessment and follow-up of patients who underwent kyphoplasty requires repetitive X-ray imaging with the known limitations of SR. Thus, VFA may facilitate the evaluation of VFs in these patients because most of the kyphoplasty patients would fulfill the criteria. In a pilot study, we measured the BMD and performed VFA in 28 patients treated with kyphoplasty. Ratios of anterior to posterior(A/P) and middle to posterior(M/P) height were measured, and Genant's method was used to classify vertebrae accordingly. Intraobserver and interobserver reliability for A/P, M/P and the Genant's method were determined. Only 1 patient did not meet the criteria for VFA. Of the 364 available vertebrae, 295 could be analyzed. Most missing data(concerning 69 vertebrae) occurred in the upper thoracic region. Three of the 69 non-eligible vertebrae were lumbar vertebrae with cement leakage from the kyphoplasty procedure. During the epidemic period, if not treated in time, the complications are high and the mortality is high. If we do not pay attention to infection prevention and control in pre-hospital emergency care, it will lead to the first time infection of medical staff and in-hospital cross infection in emergency outpatient receiving area. The correct consideration of both and the establishment of perfect pre-hospital emergency treatment and infection prevention and control synchronous strategy is an important premise to ensure the stable, orderly and safe medical treatment. Objective: To explore the effect of synchronous implementation of pre-hospital emergency care, nursing and infection pre-vention and control for senile OVCF during the epidemic. In order to improve the efficiency of pre-hospital emergency care and prevent the spread of infection. Method: A total of 92 elderly patients with OVCF who received pre-hospital treatment in 18 hospitals in Zhangjiakou City during the epidemic prevention and control period from January 2020 to November 2022 and met the inclusion criteria were selected as research objects, including 24 males and 68 females, aged 65 - 82 (74.2 ± 2.2) years. All patients were associated with concomitant injuries and underlying diseases. All patients in this group underwent predictive pre-hospital rescue and infection prevention and control procedures. Results: All the 92 elderly patients with OVCF received timely pre-hospital treatment during the epidemic period, and no aggravation occurred of the 92 patients, 35 were in the high risk area, 10 were in the medium risk area, and 47 were in the low risk area. Exclude OVCF for NCP Patients were treated according to the conventional diagnosis and treatment principles. Suspected and confirmed cases are transferred to designated surgical hospitals for treatment. All patients were followed up 3 months, 6展开更多

文摘目的研究利用双能X线骨密度仪扫描的脊柱影像进行椎体骨折评估在绝经后女性骨质疏松诊断中的应用价值。方法回顾499名年龄≥50岁的绝经后女性行骨密度检测同时行椎体侧位扫描,分析骨密度及椎体骨折评估(vertebral fracture assessment,VFA)的检测结果,收集研究对象的一般资料。结果对499名绝经后女性进行VFA检测结果分析:(1)椎体骨折诊断率提高了41.1%,骨质疏松诊断率提高了6.3%,严重骨质疏松诊断率提高了29.1%(P<0.05);(2)当骨密度达到骨质疏松阈值时,椎体骨折率明显升高,较骨密度未达到骨质疏松阈值的椎体骨折发生率提高了29.2%(P<0.05);(3)年龄增加、绝经年限延长的女性椎体骨折、骨质疏松、严重骨质疏松的患病率显著增加,骨量显著减少(P<0.05);此外,体质量指数(body mass index,BMI)降低的绝经后女性骨量显著减少,骨质疏松的患病率显著增加(P<0.05)。结论骨密度检测联合椎体骨折评估可以显著提高椎体骨折、骨质疏松、严重骨质疏松的诊断率;骨密度越低,年龄越大,绝经年限越长,椎体骨折的风险就越高;随年龄增长、绝经年限增加、BMI降低,骨密度减少,骨质疏松患病率增加。
