Patients with medium- or largesized varices can be treated for primary prophylaxis of va...Variceal bleeding is a life-threatening complication of portal hypertension with a six-week mortality rate of approximately 20%. Patients with medium- or largesized varices can be treated for primary prophylaxis of variceal bleeding using two strategies: non-selective beta-blockers(NSBBs) or endoscopic variceal ligation(EVL). Both treatments are equally effective. Patients with acute variceal bleeding are critically ill patients. The available data suggest that vasoactive drugs, combined with endoscopic therapy and antibiotics, are the best treatment strategy with EVL being the endoscopic procedure of choice. In cases of uncontrolled bleeding, transjugular intrahepatic portosystemic shunt(TIPS) with polytetrafluoroethylene(PTFE)-covered stents are recommended. Approximately 60% of the patients experience rebleeding, with a mortality rate of 30%. Secondary prophylaxis should start on day six following the initial bleeding episode. The combination of NSBBs and EVL is the recommended management, whereas TIPS with PTFE-covered stents are the preferred option in patients who fail endoscopic and pharmacologic treatment. Apart from injection sclerotherapy and EVL, other endoscopic procedures, including tissue adhesives, endoloops, endoscopic clipping and argon plasma coagulation, have been used in the management of esophageal varices. However, their efficacy and safety, compared to standard endoscopic treatment, remain to be further elucidated. There are safety issues accompanying endoscopic techniques with aspiration pneumonia occurring at a rate of approximately 2.5%. In conclusion, future research is needed to improve treatment strategies, including novel endoscopic techniques with better efficacy, lower cost, and fewer adverse events.展开更多
We collect 119 γ-ray-loud blazars (97 flat spectrum radio quasars (FSRQs) and 22 BL Lacertae objects (BL Lacs)), and investigate possible correlations between their γ-ray emission (maximum, minimum and average value...We collect 119 γ-ray-loud blazars (97 flat spectrum radio quasars (FSRQs) and 22 BL Lacertae objects (BL Lacs)), and investigate possible correlations between their γ-ray emission (maximum, minimum and average values) at 1 GeV and the radio emission at 8.4 GHz. Our main results are as follows. For the lower state γ-ray data, there is no correlation between the γ-ray and radio flux density; For the high state γ-ray data, there are good correlations for the whole 119 blazars and 97 FSRQs, and a weak correlation for the 22 BL Lac objects; For the average γ-ray data, there are good correlations. According to our analysis, we propose that the γ-rays are associated with the radio emission from the jet, and that the γ-ray emission is likely from the SSC process in this case.展开更多

Methods: One hundred and twenty patients were randomly divided into two groups,...Objective: To observe the clinical effects in the treatment of post-stroke dysfunction of lower limb by puncturing Weizhong (BL 40). Methods: One hundred and twenty patients were randomly divided into two groups, 60 cases in each group. Both groups were treated by puncturing Weizhong (BL 40) plus functional rehabilitation training, once every day. Those in the observation group were treated by Shi Xue-min's needling technique, while others in the control group were treated with routine needling technique. The therapeutic effects were assessed by the scores of lower limb functions of Lovett scale of grading muscle strength (LSGMS) and short-form FugI-Meyer assessment (SFFMA). Results: The instant and post-treatment effective rates in the observation group were 83.3% and 93.3%, versus 6.7% and 53.3% in the control group, indicating that the therapeutic effect was better in the observation group (P〈0.01). The instant and post-treatment scores of Lovett muscle strength grading scale were better in the observation group than that in the control group (P〈0.05). SFFMA score of lower limbs function was better in the observation group than that in the control group (P〈0.0s). Conclusion: Different needling methods at Weizhong (BL 40) can produce different therapeutic effects in treating post-stroke dysfunction of lower limb, and Shi's needling technique has obvious instant effect and therapeutic function.展开更多
Making use of the 2MASS Data Release, we have searched for nearinfrared (JHK) counterparts to 268 blazars from Donato et al. and obtained 238 counterparts within 5'' in the area covered by 2MASS. It provides us a ...Making use of the 2MASS Data Release, we have searched for nearinfrared (JHK) counterparts to 268 blazars from Donato et al. and obtained 238 counterparts within 5'' in the area covered by 2MASS. It provides us a sample with infrared data several times larger than the previous one of the same kind. Based on our sample and the sample by Donato et al., we have compared in detail the properties of HBLs, LBLs and FSRQs from five aspects and found that HBLs are significantly different from LBLs and FSRQs while LBLs are not obviously different from FSRQs. Our results strongly support the division of BL Lac objects into the high-frequency peaked (HBL) and low-frequency peaked (LBL) objects introduced by Padovani & Giommi and show that HBLs and LBLs are two kinds of blazar having different physical properties.展开更多

Methods: Clinical literatures related to PMS treated with acupuncture-moxibustion published from 2007 to 2016 were collected from Chinese Biomedical Literature Database(CBM), Chongqing VIP Database(CQVIP), China National Knowledge Infrastructure(CNKI), and Wanfang Academic Journal Full-text Database(Wanfang). The retrieved data underwent descriptive analysis, cluster analysis and association pattern analysis to determine the acupoints selection principle in acupuncture-moxibustion treatment of PMS. Results: The top five acupoints used in acupuncture-moxibustion treatment of PMS were Sanyinjiao(SP 6), Shenshu(BL 23), Guanyuan(CV 4), Baihui(GV 20), and Shenmen(HT 7). The leading 4 meridians were Bladder Meridian, Conception Vessel, Spleen Meridian, and Governor Vessel. Aerothermal co...The engineering computation of turbulent flows is mainly based on turbulence modeling,however,accurate aerothermal computation of hypersonic turbulent boundary layers is still a not well-solved problem. Aerothermal computation for turbulent boundary layers on a supersonic or hypersonic blunt cone with small bluntness is done firstly by using both direct numerical simulation and BL model,and seven different cases are investigated. Then the results obtained by the two methods are compared,and the reason causing the differences is found to be the incorrect assumption in the turbulence modeling that the ratio between eddy heat conductivity and eddy viscosity is constant throughout the whole boundary layer. Based on certain theoretical arguments,a method of modifying the expression of eddy heat conductivity in the region surrounding the peak location of the turbulent kinetic energy is proposed,which is verified to be effective,at least for the seven cases investigated.展开更多
Objective: To observe the effect of electroacupuncture (EA) on the expression of erythropoie-tin-producing hepatocyte receptor B2 (EphB2) in the cortex around the infracted area of middle cerebral artery occlusi...Objective: To observe the effect of electroacupuncture (EA) on the expression of erythropoie-tin-producing hepatocyte receptor B2 (EphB2) in the cortex around the infracted area of middle cerebral artery occlusion (MCAO) rats at different timing, and to reveal the possible mechanism of acupuncture in the treatment of cerebral ischemia. Methods: A total of 180 male Sprague-Dawley (SD) rats were randomly divided into a sham operation group, a model group, an acupoint group and a non-acupoint group, with 45 rats in each group. Rats in each group were further divided into three subgroups: postoperative 3 d, postoperative 14 d and postoperative 2l d groups, with 15 rats in each subgroup. The MCAO model was made by the modified occlusion method. The neurological function score, 2,3,5-triphenyl tetrazolium chloride (TFC) staining, immunohistochemistry assay, immunofluorescence double labeling method and Western blot were used to detect the corresponding indicators. Results: The neurological impairment of rats was most obvious at postoperative 3 d, and then gradually improved with time, which was more significant in the acupoint group (P〈0.05). The change of infarcted volume was consistent with the neurological function impairment. The number of EphB2 positive cells (EphB2+) around the infarcted area was decreased significantly at postoperative 3 d, and then gradually improved with time, which returned to the same level as that in the sham operation group at postoperative 21 d. The increase was most significant in the acupoint group (P〈0.05), and the positive cell number was higher than that in the sham operation group (P〈0.01). Western blot and immunohistochemistry results were basically consistent. Immunofluorescence displayed that EphB2+ and postsynaptic density-95 positive (PSD-95+) were co-expressed, after the MCAO operation, in the cortical neuron around the infracted area, and the number of co-expressing cells was increased gradually with time, which was 展开更多
