RESULTS A total of 266 patients underwent curative surgery during the study period. The median age of the study cohort was 68 year(range 26 to 91) with a followup of 7.9 years(range 4.6 to 12.6). Resection was undertaken electively in 225(84.6%) patients and emergency resection in 35(13.2%). Data on timing of surgery was missing in 6 patients. Recurrence was noted in 67(25.2%) during the study period and was predominantly early within 3 years(82.1%) and involved hepatic metastasis in 73.1%. Emergency resection(OR = 3.60, P = 0.001), T4 stage(OR = 4.33, P < 0.001) and lymphovascular invasion(LVI) (OR = 2.37, P = 0.032) were associated with higher risk of recurrent disease. Emergency resection, T4 disease and a high lymph node ratio(LNR) were strong independent predictors of adverse long-term survival. CONCLUSION Emergency surgery is associated with adverse disease free and long-term survival. T4 disease, LVI and LNR provide strong independent predictive value of longterm outcome and can inform surveillance strategies to improve outcomes.展开更多

Accurate inferences of sister group relationships provide the researcher with an historical framework within which the attributes or geographic origin of species (or supraspecific groups) evolved. Taken out of this phylogenetic context, interpretations of evolutionary processes or origins, geographic distributions, or speciation rates and mechanisms, are subject to nothing less than a biological experiment without controls. Cypriniformes is the most diverse clade of freshwater fishes with estimates of diversity of nearly 3,500 species. These fishes display an amazing array of morphological, ecological, behavioral, and geographic diversity and offer a tremendous opportunity to enhance our understanding of the biotic and abiotic factors associated with diversification and adaptation to environments. Given the nearly global distribution of these fishes, they serve as an important model group for a plethora of biological investigations, including indicator species for future cli-matic changes. The occurrence of the zebrafish, Danio rerio, in this order makes this clade a critical component in understanding and predicting the relationship between mutagenesis and phenotypic expressions in vertebrates, including humans. With the tremendous diversity in Cypriniformes, our understanding of their phylogenetic relationships has not proceeded at an acceptable rate, despite a plethora of morphological and more recent mo-lecular studies. Most studies are pre-Hennigian in origin or include relatively small numbers of taxa. Given that analyses of small numbers of taxa for molecular characters can be compromised by peculiarities of long-branch attraction and nodal-density effect, it is critical that significant progress in our understanding of the relationships of these important fishes occurs with increasing sampling of species to mitigate these potential problems. We describe a case of ectopic pancreas of the small bowel and mesentery causing recurrent episodes of pancreatitis, initially suspected on computed tomography(CT)and magnetic resonance cholangiopancreatography(MRCP),and confirmed on histological review of the resection. METHODS:A 67-year-old woman presented with clinical symptoms and biochemical evidence of pancreatitis.She had similar episodes over the past 30 years with unrevealing investigations,and was concluded to have idiopathic pancreatitis. She underwent CT and MRCP,with findings suggestive of ectopic pancreas,a diagnosis confirmed on histology of the resection. RESULTS:MRCP identified a mass in the proximal small bowel mesentery isointense to the native pancreas,with a small duct draining into a proximal jejunal loop.The resected specimen consisted of normal parenchyma with lobulated acinar tissue with scattered islets of Langerhans,an occasional ductular structure,and admixed areas of adipose tissue.The patient remained asymptomatic with normal biochemistry six months post-operatively. CONCLUSION:In an individual with abdominal pain,elevated serum amylase/lipase,but imaging findings of a normal native pancreas,ectopic pancreatitis should be considered,and can be evaluated by CT and MRCP.展开更多

For several years, control of root-knot nematodes largely depended on the...Root-knot nematodes (Meloidogvne spp.) constrain the production of high value vegetable crops, such as tomato, on smallholder farms in Kenya. For several years, control of root-knot nematodes largely depended on the use of synthetic nematicides, most of which have been banned due to their toxicity to humans and the environment. Farmer-participatory trials were established, for two consecutive growing seasons on farmers' fields infested with root-knot nematodes (A4eloidogvne javanica and Meloidogyne incognita), at two sites in central Kenya (Maragua and Kirinyaga districts), representing two contrasting group of farmers (low-input and high-input). The trials aimed at evaluating the potential of using biological control agents (BCAs), i.e. Poehonia ehlamydosporia and Pasteuria penetrans, antagonistic plants (Crotalaria grahamiana and Tagetes minuta), "'trash burning" and chemical control (dazomet 98% = Basamid~ Granular) in the management of root-knot nematodes in tomato nursery beds, and quantify the associated net benefits, compared to an untreated control. Crotalaria grahamiana, "trash burning" and dazomet, significantly (P 〈 0.05) reduced the number of second-stage juveniles (J2s) of the root-knot nematodes in the soil in nursery beds during the first season at Karigui-ini, but there was no significant (P 〉 0.05) increase in yield after transplanting tomato seedlings from the respective treatment. Similarly, one of the BCAs (P. ehlamydosporia), was effectively transferred to the field with seedlings from the BCA-treated nursery beds, but there was no significant (P 〉 0.05) increase in yield due to slow proliferation of the BCA, after transplanting. Participatory partial budgets were successfully developed with srnallholder farmers, and proved to be a vital tool in making informed decisions on management options for root-knot nematodes. Depending on the culture conditions endothelial cells were found to favor maintenance of the neuroprogenitor phenotype through the production of soluble factors, or to promote neuronal differentiation of neural progenitors through direct contact. A total of 47 Sprague-Dawley rats underwent bilateral vasectomy and were divided into groups according to length of the vas deferens affected (0.5, 1, 1.5 cm). After 8 weeks, poly-(D,L-lactide) (PDLA) grafts were used to reconnect the vas deferens, Grafts and adjoining vasa deferentia were excised 8 and 12 weeks later and evaluated microscopically. At 8 weeks, microscopic changes included a robust inflammatory response around the grafts. All grafts were still intact but in the early stages of degradation. No microtubules, indicative of vas deferens recanalization, were identified. One specimen showed evidence of healing and neovascularization at the interface zone between the vas deferens and the graft. At 12 weeks, grafts were further degraded but still present. Microscopic evaluation showed decreased inflammation. Seven specimens showed neovascularization at the interface zone; two of these showed distinct epithelialized vas deferens microcanals at the graft edges. One specimen showed a microcanal spanning the entire 0.5-cm graft. A time period of 8 weeks is not ample enough for vas deferens regeneration in the setting of a biodegradable PDLA graft; however, early evidence of re-growth was seen at 12 weeks. A longer healing time should permit further biodegradation of the graft, as well as re-growth and possible eventual reconnection of the vas deferens, allowing passage of sperm. These findings suggest a potential role for biodegradable grafts in the reconstruction ofvas deferens with long obstructed segments.展开更多

We report a method involving wipe sampling, liquid chromatography separation, and tandem mass spectrometry, which enables determination of 82 pesticides out of the 96 regulated by Health Canada. To demonstrate an application of the method, we sampled and measured pesticides in two cannabis growing facilities, representing a non-certified and a certified site. We detected 41 pesticides in surface wipe samples at the non-certified site and 6 at the certified site. This study provides the first evidence showing pesticide occurrence on common surfaces in cannabis growing facilities and points to a need for routine monitoring and strict control of pesticide use in cannabis facilities.展开更多

Design: Blinded, randomised, placebo controlled trial. Setting: Level III neonatal intensive care units in two centres. Patients: A total of 126 ventilated neonates (inclusion c riteria: postnatal age < 3 days, duration of ventilation < 8 hours, indwelling a rterial catheter for clinical purposes; exclusion criteria: severe asphyxia, sev ere intraventricular haemorrhage, major congenital anomalies, neuromuscular bloc kers). Interventions: Plasma adrenaline and noradrenaline concentrations were de termined in patients during blinded morphine (n = 60) and placebo (n = 66) infus ion (100 μg/kg plus 10 μg/kg/h). Results: Plasma concentrations at baseline (n mol/l with interquartile range in parentheses) were comparable in infants treate d with morphine (adrenaline, 0.22 (0.31); noradrenaline, 2.52 (2.99)) or pla cebo (adrenaline, 0.29 (0.46); noradrenaline, 2.44 (3.14)). During infusion, median adrenaline concentrations were 0.12 (0.28) and 0.18 (0.35)-and medi an noradrenaline concentrations were 2.8 (3.7) and 3.8 (4.0) for the morphin e and placebo treated infants respectively. Multivariate analyses showed that no radrenaline (p = 0.029), but not adrenaline (p = 0.18), concentrations were si gnificantly lower in the morphine group than the placebo group. Furthermore, nor adrenaline concentrations were related to the length of stay in the neonatal int ensive care unit. Conclusions: Continuous morphine infusion significantly decrea sed plasma noradrenaline concentrations in ventilated newborns compared with pla cebo treatment. The results of this study support the idea that routine morphine administration decreases stress responses in ventilated neonates.展开更多

The databases were searched for relevant English language articles published between January 1990 and April 2014. Only studies that conducted a quantitative evaluation of motor ability and concerned individuals aged 0-18 years were included. A separate search was conducted for each disorder(ASD, ADHD, DBD, depression) in conjunction with each of the following keywords:(psycho/perceptuo) motor/movement skill(s),(psycho/perceptuo) motor/movement abilities,(psycho/perceptuo) motor/movement impairment,(psycho/perceptuo) motor/movement problem(s), motor function, motor coordination, motor performance, motor deficit(s). To detect supplementary relevant literature, the reference lists of the retrieved articles were examined. RESULTS: The search strategy yielded 51 studies meeting the inclusion criteria. In total, 28 studies were included that examined the motor abilities of children and adolescents with ASD. All studies indicated that they performed below average on various standardized motor assessment instruments. The overall prevalence rate for impairment in motor abilities ranged from 33% to 100%. Twenty-seven studies examined the motor abilities of children and adolescents with ADHD. Depending on the type of motor assessment tool and the cut-off points used by different researchers, prevalence rates of impairment in motor abilities are highly variable and ranged from 8% to 73%. For several years, control of root-knot nematodes largely depended on the use of synthetic nematicides, most of which have been banned due to their toxicity to humans and the environment. Farmer-participatory trials were established, for two consecutive growing seasons on farmers' fields infested with root-knot nematodes (A4eloidogvne javanica and Meloidogyne incognita), at two sites in central Kenya (Maragua and Kirinyaga districts), representing two contrasting group of farmers (low-input and high-input). The trials aimed at evaluating the potential of using biological control agents (BCAs), i.e. Poehonia ehlamydosporia and Pasteuria penetrans, antagonistic plants (Crotalaria grahamiana and Tagetes minuta), "'trash burning" and chemical control (dazomet 98% = Basamid~ Granular) in the management of root-knot nematodes in tomato nursery beds, and quantify the associated net benefits, compared to an untreated control. Crotalaria grahamiana, "trash burning" and dazomet, significantly (P 〈 0.05) reduced the number of second-stage juveniles (J2s) of the root-knot nematodes in the soil in nursery beds during the first season at Karigui-ini, but there was no significant (P 〉 0.05) increase in yield after transplanting tomato seedlings from the respective treatment. Similarly, one of the BCAs (P. ehlamydosporia), was effectively transferred to the field with seedlings from the BCA-treated nursery beds, but there was no significant (P 〉 0.05) increase in yield due to slow proliferation of the BCA, after transplanting. Participatory partial budgets were successfully developed with srnallholder farmers, and proved to be a vital tool in making informed decisions on management options for root-knot nematodes. 文摘Neurovascular interactions are crucial for the normal development of the central nervous system. To study such interactions in primary cultures, we developed a procedure to simultaneously isolate neural progenitor and endothelial cell fractions from embryonic mouse brains. Depending on the culture conditions endothelial cells were found to favor maintenance of the neuroprogenitor phenotype through the production of soluble factors, or to promote neuronal differentiation of neural progenitors through direct contact. These apparently opposing effects could reflect differential cellular interactions needed for the proper development of the brain.
文摘We evaluated a biodegradable graft for reconstruction of rat vasa deferentia with long obstructed or missing segments. A total of 47 Sprague-Dawley rats underwent bilateral vasectomy and were divided into groups according to length of the vas deferens affected (0.5, 1, 1.5 cm). After 8 weeks, poly-(D,L-lactide) (PDLA) grafts were used to reconnect the vas deferens, Grafts and adjoining vasa deferentia were excised 8 and 12 weeks later and evaluated microscopically. At 8 weeks, microscopic changes included a robust inflammatory response around the grafts. All grafts were still intact but in the early stages of degradation. No microtubules, indicative of vas deferens recanalization, were identified. One specimen showed evidence of healing and neovascularization at the interface zone between the vas deferens and the graft. At 12 weeks, grafts were further degraded but still present. Microscopic evaluation showed decreased inflammation. Seven specimens showed neovascularization at the interface zone; two of these showed distinct epithelialized vas deferens microcanals at the graft edges. One specimen showed a microcanal spanning the entire 0.5-cm graft. A time period of 8 weeks is not ample enough for vas deferens regeneration in the setting of a biodegradable PDLA graft; however, early evidence of re-growth was seen at 12 weeks. A longer healing time should permit further biodegradation of the graft, as well as re-growth and possible eventual reconnection of the vas deferens, allowing passage of sperm. These findings suggest a potential role for biodegradable grafts in the reconstruction ofvas deferens with long obstructed segments.

文摘Several obstacles to the production,expansion and genetic modification of immunotherapeutic T cells in vitro have restricted the widespread use of T-cell immunotherapy.In the context of HSCT,delayed naïve T-cell recovery contributes to poor outcomes.A novel approach to overcome the major limitations of both T-cell immunotherapy and HSCT would be to transplant human T-lymphoid progenitors(HTLPs),allowing reconstitution of a fully functional naïve T-cell pool in the patient thymus.However,it is challenging to produce HTLPs in the high numbers required to meet clinical needs.Here,we found that adding tumor necrosis factor alpha(TNFα)to a DL-4-based culture system led to the generation of a large number of nonmodified or genetically modified HTLPs possessing highly efficient in vitro and in vivo T-cell potential from either CB HSPCs or mPB HSPCs through accelerated T-cell differentiation and enhanced HTLP cell cycling and survival.This study provides a clinically suitable cell culture platform to generate high numbers of clinically potent nonmodified or genetically modified HTLPs for accelerating immune recovery after HSCT and for T-cell-based immunotherapy(including CAR T-cell therapy).

文摘Determination of pesticides in cannabis facilities is increasingly important as medicinal and recreational uses of cannabis products expand rapidly. We report a method involving wipe sampling, liquid chromatography separation, and tandem mass spectrometry, which enables determination of 82 pesticides out of the 96 regulated by Health Canada. To demonstrate an application of the method, we sampled and measured pesticides in two cannabis growing facilities, representing a non-certified and a certified site. We detected 41 pesticides in surface wipe samples at the non-certified site and 6 at the certified site. This study provides the first evidence showing pesticide occurrence on common surfaces in cannabis growing facilities and points to a need for routine monitoring and strict control of pesticide use in cannabis facilities.
文摘Objectives: To determine the effects of continuous morphine infusion in ventil ated newborns on plasma concentrations of adrenaline (epinephrine) and noradrena line (norepinephrine)-and their relation to clinical outcome. Design: Blinded, randomised, placebo controlled trial. Setting: Level III neonatal intensive care units in two centres. Patients: A total of 126 ventilated neonates (inclusion c riteria: postnatal age < 3 days, duration of ventilation < 8 hours, indwelling a rterial catheter for clinical purposes; exclusion criteria: severe asphyxia, sev ere intraventricular haemorrhage, major congenital anomalies, neuromuscular bloc kers). Interventions: Plasma adrenaline and noradrenaline concentrations were de termined in patients during blinded morphine (n = 60) and placebo (n = 66) infus ion (100 μg/kg plus 10 μg/kg/h). Results: Plasma concentrations at baseline (n mol/l with interquartile range in parentheses) were comparable in infants treate d with morphine (adrenaline, 0.22 (0.31); noradrenaline, 2.52 (2.99)) or pla cebo (adrenaline, 0.29 (0.46); noradrenaline, 2.44 (3.14)). During infusion, median adrenaline concentrations were 0.12 (0.28) and 0.18 (0.35)-and medi an noradrenaline concentrations were 2.8 (3.7) and 3.8 (4.0) for the morphin e and placebo treated infants respectively. Multivariate analyses showed that no radrenaline (p = 0.029), but not adrenaline (p = 0.18), concentrations were si gnificantly lower in the morphine group than the placebo group. Furthermore, nor adrenaline concentrations were related to the length of stay in the neonatal int ensive care unit. Conclusions: Continuous morphine infusion significantly decrea sed plasma noradrenaline concentrations in ventilated newborns compared with pla cebo treatment. The results of this study support the idea that routine morphine administration decreases stress responses in ventilated neonates.
文摘Forests,trees,and agroforestry(FTA)are ecosystem hotspots.They exemplify the contributions of biodiversity to sustainable and resilient landscapes,green circular economy and to sustainable agriculture and food systems for healthy diets.However,most research on these topics have been performed separately and lack comparison.The International FTA-Kunming Conference'Forests,trees and agroforestry for diverse sustainable landscapes'22nd–24th June 2021,focused on these contributions,brought together scientists NGOs,and policy makers to further the understanding of tree diversity;provided a communication platform for scientists to share their research results;evaluated the role of tree diversity in agroecology and circular agriculture;assessed benefits of landscape restoration;and explored applied research in mountain ecosystems and food security.The goals were to gather evidence that ground the design of solutions that can contribute to the implementation of the post 2020 Global Biodiversity Framework and towards the UN Food Systems Summit,and the overall implementation of the SDGs.This paper summarizes the outcomes of the international FTA Conference in Kunming 2021 and points out the highlights of research involved in six major themes.
文摘AIM: To summarize research regarding the motor abilities of children and adolescents who suffer from a common psychiatric condition.METHODS: In order to outline the current knowledge regarding the motor abilities of children and adolescents with autism spectrum disorders(ASD), attention deficit hyperactivity disorder(ADHD), disruptive behavior disorders(DBD) and depression, a comprehensive systematic literature search was carried out using Pub Med, Medline and ERIC databases. The databases were searched for relevant English language articles published between January 1990 and April 2014. Only studies that conducted a quantitative evaluation of motor ability and concerned individuals aged 0-18 years were included. A separate search was conducted for each disorder(ASD, ADHD, DBD, depression) in conjunction with each of the following keywords:(psycho/perceptuo) motor/movement skill(s),(psycho/perceptuo) motor/movement abilities,(psycho/perceptuo) motor/movement impairment,(psycho/perceptuo) motor/movement problem(s), motor function, motor coordination, motor performance, motor deficit(s). To detect supplementary relevant literature, the reference lists of the retrieved articles were examined. RESULTS: The search strategy yielded 51 studies meeting the inclusion criteria. In total, 28 studies were included that examined the motor abilities of children and adolescents with ASD. All studies indicated that they performed below average on various standardized motor assessment instruments. The overall prevalence rate for impairment in motor abilities ranged from 33% to 100%. Twenty-seven studies examined the motor abilities of children and adolescents with ADHD. Depending on the type of motor assessment tool and the cut-off points used by different researchers, prevalence rates of impairment in motor abilities are highly variable and ranged from 8% to 73%.
