However, it remains poorly understood whether a chemically extracted acellular allogeneic nerve graft combined with neurotrophic factors provides a good local environment for neural regeneration. This study investigated the repair of injured rat sciatic nerve using a chemically extracted acellular allogeneic nerve graft combined with ciliary neurotrophic factor. An autologous nerve anastomosis group and a chemical acellular allogeneic nerve bridging group were prepared as controls. At 8 weeks after repair, sciatic functional index, evoked potential amplitude of the soleus muscle, triceps wet weight recovery rate, total number of myelinated nerve fibers and myelin sheath thickness were measured. For these indices, values in the three groups showed the autologous nerve anastomosis group 〉 chemically extracted acellular nerve graft + ciliary neurotrophic factor group 〉 chemical acellular allogeneic nerve bridging group. These results suggest that chemically extracted acellular nerve grafts combined with ciliary neurotrophic factor can repair sciatic nerve defects, and that this repair is inferior to autologous nerve anastomosis, but superior to chemically extracted acellular allogeneic nerve bridging alone.展开更多

We report the case of a 24-year-old patient, who presented with tuberculous endocarditis after mechanical mit...Background: Tuberculous endocarditis is a rare but serious complication of heart valve replacement surgery. We report the case of a 24-year-old patient, who presented with tuberculous endocarditis after mechanical mitral valve replacement, with a favorable clinical course following anti-tuberculosis treatment. Case Presentation: We report a 24-year-old male patient, admitted to the cardiac surgery department of the Fann Hospital (Dakar, Senegal), for the management of severe mixed (rheumatic and endocarditic) mitral insufficiency with associated tricuspid insufficiency. He had a history of recurrent angina and polyarthralgia in childhood, was hospitalized several times for refractory global cardiac decompensation, and for a suspected infective endocarditis a month before his admission. On admission, the clinical examination revealed signs suggestive of mitral and tricuspid insufficiency. Transthoracic echocardiography revealed severe post-endocarditic mitral insufficiency with A3 amputation, highly mobile 15 mm vegetations on the free edge of the large valve, moderate tricuspid insufficiency, and severe pulmonary artery hypertension. Mechanical mitral valve replacement and tricuspid valve annuloplasty using autologous pericardial strip were performed via median sternotomy. After ten days, the patient presented with global cardiac decompensation associated with a clinico-biological infectious syndrome, and tans-oesophageal echography revealed an abscess at the sinotubular junction, communicating with the aorta. A thoraco-abdomino-pelvic CT scan was done, which revealed a bilateral alveolar-interstitial syndrome with mediastinal lymphadenopathy. Anti-tuberculosis treatment with RHZE was initiated for 06 months. The clinical course was favorable. Conclusion: Tuberculous endocarditis in prostheses is a serious complication of heart valve replacement surgery, which may evolve favorably under medical treatment.展开更多

While historically described in veterinary pathology, it has also been found in humans, most commonly in the bladder and typically associated with urinary stasis. However, its occurrence in the renal calyx is extremely rare. We report a case of a jackstone located in the renal calyx, secondary to a large Bosniak 1 renal cyst. The simultaneous presence of the calyceal stone and renal cyst prompted us to opt for open surgery, allowing for comprehensive treatment of both conditions. The patient’s postoperative recovery was smooth, demonstrating that despite its invasiveness, open surgery remains a relevant option for managing complex stones, particularly in resource-limited settings.展开更多

Two of them were men and the first signs were noted by the mother at the birth in 2 cases. Only one case of consanguinity was observed. Clinically, light brown spots on the skin, neurofibromas, Lisch nodules were constantly observed. Histopathological’s exam confirmed neurofibromas. Moreover, cutaneous and ophthalmological manifestations lead to the diagnostic. Two cases of orthopedic complications were observed: one scoliosis and one Congenital dysplasia of the long bones. There was no specific treatment. The power system is susceptible to failures,and further interruptions by subsequent outages which may cause extended events in the formed island.Most of the literature reviewed has focused on island formation after th...The power system is susceptible to failures,and further interruptions by subsequent outages which may cause extended events in the formed island.Most of the literature reviewed has focused on island formation after the disconnection of the main grid supply.However,there is still a need to analyze the post-restoration failures to improve the resilience and minimize the pro-longed disconnection of priority customers.The current research investigates the behavior of islanding schemes under secondary collapses and provides an adaptive solution to withstand such severities.A new total active critical node finding methodology is presented which eases the rearrangement of the initially formed island when affected by multiple faults or DG failures.The concept of single islanding with the coordination of multiple DGs is also discussed,in which the DG's power-sharing capacity is not confined to the boundaries of only particular islands.An economic plan is also proposed to select the controlled type DGs to reduce the overall cost of generation by reducing fuel cost.To evaluate the network's effectiveness and robustness,the proposed adaptive method is deployed on an updated IEEE 69-bus distribution system.Index Terms-Active distribution network,distributed generation,multi-objective,post-restoration,single islanding.展开更多
Introduction: Cerebral palsy is the most common motor disorder of childhood according to the Center for Disease Control and Prevention (CDC). It is much more common in premature infants. The majority of fixed organic ...Introduction: Cerebral palsy is the most common motor disorder of childhood according to the Center for Disease Control and Prevention (CDC). It is much more common in premature infants. The majority of fixed organic lesions occur during the development of the central nervous system. Objective: To study the epidemiological, clinical, paraclinical and prognostic profile of children aged 0 to 5 years with Cerebral Palsy (CP) seen in consultation in the medical neurology department of the National Hospital of Niamey. Methods: This was a transverse descriptive study with prospective collection of patient records (0 - 5 years) over a 6-month period. Results: During our study, 100 children aged 0 to 5 years had to be consulted. The prevalence was 61%. 62.3% of patients were male. The mean age was 2.01 years. The majority of the patients were from the urban area (63.93%). Almost half of the mothers had no schooling. 4.92% of patients were from consanguineous marriages. Pregnancies were monitored with at least 4 prenatal visit in 86.98% of cases and 83.61% of children were born at term. In 75.41% of cases, patients were born by eutocic delivery. In 36.07% of cases, the patients were resuscitated at birth. Fetal distress was present in 29.5%. Seizures were present in 49.18% of the patients of which 50% were generalized. Tonicity was not good in 35.15% and among them 52.63% had hypertonia. 18.03% of patients had a motor deficit. For 34.43% of the patients the first nerve was affected, followed by the third nerve with 13.11%. 65.57% of the patients had fixed retardation and for 78.69% of the patients the retardation was homogeneous. Neuromalaria, meningitis and hydrocephalus were the most incriminated causes of delay in 63.51%, 19.67% and 18.03% of cases respectively. Anticonvulsants and psychoanaleptics were the most used in 49.18% and 32.79% of cases respectively. 32.79% of patients had received motor rehabilitation. All mothers had received psychotherapy. Power systems can suffer outages,causing complete or partial disconnection of their power supply to load centers within the distribution networks.Distributed Generation(DG)plays an essential role in power systems.DG c...Power systems can suffer outages,causing complete or partial disconnection of their power supply to load centers within the distribution networks.Distributed Generation(DG)plays an essential role in power systems.DG can be used as a back-up power source to enhance the resiliency and reliability of a power system.Island mode operations after outages in an active distributing network(ADN)is an effective way to maintain continuity of the power supply to significant loads.It is a quite complicated task for power system operators to find the power flow path.Previous studies have primarily used pre-defined guidelines to find feasible power flow paths,and have focused on multiple islands for restoration.In these studies,possible restoration pathfinding with DG was the fundamental weakness,and furthermore,the power of DG was limited to pre-defined boundaries in the form of islands.Therefore,in this study,a new algorithm has been proposed,which uses the minimum spanning tree(MST)method to find the most feasible path.The proposed algorithm starts at any random node(in this case,DG),and progresses by selecting the next node with the least cost(weight),thus considering all the nodes through which power will flow.The proposed model is formulated as a multiobjective program considering the priority of loads and minimum power loss.The effectiveness of the proposed model is tested on a modified IEEE69-bus distribution system with the penetration of multiple distributed generation sources at different nodes.Results were compared with the strategies found in literature,and the proposed method was found to be feasible and efficient.展开更多

The modes of sand transport by the wind are greatly different from those by water flow. On the other hand wind-blown sands are of a material circulation process of the earth surface. They affect wind-sand transport flux and sand ejection of a flux, the damage of grains formed cannot be neglected in engineering. Because of the complexity of windblown sand flux system, the understanding of its basic mechanics is not yet clear. The key forces in sand salutation mainly includes: the valid gravity, air drag force 'Magnus force' Saffman force 'Basset force' additional quality force and scatter force among grains. The most important force in sand salutation is the air drag force. Computation of the single sphere drag coefficient and double spheres drag coefficient is presented for the distance between two spheres being smaller than twelve times of the sphere diameter and the spheres being at different angular positions. The flow interference of two spheres was investigated for the distance s = 0.08 d to 12d, angular position 0 = 0 to 360 and Reynolds number 15≤Re≤1000.展开更多
Difluoroboronβ-diketonate(BF2bdk)compounds represent an important class of luminescent materials,whereas their synthesis requires multiple steps,which restrict their application in diverse fields.Here we report a cas...Difluoroboronβ-diketonate(BF2bdk)compounds represent an important class of luminescent materials,whereas their synthesis requires multiple steps,which restrict their application in diverse fields.Here we report a cascade reaction to prepare BF2bdk from aromatic ketones,carboxylic acids,trifluoroacetic anhydride and boron trifluoride diethyl etherate.The cascade reaction is very simple and straightforward to produce BF2bdk with desired functional groups in reasonable isolation yields.Further,when some specific BF2bdk compounds are used as luminescent dopants,organic room-temperature phosphorescence with lifetimes>1.0 s and intense afterglow under ambient conditions can be achieved with the assistance by suitable organic matrices.The dopant-matrix materials exhibit excellent processability into desired patterns and large-area panels,and function as efficient donors of excited state energy transfer for the fabrication of red afterglow materials.展开更多
Heavy atom effects and n-π*transitions have been frequently reported to enhance room-temperature organic phosphorescence efficiency but lead to shortage of phosphorescence lifetimes.Unlike these reported studies,we c...Heavy atom effects and n-π*transitions have been frequently reported to enhance room-temperature organic phosphorescence efficiency but lead to shortage of phosphorescence lifetimes.Unlike these reported studies,we conceive the incorporation of advanced charge transfer(CT)technology to boost room-temperature organic afterglow efficiency and simultaneously maintain afterglow lifetimes.Here we design difluoroboronβ-diketonate(BF2bdk)CT compounds with moderate singlet-triplet splitting energy(ΔEST)of around 0.4 e V,and relatively large spin-orbit coupling matrix elements(SOCME(S_(1)-T_(1)),1–10 cm^(-1))to achieve efficient intersystem crossing(ISC)and moderate rates of reverse intersystem crossing(kRISC,1–10 s^(-1)).The advanced CT technology,which includes multiple electron-donating groups and orthogonal donor-acceptor arrangement,have been found to narrowΔESTand enhance both ISC and RISC.Meanwhile,the organic matrices suppress nonradiative decay of BF2bdk’s T1states by their rigid microenvironment.Consequently,thermally activated delayed fluorescence(TADF)-type organic afterglow materials can be achieved with afterglow efficiency up to 83.0%,long lifetimes of 433 ms,excellent processablility,as well as advanced anti-counterfeiting and information encryption.Furthermore,with the aid of up-conversion materials and through radiative energy transfer,TADF-type afterglow materials with aqueous dispersity and near-infrared light-excitable property have been achieved,which paves the way for biomedical applications.展开更多

Seminomas are treated by orchidectomy with usually excellent outcomes. We report the occurrence of a classical seminoma in a 92-year-old man,who is currently the oldest patient with this histology reported in literature. He presented with a painful,swollen testis. Scrotal ultrasound scan revealed a testicular mass. A left inguinal orchidectomy was carried out and histological examination confirmed the diagnosis of a classical seminoma. Further staging by computerised tomography revealed pulmonary lesions suspicious of metastases. The patient declined further treatment in view of his age and co-morbidities.展开更多

The majority of the patients were from the urban area (63.93%). Almost half of the mothers had no schooling. 4.92% of patients were from consanguineous marriages. Pregnancies were monitored with at least 4 prenatal visit in 86.98% of cases and 83.61% of children were born at term. In 75.41% of cases, patients were born by eutocic delivery. In 36.07% of cases, the patients were resuscitated at birth. Fetal distress was present in 29.5%. Seizures were present in 49.18% of the patients of which 50% were generalized. Tonicity was not good in 35.15% and among them 52.63% had hypertonia. 18.03% of patients had a motor deficit. For 34.43% of the patients the first nerve was affected, followed by the third nerve with 13.11%. 65.57% of the patients had fixed retardation and for 78.69% of the patients the retardation was homogeneous. Neuromalaria, meningitis and hydrocephalus were the most incriminated causes of delay in 63.51%, 19.67% and 18.03% of cases respectively. Anticonvulsants and psychoanaleptics were the most used in 49.18% and 32.79% of cases respectively. 32.79% of patients had received motor rehabilitation. All mothers had received psychotherapy. 文摘Power systems can suffer outages,causing complete or partial disconnection of their power supply to load centers within the distribution networks.Distributed Generation(DG)plays an essential role in power systems.DG can be used as a back-up power source to enhance the resiliency and reliability of a power system.Island mode operations after outages in an active distributing network(ADN)is an effective way to maintain continuity of the power supply to significant loads.It is a quite complicated task for power system operators to find the power flow path.Previous studies have primarily used pre-defined guidelines to find feasible power flow paths,and have focused on multiple islands for restoration.In these studies,possible restoration pathfinding with DG was the fundamental weakness,and furthermore,the power of DG was limited to pre-defined boundaries in the form of islands.Therefore,in this study,a new algorithm has been proposed,which uses the minimum spanning tree(MST)method to find the most feasible path.The proposed algorithm starts at any random node(in this case,DG),and progresses by selecting the next node with the least cost(weight),thus considering all the nodes through which power will flow.The proposed model is formulated as a multiobjective program considering the priority of loads and minimum power loss.The effectiveness of the proposed model is tested on a modified IEEE69-bus distribution system with the penetration of multiple distributed generation sources at different nodes.Results were compared with the strategies found in literature,and the proposed method was found to be feasible and efficient.

文摘Winds on the earth are commonly strong enough to erode transport and deposit sediment. The modes of sand transport by the wind are greatly different from those by water flow. On the other hand wind-blown sands are of a material circulation process of the earth surface. They affect wind-sand transport flux and sand ejection of a flux, the damage of grains formed cannot be neglected in engineering. Because of the complexity of windblown sand flux system, the understanding of its basic mechanics is not yet clear. The key forces in sand salutation mainly includes: the valid gravity, air drag force 'Magnus force' Saffman force 'Basset force' additional quality force and scatter force among grains. The most important force in sand salutation is the air drag force. Computation of the single sphere drag coefficient and double spheres drag coefficient is presented for the distance between two spheres being smaller than twelve times of the sphere diameter and the spheres being at different angular positions. The flow interference of two spheres was investigated for the distance s = 0.08 d to 12d, angular position 0 = 0 to 360 and Reynolds number 15≤Re≤1000.

文摘Difluoroboronβ-diketonate(BF2bdk)compounds represent an important class of luminescent materials,whereas their synthesis requires multiple steps,which restrict their application in diverse fields.Here we report a cascade reaction to prepare BF2bdk from aromatic ketones,carboxylic acids,trifluoroacetic anhydride and boron trifluoride diethyl etherate.The cascade reaction is very simple and straightforward to produce BF2bdk with desired functional groups in reasonable isolation yields.Further,when some specific BF2bdk compounds are used as luminescent dopants,organic room-temperature phosphorescence with lifetimes>1.0 s and intense afterglow under ambient conditions can be achieved with the assistance by suitable organic matrices.The dopant-matrix materials exhibit excellent processability into desired patterns and large-area panels,and function as efficient donors of excited state energy transfer for the fabrication of red afterglow materials.

文摘Heavy atom effects and n-π*transitions have been frequently reported to enhance room-temperature organic phosphorescence efficiency but lead to shortage of phosphorescence lifetimes.Unlike these reported studies,we conceive the incorporation of advanced charge transfer(CT)technology to boost room-temperature organic afterglow efficiency and simultaneously maintain afterglow lifetimes.Here we design difluoroboronβ-diketonate(BF2bdk)CT compounds with moderate singlet-triplet splitting energy(ΔEST)of around 0.4 e V,and relatively large spin-orbit coupling matrix elements(SOCME(S_(1)-T_(1)),1–10 cm^(-1))to achieve efficient intersystem crossing(ISC)and moderate rates of reverse intersystem crossing(kRISC,1–10 s^(-1)).The advanced CT technology,which includes multiple electron-donating groups and orthogonal donor-acceptor arrangement,have been found to narrowΔESTand enhance both ISC and RISC.Meanwhile,the organic matrices suppress nonradiative decay of BF2bdk’s T1states by their rigid microenvironment.Consequently,thermally activated delayed fluorescence(TADF)-type organic afterglow materials can be achieved with afterglow efficiency up to 83.0%,long lifetimes of 433 ms,excellent processablility,as well as advanced anti-counterfeiting and information encryption.Furthermore,with the aid of up-conversion materials and through radiative energy transfer,TADF-type afterglow materials with aqueous dispersity and near-infrared light-excitable property have been achieved,which paves the way for biomedical applications.
文摘Seminoma is a germ cell tumour which primarily affects the testes. Seminomas are treated by orchidectomy with usually excellent outcomes. We report the occurrence of a classical seminoma in a 92-year-old man,who is currently the oldest patient with this histology reported in literature. He presented with a painful,swollen testis. Scrotal ultrasound scan revealed a testicular mass. A left inguinal orchidectomy was carried out and histological examination confirmed the diagnosis of a classical seminoma. Further staging by computerised tomography revealed pulmonary lesions suspicious of metastases. The patient declined further treatment in view of his age and co-morbidities.
