AIM: To assess the feasibility and efficacy of laparoscopic total mesorectal excision (LTME) of low rectal cancer with preservation of anal sphincter.METHODS: From June 2001 to June 2003, 82 patients with low rectal c...AIM: To assess the feasibility and efficacy of laparoscopic total mesorectal excision (LTME) of low rectal cancer with preservation of anal sphincter.METHODS: From June 2001 to June 2003, 82 patients with low rectal cancer underwent laparoscopic total mesorectal excision with preservation of anal sphincter. The lowest edge of tumors was below peritoneal reflection and 1.5-7 cm from the dentate line (1.5-5 cm in 48 cases, 5-7 cm in 34 cases).RESULTS: LTME with anal sphincter preservation was performed on 82 randomized patients with low rectal cancer, and 100 % sphincter preservation rate was achieved. There were 30 patients with laparoscopic low anterior resection (LLAR) at the level of the anastomosis below peritoneal reflection and 2 cm above from the dentate line; 27 patients with laparoscopic ultralow anterior resection (LULAR) at the level of anastomoses 2 cm below from the dentate line; and 25 patients with laparoscopic coloanal anastomoses (LC AA) at the level of the anastomoses at or below the dentate line. No defunctioning ileostomy was created in any case. The mean operating time was 120 minutes (ranged from 110-220 min), and the mean operative blood loss was 20 mL (ranged from 5-120 mL).Bowel function was restored and diet was resumed on day 1 or 2 after operation. The mean hospital stay was 8 days (ranged from 5-14). Postoperative analgesics were used in 45 patients. After surgery, 2 patients had urinary retention, one had anastomotic leakage, and another 2patients had local recurrence one year later. No interoperative complication was observed.CONCLUSION: LTME with preservation of anal sphincter is a feasible, safe and minimally invasive technique with less postoperative pain and rapid recovery, and importantly, it has preserved the function of the sphincter.展开更多
AIM: To clarify the expression change of Wnt-induced secreted protein-1 (WISP-l) in human rectal cancer and to determine whether it is correlated with invasion and metastasis of human rectal cancer.METHODS: Eighty...AIM: To clarify the expression change of Wnt-induced secreted protein-1 (WISP-l) in human rectal cancer and to determine whether it is correlated with invasion and metastasis of human rectal cancer.METHODS: Eighty-six paired samples of rectal cancer and surgically resected distant normal rectal tissue were collected and allocated into cancer group and control group respectively. WISP-1 mRNA was detected by relative quantitative real-time RT-PCR and WISP-1 protein was examined by immunohistochemical staining.RESULTS: WISP-1 gene overexpression was found in 65% (56/86) primary rectal cancers, 2-30 times that of the level in normal matched rectal tissues (P = 0.001). The mRNA expression level was correlated with Duke's staging, histological differentiation grade and lymph node status. The WISP-1 protein expression was in accordance with mRNA expression level. The positive degree of immunohistochemical staining in the cancer group (1.40±0.35) was different from that in control group (1.04± 0.08, P 〈 0.001). Moreover, in cancer group the positive staining degree in high-level mRNA cancers (1.46 ±0.37, n = 56) was higher than that in low-level mRNA (1.28 ± 0.28, n = 30, P = 0.018).CONCLUSION: Aberrant levels of WISP-1 expression may play a role in rectal tumorigenesis. WISP-1 may be used as a specific clinical diagnosis and prognosis marker in rectal cancer.展开更多

Whilst previous studies generally focused on flu...In recent years, nonconventional luminogens free of aromatic groups have attracted extensive attention due to their academic importance and promising wide applications. Whilst previous studies generally focused on fluorescence from aliphatic amine or carbonylcontaining systems, less attention has been paid to room temperature phosphorescence(RTP) and the systems with predominant oxygen functionalities. In this work, photophysical properties of the polyhydroxy polymers, including microcrystalline cellulose(MCC), 2-hydroxyethyl cellulose(HEC), hydroxypropyl cellulose(HPC), and cellulose acetate(CA), were studied and compared. While MCC,HEC, and HPC solids showed bright emission alongside distinct RTP, CA demonstrated relatively low intensity of solid emission without noticeable RTP. Their emissions were explained in terms of the clustering-triggered emission(CTE) mechanism and conformation rigidification. Additionally, on account of its intrinsic emission, concentrated HEC aqueous solution could be used as the probe for the detection of 2,4,6-trinitrophenol(TNP).展开更多

Nonconventional luminescent materials(NLMs)are a type of organic luminescent materials that does not contain aromatic units.Due to the simplicity of the synthesis process,mild reaction conditions,good hydrophilicity a...Nonconventional luminescent materials(NLMs)are a type of organic luminescent materials that does not contain aromatic units.Due to the simplicity of the synthesis process,mild reaction conditions,good hydrophilicity and biological compatibility,NLMs have attracted much attention.Nevertheless,numerous reports indicate that NLMs can only effectively luminesce at high concentrations and in solid state,which limits their applicability in the field of cell imaging.This study addresses this limitation by designing and synthesizing oligomers P1,P2 and P3 using ethylene glycol diglycidyl ether and amine compounds containing ethylene groups.These oligomers exhibit remarkable luminescence efficiency reaching as high as 9.2%in dilute solutions(0.1 mg/m L),making them among the best NLMs in this category.Furthermore,the synthesized oligomers exhibit excitation wavelength-dependent and concentration-dependent luminescence intensity,fluorescence response to temperature and p H changes,as well as the ability to identify Fe^(3+),Cu^(2+)and Mo^(5+)in dilute solutions.These characteristics render them potentially useful in the for cell imaging.展开更多

In order to determine how the subaqueous delta evolution depends on the water and sediment processes in the Yangtze Estuary, the amounts of water and sediment discharged into the estuary were studied. The results show...In order to determine how the subaqueous delta evolution depends on the water and sediment processes in the Yangtze Estuary, the amounts of water and sediment discharged into the estuary were studied. The results show that, during the period from 1950 to 2010, there was no significant change in the annual water discharge, and the multi-annual mean water discharge increased in dry seasons and decreased in flood seasons. However, the annual sediment discharge and the multi-annual mean sediment discharge in flood and dry seasons took on a decreasing trend, and the intra-annual distribution of water and sediment discharges tended to be uniform. The evolution process from deposition to erosion occurred at the -10 m and -20 m isobaths of the subaqueous delta. The enhanced annual water and sediment discharges had a silting-up effect on the delta, and the effect of sediment was greater than that of water. Based on data analysis, empirical curves were built to present the relationships between the water and sediment discharges over a year or in dry and flood seasons and the erosion/deposition rates in typical regions of the suhaqueous delta, whose evolution followed the pattern of silting in flood seasons and scouring in dry seasons. Notably, the Three Gorges Dam has changed the distribution processes of water and sediment discharges, and the dam's regulating and reserving functions can benefit the subaqueous delta deposition when the annual water and sediment discharges are not affected.展开更多
BACKGROUND Trocar site hernia(TSH)is a rare but potentially dangerous complication of laparoscopic surgery,and the drain-site TSH is an even rarer type.Due to the difficulty to diagnose at early stages,TSH often leads...BACKGROUND Trocar site hernia(TSH)is a rare but potentially dangerous complication of laparoscopic surgery,and the drain-site TSH is an even rarer type.Due to the difficulty to diagnose at early stages,TSH often leads to a delay in surgical intervention and eventually results in life-threatening consequences.Herein,we report an unusual case of drain-site TSH,followed by a brief literature review.Finally,we provide a novel,simple,and practical method of prevention.CASE SUMMARY A 54-year-old female patient underwent laparoscopic subtotal hysterectomy and bilateral adnexectomy for uterine fibroids 8 d ago in another hospital.She was admitted to our hospital with a 2-d history of intermittent abdominal pain,nausea,vomiting,and abdominal enlargement with an inability to pass stool and flatus.The emergency computed tomography scan revealed the small bowel herniated through a 10 mm trocar incision,which was used as a drainage port,with diffuse bowel distension and multiple air-fluid levels with gas in the small intestines.She was diagnosed with drain-site strangulated TSH.The emergency exploratory laparotomy confirmed the diagnosis.A herniorrhaphy followed by standard intestinal resection and anastomosis were performed.The patient recovered well after the operation and was discharged on postoperative day 8 and had no postoperative complications at her 2-wk follow-up visit.CONCLUSION TSH must be kept in mind during the differential diagnosis of post-laparoscopic obstruction,especially after the removal of the drainage tube,to avoid the serious consequences caused by delayed diagnosis.Furthermore,all abdomen layers should be carefully closed under direct vision at the trocar port site,especially where the drainage tube was placed.Our simple and practical method of prevention may be a novel strategy worthy of clinical promotion.展开更多

METHODS: Microsatellite markers were ...AIM: To establish the role of human T Cell Factor-4 (hTCF-4) gene exons 3-9 mutation status in association with sporadic rectal cancer with microsatellite instability (MSI). METHODS: Microsatellite markers were genotyped in 93 sporadic rectal cancer patients. Eleven cases were found to be high-frequency MSI (MSI-H). Sequence analysis of the coding region of the exons 3-9 of hTCF-4 gene was carried out for the 11 MSI-H cases and 10 controls (5 microsatellite stability (MSS) cases and 5 cases with normal mucosa). The sequencing and MSI identification were used. RESULTS: Several novel mutations and variants were revealed. In exon 4, one is a 4-position continuous alteration which caused amino acid change from Q131T and S132I (391insA, 392 G 〉 A, 393 A 〉 G and 395delC) and another nucleotide deletion (395delC) is present in MSI-H cases (5/10 and 4/10, respectively) but completely absent in the controls.CONCLUSION: Novel mutations in exon 4 of hTCF-4 gene were revealed in this study, which might be of importance in the pathogenesis of sporadic rectal cancer patients with MSI-H.展开更多

In this work,the chitosan and N-doped graphene natively grown on hierarchical porous carbon(N-PC-G/CS) nanocomposite was obtained by ultrasonic method,as a novel sensor platform for determination of tartrazine(TT)...In this work,the chitosan and N-doped graphene natively grown on hierarchical porous carbon(N-PC-G/CS) nanocomposite was obtained by ultrasonic method,as a novel sensor platform for determination of tartrazine(TT).The nanocomposite as prepared had well dispersivity in water and excellent conductivity.The N-PC-G/CS nanocomposite was characterized by scanning electron microscopy(SEM),transmission electron microscopy(TEM),nitrogen adsorption-desorption,fourier transform infrared(FTIR) and electrochemical impedance spectroscopy(EIS).The application of N-PC-G/CS for determination of tartrazine(TT) was investigated by chronocoulometry(CC),cyclic voltammetry(CV) and differential pulse voltammetry(DPV).Under optimized conditions,the sensor displayed a sensitive response to TT within a wide concentration range of 0.05-15.0 μmol/L,the detection limits is 0.036 μmol/L(S/N = 3).Furthermore,this nanocomposite could be efficiently applied for determination of TT in soft drink samples.展开更多

文摘Nonconventional luminescent materials(NLMs)are a type of organic luminescent materials that does not contain aromatic units.Due to the simplicity of the synthesis process,mild reaction conditions,good hydrophilicity and biological compatibility,NLMs have attracted much attention.Nevertheless,numerous reports indicate that NLMs can only effectively luminesce at high concentrations and in solid state,which limits their applicability in the field of cell imaging.This study addresses this limitation by designing and synthesizing oligomers P1,P2 and P3 using ethylene glycol diglycidyl ether and amine compounds containing ethylene groups.These oligomers exhibit remarkable luminescence efficiency reaching as high as 9.2%in dilute solutions(0.1 mg/m L),making them among the best NLMs in this category.Furthermore,the synthesized oligomers exhibit excitation wavelength-dependent and concentration-dependent luminescence intensity,fluorescence response to temperature and p H changes,as well as the ability to identify Fe^(3+),Cu^(2+)and Mo^(5+)in dilute solutions.These characteristics render them potentially useful in the for cell imaging.

文摘Ethyl α-(dimethylaminomethylene)-2-cyanomethyl-4-phenylquinoline-3-carboxylate(2) as new synthons directed to 1-oxo-1,2-dihydrobenzo[b][1,6]naphthyridine-4-carbonitriles was obtained by the condensation of ethyl 2-cyanomethyl-4-phenylquinoline-3-carboxylate(1) and N,N-dimethylformamide dimethyl acetal(DMFDMA).Reaction of this enamine with primary amines(3) in HOAc-DMF at120 ℃then affords 2-substituted 1-oxo-1,2-dihydrobenzo[b][1,6]naphthyridine-4-carbonitrile derivatives(4) in good yields by a tandem addition-elimination-cyclization reaction.

文摘In order to determine how the subaqueous delta evolution depends on the water and sediment processes in the Yangtze Estuary, the amounts of water and sediment discharged into the estuary were studied. The results show that, during the period from 1950 to 2010, there was no significant change in the annual water discharge, and the multi-annual mean water discharge increased in dry seasons and decreased in flood seasons. However, the annual sediment discharge and the multi-annual mean sediment discharge in flood and dry seasons took on a decreasing trend, and the intra-annual distribution of water and sediment discharges tended to be uniform. The evolution process from deposition to erosion occurred at the -10 m and -20 m isobaths of the subaqueous delta. The enhanced annual water and sediment discharges had a silting-up effect on the delta, and the effect of sediment was greater than that of water. Based on data analysis, empirical curves were built to present the relationships between the water and sediment discharges over a year or in dry and flood seasons and the erosion/deposition rates in typical regions of the suhaqueous delta, whose evolution followed the pattern of silting in flood seasons and scouring in dry seasons. Notably, the Three Gorges Dam has changed the distribution processes of water and sediment discharges, and the dam's regulating and reserving functions can benefit the subaqueous delta deposition when the annual water and sediment discharges are not affected.

文摘BACKGROUND Trocar site hernia(TSH)is a rare but potentially dangerous complication of laparoscopic surgery,and the drain-site TSH is an even rarer type.Due to the difficulty to diagnose at early stages,TSH often leads to a delay in surgical intervention and eventually results in life-threatening consequences.Herein,we report an unusual case of drain-site TSH,followed by a brief literature review.Finally,we provide a novel,simple,and practical method of prevention.CASE SUMMARY A 54-year-old female patient underwent laparoscopic subtotal hysterectomy and bilateral adnexectomy for uterine fibroids 8 d ago in another hospital.She was admitted to our hospital with a 2-d history of intermittent abdominal pain,nausea,vomiting,and abdominal enlargement with an inability to pass stool and flatus.The emergency computed tomography scan revealed the small bowel herniated through a 10 mm trocar incision,which was used as a drainage port,with diffuse bowel distension and multiple air-fluid levels with gas in the small intestines.She was diagnosed with drain-site strangulated TSH.The emergency exploratory laparotomy confirmed the diagnosis.A herniorrhaphy followed by standard intestinal resection and anastomosis were performed.The patient recovered well after the operation and was discharged on postoperative day 8 and had no postoperative complications at her 2-wk follow-up visit.CONCLUSION TSH must be kept in mind during the differential diagnosis of post-laparoscopic obstruction,especially after the removal of the drainage tube,to avoid the serious consequences caused by delayed diagnosis.Furthermore,all abdomen layers should be carefully closed under direct vision at the trocar port site,especially where the drainage tube was placed.Our simple and practical method of prevention may be a novel strategy worthy of clinical promotion.

文摘AIM: To establish the role of human T Cell Factor-4 (hTCF-4) gene exons 3-9 mutation status in association with sporadic rectal cancer with microsatellite instability (MSI). METHODS: Microsatellite markers were genotyped in 93 sporadic rectal cancer patients. Eleven cases were found to be high-frequency MSI (MSI-H). Sequence analysis of the coding region of the exons 3-9 of hTCF-4 gene was carried out for the 11 MSI-H cases and 10 controls (5 microsatellite stability (MSS) cases and 5 cases with normal mucosa). The sequencing and MSI identification were used. RESULTS: Several novel mutations and variants were revealed. In exon 4, one is a 4-position continuous alteration which caused amino acid change from Q131T and S132I (391insA, 392 G 〉 A, 393 A 〉 G and 395delC) and another nucleotide deletion (395delC) is present in MSI-H cases (5/10 and 4/10, respectively) but completely absent in the controls.CONCLUSION: Novel mutations in exon 4 of hTCF-4 gene were revealed in this study, which might be of importance in the pathogenesis of sporadic rectal cancer patients with MSI-H.

The reaction of 4-(3-aminopyrimido[4,5-d]thieno-2-yl)quinoline-2-ones, which could be obtained from Thorpe-Ziegler isomerization of 4-bromomethylquinoline-2-ones and 5-cyano-1,6-dihydro-4-methyl-2-phenyl-6-thioxopyrimidine,with aromatic aldehydes in the presence of BF3·OEt2 gives pyrimidothieno[1,6]naphthyridines in good yields.

文摘In this work,the chitosan and N-doped graphene natively grown on hierarchical porous carbon(N-PC-G/CS) nanocomposite was obtained by ultrasonic method,as a novel sensor platform for determination of tartrazine(TT).The nanocomposite as prepared had well dispersivity in water and excellent conductivity.The N-PC-G/CS nanocomposite was characterized by scanning electron microscopy(SEM),transmission electron microscopy(TEM),nitrogen adsorption-desorption,fourier transform infrared(FTIR) and electrochemical impedance spectroscopy(EIS).The application of N-PC-G/CS for determination of tartrazine(TT) was investigated by chronocoulometry(CC),cyclic voltammetry(CV) and differential pulse voltammetry(DPV).Under optimized conditions,the sensor displayed a sensitive response to TT within a wide concentration range of 0.05-15.0 μmol/L,the detection limits is 0.036 μmol/L(S/N = 3).Furthermore,this nanocomposite could be efficiently applied for determination of TT in soft drink samples.

文摘We prove some sharp Hardy inequality associated with the gradient △γ=(△x,|x|γ [x△y) by a direct and simple approach. Moreover, similar method is applied to ob- tain some weighted sharp Rellich inequality related to the Grushin operator in the setting of L^p. We also get some weighted Hardy and Rellich type inequalities related to a class of Greiner type operators.
