The Study of Nurse’s Work Alienation and Its Influence on Turnover Intention

Background: Nurse turnover is a significant cause of human resource shortage, which not only increases the workload of existing nurses, reduces job satisfaction, but also affects the quality of care. Therefore, maintaining the stability of nursing human resources becomes a great concern. Objective: To understand the current status of nurse work alienation and turnover intention and its correlationship. Methods: By convenience sampling, from May to August 2021, 1112 cases were selected and investigated from three hospitals in Wuhan by using general demographic scale, the work alienation scale and turnover intention scale. Results: The average score of nurse’s work alienation was (30.32 ± 9.84);turnover intention score was (14.83 ± 3.99). The dimensions of work alienation were positively correlated with turnover intention (P Conclusion: Nurse’s work alienation and turnover intention are in the moderate level. It is recommended that manager should focus on the nurse’s mental environments, and take active and effective measures to reduce the level of nurse’s work alienation, improving the job satisfaction of nurse and the quality of nursing work. 展开更多

Investigation and Analysis of Catheter-Associated Skin Impairment in PICC’s Patients in Jingzhou City 被引量:1

Objective: To understand the status quo of PICC Catheter-associated Skin Impairment (CASI) in some hospitals at all levels in Jingzhou city, and to explore the risk factors for PICC Catheter-associated Skin Impairment. Methods: From June 2019 to July 2020, a self-made questionnaire for CASI status was used to facilitate the extraction of CASI patients in PICC in some hospitals at all levels in Jingzhou city. Local skin lesions were collected and classified during PICC maintenance, and the damage types, occurrence frequency and related influencing factors of CASI were counted to identify independent risk factors. Results: In this study, there were 173 cases of CASI in PICC’s patients including local infection 56.1% (97/173), exudation at the puncture site 16.8% (29/173), irritant dermatitis 21.4% (37/173), allergic dermatitis 4.6% (8/173), and tension injury 1.2% (2/173). After univariate analysis, there were statistically significant differences between CASI occurrence type and duration of catheter wear, age, underlying disease, skin moisture and hyperhidrosis, PICC maintenance as required, hospital grade maintenance and intermittent chemotherapy (P < 0.05). Then, disordered multiple Logistic regression analysis was performed, and exudation at the puncture site was used as the reference group. The results showed that patients with PICC were more likely to have CASI if they were aged, long catheter time, had diabetes, had moist and sweaty skin, and were in the intermittent stage of chemotherapy. Conclusion: The incidence of CASI is high in patients aged 70 - 79 years, and local infection is the most common. PICC’s patients with diabetes are at high risk of CASI. PICC’s patients with excessive sweating are more likely to have CASI. The intermittent stage of chemotherapy is the excessive CASI;time with tube affects CASI occurrence. 展开更多

The Status Quo and Influencing Factors of the Moral Distress in Nurses in Tertiary Grade A Hospitals in Wuhan

Background: As medical technology has advanced, it has also made it possible to maintain end-stage life support for longer periods of time, but it has also been accompanied by a debate about ineffective care, nursing is considered to be an ethically important profession, and nurses aim to achieve ethical goals such as providing the best possible care to patients, achieving high quality outcomes, but it is common when there are insufficient numbers of staff, inadequately trained staff, and organizational policies and procedures that make it difficult, or even impossible, for nurses to meet the needs of patients and their families. This conflict results in moral distress for nurses, yet limited attention has been paid to this phenomenon. Objective: To explore the current phenomenon of moral distress and its triggering factors in nurses in tertiary grade A hospitals in Wuhan, by targeting root causes and understanding the interplay between nurses and settings where moral distress occurs, interventions can be tailored to minimize moral distress with the ultimate goal of enhancing patient care. Method: Totally 384 nurses from clinical departments in 2 tertiary Grade A hospitals in Wuhan were investigated with the Chinese version Moral Distress Scale-Revised (MDS-R). Result: The total score of moral distress was 47.41 ± 27.14, and the mean scores of moral distress frequency and intensity were 1.01 ± 0.53 and 1.19 ± 0.61, which were at a lower level. The main source of moral distress for nurses is related to end-of-life care and medical decision communication;Nurses’ moral distress scores were statistically significant (P Conclusion: Hospital facility leaders and nursing managers need to train nurses to develop competency development such as reflection, empathy, communication, positive thinking, and emotional intelligence to practice ethical dilemma response, and facilitate collaborative communication among healthcare members, so as to alleviate moral distress in nurses. 展开更多
