Most patients have personal and/or family history of atopic diseases. Several criteria have been proposed for the diagnosis of AD. Although the clinical features of childhood AD have been widely studied, there has been less large-scale study on adult/adolescent AD. The aim of this study was to investigate the clinical features of adult/adolescent patients with chronic symmetrical eczemaJAD and to propose Chinese diagnostic criteria for adult/adolescent AD. Methods: A hospital-based study was performed. Forty-two dermatological centers participated in this study. Adult and adolescent patients (12 years and over) with chronic symmetrical eczema or AD were included in this study. Questionnaires were completed by both patients and-investigators. The valid questionnaires were analyzed using EpiData 3.1 and SPSS 17.0 software. Results: A total of 2662 valid questionnaires were collected (1369 male and 1293 female). Of all 2662 patients, 2062 (77.5%) patients had the disease after 12 years old, while only 600 (22.5%) patients had the disease before 12 years old, suggesting late-onset eczema/AD is common. Two thousand one hundred and thirty-nine (80.4%) patients had the disease for more than 6 months. One thousand one hundred and forty-four (43.0%) patients had a personal and/or family history of atopic diseases. One thousand five hundred and forty-eight (58.2%) patients had an elevated total serum IgE and/or eosinophilia and/or positive allergen-specific IgE. Based on these clinical and laboratory features, we proposed Chinese criteria for adult/adolescent AD. Of all 2662 patients, 60.3% were satisfied with our criteria, while only 48.2% satisfied with Hanifin Rajka criteria and 32.7% satisfied with Williams criteria, suggesting a good sensitivity of our criteria in adult/adolescent AD patients. METHODS: Fourty adult male Wistar rats were randomly divided into two groups, the model group (20) was administered alcohol intragastrically plus an Oliver oil diet to establish an ALD model, and the control group (20) was given an equal amount of normal saline. The ultramicrostructural changes of mitochondria were observed under electron microscopy. Mitochondria of liver was extracted, and patency of PTP, mitochondrial membrane potential (ΔΨm), mitochondrial mass and intracellular calcium concentration of isolated hepacytes were detected by flow cytometry using rhodamine123 (Rh123), Nonyl-Acridine Orange and calcium fluorescent probe Fluo-3/AM, respectively. RESULTS: Membrane and cristae were broken or disappeared in mitochondria in different shapes under electron microscopy. Some mitochondria showed U shape or megamitochondrion. In the model group, liver mitochondria PTP was broken, and mitochondria swelled, the absorbance at 450 nm, A540 decreased (0.0136 ± 0.0025 vs 0.0321 ± 0.0013, model vs control, P < 0.01); mitochondria transmembrane potential (239.4638 ± 12.7263 vs 377.5850 ± 16.8119, P < 0.01) was lowered; mitochondrial mass (17.4350 ± 1.9880 vs 31.6738 ± 3.4930, P < 0.01); and [Ca2+]i was increased in liver cells (7.0020 ± 0.5008 vs 10.2050 ± 0.4701, P < 0.01).CONCLUSION: Chronic alcohol intake might lead to broken mitochondria PTP, decreased mitochondria membrane potential and injury, and elevated intracellular Ca2+ production. In this study, we defined that the basic soil productivity (BSP) is the production capacity of a farmland soil with its own physical and chemical properties for a specific crop season under local environment and field management. Based on 22-yr (1990-2011) long-term experimental data on black soil (Typic hapludoll) in Gongzhuling, Jilin Province, Northeast China, the decision support system for an agro-technology transfer (DSSAT)-CERES-Maize model was applied to simulate the yield by BSP of spring maize (Zea mays L.) to examine the effects of long-term fertilization on changes of BSP and explore the mechanisms of BSP increasing. Five treatments were examined: (1) no-fertilization control (control); (2) chemical nitrogen, phosphorus, and potassium (NPK); (3) NPK plus farmyard manure (NPKM); (4) 1.5 time of NPKM (1.5NPKM) and (5) NPK plus straw (NPKS). Results showed that after 22-yr fertilization, the yield by BSP of spring maize significantly increased 78.0, 101.2, and 69.4% under the NPKM, 1.5NPKM and NPKS, respectively, compared to the initial value (in 1992), but not significant under NPK (26.9% increase) and the control (8.9% decrease). The contribution percentage of BSP showed a significant rising trend (P〈0.05) under 1.5NPKM. The average contribution percentage of BSP among fertilizations ranged from 74.4 to 84.7%, and ranked as 1.5NPKM〉NPKM〉NPK〉NPKS, indicating that organic manure combined with chemical fertilizers (I.5NPKM and NPKM) could more effectively increase BSP compared with the inorganic fertilizer application alone (NPK) in the black soil. This study showed that soil organic matter (SOM) was the key factor among various fertility factors that could affect BSP in the black soil, and total N, total P and/or available P also played important role in BSP increasing. Background Data on the epidemiology of hypertension in Chinese non-dialysis chronic kidney disease (CKD) patients are limited.The aim of the present study was to investigate the prevalence,awareness,treatment,and co...Background Data on the epidemiology of hypertension in Chinese non-dialysis chronic kidney disease (CKD) patients are limited.The aim of the present study was to investigate the prevalence,awareness,treatment,and control of hypertension in the non-dialysis CKD patients through a nationwide,multicenter study in China.Methods The survey was performed in 61 tertiary hospitals in 31 provinces,municipalities,and autonomous regions in China (except Hong Kong,Macao,and Taiwan).Trained physicians collected demographic and clinical data and measured blood pressure (BP) using a standardized protocol.Hypertension was defned as systolic BP ≥140 mmHg and/or diastolic BP ≥90 mmHg,and/or use of antihypertensive medications.BP 〈140/90 mmHg and 〈130/80 mmHg were used as the 2 thresholds of hypertension control.In multivariate logistic regression with adjustment for sex and age,we analyzed the association between CKD stages and uncontrolled hypertension in non-dialysis CKD patients.Results The analysis included 8927 non-dialysis CKD patients.The prevalence,awareness,and treatment of hypertension in non-dialysis CKD patients were 67.3%,85.8%,and 81.0%,respectively.Of hypertensive CKD patients,33.1% and 14.1% had controlled BP to 〈140/90 mmHg and 〈130/80 mmHg,respectively.With successive CKD stages,the prevalence of hypertension in non-dialysis CKD patients increased,but the control of hypertension decreased (P〈0.001).When the threshold of BP 〈130/80 mmHg was considered,the risk of uncontrolled hypertension in CKD 2,3a,3b,4,and 5 stages increased 1.3,1.4,1.4,2.5,and 4.0 times compared with CKD 1 stage,respectively (P〈0.05).Using the threshold of 〈140/90 mmHg,the risk of uncontrolled hypertension increased in advanced stages (P〈0.05).Conclusions The prevalence of hypertension Chinese non-dialysis CKD patients was high,and the hypertension control was suboptimal.With successive CKD stages,the risk of uncontrolled hypertension increased.展开更多

However, it is often difficult to accurately diagnose pediatric tuberculosis (TB). The tuberculin test (TST) may have a low specificity because of cross-reactivity with antigens present in Mycobacterium bovis bacillus Calmette-Guerin (BCG) and other mycobacteria, especially in China with a predominantly BCG-vaccinated population.Early-secreted antigenic target 6-kDa protein (ESAT-6) and culture filtrate protein 10 (CFP-10), stand out as suitable antigens that induce an interferon-gamma (IFN-γ) secreting, T-cell-mediated immune response to infection. While,considered the higher costs and complexity of the IFN-γ release assay (TSPOT), we aimed to evaluate the TSPOT and TST test in the clinical diagnosis of pediatric tuberculosis and to establish a diagnostic process suitable for China.Methods The sensitivity and specificity of the assay were evaluated in total seventy four children with active tuberculosis and fifty one nontuberculous children with other disease, and then the results were compared with TST.Logistic regression models were used to identify variables that were associated with positive results for each assay. The independent variables included sex, age, birth place, vaccination history, close contract with an active TB patient.Results The sensitivity of TSPOT was higher than TST in active TB children with or without BCG vaccination, as well as in children with culture-confirmed TB. But the difference was not significant statistically. Combining results of the TSPOT and TST improved the sensitivity to 94.6%. Agreement of the TST and TSPOT was low (77.0%, k=0.203) in active TB patients. The difference in specificity between TSPOT and TST test was statistically significant (94.1% vs.70.6%, P=0.006). The recurrence of colorectal adenoma(CRA)is high.Although there are guidelines for colonoscopy surveillance after polypectomy in other countries,little is known about its recurrence rate and recurrence peak,especially...The recurrence of colorectal adenoma(CRA)is high.Although there are guidelines for colonoscopy surveillance after polypectomy in other countries,little is known about its recurrence rate and recurrence peak,especially in China.The aim of the present research is to investigate how long after polypectomy follow-up should take and to analyze risk factors of recurrence.1208 patients who received polypectomies fromfive clinical research centers in four regions of China(Shanghai,Guangzhou,Nanjing and Beijing)were included.They were divided into 4 groups:group A(follow-up≤1 year after poly-pectomy),group B(follow-up 2–3 years after polypect-omy),group C(follow-up 4–5 years after polypectomy),and group D(follow-up>5 years after polypectomy).The sex,age,adenoma location,size,number,and pathological characteristics were compared.On the whole,the recur-rence rate was 59.46%in group A,61.09%in group B,78.07%in group C,and 87.12%in group D,which indicated an increased tendency with a prolonged follow-up duration.There was a significant difference between group A and C or D,and between group B and C or D(P<0.01),but there was no statistical difference between group A and B.Additionally,the recurrent patients in thefirst year had a recurrence rate of 97.33%in thefirst three years(59.46/61.09),which means that the peak of recurrence was almost entirely concentrated in thefirst year.The recurrence rate was higher in males and the elder.The risk factors included multiple numbers,villous feature,high-grade dysplasia of medium or smaller size and location in the distal colon.In conclusion,the peak of recurrence was almost totally concentrated in thefirst year;meanwhile,thefirst year follow-up is of critical importance in China.It may not be necessary to do the follow-up examination during the second and third years,but after three years,another colonoscopy should be undertaken.展开更多
