METHODS: Peripheral blood mononuclear cell (PBMC) from responders and non-responders were stimulated with or with out recombinant HBsAg or PHA. Broad spectrum of cytokines viz (Th1) IFN-γ, IL-2, TNF-α, IL-12 and (Th2) IL-10, IL-4 were measured after in vitro stimulation with recombinant HBsAg and were compared with respective antibody titers. RESULTS: A significant decrease (P = 0.001) in Th1 and Th2 cytokines namely, IL-2, INF-γ, TNF-α and IL-10in non-responders was observed. The level of IL-4 was not significant between the three groups. Furthermore, despite a strong Th1 and Th2 cytokine response, the level of IL-12 was elevated in high-responders compared to other groups (P = 0.001) and demonstrated a positive correlation with anti-HBs titers and Th1 cytokine response. CONCLUSION: Our findings suggest that unrespon-siveness to recombinant hepatitis B vaccines (rHB) is multifactorial, including specific failure of antigen presentation or the lack of both T helper Th1 and Th2 response.展开更多

Objective: Specific objectives include evaluating the mechanical properties and structural behaviour of steel and GFRP-reinforced one-way slabs and comparing experimental and theoretical predictions. Methods: Four different mix proportions were arrived at, comprising both conventional concrete and Alccofine-based concrete. In each formulation, a combination of normal river sand and M-sand was utilized. Results: Concrete with Alccofine exhibits superior mechanical properties, while M-sand incorporation minimally affects strength but reduces reliance on natural sand. GFRP-reinforced slabs display distinct brittle behaviour with significant deflections post-cracking, contrasting steel-reinforced slabs’ gradual, ductile failure. Discrepancies between experimental data and design recommendations underscore the need for guideline refinement. Conclusion: Alccofine and M-sand enhance concrete properties, but reinforcement type significantly influences slab behaviour. The present work pertains to the study of microstructure and mechanical properties of explosively cladded commercially pure titanium(cpTi) with austenitic stainless steel(SS 304L) subjected to varied process parameter...The present work pertains to the study of microstructure and mechanical properties of explosively cladded commercially pure titanium(cpTi) with austenitic stainless steel(SS 304L) subjected to varied process parameters, viz., loading ratios(mass of explosive or mass of flyer plate) and preset angles. The microstructural study reveals the transformation from straight to wavy interface while increasing loading ratios. Vickers hardness increases with loading ratios, and the maximum hardness is witnessed in the closer proximity of collision interface. Ram tensile and shear strength of explosive claddings are higher than that of parent plates. While the base plate fails in impact test, the flyer plate is deformed, indicating good impact strength. Increased mass of claddings, due to oxide formation, is witnessed in corrosion tests, which confirms the superiority of explosive claddings in corrosive environment. Triaxial weldability window, an analytical estimation for Ti-SS 304L explosive claddings, is developed and correlated.展开更多

The prevalence of resistant hypertension is often under estimated due to various reaso...Resistant hypertension is associated with chronic activation of the sympathetic nervous system resulting in various comorbidities. The prevalence of resistant hypertension is often under estimated due to various reasons. Activation of sympathetic nervous system at the renal-as well as systemic-level contributes to the increased level of catecholamines and resulting increase in the blood pressure. This increased activity was demonstrated by increased muscle sympathetic nerve activity and renal and total body noradrenaline spillover. Apart from the hypertension, it is hypothesized to be associated with insulin resistance, congestive heart failure and obstructive sleep apnea. Renal denervation is a novel procedure where the sympathetic afferent and efferent activity is reduced by various techniques and has been used successfully to treat drug-resistant hypertension improvement of various metabolic derangements.Renal denervation has the unique advantage of offering the denervation at the renal level, thus mitigating the systemic side effects. Renal denervation can be done by various techniques including radiofrequency ablation, ultrasound guided ablation and chemical ablation. Various trials evaluated the role of renal denervation in the management of resistant hypertension and have found promising results. More studies are underway to evaluate the role of renal denervation in patients presenting with resistant hypertension in different scenarios. Appropriate patient selection might be the key in determining the effectiveness of the procedure.展开更多
In the present study,anti-radical,anti-diabetic and antimicrobial activities of different solvent extracts of Passiflora ligularis fruits were investigated.Among the various solvents,acetone extract displayed maximum ...In the present study,anti-radical,anti-diabetic and antimicrobial activities of different solvent extracts of Passiflora ligularis fruits were investigated.Among the various solvents,acetone extract displayed maximum total phenolic(640.70 mg GAE/g extract),tannin(214.30 mg GAE/g extract)and flavonoid contents(387.33 mg RE/g extract).Results of antioxidant studies revealed that the acetone extract of fruits possessed an efficient 2,2-diphenyl-1-picryl-hydrazyl(DPPH•)(IC5019.13g/mL),2,2-azinobis(3-ethylbenzothiozoline-6-sulfonic acid)(ABTS•+)(9800.94mol/L trolox equi/g extract),superoxide(78.27%)and nitric oxide(79.95%)radical scavenging activities,ferric reducing antioxidant power(43.06 mmol Fe(II)/mg extract),metal chelating(134.53 mg EDTA/g extract)ability.The acetone extract of P.ligularis fruits also exhibited significant(P<0.005)inhibition activities onα-amylase(82.56%)andα-glucosidase(75.36%)enzymes.Furthermore,the fruits of P.ligularis also demonstrated antibacterial activities against Gram(+)and Gram(−)bacteria and inhibited the fungal strains Candida albicans(14.85 mm)and Aspergillus niger(13.91 mm)in the disc diffusion method.Quantification of polyphenolics by HPLC showed the presence of ellagic acid,gallic acid,and rutin.Therefore,the results indicate that the fruit pulp of P.ligularis can serve as a potential antioxidative and antimicrobial agent in food and pharmaceutical industries.展开更多

The importance of UC is increasing with the constantly varying demands. There- fore, there is an urgent need in the power sector to keep track of the latest methodologies to further optimize the working criterions of the generating units. This paper focuses on providing a clear review of the latest techniques employed in optimizing UC problems for both stochastic and deterministic loads, which has been acquired from many peer reviewed published papers. It has been divided into many sections which include various constraints based on profit, security, emission and time. BACKGROUND Hepatosplenic T-cell lymphoma(HSTCL)is a rare and aggressive peripheral T-cell lymphoma with historically dismal outcomes,representing less than one percent of non-Hodgkin lymphomas.Given its rarity,the tru...BACKGROUND Hepatosplenic T-cell lymphoma(HSTCL)is a rare and aggressive peripheral T-cell lymphoma with historically dismal outcomes,representing less than one percent of non-Hodgkin lymphomas.Given its rarity,the true incidence of HSTCL is unknown and most data have been extrapolated through case reports.To the best of our knowledge,the largest and most up to date study addressing the epidemiology and outcomes of patients with HSTCL in the United States covered a period from 1996 to 2014,with a sample size of 122 patients.AIM To paint the most updated epidemiological picture of HSTCL.METHODS A total of 186 patients diagnosed with HSTCL,between 2000 and 2017,were ultimately enrolled in our study by retrieving data from the Surveillance,Epidemiology,and End Results database.We analyzed demographics,clinical characteristics,and overall mortality(OM)as well as cancer-specific mortality(CSM)of HSTCL.Variables with a P value<0.01 in the univariate Cox regression were incorporated into the multivariate Cox model to determine the independent prognostic factors,with a hazard ratio of greater than 1 representing adverse prog-nostic factors.CONCLUSION Overall,the outlook for this rare malignancy is very grim.In this retrospective cohort study of the United States population,non-Hispanic blacks and the elderly had a higher CSM.This data highlights the need for larger prospective studies to investigate factors associated with worse prognosis in one ethnic group,such as treatment delays,which have been shown to increase mortality in this racial/ethnic group for other cancers.展开更多

METHODS: Five hundred consecutive HIV infected patients w...AIM: To screen for the co-infection of hepatitis B (HBV) and hepatitis C virus (HCV) in human immunodeficiency virus (HIV) infected patients in southern India. METHODS: Five hundred consecutive HIV infected patients were screened for Hepatitis B Virus (HBsAg and HBV-DNA) and Hepatitis C virus (anti-HCV and HCV-RNA) using commercially available ELISA kits; HBsAg, HBeAg/ anti-HBe (Biorad laboratories, USA) and anti-HCV (Murex Diagnostics, UK). The HBV-DNA PCR was performed to detect the surface antigen region (pre S-S). HCV-RNA was detected by RT-PCR for the detection of the constant 5' putative non-coding region of HCV. RESULTS: HBV co-infection was detected in 45/500 (9%) patients and HCV co-infection in 11/500 (2.2%) subjects. Among the 45 co-infected patients only 40 patients could be studied, where the detection rates of HBe was 55% (22/40), antiHBe was 45% (18/40) and HBV-DNA was 56% (23/40). Among 11 HCV co-infected subjects, 6 (54.5%) were anti-HCV and HCV RNA positive, while 3 (27.2%) were positive for anti-HCV alone and 2 (18%) were positive for HCV RNA alone. CONCLUSION: Since the principal routes for HIV transmission are similar to that followed by the hepatotropic viruses, as a consequence, infections with HBV and HCV are expected in HIV infected patients. Therefore, it would be advisable to screen for these viruses in all the HIV infected individuals and their sexual partners at the earliest.展开更多

The current large-scale Internet of Things(IoT)networks typically generate high-velocity network traffic streams.Attackers use IoT devices to create botnets and launch attacks,such as DDoS,Spamming,Cryptocurrency mini...The current large-scale Internet of Things(IoT)networks typically generate high-velocity network traffic streams.Attackers use IoT devices to create botnets and launch attacks,such as DDoS,Spamming,Cryptocurrency mining,Phishing,etc.The service providers of large-scale IoT networks need to set up a data pipeline to collect the vast network traffic data from the IoT devices,store it,analyze it,and report the malicious IoT devices and types of attacks.Further,the attacks originating from IoT devices are dynamic,as attackers launch one kind of attack at one time and another kind of attack at another time.The number of attacks and benign instances also vary from time to time.This phenomenon of change in attack patterns is called concept drift.Hence,the attack detection system must learn continuously from the ever-changing real-time attack patterns in large-scale IoT network traffic.To meet this requirement,in this work,we propose a data pipeline with Apache Kafka,Apache Spark structured streaming,and MongoDB that can adapt to the ever-changing attack patterns in real time and classify attacks in large-scale IoT networks.When concept drift is detected,the proposed system retrains the classifier with the instances that cause the drift and a representative subsample instances from the previous training of the model.The proposed approach is evaluated with the latest dataset,IoT23,which consists of benign and several attack instances from various IoT devices.Attack classification accuracy is improved from 97.8%to 99.46%by the proposed system.The training time of distributed random forest algorithm is also studied by varying the number of cores in Apache Spark environment.展开更多

文摘In the present study,anti-radical,anti-diabetic and antimicrobial activities of different solvent extracts of Passiflora ligularis fruits were investigated.Among the various solvents,acetone extract displayed maximum total phenolic(640.70 mg GAE/g extract),tannin(214.30 mg GAE/g extract)and flavonoid contents(387.33 mg RE/g extract).Results of antioxidant studies revealed that the acetone extract of fruits possessed an efficient 2,2-diphenyl-1-picryl-hydrazyl(DPPH•)(IC5019.13g/mL),2,2-azinobis(3-ethylbenzothiozoline-6-sulfonic acid)(ABTS•+)(9800.94mol/L trolox equi/g extract),superoxide(78.27%)and nitric oxide(79.95%)radical scavenging activities,ferric reducing antioxidant power(43.06 mmol Fe(II)/mg extract),metal chelating(134.53 mg EDTA/g extract)ability.The acetone extract of P.ligularis fruits also exhibited significant(P<0.005)inhibition activities onα-amylase(82.56%)andα-glucosidase(75.36%)enzymes.Furthermore,the fruits of P.ligularis also demonstrated antibacterial activities against Gram(+)and Gram(−)bacteria and inhibited the fungal strains Candida albicans(14.85 mm)and Aspergillus niger(13.91 mm)in the disc diffusion method.Quantification of polyphenolics by HPLC showed the presence of ellagic acid,gallic acid,and rutin.Therefore,the results indicate that the fruit pulp of P.ligularis can serve as a potential antioxidative and antimicrobial agent in food and pharmaceutical industries.
文摘Unit commitment (UC) is an optimization problem used to determine the operation schedule of the generating units at every hour interval with varying loads under different constraints and environments. Many algorithms have been invented in the past five decades for optimization of the UC problem, but still researchers are working in this field to find new hybrid algorithms to make the problem more realistic. The importance of UC is increasing with the constantly varying demands. There- fore, there is an urgent need in the power sector to keep track of the latest methodologies to further optimize the working criterions of the generating units. This paper focuses on providing a clear review of the latest techniques employed in optimizing UC problems for both stochastic and deterministic loads, which has been acquired from many peer reviewed published papers. It has been divided into many sections which include various constraints based on profit, security, emission and time. It emphasizes not only on deregulated and regulated environments but also on renewable energy and distributed generating systems. In terms of contributions, the detailed analysis of all the UC algorithms has been discussed for the benefit of new researchers interested in working in this field.

文摘Objective:To investigate the analgesic and anti-inflammatory activities of ethanol extract of Passiflora foetida(P.foetida) leaves.Methods:Ethanol extract of P.foetida leaf was evaluated for analgesic action by acetic acid-induced writhing and hot plate method in albino mice.The anti-inflammatory property of ethanolic leaf extract was tested by carrageenan induced acute paw edema and histamine induced acute paw edema in rats.Results:The dose 200 mg/kg of P. foetida leaf extract exhibited highest significant analgesic activity[(13.50±0.43) min]at a reaction time of 20 min in hot plate method in mice.The ethanol extract of leaf dose 100 mg/kg produced a highly significant anti inflammatory effect[(1.302±0.079) mL]in rats.Conclusions:It is very clear that P.foetida also has analgesic and anti-inflammatory activities for the pharmaceuticals.
文摘BACKGROUND Hepatosplenic T-cell lymphoma(HSTCL)is a rare and aggressive peripheral T-cell lymphoma with historically dismal outcomes,representing less than one percent of non-Hodgkin lymphomas.Given its rarity,the true incidence of HSTCL is unknown and most data have been extrapolated through case reports.To the best of our knowledge,the largest and most up to date study addressing the epidemiology and outcomes of patients with HSTCL in the United States covered a period from 1996 to 2014,with a sample size of 122 patients.AIM To paint the most updated epidemiological picture of HSTCL.METHODS A total of 186 patients diagnosed with HSTCL,between 2000 and 2017,were ultimately enrolled in our study by retrieving data from the Surveillance,Epidemiology,and End Results database.We analyzed demographics,clinical characteristics,and overall mortality(OM)as well as cancer-specific mortality(CSM)of HSTCL.Variables with a P value<0.01 in the univariate Cox regression were incorporated into the multivariate Cox model to determine the independent prognostic factors,with a hazard ratio of greater than 1 representing adverse prog-nostic factors.CONCLUSION Overall,the outlook for this rare malignancy is very grim.In this retrospective cohort study of the United States population,non-Hispanic blacks and the elderly had a higher CSM.This data highlights the need for larger prospective studies to investigate factors associated with worse prognosis in one ethnic group,such as treatment delays,which have been shown to increase mortality in this racial/ethnic group for other cancers.
文摘Tenosynovitis represents a common clinical condition characterized by inflam-mation of the synovium that encases the tendon sheath.Although tenosynovities may be noted in any tendon in the body,extremities such as hand,and foot remain the sites of high predilection to acquire this condition.The predominant cause of this predilection rests in the intricate tendon arrangements in these extremities that permit fine motor actions.This editorial explores the common causes and the complications associated with this condition to improve the understanding of the readers of this common condition encountered in our everyday clinical practice.

文摘The extensive research and development in perovskite solar cells (PSCs) have rekindled the hopes of converting solar energy into electricity.An elusive understanding of underlying mechanisms is required for the development of efficient PSCs.Over the years,Impedance Spectroscopy (IS) characterization,along with complementary techniques,has proven to be an effective way to understand and analyze the charge transport and recombination at interface and bulk of PSCs.The IS of PSCs have been analyzed,interpreted,and improvised continuously,revealing intricate details about the work.However,there is a lack of centralized source of these details,which make it tougher to account for the generalized approach to understand the device properties.The present work is focused on compiling the research done on various PSC device architectures via IS to construct a comprehensive foundation of information on impedance plots,equivalent circuits,and associated processes.
文摘AIM: To screen for the co-infection of hepatitis B (HBV) and hepatitis C virus (HCV) in human immunodeficiency virus (HIV) infected patients in southern India. METHODS: Five hundred consecutive HIV infected patients were screened for Hepatitis B Virus (HBsAg and HBV-DNA) and Hepatitis C virus (anti-HCV and HCV-RNA) using commercially available ELISA kits; HBsAg, HBeAg/ anti-HBe (Biorad laboratories, USA) and anti-HCV (Murex Diagnostics, UK). The HBV-DNA PCR was performed to detect the surface antigen region (pre S-S). HCV-RNA was detected by RT-PCR for the detection of the constant 5' putative non-coding region of HCV. RESULTS: HBV co-infection was detected in 45/500 (9%) patients and HCV co-infection in 11/500 (2.2%) subjects. Among the 45 co-infected patients only 40 patients could be studied, where the detection rates of HBe was 55% (22/40), antiHBe was 45% (18/40) and HBV-DNA was 56% (23/40). Among 11 HCV co-infected subjects, 6 (54.5%) were anti-HCV and HCV RNA positive, while 3 (27.2%) were positive for anti-HCV alone and 2 (18%) were positive for HCV RNA alone. CONCLUSION: Since the principal routes for HIV transmission are similar to that followed by the hepatotropic viruses, as a consequence, infections with HBV and HCV are expected in HIV infected patients. Therefore, it would be advisable to screen for these viruses in all the HIV infected individuals and their sexual partners at the earliest.

文摘The current large-scale Internet of Things(IoT)networks typically generate high-velocity network traffic streams.Attackers use IoT devices to create botnets and launch attacks,such as DDoS,Spamming,Cryptocurrency mining,Phishing,etc.The service providers of large-scale IoT networks need to set up a data pipeline to collect the vast network traffic data from the IoT devices,store it,analyze it,and report the malicious IoT devices and types of attacks.Further,the attacks originating from IoT devices are dynamic,as attackers launch one kind of attack at one time and another kind of attack at another time.The number of attacks and benign instances also vary from time to time.This phenomenon of change in attack patterns is called concept drift.Hence,the attack detection system must learn continuously from the ever-changing real-time attack patterns in large-scale IoT network traffic.To meet this requirement,in this work,we propose a data pipeline with Apache Kafka,Apache Spark structured streaming,and MongoDB that can adapt to the ever-changing attack patterns in real time and classify attacks in large-scale IoT networks.When concept drift is detected,the proposed system retrains the classifier with the instances that cause the drift and a representative subsample instances from the previous training of the model.The proposed approach is evaluated with the latest dataset,IoT23,which consists of benign and several attack instances from various IoT devices.Attack classification accuracy is improved from 97.8%to 99.46%by the proposed system.The training time of distributed random forest algorithm is also studied by varying the number of cores in Apache Spark environment.
文摘Clavicle fractures are among the most prevalent types of fractures with numerous treatment strategies that have evolved over time.In the realm of lateral-third clavicle fracture management,several surgical methods are available,with plate and screw constructs being one of the most frequently employed options.Within this construct,numerous choices exist for fixing the fracture.This editorial provides an overview of the common plate options utilized in the management of distal third clavicle fractures underscoring the critical considerations and approaches that guide clinicians in selecting the most appropriate fixation techniques,considering the complex landscape of clavicle fractures and their challenging management.
文摘Introduction:Awake fiberoptic bronchoscopy has long been considered the criterion standard for the management of difficult airways because of large thyroid malignancies.After an unsuccessful attempt to intubate with a fiberoptic bronchoscope in the supine position,the decision to switch to the left lateral position was made.We present this case to propose the utilization of the lateral position for awake intubation.Case presentation:We report a rare case of a patient with a large papillary carcinoma of the thyroid planned for surgical excision.The patient presented to our hospital with complaints of difficulty breathing while lying down in the supine position and oozing blood from the necrotic tissue.The breathing difficulties subsided in the lateral position.Conclusion:We demonstrated how the left lateral position with an adjunct maneuver facilitates intubation in large thyroid malignancies.
文摘The Unified Power Quality Conditioner (UPQC) plays an important role in the constrained delivery of electrical power from the source to an isolated pool of load or from a source to the grid. The proposed system can compensate voltage sag/swell, reactive power compensation and harmonics in the linear and nonlinear loads. In this work, the off line drained data from conventional fuzzy logic controller. A novel control system with a Combined Neural Network (CNN) is used instead of the traditionally four fuzzy logic controllers. The performance of combined neural network controller compared with Proportional Integral (PI) controller and Fuzzy Logic Controller (FLC). The system performance is also verified experimentally.
