Patient-specific template (PST) is a technique that uses computer technology for the planning, deigning and production of cutting guides. This study aims to compare PST to conventional technique in terms of mechanical axis alignment and component positioning. Patients and method: 109 TKA were performed for 78 patients in 2 groups. Group A included 69 conventional TKA in 55 patients and Group B included 40 patient-specific TKA in 23 patient. Postoperative long-film X-rays were done for all patients to observe the mechanical axis, anatomical axis, lateral distal femoral mechanical angle and medial proximal tibial angle. Results: No statistically significant difference was found between the two groups regarding alignment or component positioning. Conclusion: Both techniques have shown similar results in restoring the mechanical axis and alignment after TKA. However, PST had the advantages of reduced blood loss and shorter operative time.展开更多

The adsorption properties of NO on homonuclear, Pd2, Rh2, and heteronuclear transition metal dimers, PdRh, that deposited on MgO(001) surface have been studied by means of hybrid density functional theory calculations and embedded cluster model. The most stable NO chemisorption geometry is in a bridge position on Pd2 and a top configuration of Rh2 and PdRh with N-down oriented. NO prefers binding to Rh site when both Rh and Pd atoms co-exist in the PdRh. The natural bond orbital analysis (NBO) reveals that the electronic structure of the adsorbed metal represents a qualitative change with respect to that of the free metal. The adsorption properties of NO have been analyzed with reference to the NBO, charge transfer, band gaps, pairwise and non-pairwise additivity. The binding of NO precursor is dominated by the E(i)Mx-NO pairwise additive components and the role of the support was not restricted to supporting the metal. The adsorbed dimers on the MgO surface lose most of the metal-metal interaction due to the relatively strong bond with the substrate. Spin polarized calculations were performed and the results concern the systems in their more stable spin states. Spin quenching occurs for Rh atom, Pd2, Rh2 and PdRh complexes at the terrace and defective surfaces. The adsorption energies of the low spin states of spin quenched complexes are always greater than those of the high spin states. The metal-support and dimer-support interactions stabilize the low spin states of the adsorbed metals with respect to the isolated metals and dimers. Salvage procedure as hemiarthroplasty or total shoulder arthroplasty used in huge non-constructable defect or very old neglected dislocation. In our case series, we treated 9 cases (2 females) of locked posterior shoulder dislocation with antero-medial humeral head defects ranging between 30% - 50% of head size. Open reduction of dislocation followed by transfer of the lesser tuberosity together with subscapularis tendon for reconstruction of the humeral head defect. The transfer was fixed with Ethibond suture size 5-0 (Ethicon, Inc. Somerville, New Jersy). The mean follow-up period was 14.5 months (range, 12 - 25 months). Seven cases had no pain or restriction of activities of daily living. No patient had symptoms of instability of the shoulder. According to UCLA Shoulder rating scale, there were 3 cases rated excellent, 4 cases rated good, one case rated fair and one case rated poor. It is concluded that reconstruction of the humeral head defect provides good pain relief, stability and function for patients with a locked posterior dislocation where the defect involves between 30% - 50% of the articular surface circumference. Serum levels of 25(OH) D3, calcium, phosphorus, alkaline phosphatase and plasma level of parathormone were measured. Quantitative PCR for HCV was performed Bone density was determined by dual energy X-ray absorptiometry. All cases received conventional therapy, and only 33 patients received vitamin D supplementation.RESULTS: Children with HCV showed significantly increased levels of HCV RNA(P < 0.001), parathormone(P < 0.01) and decreased vitamin D levels(P < 0.05)(33.3% deficient and 43.3% insufficient) compared with controls. Abnormal bone status(Z score-1.98 ± 0.75) was found in ribs, L-spine, pelvis and total body. Cases treated with vitamin D showed significant higher early(P < 0.04) and sustained(P < 0.05) virological response. There was a high frequency of vitamin D deficiency among the Egyptian HCV children, with significant decrease in bone density. The vitamin D level should be assessed before the start of antiviral treatment with the correction of any detected deficiency. CONCLUSION: Adding vitamin D to conventional Peg/RBV therapy significantly improved the virological response and helped to prevent the risk of emerging bone fragility.展开更多

Patients and Method: 48 patients (19 females) with monocompartmental gonarthrosis of the knee combined with varus malalignment, 41 with bilateral affection and 7 cases with unilateral gonarthritis (total 89 knees). Mean age: 53 years (range from 42 to 61 years). They were treated and followed from June 2010 to May 2014 (35 months). Biplanar opening high tibial osteotomy technique was used and fixed by low profile locked T-plate (21 cases) and low profile anatomical proximal tibial plate (27cases). Pre-operative mean varus deformity was 16.5° (9° - 19°). We utilized Lysholm score and Knee Society Score for evaluation of preoperative and follow-up data. Results: The average correction angle was 11.50 ± 2.50° (7° - 19°). Preoperative posterior tibial slope was 8.04° ± 1.30° and at last follow-up 9.15° ± 1.60°, respectively (p = 0.437). The joint space distance increased from 4.05 ± 1.30 mm to 4.83 ± 1.33 mm (p < 0.001). The average time to complete bone union was 14.69 ± 1.5 weeks. There were no cases of delayed union or non-union. No major complications like broken plate, nerve injury, or blood vessel injury occurred. No progression of degenerations developed in the three compartments of the knee at final follow-up. The mean score on the Lysholm-Gillquist knee functional scoring scale was 45.5 ± 21.7 preoperatively, and it improved to 77.0 ± 23.9 (p < 0.001). There was no obvious difference in the range of motion before and after operation. The average knee score improved from 51.19 ± 11.82 to 93.49 ± 5.10. Microscopic ...Egyptian beach ilmenite occurs in a relatively high content in the naturally highly concentrated superficial black sand deposits at specific beach zones in the northern parts of the Nile Delta at Rosetta. Microscopic study shows that the ilmenite occurs as fresh homogeneous black or heterogeneous multicoloured altered grains and exhibits three types (homogeneous, exsolved and altered) of ilmenite varieties. XRD data of ilmenite indicates their association with minor hematite and quartz, whereas leucoxene shows its association with Nb-rutUe, pseudorutile and hematite. Grain size distribution suggests a very fine sand size of 〉89% and 80% and a fine sand size of 10.5% and 18.3% for fresh and altered ilmenites, respectively. The density of fresh, altered ilmenite and leucoxene concentrates varies from 2.70, 2.50 to 2.40 ton/m^3, suggesting a gradual decrease from high grade fresh to leucoxene and consistent with variation in magnetic susceptibility as a consequence of the leaching of iron. Mass magnetic susceptibility reveals 97.6% of ilmenite and 92% of the altered form are obtained at 0.20 and 0.48 ampere. Fresh iimenite exhibits variable TiO2 (47.18%) and Fe2O3^T (46.10%) with minor MnO, MgO and Cr2O3 (1.22, 1.10 and 0.51%). The altered ilmenite is higher in TiO2 (76.16%) and SiO2 (4.68%) and lower in Fe2O3^T (14.45%), MnO, MgO and Cr2O3 (0.39, 0.52 and 0.11%) compared with the fresh form. Three concentrates of ilmenites (G1, G2 and G3) were prepared from crude ore using a Reading cross belt magnetic separator under different conditions, revealing a gradual increase of TiO2, SiO2, Al2O3 and CaO accompanied by a decrease of Fe2O3^T, MgO and Cr2O3 with repetition of the separation processes. Several ore dressing techniques were carried out to upgrade the iimenite concentrate.展开更多

These actions may be represented by sheltering, evacuation, distribution of stable iodine tablets and/or relocation. This study represents a comprehensive probabilistic study to investigate the role of the adoption of the countermeasures in case of a hypothetical accident of type LOCA for nuclear power plant of PWR (1000 Mw). The effective doses in different organs, short and long health effects, and the associated risks are calculated with and without countermeasures. In addition, the overall costs of the accident and the costs of countermeasures are estimated which represent our first trials to know how much the proposed accident cost. The results showed that, the area around the site requires early and late countermeasures action after the accident especially in the downwind sectors. For late countermeasures, the duration time of relocation ranged from about two to 10 years. The adoption of the countermeasures increases the costs of emergency plan by 40% but reduces the risk associated the accident.展开更多

However, MOWHTO tends to decrease patellar height and increase the posterior tibial slope, which can affect the knee joint stability. Objective: The aim of this study was to describe the technique of biplanar MOWHTO and fixation by TomoFix plate, as well as to evaluate the accuracy of the planned correction and the postoperative tibial slope. Patients and Methods: This prospective cohort study was conducted on patients who presented with varus knee deformity and underwent biplanar MOWHTO and fixation by TomoFix plate during the period from March 2016 to March 2017. Assessment of patients included pre- and postoperative Knee and function scores, mechanical femorotibial angle (mFTA), posterior tibial slope angle (pTSA), range of motion (ROM) and radiological evaluation of the healing of the osteotomy site. Results: The recruited patients were 13 (8 men and 5 women), with average age 31.7 years old. The knee and functional scores improved from the preoperative mean of 45 and 41 to the postoperative mean of 85 and 72 points, respectively. The average knee flexion was 115 degrees, which at the final follow-up remained unchanged except for one case. The mean preoperative mFTA was 13.5°varus and decreased post-operatively to a mean of 3°valgus. The average postoperative follow-up period was 12 months (10 - 18 M). Conclusion: The biplanar MOWHTO allows preservation of posterior tibial slope while correcting the varus knee adequately.展开更多

This paper examines methods of calculating rates of growth and thus calculating future population growth. This paper is concerned about regulators making sure that nuclear power plants are not located near densely populated centers and that population centers located near nuclear power plants do not become densely populated. In the case of El Dabaa city in the north coast of Egypt, south west of the nominated plant site, three rates of growth 1.5%, 2.5% and 3% were identified. The first is the current population growth rate of the city, the second is the average national growth rate and the third is the expected growth rate of the city when the nuclear power plant gets constructed and operated. In all three cases, this city is a population center and rules of distance from the nuclear power plant shall apply. Thus the further natural growth of this city should be controlled and directed away from the downwind of the plant.展开更多

The X-ray diffrac-tion profiles revealed that these nanoparticles were crystallized well and the volume of mullite unit cell was increased as a function of the ionic radius of dopant ion. TEM images showed regular orthorhombic crystal morphology for the pure mullite sample. Meanwhile, the doped samples exhibited slightly distorted crystal morphology of larger particle sizes. DSC thermograms evinced that the exo-thermic peak temperature of mullite was shifted to the lower value with M3+ion insertion. The photoluminescence spectra were stud-ied for mullite samples, and it was found that the intensity of the emission spectra was affected by the M3+ion type. It was found that, Y3+doped mullite achieved the minimum dielectric loss value of 0.01 in the radio wave frequency region (1 MHz). Meanwhile, Gd3+doped mullite achieved the minimum dielectric loss value of 0.09 in the microwave frequency region (1 GHz).展开更多

Also this technique can be applied for medial compartment degeneration of the knee without deformity with good functional outcom
文摘Ophiolitic peridotites exposed in the Eastern Desert(ED)of Egypt record multiple stages of evolution, including different degrees of partial melting and melt extraction, serpentinization, carbonatization and metamorphism. The present study deals with metaperidotites at two selected localities in the central and southern ED,namely Wadi El-Nabáand Wadi Ghadir, respectively. They represent residual mantle sections of a Neoproterozoic dismembered ophiolite that tectonically emplaced over a volcano-sedimentary succession that represents island–arc assemblages. The studied metaperidotites are serpentinized, with the development of talc-carbonate and quartz-carbonate rocks, especially along shear and fault planes.Fresh relics of primary minerals(olivine, orthopyroxene and Cr-spinel)are preserved in a few samples of partiallyserpentinized peridotite. Most of the Cr-spinel crystals have fresh cores followed by outer zones of ferritchromite and Crmagnetite, which indicates that melt extraction from the mantle protolith took place under oxidizing conditions. The protoliths of the studied metaperidotites were dominated by harzburgites, which is supported by the abundance of mesh and bastite textures in addition to some evidence from mineral and whole-rock chemical compositions. The high Cr#(0.62–0.69;Av.0.66)and low TiO_(2)(<0.3 wt%)contents of the fresh Cr-spinels,the higher Fo(89–92;Av.91)and NiO(0.24–0.54 wt%,Av.0.40)contents of the primary olivine relics, together with the high Mg#(0.91–0.93;Av.91)and low CaO,Al2 O3 and TiO_(2)of the orthopyroxene relics, are all comparable with depleted to highly depleted forearc harzburgite from a suprasubduction zone setting. The investigated peridotites have suffered subsequent phases of metasomatism, from oceanfloor hydrothermal alteration(serpentinization)to magmatic hydrothermal alteration. The enrichment of the studied samples in light rare earth elements(LREEs)relative to the heavy ones(HREEs)is attributed to most probably be due to the contamination of their mantle source with granitic
文摘The present study is aimed to serve a small community living on Stand-Alone Solar-Energy (S.A.S.E.S) system. As a basis for the study 1 cubic meter of hydrogen is to be produced by electrolysis in 5 hrs that requires energy input of 5 KW-hr. The proposed system consists of the following main components: photovoltaic module, water electrolyzer and fuel cell. Solar hydrogen production by water electrolysis is described and design parameters are specified. Economic feasibility of the proposed system is evaluated. The projected cost of hydrogen is calculated and found to be 5 cents/ft3.

文摘Egyptian beach ilmenite occurs in a relatively high content in the naturally highly concentrated superficial black sand deposits at specific beach zones in the northern parts of the Nile Delta at Rosetta. Several ore dressing techniques were carried out to upgrade the iimenite concentrate.
文摘The countermeasures are the actions that should be taken, after the occurrence of a nuclear accident to protect the public against the associated risks. The adoption of the countermeasures increases the costs of emergency plan by 40% but reduces the risk associated the accident.
文摘Background: Biplanar medial opening wedge high tibial osteotomy (MOWHTO) is a treatment method that allows adequate correction of symptomatic varus knee deformity. Conclusion: The biplanar MOWHTO allows preservation of posterior tibial slope while correcting the varus knee adequately.
文摘One of the roles of nuctear regulators is to review population projections calculated by applicants for populations lying in the vicinity of nuclear power plants. Thus the further natural growth of this city should be controlled and directed away from the downwind of the plant.

文摘Dielectric ceramics of M(x)Al6(1-x)Si2O13 doped mullite were synthesized via co-precipitation technique. Meanwhile, Gd3+doped mullite achieved the minimum dielectric loss value of 0.09 in the microwave frequency region (1 GHz).
文摘We report on the experimental study of the visible and ultraviolet (UV) fluorescence emission in a (1% molar) Thulium (Tm+3)-doped Zirconium fluoride glass sample. From red at 650 nm, blue (453 & 480 nm) and UV at 365 & 284 nm, have been observed under excitation of a Nd:YAG laser at 1.064 μm. The responsible up-conversion mechanisms are investigated based on experimental data. The related problem of photo-degradation associated with formation of color centers is also addressed.
文摘The present work is a comprehensive of drinking water quality. Eleven groundwater samples were taken from various rural regions of Egypt, the groundwater samples were investigated for chemical, radiometric and heavy metals analyses, the major cations including;sodium ( ), potassium ( ), calcium ( ) and magnesium ( ) ions species, the major anions of chloride ( ), sulphate ( ), nitrite ( ), phosphate ( ). Radiometric analyses in water expressed as the gross alpha and beta activity concentrations, heavy metals analyses including arsenic ( ), lead ( ), cobalt ( ), manganese ( ), iron ( ) and cadmium ( ) ions. The groundwater samples were found to contain high concentrations of heavy metals than the limited values of the world health organization (WHO). Heavy metals speciation were performed using MinteqA2 geochemical code. A modified exchange resin was prepared by polymerization of the condensed dioxalayl p-sulphanilamide with phenol, this ion exchange resin was examined by the different techniques such as;x-ray diffraction, infra red spectra (IR), and electronic microscopic, it was found a good adsorbent material that used for the reduction of heavy metals from contaminated groundwater samples.
