AIM:To compare results of liver stiffness measurements by transient elastography(TE) obtained in our patients population with that used in a recently published meta-analysis.METHODS:This was a single center cross-sect...AIM:To compare results of liver stiffness measurements by transient elastography(TE) obtained in our patients population with that used in a recently published meta-analysis.METHODS:This was a single center cross-sectional study.Consecutive patients with chronic viral hepatitis scheduled for liver biopsy at the outpatient ward of our Infectious Diseases Department were enrolled.TE was carried out by using FibroScan(Echosens,Paris,France).Liver biopsy was performed on the same day as TE,as day case procedure.Fibrosis was staged according to the Metavir scoring system.The diagnostic performance of TE was assessed by using receiver operating characteristic(ROC) curves and the area under the ROC curve analysis.RESULTS:Two hundred and fifty-two patients met the inclusion criteria.Six(2%) patients were excluded due to unreliable TE measurements.Thus,246(171 men and 75 women) patients were analyzed.One hundred and ninety-five(79.3%) patients had chronic hepatitis C,41(16.7%) had chronic hepatitis B,and 10(4.0%) were coinfected with human immunodeficiency virus.ROC curve analysis identified optimal cut-off value of TE as high as 6.9 kPa forF ≥ 2;7.9 kPa forF ≥ 3;9.6 kPa for F = 4 in all patients(n = 246),and as high as 6.9 kPa for F ≥ 2;7.3 kPa for F ≥ 3;9.3 kPa for F = 4 in patients with hepatitis C(n = 195).Cut-off values of TE obtained by maximizing only the specificity were as high as 6.9 kPa for F ≥ 2;9.6 kPa for F ≥ 3;12.2 kPa for F = 4 in all patients(n = 246),and as high as 7.0 kPa forF ≥ 2;9.3 kPa forF ≥ 3;12.3 kPa forF = 4 in patients with hepatitis C(n = 195).CONCLUSION:The cut-off values of TE obtained in this single center study are comparable to that obtained in a recently published meta-analysis that included up to 40 studies.展开更多

METHODS: We used the MEDLINE database to investigate studies between 1996 and 2010 with greater than 10 patients that ...AIM: To systematically review these minimally invasive approaches to infected pancreatic necrosis. METHODS: We used the MEDLINE database to investigate studies between 1996 and 2010 with greater than 10 patients that examined these techniques. Using a combination of Boolean operators, reports were retrieved addressing percutaneous therapy (341 studies), endoscopic necrosectomy (574 studies), laparoscopic necrosectomy via a transperitoneal approach (148 studies), and retroperitoneal necrosectomy (194 studies). Only cohorts with at least 10 or more patients were included. Non-English papers, letters, animal studies, duplicate series and reviews without original data were excluded, leaving a total of 27 studies for analysis. RESULTS: Twenty-seven studies with 947 patients total were examined (eight studies on percutaneous approach; ten studies on endoscopic necrosectomy; two studies on laparoscopic necrosectomy via a transperitoneal approach; five studies on retroperitoneal necrosectomy; and two studies on a combined percutaneous-retroperitoneal approach). Success rate, complications, mortality, and number of procedures were outcomes that were included in the review. We found that most published reports were retrospective in nature, and thus, susceptible to selection and publication bias. Few reports examined these techniques in a comparative, prospective manner. CONCLUSION: Each minimally invasive approach though was found to be safe and feasible in multiple reports. With these new techniques, treatment of infected pancreatic necrosis remains a challenge. We advocate a multidisciplinary approach to this complex problem with treatment individualized to each patient.展开更多

Methods: In vitro antibacterial activity of the aqueous and ethanol extract of the plant was investigated using the agar cup plate diffusion method. Results: The ethanol extract of the plant showed antibacterial activity against Escherichia coli, Streptococcus faecalis, Shigella dysenterme, Slaphylococcus aureus (S. aureus) , Salmonella sp. , Pseudomonas aeruginosa and Neisseria gonor-r/iaeae, while the water extract was active against Escherichia coli, Streptococcus faecalis, Shigella dysenterme, Sta-phylococcus aureus and. Pseudomonas aeruginosa. The etlianol extract exhibited higher antibacterial activity than the water extract. The minimum inhibitory concentration (MIC) and minimum bactericidal, concentration (MBC} of the ethanol extract were 0. 65 mg/mt and 0. 85 mg/ml, respectively , against S. aureus. Treatment of the extract at higher temperature, 60℃ increased the sensitivity of the test organisms to the plant extract. Phytochemical analysis indicated t/tat the plant possesses tannins, saponins as well as phenols. Conclusion: A scientific basis exists that the plant possesses antibacterial activity and it could be a probable source of therapeutic agent.展开更多

The present study was carried out...Monitoring of heavy metals contamination of agricultural products and their transfer and bioaccumulation in crops like rice has become a hot topic worldwide over the last two decades. The present study was carried out to determine the accumulation of heavy metals in rice fields and their transfer to rice grains. Soil, irrigation water and rice grains samples were gathered in Maga-Pouss, Far-North, Cameroon. Concentrations of six heavy metals (lead, cadmium, zinc, copper, iron and mercury) were evaluated by Atomic Absorption Spectrophotometer (AAS). Mercury was not detected in this study. Average concentrations of metals were in this order (in mg/kg): Fe (188.60 ± 97.06) > Pb (63.63 ± 7.11) > Cd (2.59 ± 0.29) > Zn (1.10 ± 1.05) > Cu (0.80 ± 0.73) in water and Pb (105.50 ± 31.11) > Fe (105.50 ± 31.11) > Cu (45.93 ± 14.39) > Zn (22.52 ± 6.40) > Cd (3.15 ± 0.49) in soil. Water in Maga-Pouss rice fields appears to be more harmful than the soil, notably for lead, cadmium and copper. In rice grains, heavy metals were found in this order (mg/kg): Fe (188.01 ± 82.62) > Cu (27.20 ± 0.00) > Zn (23.61 ± 12.42) > Pb (19.50 ± 19.91) > Cd (2.02 ± 1.05). The mean bioconcentration factor (BCF) of metals from soil to rice grains was in the following order: Fe (2.60) > Zn (1.05) > Cd (0.64) > Cu (0.59) > Pb (0.18). From water to rice grains, the order is: Cu (37.26) > Zn (22.49) > Cd (6.97) > Pb (2.74) > Fe (1.94). Rice field pH and electrical conductivity favored the uptake of lead, copper and cadmium by rice grains. The findings of this study will be good documentation for risk assessment, and decision-making by environmental managers in this region.展开更多

Objective: The objectives of this study were to elucidate the current epidemiology and describe the clinical, diagnostic and therapeutic features of ICMET in Yaounde. Method and findings: A descriptive cross-sectional study was done in the neurosurgery departments of the General and Central Hospitals of Yaounde during the period from January 2016 to December 2022. We included all medical booklets of patients admitted for a tumoral intracranial expansive process with our target population being patients with histological evidence of ICMET, and did a retrospective inclusion of data using a pre-established technical form aimed at collecting sociodemographic data, clinical data, paraclinical data, and the treatment procedures. Analysis was done using the SPSS statistical software. A total of 614 cases of intracranial tumors were included among whom 35 presented histological evidence of ICMET. This gives a frequency of 5.7%. The sex ratio was 0.94, the mean age was 55.68 +/- 14.4 years, extremes 28 and 86 years and the age range 50 - 59 was affected in 28.57% of cases. The clinical presentation included signs of raised intracranial pressure (headache, blurred vision, vomiting) in 26 cases (74.3%), motor deficit 48.6%, seizures 17.1%. The mode of onset was metachronous in 71.4% and synchronous in 28.6%. The imaging techniques were cerebral CT scan in 82.9%, cerebral MRI in 40%, TAP scan in 22.9%. The metastatic lesions were supratentorial in 94.3% and single in 62.9%. The primary cancers found were breast cancer (31.4%), lung cancer (25.7%), prostate cancer (17.1%), thyroid cancer (5.7%), colon cancer (2.9%), and melanoma (2.9%). The therapeutic modalities were total resection (68.6%), radiotherapy (37.1%). Conclusion: Intracranial metastases are relatively frequent. There is a female sex predominance and the age group 50 - 59 years is the most affected. Brai展开更多

This non-union is a major treatment challenge with increased cost of care and morbidity in this part of the world. Humeral shaft non-union can be treated with locked intra-medullary nailing (LIMN) or dynamic compression plating (DCP). Study on comparison of these methods of fixation in this part of the world is scarce in literature search, hence the reason for this study. Objective: The objectives of this study are: (1) to compare early clinical outcome following fixation of humeral shaft fracture nonunion with DCP versus LIMN;(2) to compare the time of radiologic fracture union of DCP with LIMN;(3) to compare complications following fixation of humeral shaft fracture nonunion with DCP versus LIMN. Patients and Methods: This was a randomized control study done for 2 years in which fifty adult patients with humeral shaft non-union were recruited. The patients were grouped into 2 (P = DCP & N = LIMN). Forty five of the patients completed the follow up periods of the study and then analyzed. The P group had ORIF with DCP while the N group had ORIF with LIMN. Both groups had grafting with cancellous bones. Each patient was followed up for a period of 6 months at the time which radiographic union is expected. Any patient without clinical and/or radiographic evidence of union after six months of surgery was diagnosed as having recurrent non-union. The data generated was analyzed using SPSS Version 23. The results were presented in charts and tables. The paired t-test was used while considering p value Result: Forty five patients completed follow up. There was a male preponderance (4:1), right humerus predominated (3:2). Motor vehicular accidents were the commonest cause of the fractures (62%). The obj...Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and Plasmodium species are the causative agents of coronavirus disease 2019 (COVID-19) and malaria respectively with similar clinical presentations. The objective of this study is to determine the burden of co-infection of SARS-CoV-2 and malaria in the general population. Five (5 mLs) of blood samples were collected for SARS-CoV-2 and malaria parasite test. The malaria test was performed using a commercially available one-step malaria antigen Plasmodium falciparum histidine-rich protein 2 (Pf HRP-II) rapid test kit. The results of the study showed that the participants that were coinfected with SARS-CoV-2 IgG and malaria were 13 (2.5%) in Lagos, 1114 (39.1%) in Delta and 49 (2.3%) in Sokoto States. The prevalence of coinfection of SARS-CoV-2 and malaria in urban areas in Lagos, Delta and Sokoto States were 7 (2.2%), 1373 (48.1%), and 5 (0.2%) respectively. In rural areas, the prevalence of coinfection of SARS-CoV-2 and malaria in Lagos, Delta and Sokoto States were 6 (0.3%), 365 (12.8%), and 44 (2.1%) respectively in this study. This suggests that participants in the urban areas were more prone to co-infections than the rural areas in Lagos and Delta states, while it was otherwise in Sokoto State. In conclusion, the co-infection of SARS-CoV-2 and malaria was very high in Delta State compared to the other States. It is important for clinics to screen for both diseases when patients present with symptoms of malaria. This is because the infections have similar symptoms and the public is quick to assume malaria infection without diagnosing for COVID-19 and vice versa.展开更多
Introduction: Although cervical myelopathy is the most common degenerative cervical spine pathology in adults, the indications and approaches of surgical treatment have not yet been clearly defined in the literature. ...Introduction: Although cervical myelopathy is the most common degenerative cervical spine pathology in adults, the indications and approaches of surgical treatment have not yet been clearly defined in the literature. Very few studies exist regarding these aspects in our setting, and they are mostly outdated. This study aimed to describe the diagnostic aspects, current surgical treatment with technical improvements, and progression in patients. Patients and Methods: We conducted a multicentre retrospective descriptive study over a 10-year period between January 2011 and January 2020 in three referral centres in Yaoundé. All patients who underwent surgery for cervical myelopathy were included. Results: Fifty-two patients were recruited. The rate of operated cervical myelopathies among all degenerative spinal pathologies was 14.05%. The M/F sex ratio was 3/1, and the average age was 52 ± 10 years. All included patients had gait problems, 90.38% demonstrated motor deficiency, and 67.30% experienced at least three levels of compression. Surgery was decided based on the Nurick grade;a posterior approach was applied to 86.54% of patients. Postoperative progression showed a neurological improvement of 82% with an average follow-up of 4 years. Conclusion: In this study, the patients who underwent surgery were relatively young, their clinical presentations were mostly advanced, and surgical management showed good results in well-selected cases.展开更多

It addresses a significant research gap in understanding metro passengers’ dynamics across cultural and geographical contexts. It employs questionnaires, field observations, and advanced data analysis techniques like association rule mining and neural network modeling. Key findings include a correlation between rainy weather, shorter waiting times, and higher arrival volumes. Neural network models showed high predictive accuracy, with waiting time, metro satisfaction, and weather being significant factors in Lagos Light Rail Blue Line Metro. In contrast, arrival patterns, weather, and time of day were more influential in Wuhan Metro Line 5. Results suggest that improving metro satisfaction and reducing waiting times could increase arrival volumes in Lagos Metro while adjusting schedules for weather and peak times could optimize flow in Wuhan Metro. These insights are valuable for transportation planning, passenger arrival volume management, and enhancing user experiences, potentially benefiting urban transportation sustainability and development goals.展开更多

Evidences have shown that antrochoanal polyp often times coexists with adenoid enlargement especially in the young which was in sharp contrast to ethmoidal polyposis occurring in the presence of adenoid enlargement. Moreover, the hidden location of nasopharynx housing the adenoids, coupled with the masking effect of bilateral sinonasal polyps, leads to inadvertent misdiagnosis of this complex pathology with subsequent difficulty in providing effective management. Case Presentation: A 24 year-old lady presented with recurrent bilateral nasal obstruction that became persistent associated with nasal discharge and anosmia. She also had recurrent right otorrhoea with associated hearing loss. Diagnostic rigid nasal endoscopy revealed mucoid discharge with bilateral polypoid masses filling both nasal cavities. Otoendoscopic finding revealed a small (about 5%) central tympanic membrane perforation. A clinical assessment of chronic rhinosinusitis with nasal polyposis complicated by chronic suppurative otitis media (CSOM) was made. A non-contrast CT scan of the paranasal sinuses showed isodense lesions in the nasal cavities, all paranasal sinuses and the entire nasopharynx. She had endoscopic sinus surgery and a nasopharyngeal clearance biopsy. The nasal, paranasal and nasopharyngeal masses had histologic confirmation of inflammatory nasal polyps and lymphoid (adenoid) hyperplasia respectively. Her condition improved remarkably with subsequent medical treatment. She was followed up for 8 months and no recurrence was observed. Conclusion: Sinonasal polyposis can coexist with adenoid hypertrophy and middle ear disease as a single pathological condition. Hence, a high index of suspicion and thorough evaluation become necessary for making timely diagnoses and instituting effective management.展开更多

Today a laparoscopic operation is the treatment of choice for esophageal achalasia and for most patients with gastroesophageal reflux disease. Because the pathogenesis of achalasia is unknown, treatment is palliative and aims to improve esophageal emptying by decreasing the functional obstruction at the level of the gastro-esophageal junction. The refinement of minimally invasive techniques accompanied by large, multiple randomized control trials with long-term outcome has allowed the laparoscopic Heller myotomy and partial fundoplication to become the treatment of choice for achalasia compared to endoscopic procedures, including endoscopic botulinum toxin injection and pneumatic dilatation. Patients with suspected gastroesophageal reflux need to undergo a thorough preoperative workup. After establishing diagnosis, treatment for gastroesophageal reflux should be individualized to patient characteristics and a decision about an operation made jointly between surgeon and patient. The indications for surgery have changed in the last twenty years. In the past, surgery was often considered for patients who did not respond well to acid reducing medications. Today, the best candidate for surgery is the patient who has excellent control of symptoms with proton pump inhibitors. The minimally invasive approach to antireflux surgery has allowed surgeons to control reflux in a safe manner, with excellent long term outcomes. Like achalasia and gastroesophageal reflux, the treatment of patients with paraesophageal hernias has also seen a major evolution. The laparoscopic approach has been shown to be safe, and durable, with good relief of symptoms over the long-term. The most significant controversy with laparoscopic paraesophageal hernia repair is the optimal crural repair. This manuscript reviews the evolution of these techniques.展开更多
Jiama,with more than 11 Mt of copper metal,is the largest porphyry-skarn copper system in the Gangdese metallogenic belt,Tibet,China,creating ideal conditions for deciphering the origin of porphyry ores in a collision...Jiama,with more than 11 Mt of copper metal,is the largest porphyry-skarn copper system in the Gangdese metallogenic belt,Tibet,China,creating ideal conditions for deciphering the origin of porphyry ores in a collision setting.Despite massive studies of the geology,chronology,petrogenesis,and ore-related fluids and their sources in Jiama,there is a lack of systematic summaries and reviews of this system.In contrast to traditional porphyry copper systems in a subduction setting,recent studies and exploration suggest that the Jiama deposit includes porphyry-type Mo-Cu,skarn-type Cu polymetallic,vein-type Au and manto orebodies.This paper reviews the latest studies on the geology,chronology,petrogenesis,fluid inclusions,and isotopic geochemistry(hydrogen,oxygen,sulfur,and lead)of the Jiama deposit.Accordingly,a multi-center complex mineralization model was constructed,indicating that multi-phase intrusions from the same magma reservoir can form multiple hydrothermal centers.These centers are mutually independent and form various orebodies or are superimposed on each other and form thick,high-grade orebodies.Finally,a new comprehensive exploration model was established for the Jiama porphyry copper system.Both models established in this study help to refine the theories on continental-collision metallogeny and porphyry copper systems.展开更多

RESULTS: Twenty-seven studies with 947 patients total were examined (eight studies on percutaneous approach; ten studies on endoscopic necrosectomy; two studies on laparoscopic necrosectomy via a transperitoneal approach; five studies on retroperitoneal necrosectomy; and two studies on a combined percutaneous-retroperitoneal approach). Success rate, complications, mortality, and number of procedures were outcomes that were included in the review. We found that most published reports were retrospective in nature, and thus, susceptible to selection and publication bias. Few reports examined these techniques in a comparative, prospective manner. CONCLUSION: Each minimally invasive approach though was found to be safe and feasible in multiple reports. With these new techniques, treatment of infected pancreatic necrosis remains a challenge. We advocate a multidisciplinary approach to this complex problem with treatment individualized to each patient.

文摘To investigate the scientific bases for t/te traditional use of Stachy-tarpheta angustifolia. Methods: In vitro antibacterial activity of the aqueous and ethanol extract of the plant was investigated using the agar cup plate diffusion method. Results: The ethanol extract of the plant showed antibacterial activity against Escherichia coli, Streptococcus faecalis, Shigella dysenterme, Slaphylococcus aureus (S. aureus) , Salmonella sp. , Pseudomonas aeruginosa and Neisseria gonor-r/iaeae, while the water extract was active against Escherichia coli, Streptococcus faecalis, Shigella dysenterme, Sta-phylococcus aureus and. Pseudomonas aeruginosa. The etlianol extract exhibited higher antibacterial activity than the water extract. The minimum inhibitory concentration (MIC) and minimum bactericidal, concentration (MBC} of the ethanol extract were 0. 65 mg/mt and 0. 85 mg/ml, respectively , against S. aureus. Treatment of the extract at higher temperature, 60℃ increased the sensitivity of the test organisms to the plant extract. Phytochemical analysis indicated t/tat the plant possesses tannins, saponins as well as phenols. Conclusion: A scientific basis exists that the plant possesses antibacterial activity and it could be a probable source of therapeutic agent.
文摘Monitoring of heavy metals contamination of agricultural products and their transfer and bioaccumulation in crops like rice has become a hot topic worldwide over the last two decades. The present study was carried out to determine the accumulation of heavy metals in rice fields and their transfer to rice grains. Soil, irrigation water and rice grains samples were gathered in Maga-Pouss, Far-North, Cameroon. Concentrations of six heavy metals (lead, cadmium, zinc, copper, iron and mercury) were evaluated by Atomic Absorption Spectrophotometer (AAS). Mercury was not detected in this study. Average concentrations of metals were in this order (in mg/kg): Fe (188.60 ± 97.06) > Pb (63.63 ± 7.11) > Cd (2.59 ± 0.29) > Zn (1.10 ± 1.05) > Cu (0.80 ± 0.73) in water and Pb (105.50 ± 31.11) > Fe (105.50 ± 31.11) > Cu (45.93 ± 14.39) > Zn (22.52 ± 6.40) > Cd (3.15 ± 0.49) in soil. Water in Maga-Pouss rice fields appears to be more harmful than the soil, notably for lead, cadmium and copper. In rice grains, heavy metals were found in this order (mg/kg): Fe (188.01 ± 82.62) > Cu (27.20 ± 0.00) > Zn (23.61 ± 12.42) > Pb (19.50 ± 19.91) > Cd (2.02 ± 1.05). The mean bioconcentration factor (BCF) of metals from soil to rice grains was in the following order: Fe (2.60) > Zn (1.05) > Cd (0.64) > Cu (0.59) > Pb (0.18). From water to rice grains, the order is: Cu (37.26) > Zn (22.49) > Cd (6.97) > Pb (2.74) > Fe (1.94). Rice field pH and electrical conductivity favored the uptake of lead, copper and cadmium by rice grains. The findings of this study will be good documentation for risk assessment, and decision-making by environmental managers in this region.
文摘Background: The incidence of intracranial metastases (ICMET) has been steadily rising, and its frequency with respect to primary brain tumours is relatively high. Objective: The objectives of this study were to elucidate the current epidemiology and describe the clinical, diagnostic and therapeutic features of ICMET in Yaounde. Method and findings: A descriptive cross-sectional study was done in the neurosurgery departments of the General and Central Hospitals of Yaounde during the period from January 2016 to December 2022. We included all medical booklets of patients admitted for a tumoral intracranial expansive process with our target population being patients with histological evidence of ICMET, and did a retrospective inclusion of data using a pre-established technical form aimed at collecting sociodemographic data, clinical data, paraclinical data, and the treatment procedures. Analysis was done using the SPSS statistical software. A total of 614 cases of intracranial tumors were included among whom 35 presented histological evidence of ICMET. This gives a frequency of 5.7%. The sex ratio was 0.94, the mean age was 55.68 +/- 14.4 years, extremes 28 and 86 years and the age range 50 - 59 was affected in 28.57% of cases. The clinical presentation included signs of raised intracranial pressure (headache, blurred vision, vomiting) in 26 cases (74.3%), motor deficit 48.6%, seizures 17.1%. The mode of onset was metachronous in 71.4% and synchronous in 28.6%. The imaging techniques were cerebral CT scan in 82.9%, cerebral MRI in 40%, TAP scan in 22.9%. The metastatic lesions were supratentorial in 94.3% and single in 62.9%. The primary cancers found were breast cancer (31.4%), lung cancer (25.7%), prostate cancer (17.1%), thyroid cancer (5.7%), colon cancer (2.9%), and melanoma (2.9%). The therapeutic modalities were total resection (68.6%), radiotherapy (37.1%). Conclusion: Intracranial metastases are relatively frequent. 文摘Background: Fractures of humeral shaft in adults are common injuries. Humeral shafts non-union either from late presentation after initial treatment by traditional bone setters or failed non-operative orthodox care is a major problem in this part of the world. This non-union is a major treatment challenge with increased cost of care and morbidity in this part of the world. Humeral shaft non-union can be treated with locked intra-medullary nailing (LIMN) or dynamic compression plating (DCP). Study on comparison of these methods of fixation in this part of the world is scarce in literature search, hence the reason for this study. Objective: The objectives of this study are: (1) to compare early clinical outcome following fixation of humeral shaft fracture nonunion with DCP versus LIMN;(2) to compare the time of radiologic fracture union of DCP with LIMN;(3) to compare complications following fixation of humeral shaft fracture nonunion with DCP versus LIMN. Patients and Methods: This was a randomized control study done for 2 years in which fifty adult patients with humeral shaft non-union were recruited. The patients were grouped into 2 (P = DCP & N = LIMN). Forty five of the patients completed the follow up periods of the study and then analyzed. The P group had ORIF with DCP while the N group had ORIF with LIMN. Both groups had grafting with cancellous bones. Each patient was followed up for a period of 6 months at the time which radiographic union is expected. Any patient without clinical and/or radiographic evidence of union after six months of surgery was diagnosed as having recurrent non-union. The data generated was analyzed using SPSS Version 23. The results were presented in charts and tables. The paired t-test was used while considering p value Result: Forty five patients completed follow up. 文摘Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and Plasmodium species are the causative agents of coronavirus disease 2019 (COVID-19) and malaria respectively with similar clinical presentations. The objective of this study is to determine the burden of co-infection of SARS-CoV-2 and malaria in the general population. Five (5 mLs) of blood samples were collected for SARS-CoV-2 and malaria parasite test. The malaria test was performed using a commercially available one-step malaria antigen Plasmodium falciparum histidine-rich protein 2 (Pf HRP-II) rapid test kit. The results of the study showed that the participants that were coinfected with SARS-CoV-2 IgG and malaria were 13 (2.5%) in Lagos, 1114 (39.1%) in Delta and 49 (2.3%) in Sokoto States. The prevalence of coinfection of SARS-CoV-2 and malaria in urban areas in Lagos, Delta and Sokoto States were 7 (2.2%), 1373 (48.1%), and 5 (0.2%) respectively. In rural areas, the prevalence of coinfection of SARS-CoV-2 and malaria in Lagos, Delta and Sokoto States were 6 (0.3%), 365 (12.8%), and 44 (2.1%) respectively in this study. This suggests that participants in the urban areas were more prone to co-infections than the rural areas in Lagos and Delta states, while it was otherwise in Sokoto State. In conclusion, the co-infection of SARS-CoV-2 and malaria was very high in Delta State compared to the other States. It is important for clinics to screen for both diseases when patients present with symptoms of malaria. This is because the infections have similar symptoms and the public is quick to assume malaria infection without diagnosing for COVID-19 and vice versa.
文摘Introduction: Although cervical myelopathy is the most common degenerative cervical spine pathology in adults, the indications and approaches of surgical treatment have not yet been clearly defined in the literature. Very few studies exist regarding these aspects in our setting, and they are mostly outdated. This study aimed to describe the diagnostic aspects, current surgical treatment with technical improvements, and progression in patients. Patients and Methods: We conducted a multicentre retrospective descriptive study over a 10-year period between January 2011 and January 2020 in three referral centres in Yaoundé. All patients who underwent surgery for cervical myelopathy were included. Results: Fifty-two patients were recruited. The rate of operated cervical myelopathies among all degenerative spinal pathologies was 14.05%. The M/F sex ratio was 3/1, and the average age was 52 ± 10 years. All included patients had gait problems, 90.38% demonstrated motor deficiency, and 67.30% experienced at least three levels of compression. Surgery was decided based on the Nurick grade;a posterior approach was applied to 86.54% of patients. Postoperative progression showed a neurological improvement of 82% with an average follow-up of 4 years. Conclusion: In this study, the patients who underwent surgery were relatively young, their clinical presentations were mostly advanced, and surgical management showed good results in well-selected cases.
文摘This study explores the factors influencing metro passengers’ arrival volume in Wuhan, China, and Lagos, Nigeria, by examining weather, time of day, waiting time, travel behavior, arrival patterns, and metro satisfaction. It addresses a significant research gap in understanding metro passengers’ dynamics across cultural and geographical contexts. It employs questionnaires, field observations, and advanced data analysis techniques like association rule mining and neural network modeling. Key findings include a correlation between rainy weather, shorter waiting times, and higher arrival volumes. Neural network models showed high predictive accuracy, with waiting time, metro satisfaction, and weather being significant factors in Lagos Light Rail Blue Line Metro. In contrast, arrival patterns, weather, and time of day were more influential in Wuhan Metro Line 5. Results suggest that improving metro satisfaction and reducing waiting times could increase arrival volumes in Lagos Metro while adjusting schedules for weather and peak times could optimize flow in Wuhan Metro. These insights are valuable for transportation planning, passenger arrival volume management, and enhancing user experiences, potentially benefiting urban transportation sustainability and development goals.
文摘Background: Ethmoidal polyposis coexisting with adenoid enlargement and tympanic membrane perforation is a rare condition in otolaryngological practice. Evidences have shown that antrochoanal polyp often times coexists with adenoid enlargement especially in the young which was in sharp contrast to ethmoidal polyposis occurring in the presence of adenoid enlargement. Moreover, the hidden location of nasopharynx housing the adenoids, coupled with the masking effect of bilateral sinonasal polyps, leads to inadvertent misdiagnosis of this complex pathology with subsequent difficulty in providing effective management. Case Presentation: A 24 year-old lady presented with recurrent bilateral nasal obstruction that became persistent associated with nasal discharge and anosmia. She also had recurrent right otorrhoea with associated hearing loss. Diagnostic rigid nasal endoscopy revealed mucoid discharge with bilateral polypoid masses filling both nasal cavities. Otoendoscopic finding revealed a small (about 5%) central tympanic membrane perforation. A clinical assessment of chronic rhinosinusitis with nasal polyposis complicated by chronic suppurative otitis media (CSOM) was made. A non-contrast CT scan of the paranasal sinuses showed isodense lesions in the nasal cavities, all paranasal sinuses and the entire nasopharynx. She had endoscopic sinus surgery and a nasopharyngeal clearance biopsy. The nasal, paranasal and nasopharyngeal masses had histologic confirmation of inflammatory nasal polyps and lymphoid (adenoid) hyperplasia respectively. Her condition improved remarkably with subsequent medical treatment. She was followed up for 8 months and no recurrence was observed. Conclusion: Sinonasal polyposis can coexist with adenoid hypertrophy and middle ear disease as a single pathological condition. Hence, a high index of suspicion and thorough evaluation become necessary for making timely diagnoses and instituting effective management.
文摘Thanks to the development of minimally invasive surgery, the last 20 years have witnessed a change in the treatment algorithm of benign esophageal disorders. Today a laparoscopic operation is the treatment of choice for esophageal achalasia and for most patients with gastroesophageal reflux disease. Because the pathogenesis of achalasia is unknown, treatment is palliative and aims to improve esophageal emptying by decreasing the functional obstruction at the level of the gastro-esophageal junction. The refinement of minimally invasive techniques accompanied by large, multiple randomized control trials with long-term outcome has allowed the laparoscopic Heller myotomy and partial fundoplication to become the treatment of choice for achalasia compared to endoscopic procedures, including endoscopic botulinum toxin injection and pneumatic dilatation. Patients with suspected gastroesophageal reflux need to undergo a thorough preoperative workup. After establishing diagnosis, treatment for gastroesophageal reflux should be individualized to patient characteristics and a decision about an operation made jointly between surgeon and patient. The indications for surgery have changed in the last twenty years. In the past, surgery was often considered for patients who did not respond well to acid reducing medications. Today, the best candidate for surgery is the patient who has excellent control of symptoms with proton pump inhibitors. The minimally invasive approach to antireflux surgery has allowed surgeons to control reflux in a safe manner, with excellent long term outcomes. Like achalasia and gastroesophageal reflux, the treatment of patients with paraesophageal hernias has also seen a major evolution. The laparoscopic approach has been shown to be safe, and durable, with good relief of symptoms over the long-term. The most significant controversy with laparoscopic paraesophageal hernia repair is the optimal crural repair. This manuscript reviews the evolution of these techniques.

文摘Jiama,with more than 11 Mt of copper metal,is the largest porphyry-skarn copper system in the Gangdese metallogenic belt,Tibet,China,creating ideal conditions for deciphering the origin of porphyry ores in a collision setting.Despite massive studies of the geology,chronology,petrogenesis,and ore-related fluids and their sources in Jiama,there is a lack of systematic summaries and reviews of this system.In contrast to traditional porphyry copper systems in a subduction setting,recent studies and exploration suggest that the Jiama deposit includes porphyry-type Mo-Cu,skarn-type Cu polymetallic,vein-type Au and manto orebodies.This paper reviews the latest studies on the geology,chronology,petrogenesis,fluid inclusions,and isotopic geochemistry(hydrogen,oxygen,sulfur,and lead)of the Jiama deposit.Accordingly,a multi-center complex mineralization model was constructed,indicating that multi-phase intrusions from the same magma reservoir can form multiple hydrothermal centers.These centers are mutually independent and form various orebodies or are superimposed on each other and form thick,high-grade orebodies.Finally,a new comprehensive exploration model was established for the Jiama porphyry copper system.Both models established in this study help to refine the theories on continental-collision metallogeny and porphyry copper systems.
文摘Protein phosphorylation,one of the major post-translational modifications,plays a crucial role in cell signaling,DNA replication,gene expression and differentiation;and alters enzyme activity and other biological activities;and regulates cell proliferation and enlargement,phytohormone biosynthesis and signaling,plant disease resistance,and grain filling and quality during rice seed development.Research work on protein phosphorylation started in the 1950 s with the discovery of phosphorylase a and phosphorylase b which are phospho and dephospho forms of the same enzyme.Over the last decade,rice proteomics has accomplished tremendous progress in setting up techniques to proteome nearly all tissues,organs and organelles.The progress made in this field is evident in number of research works.However,research on rice protein phosphorylation is still at its infancy and there are still many unanswered questions.In this review,the general description of protein phosphorylation,including history,structure,frequency of occurrence and function,are discussed.This work also elucidates the different methods for identification,qualification and finally,the progress in rice phosphoproteome research and perspectives.

文摘Reversible protonic ceramic cells(RePCCs)hold promise for efficient energy storage,but their practicality is hindered by a lack of high-performance air electrode materials.Ruddlesden-Popper perovskite Sr_(3)Fe_(2)O_(7−δ)(SF)exhibits superior proton uptake and rapid ionic conduction,boosting activity.However,excessive proton uptake during RePCC operation degrades SF’s crystal structure,impacting durability.This study introduces a novel A/B-sites co-substitution strategy for modifying air electrodes,incorporating Sr-deficiency and Nb-substitution to create Sr_(2.8)Fe_(1.8)Nb_(0.2)O_(7−δ)(D-SFN).Nb stabilizes SF’s crystal,curbing excessive phase formation,and Sr-deficiency boosts oxygen vacancy concentration,optimizing oxygen transport.The D-SFN electrode demonstrates outstanding activity and durability,achieving a peak power density of 596 mW cm^(−2)in fuel cell mode and a current density of−1.19 A cm^(−2)in electrolysis mode at 1.3 V,650℃,with excellent cycling durability.This approach holds the potential for advancing robust and efficient air electrodes in RePCCs for renewable energy storage.
