Changing profiles of cancer burden worldwide and in China: a secondary analysis of the global cancer statistics 2020 被引量:1491

Background:Cancer is one of the leading causes of death globally,but its burden is not uniform.GLOBOCAN 2020 has newly updated the estimates of cancer burden.This study summarizes the most recent changing profiles of cancer burden worldwide and in China and compares the cancer data of China with those of other regions.Methods:We conducted a descriptive secondary analysis of the GLOBOCAN 2020 data.To depict the changing global profile of the leading cancer types in 2020 compared with 2018,we extracted the numbers of cases and deaths in 2018 from GLOBOCAN 2018.We also obtained cancer incidence and mortality from the 2015 National Cancer Registry Report in China when sorting the leading cancer types by new cases and deaths.For the leading cancer types according to sex in China,we summarized the estimated numbers of incidence and mortality,and calculated China’s percentage of the global new cases and deaths.Results:Breast cancer displaced lung cancer to become the most leading diagnosed cancer worldwide in 2020.Lung,liver,stomach,breast,and colon cancers were the top five leading causes of cancer-related death,among which liver cancer changed from the third-highest cancer mortality in 2018 to the second-highest in 2020.China accounted for 24%of newly diagnosed cases and 30%of the cancer-related deaths worldwide in 2020.Among the 185 countries included in the database,China’s age-standardized incidence rate(204.8 per 100,000)ranked 65th and the age-standardized mortality rate(129.4 per 100,000)ranked 13th.The two rates were above the global average.Lung cancer remained the most common cancer type and the leading cause of cancer death in China.However,breast cancer became the most frequent cancer type among women if the incidence was stratified by sex.Incidences of colorectal cancer and breast cancer increased rapidly.The leading causes of cancer death varied minimally in ranking from 2015 to 2020 in China.Gastrointestinal cancers,including stomach,colorectal,liver,and esophageal cancers,contributed to a massive bu 展开更多

Land-use/land-cover changes (LUCCs) have links to both human and nature inter- actions. China's Land-Use/cover Datasets (CLUDs) were updated regularly at 5-year inter- vals from the late 1980s to 2010, with standard procedures based on Landsat TM/ETM+ im- ages. A land-use dynamic regionalization method was proposed to analyze major land-use conversions. The spatiotemporal characteristics, differences, and causes of land-use changes at a national scale were then examined. The main findings are summarized as fol- lows. Land-use changes (LUCs) across China indicated a significant variation in spatial and temporal characteristics in the last 20 years (1990-2010). The area of cropland change de- creased in the south and increased in the north, but the total area remained almost un- changed. The reclaimed cropland was shifted from the northeast to the northwest. The built-up lands expanded rapidly, were mainly distributed in the east, and gradually spread out to central and western China. Woodland decreased first, and then increased, but desert area was the opposite. Grassland continued decreasing. Different spatial patterns of LUC in China were found between the late 20th century and the early 21st century. The original 13 LUC zones were replaced by 15 units with changes of boundaries in some zones. The main spatial characteristics of these changes included (1) an accelerated expansion of built-up land in the Huang-Huai-Hai region, the southeastern coastal areas, the midstream area of the Yangtze River, and the Sichuan Basin; (2) shifted land reclamation in the north from northeast China and eastern Inner Mongolia to the oasis agricultural areas in northwest China; (3) continuous transformation from rain-fed farmlands in northeast China to paddy fields; and (4) effective- ness of the "Grain for Green" project in the southern agricultural-pastoral ecotones of Inner Mongolia, the Loess Plateau, and southwestern mountainous areas. Background: Nontraumatic osteonecrosis of the femoral head (NONFH) is a debilitating disease that represents a significant financial burden for both individuals and healthcare systems. Despite its significance, however, its prevalence in the Chinese general population remains unknown. This study aimed to investigate the prevalence of NONFH and its associated risk factors in the Chinese population. Methods: A nationally representative survey of 30,030 respondents was undertaken from June 2012 to August 2013. All participants underwent a questionnaire investigation, physical examination of hip, and bilateral hip joint X-ray and/or magnetic resonance imaging exalnination. Blood samples were taken after overnight fasting to test serum total cholesterol, triglyceride, and high-density lipoprotein (HDL) and low-density lipoprotein (LDL) levels. We then used multivariate logistic regression analysis to investigate the associations between various metabolic, demographic, and lifestyle-related variables and NONFH. Results: NONFH was diagnosed in 218 subjects (0.725%) and the estimated NONFH cases were 8.12 million among Chinesepeople aged 15 years and over. The prevalence of NONFH was significantly higher in males than in females (1.02% vs. 0.51%, x^2 = 24.997, P 〈 0.001 ). Among NONFH patients, North residents were subjected to higher prevalence of NONFH than that of South residents (0.85% vs. 0.61%,x^2= 5.847, P = 0.016). Our multivariate regression analysis showed that high blood levels oftriglycerides, total cholesterol, LDL-cholesterol, and non-H DL-cholesterol, male, urban residence, family history of osteonecrosis of the femoral head. heavy smoking, alcohol abuse and glucocorticoid intake, overweight, and obesity were all significantly associated with an increased risk of NONFH. Conclusions: Our findings highlight that NONFH is a significantpublic health challenge in China and underscore the need for policy measures on the national level. Land use and land cover change as the core of coupled human-environment systems has become a potential field of land change science (LCS) in the study of global environmental change. Based on remotely sensed data of land use change with a spatial resolution of 1 km × 1 km on national scale among every 5 years, this paper designed a new dynamic regionalization according to the comprehensive characteristics of land use change including regional differentiation, physical, economic, and macro-policy factors as well. Spatial pattern of land use change and its driving forces were investigated in China in the early 21 st century. To sum up, land use change pattern of this period was characterized by rapid changes in the whole country. Over the agricultural zones, e.g., Huang-Huai-Hai Plain, the southeast coastal areas and Sichuan Basin, a great proportion of fine arable land were engrossed owing to considerable expansion of the built-up and residential areas, resulting in decrease of paddy land area in southern China. The development of oasis agriculture in Northwest China and the reclamation in Northeast China led to a slight increase in arable land area in northern China. Due to the "Grain for Green" policy, forest area was significantly increased in the middle and western developing regions, where the vegetation coverage was substantially enlarged, likewise. This paper argued the main driving forces as the implementation of the strategy on land use and regional development, such as policies of "Western Development", "Revitalization of Northeast", coupled with rapidly economic development during this period. 展开更多

NF-κB pathway consists of canonical and non-canonical pathways.The canonical NF-κB is activated by various stimuli,transducing a quick but transient transcriptional activity,to regulate the expression of various proinflammatory genes and also serve as the critical mediator for inflammatory response.Meanwhile,the activation of the non-canonical NF-κB pathway occurs through a handful of TNF receptor superfamily members.Since the activation of this pathway involves protein synthesis,the kinetics of non-canonical NF-κB activation is slow but persistent,in concordance with its biological functions in the development of immune cell and lymphoid organ,immune homeostasis and immune response.The activation of the canonical and non-canonical NF-κB pathway is tightly controlled,highlighting the vital roles of ubiquitination in these pathways.Emerging studies indicate that dysregulated NF-κB activity causes inflammation-related diseases as well as cancers,and NF-κB has been long proposed as the potential target for therapy of diseases.This review attempts to summarize our current knowledge and updates on the mechanisms of NF-κB pathway regulation and the potential therapeutic application of inhibition of NF-κB signaling in cancer and inflammatory diseases. 展开更多

The genome sequence of the Severe Acute Respiratory Syndrome (SARS)-associated virus provides essential information for the identification of pathogen(s), exploration of etiology and evolution, interpretation of transmission and pathogenesis, development of diagnostics, prevention by future vaccination, and treatment by developing new drugs. We report the complete genome sequence and comparative analysis of an isolate (BJ01) of the coronavirus that has been recognized as a pathogen for SARS. The genome is 29725 nt in size and has 11 ORFs (Open Reading Frames). It is composed of a stable region encoding an RNA-dependent RNA polymerase (composed of 2 ORFs) and a variable region representing 4 CDSs (coding sequences) for viral structural genes (the S, E, M, N proteins) and 5 PUPs (putative uncharacterized proteins). Its gene order is identical to that of other known coronaviruses. The sequence alignment with all known RNA viruses places this virus as a member in the family of Coronaviridae. Thirty putative substitutions have been identified by comparative analysis of the 5 SARS- associated virus genome sequences in GenBank. Fifteen of them lead to possible amino acid changes (non-synonymous mutations) in the proteins. Three amino acid changes, with predicted alteration of physical and chemical features, have been detected in the S protein that is postulated to beinvolved in the immunoreactions between the virus and its host. Two amino acid changes have been detected in the Mprotein, which could be related to viral envelope formation. Phylogenetic analysis suggests the possibility of non-human origin of the SARS-associated viruses but provides noevidence that they are man-made. Further efforts should focus on identifying the etiology of the SARS-associated virus and ruling out conclusively the existence of otherpossible SARS-related pathogen(s). 展开更多

AIM:To determine the resistance patterns of Helicobacter pylori(H.pylori) strains isolated from patients in Beijing and monitor the change of antibiotic resistance over time.METHODS:In this prospective,serial and crosssectional study,H.pylori cultures were successfully obtained from 371 and 950 patients(never receiving eradication) during 2009-2010 and 2013-2014,respectively.Resistance to amoxicillin,clarithromycin,metronidazole,levofloxacin,tetracycline,and rifampicin was determined by Epsilometer test.RESULTS:The resistance rates of isolates obtained during 2009-2010 were 66.8%,39.9%,34.5%,15.4%,6.7%,and 4.9% to metronidazole,clarithromycin,levofloxacin,rifampicin,amoxicillin and tetracycline,respectively; and the corresponding rates for isolates during 2013-2014 were 63.4%,52.6%,54.8%,18.2%,4.4% and 7.3%,respectively.The resistance rates to clarithromycin and levofloxacin were significantly increased after four years.In 2009-2010,14.6% of H.pylori isolates were susceptible to all tested antibiotics,with mono(33.7%),double(28.3%),triple(16.7%),quadruple(6.2%),quintuple(0.3%) and sextuple resistance(0.3%) also being detected.In 2013-2014,9.4% were susceptible to all tested antibiotics,and mono(27.6%),double(28.4%),triple(24.9%),quadruple(7.3%),quintuple(2.3%) and sextuple resistance(0.1%) was also observed.More multiple resistant H.pylori isolates were found during 2013-2014.Gender(to levofloxacin and metronidazole),age(to levofloxacin) and endoscopic findings(to clarithromycin) were independent factors influencing antibiotic resistance.CONCLUSION:H.pylori resistance to commonly used antibiotics in Beijing is high with increased multiple antibiotic resistance. 展开更多

Based on the concept of "active blocks" and spatial distribution of historical earthquakes with surface ruptures as well as major and subordinate active faults. The Sichuan-Yunnan region can be divided into four first-order blocks. They are the Markam block (I), the Sichuan-Yunnan rhombic block (II), Baoshan-Pu'er block (III), and Mizhina-Ximeng block (IV). Cut by sub-ordinate NE-trending active faults, the Sichuan-Yunnan rhombic block (II) can be further divided into two sub-blocks: the northwestern Sichuan sub-block (II1) and the middle Yunnan sub-block (II2), while the Baoshan- Pu'er block (III) can be further divided into three sub-blocks: Baoshan sub-block (III1), Jinggu sub-block (III2), and Mengla sub-block (III3). A quantitative study of offset landforms is carried out and the basic types of active faults and their long-term slip rates along the major boundaries of active blocks of different orders in the Sichuan-Yunnan region are determined, through slip vector analysis, the motion states of the active blocks are clarified and the deformation coordination on the block margins is discussed. It is suggested that the tectonic motion of the blocks in this region is a complex or superimposition of three basic types of motions: southeastward sliding, rotating on vertical axis, and uplifting. The Markam block (I), the northwestern Sichuan sub-block (II1), and middle Yunnan sub-block (II2) have a southeastward horizontal sliding rate of 1-5 mm/a, clockwise rotating angular rate of 1.4-4(/Ma, and uplifting rate of about 1 mm/a. The Baoshan-Pu'er (III) and Mizhina-Ximeng (IV) blocks have also been extensively clockwise rotated. This pattern of motion is a strain response to the collision between the Indian and Eurasian plates and the localized deformation and differential slip on the block margins associated with the northward motion of the Indian Plate. Tea is the world's oldest and most popular caffeine-containing beverage with immense economic, medicinal, and cultural importance. Here, we present the first high-quality nucleotide sequence of the repeat-rich (80.9%), 3.02-Gb genome of the cultivated tea tree Camellia sinensis. We show that an extraordinarily large genome size of tea tree is resulted from the slow, steady, and long-term amplification of a few LTR retrotransposon families. In addition to a recent whole-genome duplication event, lineage-specific expansions of genes associated with flavonoid metabolic biosynthesis were discovered, which enhance catechin production, terpene enzyme activation, and stress tolerance, important features for tea flavor and adaptation. We demonstrate an independent and rapid evolution of the tea caffeine synthesis pathway relative to cacao and coffee. A comparative study among 25 Camellia species revealed that higher expression levels of most flavonoid- and caffeinebut not theanine-related genes contribute to the increased production of catechins and caffeine and thus enhance tea-processing suitability and tea quality. These novel findings pave the way for further metabolomic and functional genomic refinement of characteristic biosynthesis pathways and will help develop a more diversified set of tea flavors that would eventually satisfy and attract more tea drinkers worldwide. 展开更多

Abiotic stresses and soil nutrient limitations are major environmental conditions that reduce plant growth,productivity and quality.Plants have evolved mechanisms to perceive these environmental challenges,transmit the stress signals within cells as well as between cells and tissues,and make appropriate adjustments in their growth and development in order to survive and reproduce.In recent years,significant progress has been made on many fronts of the stress signaling research,particularly in understanding the downstream signaling events that culminate at the activation of stress-and nutrient limitation-responsive genes,cellular ion homeostasis,and growth adjustment.However,the revelation of the early events of stress signaling,particularly the identification of primary stress sensors,still lags behind.In this review,we summarize recent work on the genetic and molecular mechanisms of plant abiotic stress and nutrient limitation sensing and signaling and discuss new directions for future studies. 展开更多

AIM:To sum up the clinical and pathological characte- ristics of solid pseudopapillary tumor (SPT) and the experience with it.METHODS: A total of 553 SPT patients reported in Chinese literature between January 1996 and January 2009 were retrospectively reviewed and analyzed. RESULTS: The mean age of the 553 SPT patients included in this review was 27.2 years, and the male to female ratio was 1:8.37. Their symptoms were non-specific, and nearly one third of the patients were asymptomatic. Computed tomography and ultraso-nography were performed to show the nature and location of SPT. Most of the tumors were distributed in the pancreatic head (39.8%), tail (24.1%), body andtail (19.5%). Forty-five patients (9.2%) were diagnosed as malignant SPT with metastasis or invasion. None of the clinical factors was closely related to the malignant potential of SPT. Surgery was the main therapeutic modality for SPT. Local resection, distal pancreatectomy and pancreatoduodenectomy were the most common surgical procedures. Local recurrence and hepatic metastasis were found in 11 and 2 patients, respectively, after radical resection. Four patients died of tumor progression within 4 years after palliative resection of SPT. The prognosis of SPT patients was good with a 5-year survival rate of 96.9%.CONCLUSION: SPT of the pancreas is a rare indolent neoplasm that typically occurs in young females. It is a low-grade malignancy and can be cured with extended resection. The prognosis of such patients is good although the tumor may recur and metastasize. 展开更多

Background: Transarterial chemoembolization(TACE) is recommended as the standard care for unresectable hepatocellular carcinoma(HCC) at Barcelona Clinic Liver Cancer(BCLC) stage A-B. However, the efficacy of TACE on large(> 10 cm) stage A-B HCC is far from satisfactory, and it is proposed that hepatic artery infusion chemotherapy(HAIC)might be a better first-line treatment of this disease. Hence, we compared the safety and efficacy of HAIC with the modified FOLFOX(mFOLFOX) regimen and those ofTACE in patients with massive unresectable HCC.Methods: A prospective, non-randomized, phase II study was conducted on patients with massive unresectable HCC. The protocol involved HAIC with the mFOLFOX regimen(oxaliplatin, 85 mg/m^2 intra-arterial infusion; leucovorin,400 mg/m^2 intra-arterial infusion; and fluorouracil, 400 mg/m2 bolus infusion and 2400 mg/m^2 continuous infusion)every 3 weeks and TACE with 50 mg of epirubicin, 50 mg of lobaplatin, 6 mg of mitomycin, and lipiodol and polyvinyl alcohol particles. The tumor responses, time-to-progression(TTP), and safety were assessed.Results: A total of 79 patients were recruited for this study: 38 in the HAIC group and 41 in the TACE group. The HAIC group exhibited higher partial response and disease control rates than did the TACE group(52.6% vs. 9.8%, P < 0.001;83.8% vs. 52.5%, P = 0.004). The median TTPs for the HAIC and TACE groups were 5.87 and 3.6 months(hazard radio[HR] = 2.35,95% confidence interval [CI] = 1.16-4.76, P = 0.015). More patients in the HAIC group than in the TACE group underwent resection(10 vs. 3,P = 0.033). The proportions of grade 3-4 adverse events(AE) and serious adverse events(SAE) were lower in the HAIC group than in the TACE group(grade 3-4 AEs: 13 vs. 27, P = 0.007;SAEs: 6 vs. 15,p = 0.044). Although a number of petrographic observations and isotopic data suggest that magma mixing is common in genesis of many granite plutons, it is still controversial whether the mantle-derived magmas were involved in granites. We carried out in this study a systematic analysis of in situ zircon Hf-O isotopes for three early Yanshanian intrusions dated at ca. 160 Ma from the Nanling Range of Southeast China. The Qinghu monzonite has very homogeneous zircon Hf-O isotopic compositions, εHf(t) =11.6±0.3 and δ18O=5.4‰±0.3‰. In combination with whole-rock geochemical and Sr-Nd isotopic data, the parental magma of the Qinghu monzonite were likely derived from the partial melting of recently-metasomatized, phlogopite-bearing lithospheric mantle without appreciable crustal contamination. The Lisong and Fogang granites and the mafic microgranular enclaves (MME) within the Lisong granites have a wide range of zircon Hf-O isotopic compositions, with Hf and O isotopes being negatively correlated within each pluton. The Lisong MMEs were crystallized from a mantle-derived magma, similar to the parental magma of the Qinghu monzonite, with small amount of crustal assimilation. The Lisong and Fogang granites were formed by reworking of meta-sedimentary materials by mantle-derived magmas and mixing of the mantle-and sediment-derived melts to varying degrees. It is thus concluded that these two Yanshanian granites in the Nanling Range were formed associated with growth and differentiation of continental crust. 展开更多

ENSO induces coherent climate anomalies over the Indo-western Pacific, but these anomalies outlast SST anomalies of the equatorial Pacific by a season, with major effects on the Asian summer monsoon. This review provides historical accounts of major milestones and synthesizes recent advances in the endeavor to understand summer variability over the Indo-Northwest Pacific region. Specifically, a large-scale anomalous anticyclone (AAC) is a recurrent pattern in post-E1 Nifio summers, spanning the tropical Northwest Pacific and North Indian oceans. Regarding the ocean memory that anchors the summer AAC, competing hypotheses emphasize either SST cooling in the easterly trade wind regime of the Northwest Pacific or SST warming in the westerly monsoon regime of the North Indian Ocean. Our synthesis reveals a coupled ocean- atmosphere mode that builds on both mechanisms in a two-stage evolution. In spring, when the northeast trades prevail, the AAC and Northwest Pacific cooling are coupled via wind-evaporation-SST feedback. The Northwest Pacific cooling persists to trigger a summer feedback that arises from the interaction of the AAC and North Indian Ocean warming, enabled by the westerly monsoon wind regime. This Indo-western Pacific ocean capacitor (IPOC) effect explains why E1 Nifio stages its last act over the monsoonal Indo-Northwest Pacific and casts the Indian Ocean warming and AAC in leading roles. The IPOC displays interdecadal modulations by the ENSO variance cycle, significantly correlated with ENSO at the turn of the 20th century and after the 1970s, but not in between. Outstanding issues, including future climate projections, are also discussed. 展开更多

Especially, some important integralproperties are kept unchanged, such as the anti-symmetries of thehorizontal advection operators and the vertical convectionoperator, the mass conservation, the effective energy conservationunder the standard stratification approximation, and so on.
