Pancreatic injury is mild in 80%-90% of patients who recover without complications. The early prediction of the severity of the disease is an important goal for physicians in management of patients with acute pancreatitis in order to optimize the therapy and to prevent organ dysfunction and local complications. For that purpose, multiple clinical scale scores have been applied to patients with acute pancreatitis. Recently, a new problem has emerged: the increased severity of the disease in obese patients. However, the mechanisms by which obesity increases the severity of acute pancreatitis are unclear. Several hypotheses have been suggested: (1) obese patients have an increased inflammation within the pancreas; (2) obese patients have an increased accumulation of fat within and around the pancreas where necrosis is often located; (3) increase in both peri- and intra-pancreatic fat and inflammatory cells explain the high incidence of pancreatic inflammation and necrosis in obese patients; (4) hepatic dysfunction associated with obesity might enhance the systemic inflammatory response by altering the detoxiflcation of inflammatory mediators; and (5) ventilation/perfusion mismatch leading to hypoxia associated with a low pancreatic flow might reduce the pancreatic oxygenation and further enhance pancreatic injury. Recent experimental investigations also show an increased mortality and morbidity in obese rodents with acute pancreatitis and the implication of the adipokines leptin and adiponectin. Such models are important to investigate whether the inflammatory response of the disease is enhanced by obesity. METHODS: A retrospective analysis of 93 patients, who had undergone elective ileostomy closure between 2009 and 2013 was performed. Demographic, clinical and surgical variables were reviewed for analysis. All complications were recorded, and classified according to the Clavien-Dindo Classification. Statistical univariate and multivariate analysis was performed, setting a P value of 0.05 for significance.RESULTS: The patients had a mean age of 60.3 years, 58% male. The main procedure for ileostomy cre-ation was rectal cancer(56%), and 37% had received preoperative chemo-radiotherapy. The average delay from creation to closure of the ileostomy was 10.3 mo. Postoperative complications occurred in 40% of the pa-tients, with 1% mortality. The most frequent were ileus(13%) and wound infection(13%). Pseudomembra-nous colitis appeared in 4%. Increased postoperative complications were associated with delay in ileostomyclosure(P = 0.041). Male patients had more complica-tions(P = 0.042), mainly wound infections(P = 0.007). Pseudomembranous colitis was also associated with the delay in ileostomy closure(P = 0.003). End-to-end in-testinal anastomosis without resection was significantly associated with postoperative ileus(P = 0.037). CONCLUSION: Although closure of a protective il-eostomy is a fairly common surgical procedure, it has a high rate of complications, and this must be taken into account when the indication is made. The delay in stoma closure can increase the rate of complications in general, and specifically wound infections and colitis.展开更多

Wheat, soy protein concentrate, wheat gluten, fishmeal and crystalline amino acids (AAs) were the protein sources in the experimental diets with Lys as limiting AA. Following an adaptation period of five days, two consecutive excreta collection periods (2 × 5 d) were conducted: 10 - 20 d of age (starter period) and 25 - 35 d of age (grower period). Statistical evaluation of N balance data utilized an exponential modelling approach. Based on different dietary Lys efficiency, Lys requirement data were derived by modelling depending on average body weight (BW) during starter and grower period and targeted body protein deposition (PD), respectively. In addition, the influence of graded feed intake was taken into account. For the starter period at 600 g BW and assumed 10 g daily body PD, Lys requirement data between 741 mg and 823 mg per day were observed. The corresponding Lys in-feed concentration was 1.06% and 1.18%, dependent on supposed Lys efficiency at 70 g daily feed intake. For the grower period (average BW 1800 g), 1272 mg to 1473 mg Lys per day was needed to yield 16.5 g daily PD. The corresponding required Lys in-feed concentration was between 0.85% and 0.94% Lys for 150 g daily feed intake.展开更多

The existence of HBV variants with primary antiviral resistance may be important for treatment choice. OO-Method is an automatic software production method. The contribution of this work is to systematically define a set of rules that allows estimating the functional size at an early stage of the software production process, in accordance with COSMIC-FFP. To do this, we describe the design, the application, and the analysis of the proposed measurement procedure following the steps of a process model for software measurement. We also report initial results on the evaluation of RmFFP in terms of its reproducibility.展开更多

METHODS: Between February 2000 and February 2003, 279 consecutive patients with acute pancreatitis were prospectively enrolled in our study. In this report, 40 patients with mild and 40 patients with severe pancreatitis were randomly studied, sCD40L concentrations were measured 48 hours after admission. RESULTS: sCD40L levels were significantly higher 48 hours after admission in severe pancreatitis than in mild pancreatitis. Using a cutoff of 1000 pg/L, the sensitivity and specificity of sCD40L to detect a severe course of the disease were 78% and 62% respectively compared to 72% and 81% for CRP. Logistic regression analysis found that CRP was the only statistically significant marker able to detect a severe course of the disease. CONCLUSION: These findings indicate that CRP remains a valuable marker to determine the severity and prognosis of acute pancreatitis whereas sCD40L levels should be assessed in further studies.展开更多

The aim of the present study was to assess lysine (Lys) requirement of growing chicken both by classical supplementation technique and principles of diet dilution technique as applied with “Goettingen approach”. During the starter period (1 - 21 d), a growth study with male meat type chicken (Ross 308) was conducted making use of five graded dietary Lys-levels (3 repetition boxes with 3 birds/box). L-Lys×HCl was gradually added to a diet based on wheat, soybean protein concentrate, wheat gluten and fishmeal to yield 80%, 87.5%, 95%, 102.5% and 110% of the expected requirement level (13 g Lys/kg as fed). Diets were iso-energetic (12.8 MJME/kg) and iso-nitrogenous (21.65% crude protein). Birds were fed on free choice level also to assess the feed intake (FI) effects as important factor on traditional response criteria. Analyzed body composition at start and end of the growth study yielded N deposition (ND) data for further data assessment using exponential approximations depending on dietary Lys content or observed Lys intake. The results indicated significant differences (p < 0.05) in response on body weight gain (BWG) and observed dietary protein quality with unexpected consequences for the derived Lys requirement data. According to the independent variable (Lys in % of diet versus daily Lys intake) and aimed level of daily ND, the needed in-feed content of Lys varied between 1.24% and 1.46%. Application of the exponential modelling by “Goettingen approach” overcame these misleading conclusions by modelling the relationship between required Lys intake and observed response data (BWG, ND) taking also into account the expected real feed intake to formulate the needed in-feed concentration.展开更多

A quantitative, correlational, and cross-sectional study was conducted from May to November 2012. By stratified random sampling, 290 students in the health field were selected. Questionnaire was used to identify factors associated with obesity. The obesity and overweight prevalence was determined, calculating body mass index through anthropometric measurement. The strength of the association between variables was measured using Chi squared and odds ratio tests. The proportion of students with overweight or obesity was 28.3% and was higher in men (36.8%) than in women (24.6%), with statistical significance (p < 0.01). Greater obesity or overweight was identified in psychology students (p < 0.01) and in those who reported an average appetite (79.3%) (p < 0.05). In total, 66.6% of the students decreased their physical activity upon joining the university, which was a factor significantly associated with obesity and overweight (p < 0.05). The findings show that the school environment can be a conditioning factor for the development of obesity and overweight in students. Implementation of a program is recommended to prevent and control such problems.展开更多

Animal production is one of the most expressive activities of Brazilian agribusiness. In order to ensure the productivity and competitiveness of the sector, the use of drugs for therapeutic and prophylactic purposes is a common practice. Due to the continuous release of antimicrobials into the environment, the aim of this study was to compare the frequency of detection of tetracyclines and sulfonamides in surface water collected from rural areas in Lidice District of Rio Claro, in the State of Rio de Janeiro, Brazil. An investigative study was conducted with 24 river water samples analyzed by high-performance liquid chromatography coupled to tandem mass spectrometry and the aim of this study was to determine residues of sulfonamides and tetracyclines based on the USEPA method 1694. The results indicated the presence of sulfamethoxazole and oxytetracycline concentrations at the ng·L-1 level. The applied method showed overall good performance with recoveries above 57%, method detection limits ≤ 7.17 ng·L-1, method quantification limits ≤ 23.90 ng·L-1 and good linearity.展开更多

Metabolic syndrome implies a variety of risk factors that are based on abdominal obesity and insulin resistance. Moreover, the importance of metabolic syndrome is emphasized due to the fact that it presents an increase in oxidative stress, which is produced by the sum of alterations that each risk factor produces within the metabolic syndrome pathology. Reducing oxidative stress in these patients is currently one of the most interesting challenges of cardiovascular and metabolic therapeutics, because it is a molecular biology alteration that is not generally diagnosed and, therefore, not treated. The increasing incidence of overweight and obesity cause an increase in the incidence of metabolic syndrome, thus turning into a huge problem that keeps growing at alarming proportions. This syndrome's incidence oscillates between 20 and 40%, depending on the gender, age, ethnic group, and diagnostic criteria used for the definition of the disease. Prospective studies show that metabolic syndrome doubles the risk of cardiovascular disease and causes a five-fold increase in the risk of developing type 2 diabetes. With the aim of decreasing the oxidative stress caused by metabolic syndrome, we investigated the effect of antioxidant protection in DNA repair and cell membranes through the use of blueberry extract, which is the fruit with the highest antioxidant capacity, in patients with metabolic syndrome. Thirty patients were studied for a period of 6 months of intervention, and it could be demonstrated that they showed a highly significant decrease in the damage produced to the DNA, which was measured by the urinary excretion of 8-hydroxy-2'-deoxyguanosine (8-OHdG) and the damage caused to the vascular endothelium and cell membranes, which was measured through the urinary excretion of F2展开更多

We found that the temperature varied highly by year, month, peatland type, soil depth, HT and WL manipulations. The highest effect of HT on the temperature at 25 cm depth was found in June for the bog mesocosms (3.34-4.27 ℃) but in May for the fen mesocosms (2.32-4.33 ℃) over the 2-year study period. The effects of WL in the bog mesocosms were only found between August and January, with the wet mesocosms warmer than the dry mesocosms by 0.48-2.03 ℃ over the 2-year study period. In contrast, wetter fen mesocosms were generally cooler by 0.16-3.87℃. Seasonal changes of temperatures elevated by the HT also varied by depth and ecosystem type, with temperature differences at 5 cm and 10 cm depth showing smaller seasonal fluctuations than those at 25 cm and 40 cm in the bog mesocosms. However, increased HT did not always lead to warmer soil, especially in the fen mesocosms. Both HT and WL manipulations have also changed the length of the non-frozen season.展开更多

文摘Vascular endothelial growth factor(VEGF)was discovered by its angiogenic activity. However, during evolution,it appeared earlier as a neurotrophic factor required for the development of the nervous system in invertebrates lacking a circulatory system. We aimed at reviewing recent evidence indicating that VEGF has neuroprotective effects in neurons exposed to a variety of insults.Of particular interest is the link established between VEGF and motoneurons, especially after the design of the VEGFδ/δmutant mice. These mice are characterized by low levels of VEGF and develop muscle weakness and motoneuron degeneration resembling amyotrophic lateral sclerosis. The administration of VEGF through several routes to animal models of amyotrophic lateral sclerosis delays motor impairment and motoneuron degeneration and increases life expectancy. There are new recent advances in the role of VEGF in the physiology of motoneurons. Our experimental aims use the extraocular(abducens)motoneurons lesioned by axotomy as a model for studying VEGF actions.Axotomized abducens motoneurons exhibit severe alterations in their discharge activity and a loss of synaptic boutons. The exogenous administration of VEGF to axotomized abducens motoneurons, either from the transected nerve or intraventricularly, fully restores the synaptic and discharge properties of abducens motoneurons, despite being axotomized. In addition, when an anti-VEGF neutralizing antibody is delivered from the muscle to intact, uninjured abducens motoneurons, these cells display alterations in their discharge pattern and a loss of synaptic boutons that resemble the state of axotomy. All these data indicate that VEGF is an essential neurotrophic factor for motoneurons.

文摘This paper introduces a measurement procedure, called RmFFP, which describes a set of operations for modelling and estimating the size of object-oriented software systems from high-level specifications using the OO-Method Requirement Model. We also report initial results on the evaluation of RmFFP in terms of its reproducibility.

文摘AIM: TO assess the early predictability of the soluble CD40L (sCD40L) in pancreatitis severity. CONCLUSION: These findings indicate that CRP remains a valuable marker to determine the severity and prognosis of acute pancreatitis whereas sCD40L levels should be assessed in further studies.
文摘Validated procedures play an important role to obtain accurate information about individual amino acid requirement data. Application of the exponential modelling by “Goettingen approach” overcame these misleading conclusions by modelling the relationship between required Lys intake and observed response data (BWG, ND) taking also into account the expected real feed intake to formulate the needed in-feed concentration.
文摘The aim for this research is to determine the association between the risk factors and prevalence of overweight and obesity in students in the health field in the Autonomous University of San Luis Potosí (UASLP in Spanish). Implementation of a program is recommended to prevent and control such problems.
文摘Cystic pancreatic lesions are common, and about 90%of these lesions are pseudocysts or retention cysts.True cysts of the pancreas are rare. The majority of pancreatic serous cystadenomas are microcystic.Macrocystic serous cystadenoma is an unusual and essentially benign pancreatic tumor. The radiologic features of this subtype may resemble a pseudocyst or a mucinous cystadenoma. Thus,it is often difficult to make a correct diagnosis[1,2].Herein,we report on a macrocystic serous cystadenoma of the pancreas in a 46-year-old female patient, with a preoperative diagnosis of pancreatic pseudocyst by radiologic imaging.
文摘The occurrence of antimicrobials in the aquatic environment and drinking water has raised the question of their impact on the environment and public health. The applied method showed overall good performance with recoveries above 57%, method detection limits ≤ 7.17 ng·L-1, method quantification limits ≤ 23.90 ng·L-1 and good linearity.
文摘Oxidative stress is a health condition that could potentially harm the patient, and it is defined as the lack of balance between the production of oxygen free radicals, which rises, and antioxidant defenses, which are in decrease. Thirty patients were studied for a period of 6 months of intervention, and it could be demonstrated that they showed a highly significant decrease in the damage produced to the DNA, which was measured by the urinary excretion of 8-hydroxy-2'-deoxyguanosine (8-OHdG) and the damage caused to the vascular endothelium and cell membranes, which was measured through the urinary excretion of F2

文摘We initiated a multi-factor global change experiment to explore the effects of infrared heat loading (HT) and water table level (WL) treatment on soil temperature (T) in bog and fen peatland mesocosms. Both HT and WL manipulations have also changed the length of the non-frozen season.
