AIM To report the results of the International Nosocomial Infection Control Consortium(INICC) study conducted in Quito, Ecuador.METHODS A device-associated healthcare-acquired infection(DAHAI) prospective surveillance...AIM To report the results of the International Nosocomial Infection Control Consortium(INICC) study conducted in Quito, Ecuador.METHODS A device-associated healthcare-acquired infection(DAHAI) prospective surveillance study conducted from October 2013 to January 2015 in 2 adult intensive care units(ICUs) from 2 hospitals using the United States Centers for Disease Control/National Healthcare Safety Network(CDC/NHSN) definitions and INICC methods. RESULTS We followed 776 ICU patients for 4818 bed-days. The central line-associated bloodstream infection(CLABSI) rate was 6.5 per 1000 central line(CL)-days, the ventilator-associated pneumonia(VAP) rate was 44.3 per 1000 mechanical ventilator(MV)-days, and the catheterassociated urinary tract infection(CAUTI) rate was 5.7 per 1000 urinary catheter(UC)-days. CLABSI and CAUTI rates in our ICUs were similar to INICC rates [4.9(CLABSI) and 5.3(CAUTI)] and higher than NHSN rates [0.8(CLABSI) and 1.3(CAUTI)]- although device use ratios for CL and UC were higher than INICC and CDC/NSHN's ratios. By contrast, despite the VAP rate was higher than INICC(16.5) and NHSN's rates(1.1), MV DUR was lower in our ICUs. Resistance of A. baumannii to imipenem and meropenem was 75.0%, and of Pseudomonas aeruginosa to ciprofloxacin and piperacillin-tazobactam was higher than 72.7%, all them higher than CDC/NHSN rates. Excess length of stay was 7.4 d for patients with CLABSI, 4.8 for patients with VAP and 9.2 for patients CAUTI. Excess crude mortality in ICUs was 30.9% for CLABSI, 14.5% for VAP and 17.6% for CAUTI. CONCLUSION DA-HAI rates in our ICUs from Ecuador are higher than United States CDC/NSHN rates and similar to INICC international rates.展开更多

Purpose:The family caregiver's role is an important influence factor of patient's adaptation behaviour and quality of life after stroke.The purpose of this research was to identify the effect of caregiver empo...Purpose:The family caregiver's role is an important influence factor of patient's adaptation behaviour and quality of life after stroke.The purpose of this research was to identify the effect of caregiver empowerment program based on the adaptation model(CEP-BAM)on functional capacity and quality of life of patients after stroke.Methods:This research was a quasi-experimental research with a pre-and post-test control group design.The total participants who completed the research were 80 patients and their families,40 participants in the intervention group and 40 participants in the control group.We conducted home visits to perform the intervention.The measurement of functional capacity and quality of life performed four times(pre-test and three times post-test).Data analysis was performed using the repeated measurement ANOVA or general linear model repeated measure(GLM-RM).Results:This research proves a significant difference in functional capacity and quality of life between the two groups and between pre-test and sixth months after intervention(P<0.05).The quality of life of the intervention group in the sixth month after intervention was better than that of the control group(33.40±3.65 vs 30.60±2.78)with a significant difference(P<0.05).Conclusion:It is concluded that CEP-BAM effectively increased patients'functional capacity and quality of life after a stroke in the sixth month after intervention.展开更多

They are responsible for severe sequelae and the majority of deaths. In Gabon, no study on stroke mortality has been conducted. Objective: To determine short, medium, long-term mortality and factors associated with long-term stroke mortality. Method: Our study took place in the Neurology Department of the University Hospital Center of Libreville (UHCL). It was a historical cohort study with descriptive and analytical purposes covering the period from June 1 to August 31, 2018 and taking into account hospitalized patients from January 1, 2013 to December 31, 2017. We conducted a comprehensive systematic recruitment of patients with stroke, 18 years old and more, and had been agreed to give information. We included all patients meeting the inclusion criteria. The data was gathered using Epi-Info 7 software. The CHI-2 test was used for the comparison of frequencies and the Student’s test, for comparison of means. Multivariate analysis with logistic regression allowed us to look for factors associated with long-term mortality. A result was statistically significant for a p 0.05 value. Results: At 3 months, 28 patients (18.1%) died, at 6 months thirty-one patients or 20.1% died. At 5 years old, fifty-three patients or 34.4% had died. The factors associated with long-term mortality, if the lost of sight were all alive were tobacco (p = 0.01) and stroke (p = 0.008). If all those who were lost to sight had died, no factor was associated with 5-year mortality. Conclusion: Stroke mortality must not be taken for granted, it can be underestimated because of the large number of lost sight. Measures must be put in place to strengthen post-stroke monitoring. .展开更多

This study aimed ...Introduction: Adolescence is a period of transition to adulthood, including for HIV-infected adolescents (HIA), when sexual problems emerge. Few studies have been carried out on the sexuality of HIA. This study aimed to assess the sexual behavior of HIV-infected adolescents in Cameroon. Methodology: A cross-sectional study was conducted in three hospitals in the cities of Yaoundé and Douala, from November 2019 to June 2020. All HIA aged 13 to 19 years followed in the study who knew their HIV status were included in the study after obtaining their assent and their parent’s consent. Socio-demographic and clinical characteristics were collected, as well as sexual practices and the determinants of their sexual activity. A multivariate analysis was performed to explore the relationship between the different variables studied and the sexual activity of these adolescents. Results: Of 204 HIA enrolled in the study, 64 (31.7%) were sexually active, the mean age at first sexual intercourse was 15.6 ± 2.8 years and the sex ratio was 0.94. Of the adolescents in the study, 75.7% of girls and 61.2% of boys had regular sexual activity, while 21.3% of girls and 35.4% of boys had multiple sexual relationships in the six months preceding the study. Nearly 16 (25.0%) of the sexually active HIA had more than 2 sexual encounters per month, 6 (9.3%) of these encounters had been paid for and 9 (27.2%) of the girls had already had at least one pregnancy. More than 7 out of 10 HIA (79.6%) had used a condom the last time they had sex. Age less than 18 years [OR = 11.1 (95% CI: 3.1 - 39.4), p = 0.001], lack of remuneration [OR = 9.8 (95% CI: 2.0 - 47.4), p = 0.001] and self-care were significantly associated with sexual activity in HIA, while school attendance was a protective factor. Self-funded HIAs were 21 times more likely to be sexually active [OR = 21.6 (95% CI: 2.3 - 179), p = 0.004]. Conclusion: More than a third of HIV-infected adolescents were sexually active and risky sexual practices were not negligible.展开更多

Methods: The examined patients were classified into 2 groups: group 1—eyes with established retinal vein occlusion (n = 29) and group 2—unaffected fellow eyes of patients with retinal vein occlusion (n = 24). The scanning protocol “Angiography 3 × 3 mm” of Zeiss Cirrus HD-OCT 6000, AngioPlex Metrix was used to evaluate the retinal superficial capillary plexus. The analyzed parameters were vascular density and perfusion density, as well as the area, perimeter, and circularity of the foveolar avascular zone (FAZ). Results: The comparative analysis of FAZ parameters at the superficial capillary plexus (SCP) between group 1 (eyes with retinal vein occlusion) and group 2 (unaffected fellow eyes) showed significant results for the three parameters, respectively area (p = 0.003), perimeter (p ≤ 0.001), and circularity (p = 0.011) of FAZ. The comparative analysis of the vascular network at SCP in patients with diagnosed retinal vein occlusion and unaffected fellow eyes showed significant results for vascular density (VD) in the central (p = 0.038) and inner (p ≤ 0.001) zones as well as total VD (p ≤ 0.001) were statistically significant. Moreover, the results obtained in the study of vascular perfusion (VP) indicated significant results in the inner zone (p ≤ 0.001) and total VP (p = 0.001). Vascular perfusion in the central zone (p = 0.116) was the only parameter not to meet significant results. Conclusion: The current study observed a significant enlargement of the FAZ and loss of its circularity, along with a reduction in vascular network parameters at the superficial retinal capillary plexus level.展开更多

We were interested in any newborn, born alive, vaginally or by cesarean section, of low weight, seen in the first 24 hours from a full-term single-fetal pregnancy whose mother had given her consent. Our sampling was consecutive and non-exhaustive. We excluded newborns whose term was unclear and those with congenital malformations or signs of embryo-foetopathy. Data collection was done using survey sheets. Statistical analyzes were carried out with CS Pro 7.3 and SPSS version 25.0 software. As they reach adolescence, they are confronted with various constraints related to the infection and its treatment, in addition to those of the growth period they are going through. The main aim of the study was to assess the acceptance and describe the experience of HIV infection by infected adolescents but also to investigate the factors associated with good acceptance and a positive experience. Methodology: The cross-sectional analytic study concerned HIV-infected adolescents aged 15 to 19 followed up at the Chantal Biya Foundation-Mother and Child Centre (CME-FCB) and the Yaoundé University Hospital Centre (CHUY) between February 2020 and June 2020. The study saw participants complete a questionnaire containing socio-demographic data and assessing acceptance and experience with the infection. Data analysis was accomplished using Epi info software version 7.2.2.6. Results: One hundred and thirteen HIV-infected adolescents were included in the study. The sex ratio was 0.68 and the mean age was 17 years. More than half of the adolescents had a good acceptance and positive experience with the infection. Related factors were the adolescent’s perception of good health and participation in an association with other infected adolescents. Conclusion: Emphasizing the psychological and educational follow-up of infected adolescents and encouraging their participation in associations for adolescents living with HIV could reduce the consequences of poor acceptance and ensure a better transition to adulthood. .展开更多

The pediatric department at Spécialisé Mère-Enfant Blanche Gomes Hospital (HSMEBG) is divided into two sectors, one catering to children aged one month to four years and the other dedicated to children aged five to 17 years. According to department records, over the past three years, there has been an average of 1050 hospitalizations per year, with an average duration of five days. Objectives: This study aims to describe the socio-demographic characteristics of children who died while in the pediatrics department of the HSMEBG and analyze the factors associated with their deaths. Methodology: A retrospective analytical cross-sectional study was conducted, collecting data over a three-year period, covering the years 2019, 2020 and 2021. Data were collected from medical records of deceased children using Excel software version 2016, and statistical calculations and logistic regression were performed using Epi info software version 7.2.5.0. Results: During the three years of operation, the pediatric department at HSMEBG recorded 3060 new admissions, of which 271 resulted in death, representing an overall frequency of 8.8%. December and January had the highest mortality rates, accounting for 15.5% and 12.5%, respectively. Out of the 271 recorded deaths, 143 (52.77%) occurred in children under the age of one, and 230 (84.87%) occurred in children under the age of five. The average age at death was 2.4 years, ranging from one month to 17 years. The sex ratio was 1. More than half of the deaths (51.66%) occurred during the night, and 165 (60.89%) sought medical help more than three days after the onset of symptoms. This research paper performs a numerical simulation using the finite element method (FEM) to study electromagnetic propagation through both conductive and dielectric media. The simulations are made using the COMSOL Multiphysics software which notably implements the finite element method. The microwave is produced by a Vivaldi antenna at the respective frequencies of 2.6 and 5 GHz and the propagation equation is formulated from Maxwell’s equations. The results obtained show that in the air, strong electric fields are observed in the slot and the micro-strip line for the two frequencies, they are even greater when the wave propagates in the glass and very weak for the copper. The 3D evolutions of the wave in air and glass present comparable values at equal frequencies, the curves being more regular in air (dielectric). The radiation patterns produced for air and glass are directional, with a large main lobe, which is narrower at 5 GHz. For copper, the wave propagation is quite uniform in space, and the radiation patterns show two main lobes with a much larger size at 2.6 GHz than at 5 GHz. The propagation medium would therefore influence the range of values of the gain of the antenna.展开更多

The Pennsylvanian unconformity,which is a detrital surface,separates the beds of the Permian-aged strata from the Lower Paleozoic in the Central Basin Platform.Seismic data interpretation indicates that the unconformi...The Pennsylvanian unconformity,which is a detrital surface,separates the beds of the Permian-aged strata from the Lower Paleozoic in the Central Basin Platform.Seismic data interpretation indicates that the unconformity is an angular unconformity,overlying multiple normal faults,and accompanied with a thrust fault which maximizes the region's structural complexity.Additionally,the Pennsylvanian angular unconformity creates pinch-outs between the beds above and below.We computed the spectral decomposition and reflector convergence attributes and analyzed them to characterize the angular unconformity and faults.The spectral decomposition attribute divides the broadband seismic data into different spectral bands to resolve thin beds and show thickness variations.In contrast,the reflector convergence attribute highlights the location and direction of the pinch-outs as they dip south at angles between 2° and 6°.After reviewing findings from RGB blending of the spectrally decomposed frequencies along the Pennsylvanian unconformity,we observed channel-like features and multiple linear bands in addition to the faults and pinch-outs.It can be inferred that the identified linear bands could be the result of different lithologies associated with the tilting of the beds,and the faults may possibly influence hydrocarbon migration or act as a flow barrier to entrap hydrocarbon accumulation.The identification of this angular unconformity and the associated features in the study area are vital for the following reasons:1)the unconformity surface represents a natural stratigraphic boundary;2)the stratigraphic pinch-outs act as fluid flow connectivity boundaries;3)the areal extent of compartmentalized reservoirs'boundaries created by the angular unconformity are better defined;and 4)fault displacements are better understood when planning well locations as faults can be flow barriers,or permeability conduits,depending on facies heterogeneity and/or seal effectiveness of a fault,which can affect hydrocarbon production.The methodol展开更多

Labeo victorianus(Boulenger,1901)is one of the endemic fishes in Lake Victoria Basin(LVB)but is now threatened by multiple stressors caused by human activities.We investigated spatial and temporal variability in food ...Labeo victorianus(Boulenger,1901)is one of the endemic fishes in Lake Victoria Basin(LVB)but is now threatened by multiple stressors caused by human activities.We investigated spatial and temporal variability in food composition and condition of L.victorianus in influent rivers of Lake Victoria,Kenya.Sampling was done during the dry and wet seasons by electrofishing.Food composition analysis showed that L.victorianus is a benthophagus and omnivorous species whose diet is dominated by detritus,periphyton and insects.There were differences in food composition among rivers,with significant river X season interactions(PERMANOVA F=11.6,df=4,p=0.001),suggesting that the diet depended on prevailing environmental conditions.In turbid rivers,the diet was dominated by detritus while in less turbid rivers it was dominated by insects and periphyton.Sand and mud also formed a significant part of the diet,which was an indication of a limited occurrence of preferable food items.There were ontogenetic shifts in food composition(PERMANOVA F=4.6,df=3,p=0.001),but also with a spatial interaction(PERMANOVA F=5.6,df=7,p=0.001),further indicating the role of environmental conditions in determining the diet for different size classes.Interestingly,the fish condition did not differ among rivers.This study shows that turbidity and organic matter and nutrient loading determine the diet of L.victorianus in LVB rivers,and provides further justification for the maintenance of water quality as a conservation measure for threatened species.展开更多

文摘AIM To report the results of the International Nosocomial Infection Control Consortium(INICC) study conducted in Quito, Ecuador.METHODS A device-associated healthcare-acquired infection(DAHAI) prospective surveillance study conducted from October 2013 to January 2015 in 2 adult intensive care units(ICUs) from 2 hospitals using the United States Centers for Disease Control/National Healthcare Safety Network(CDC/NHSN) definitions and INICC methods. RESULTS We followed 776 ICU patients for 4818 bed-days. The central line-associated bloodstream infection(CLABSI) rate was 6.5 per 1000 central line(CL)-days, the ventilator-associated pneumonia(VAP) rate was 44.3 per 1000 mechanical ventilator(MV)-days, and the catheterassociated urinary tract infection(CAUTI) rate was 5.7 per 1000 urinary catheter(UC)-days. CLABSI and CAUTI rates in our ICUs were similar to INICC rates [4.9(CLABSI) and 5.3(CAUTI)] and higher than NHSN rates [0.8(CLABSI) and 1.3(CAUTI)]- although device use ratios for CL and UC were higher than INICC and CDC/NSHN's ratios. By contrast, despite the VAP rate was higher than INICC(16.5) and NHSN's rates(1.1), MV DUR was lower in our ICUs. Resistance of A. baumannii to imipenem and meropenem was 75.0%, and of Pseudomonas aeruginosa to ciprofloxacin and piperacillin-tazobactam was higher than 72.7%, all them higher than CDC/NHSN rates. Excess length of stay was 7.4 d for patients with CLABSI, 4.8 for patients with VAP and 9.2 for patients CAUTI. Excess crude mortality in ICUs was 30.9% for CLABSI, 14.5% for VAP and 17.6% for CAUTI. CONCLUSION DA-HAI rates in our ICUs from Ecuador are higher than United States CDC/NSHN rates and similar to INICC international rates.

文摘Purpose:The family caregiver's role is an important influence factor of patient's adaptation behaviour and quality of life after stroke.The purpose of this research was to identify the effect of caregiver empowerment program based on the adaptation model(CEP-BAM)on functional capacity and quality of life of patients after stroke.Methods:This research was a quasi-experimental research with a pre-and post-test control group design.The total participants who completed the research were 80 patients and their families,40 participants in the intervention group and 40 participants in the control group.We conducted home visits to perform the intervention.The measurement of functional capacity and quality of life performed four times(pre-test and three times post-test).Data analysis was performed using the repeated measurement ANOVA or general linear model repeated measure(GLM-RM).Results:This research proves a significant difference in functional capacity and quality of life between the two groups and between pre-test and sixth months after intervention(P<0.05).The quality of life of the intervention group in the sixth month after intervention was better than that of the control group(33.40±3.65 vs 30.60±2.78)with a significant difference(P<0.05).Conclusion:It is concluded that CEP-BAM effectively increased patients'functional capacity and quality of life after a stroke in the sixth month after intervention.
文摘Strokes are common around the world and especially in sub-Saharan Africa. They are responsible for severe sequelae and the majority of deaths. In Gabon, no study on stroke mortality has been conducted. Objective: To determine short, medium, long-term mortality and factors associated with long-term stroke mortality. Method: Our study took place in the Neurology Department of the University Hospital Center of Libreville (UHCL). It was a historical cohort study with descriptive and analytical purposes covering the period from June 1 to August 31, 2018 and taking into account hospitalized patients from January 1, 2013 to December 31, 2017. We conducted a comprehensive systematic recruitment of patients with stroke, 18 years old and more, and had been agreed to give information. We included all patients meeting the inclusion criteria. The data was gathered using Epi-Info 7 software. The CHI-2 test was used for the comparison of frequencies and the Student’s test, for comparison of means. Multivariate analysis with logistic regression allowed us to look for factors associated with long-term mortality. A result was statistically significant for a p 0.05 value. Results: At 3 months, 28 patients (18.1%) died, at 6 months thirty-one patients or 20.1% died. At 5 years old, fifty-three patients or 34.4% had died. The factors associated with long-term mortality, if the lost of sight were all alive were tobacco (p = 0.01) and stroke (p = 0.008). If all those who were lost to sight had died, no factor was associated with 5-year mortality. Conclusion: Stroke mortality must not be taken for granted, it can be underestimated because of the large number of lost sight. Measures must be put in place to strengthen post-stroke monitoring. .
文摘Introduction: Adolescence is a period of transition to adulthood, including for HIV-infected adolescents (HIA), when sexual problems emerge. Few studies have been carried out on the sexuality of HIA. This study aimed to assess the sexual behavior of HIV-infected adolescents in Cameroon. Methodology: A cross-sectional study was conducted in three hospitals in the cities of Yaoundé and Douala, from November 2019 to June 2020. All HIA aged 13 to 19 years followed in the study who knew their HIV status were included in the study after obtaining their assent and their parent’s consent. Socio-demographic and clinical characteristics were collected, as well as sexual practices and the determinants of their sexual activity. A multivariate analysis was performed to explore the relationship between the different variables studied and the sexual activity of these adolescents. Results: Of 204 HIA enrolled in the study, 64 (31.7%) were sexually active, the mean age at first sexual intercourse was 15.6 ± 2.8 years and the sex ratio was 0.94. Of the adolescents in the study, 75.7% of girls and 61.2% of boys had regular sexual activity, while 21.3% of girls and 35.4% of boys had multiple sexual relationships in the six months preceding the study. Nearly 16 (25.0%) of the sexually active HIA had more than 2 sexual encounters per month, 6 (9.3%) of these encounters had been paid for and 9 (27.2%) of the girls had already had at least one pregnancy. More than 7 out of 10 HIA (79.6%) had used a condom the last time they had sex. Age less than 18 years [OR = 11.1 (95% CI: 3.1 - 39.4), p = 0.001], lack of remuneration [OR = 9.8 (95% CI: 2.0 - 47.4), p = 0.001] and self-care were significantly associated with sexual activity in HIA, while school attendance was a protective factor. Self-funded HIAs were 21 times more likely to be sexually active [OR = 21.6 (95% CI: 2.3 - 179), p = 0.004]. Conclusion: More than a third of HIV-infected adolescents were sexually active and risky sexual practices were not negligible.
文摘Purpose: To evaluate optical coherence tomography angiography (OCT-A) data obtained from the superficial retinal capillary plexus of patients with retinal vein occlusion and comparative analysis with data registered from unaffected fellow eyes. Methods: The examined patients were classified into 2 groups: group 1—eyes with established retinal vein occlusion (n = 29) and group 2—unaffected fellow eyes of patients with retinal vein occlusion (n = 24). The scanning protocol “Angiography 3 × 3 mm” of Zeiss Cirrus HD-OCT 6000, AngioPlex Metrix was used to evaluate the retinal superficial capillary plexus. The analyzed parameters were vascular density and perfusion density, as well as the area, perimeter, and circularity of the foveolar avascular zone (FAZ). Results: The comparative analysis of FAZ parameters at the superficial capillary plexus (SCP) between group 1 (eyes with retinal vein occlusion) and group 2 (unaffected fellow eyes) showed significant results for the three parameters, respectively area (p = 0.003), perimeter (p ≤ 0.001), and circularity (p = 0.011) of FAZ. The comparative analysis of the vascular network at SCP in patients with diagnosed retinal vein occlusion and unaffected fellow eyes showed significant results for vascular density (VD) in the central (p = 0.038) and inner (p ≤ 0.001) zones as well as total VD (p ≤ 0.001) were statistically significant. Moreover, the results obtained in the study of vascular perfusion (VP) indicated significant results in the inner zone (p ≤ 0.001) and total VP (p = 0.001). Vascular perfusion in the central zone (p = 0.116) was the only parameter not to meet significant results. Conclusion: The current study observed a significant enlargement of the FAZ and loss of its circularity, along with a reduction in vascular network parameters at the superficial retinal capillary plexus level.

It constitutes a major public health problem, both in developed and developing countries, due to its magnitude and its strong association with infant morbidity and mortality. Methodology: We carried out a cross-sectional analytical study with prospective data collection using a technical pretested sheet in the maternity wards of the Douala General Hospital, the Laquintinie Hospital, and the District hospitals of Deido, Nylon and Bonassama over a period of 4 months (January to April 2020). We were interested in any newborn, born alive, vaginally or by cesarean section, of low weight, seen in the first 24 hours from a full-term single-fetal pregnancy whose mother had given her consent. Our sampling was consecutive and non-exhaustive. We excluded newborns whose term was unclear and those with congenital malformations or signs of embryo-foetopathy. Data collection was done using survey sheets. Statistical analyzes were carried out with CS Pro 7.3 and SPSS version 25.0 software. The Student, Chi-square and Fischer tests were used to compare the means of the variables, the percentages with a significance threshold P value Results: During the study period, 305 full-term newborns were included, divided into 172 boys and 133 girls. 文摘Introduction: Access to antiretroviral drugs has improved the survival of children infected with the Human Immunodeficiency Virus (HIV). As they reach adolescence, they are confronted with various constraints related to the infection and its treatment, in addition to those of the growth period they are going through. The main aim of the study was to assess the acceptance and describe the experience of HIV infection by infected adolescents but also to investigate the factors associated with good acceptance and a positive experience. Methodology: The cross-sectional analytic study concerned HIV-infected adolescents aged 15 to 19 followed up at the Chantal Biya Foundation-Mother and Child Centre (CME-FCB) and the Yaoundé University Hospital Centre (CHUY) between February 2020 and June 2020. The study saw participants complete a questionnaire containing socio-demographic data and assessing acceptance and experience with the infection. Data analysis was accomplished using Epi info software version 7.2.2.6. Results: One hundred and thirteen HIV-infected adolescents were included in the study. The sex ratio was 0.68 and the mean age was 17 years. More than half of the adolescents had a good acceptance and positive experience with the infection. Related factors were the adolescent’s perception of good health and participation in an association with other infected adolescents. Conclusion: Emphasizing the psychological and educational follow-up of infected adolescents and encouraging their participation in associations for adolescents living with HIV could reduce the consequences of poor acceptance and ensure a better transition to adulthood. .
文摘BACKGROUND Ameloblastomas are common benign epithelial odontogenic neoplasms that present an aggressive and unpredictable behavior that may modify treatment strategies.Different signaling pathways that participate in the progression of these tumors have been identified.B-raf proto-oncogene serine/threonine kinase(BRAF)is a protein involved in the behavior of ameloblastomas,and it is related to many cell mechanisms.BRAF gene mutations have been identified in ameloblastomas,of which the BRAF V600E(valine substituted by glutamic acid at amino acid 600)mutation has been the most common and can be present concomitantly with other mutations that may be involved in its behavior.Targeted therapies have been used as an alternative in the case of resistance or contraindications to conventional treatments.AIM To document the presence of BRAF V600E and additional mutations,their behavior,and targeted therapies in these tumors.METHODS An electronic literature search was conducted according to PRISMA guidelines in PubMed/MEDLINE,Cochrane,EMBASE,and SpringerLink using the terms“ameloblastomas”,“BRAF V600E”,“additional mutations”,and“targeted therapies”.Ameloblastomas were classified according to WHO guidelines.Inclusion criteria were articles in English,published not more than 10 years ago,and studies with laboratory works related to BRAF V600E.Articles were evaluated by two independent reviewers and retrieved for full-text evaluation.The EBLIP Critical Appraisal Checklist was used to evaluate the quality of the eligible studies.Descriptive statistical analysis was performed.RESULTS Two independent reviewers,with a substantial concordance indicated by a kappa coefficient of k=0.76,evaluated a total of 19 articles that were included in this study.The analysis registered 521 conventional ameloblastomas(AM),81 unicystic ameloblastomas(UA),13 ameloblastic carcinomas(AC),three metastatic ameloblastomas(MA),and six peripheral ameloblastomas(PA),of which the histopathological type,anatomic location,laboratory tests,exp
文摘Introduction: The United Nations Sustainable Development Goals (SDGs) aim to decrease the global maternal mortality ratio to below 70 per 100,000 live births and eliminate preventable deaths of newborns and children under the age of five in all countries by 2030. The pediatric department at Spécialisé Mère-Enfant Blanche Gomes Hospital (HSMEBG) is divided into two sectors, one catering to children aged one month to four years and the other dedicated to children aged five to 17 years. According to department records, over the past three years, there has been an average of 1050 hospitalizations per year, with an average duration of five days. Objectives: This study aims to describe the socio-demographic characteristics of children who died while in the pediatrics department of the HSMEBG and analyze the factors associated with their deaths. Methodology: A retrospective analytical cross-sectional study was conducted, collecting data over a three-year period, covering the years 2019, 2020 and 2021. Data were collected from medical records of deceased children using Excel software version 2016, and statistical calculations and logistic regression were performed using Epi info software version 7.2.5.0. Results: During the three years of operation, the pediatric department at HSMEBG recorded 3060 new admissions, of which 271 resulted in death, representing an overall frequency of 8.8%. December and January had the highest mortality rates, accounting for 15.5% and 12.5%, respectively. Out of the 271 recorded deaths, 143 (52.77%) occurred in children under the age of one, and 230 (84.87%) occurred in children under the age of five. The average age at death was 2.4 years, ranging from one month to 17 years. The sex ratio was 1. More than half of the deaths (51.66%) occurred during the night, and 165 (60.89%) sought medical help more than three days after the onset of symptoms. 文摘The simulation of the electromagnetic wave propagation plays an important role in predicting the performance of wireless transmission and communication systems. This research paper performs a numerical simulation using the finite element method (FEM) to study electromagnetic propagation through both conductive and dielectric media. The simulations are made using the COMSOL Multiphysics software which notably implements the finite element method. The microwave is produced by a Vivaldi antenna at the respective frequencies of 2.6 and 5 GHz and the propagation equation is formulated from Maxwell’s equations. The results obtained show that in the air, strong electric fields are observed in the slot and the micro-strip line for the two frequencies, they are even greater when the wave propagates in the glass and very weak for the copper. The 3D evolutions of the wave in air and glass present comparable values at equal frequencies, the curves being more regular in air (dielectric). The radiation patterns produced for air and glass are directional, with a large main lobe, which is narrower at 5 GHz. For copper, the wave propagation is quite uniform in space, and the radiation patterns show two main lobes with a much larger size at 2.6 GHz than at 5 GHz. The propagation medium would therefore influence the range of values of the gain of the antenna.
文摘Chronic kidney disease is commonly diagnosed in dogs,and clinical signs may be aggravated when infected agents are involved.In this case report,33 dogs with chronic kidney disease were clinically evaluated and serologically tested for Leptospira spp.,Ehrlichia canis,and Anaplasma phagocytophilum.The seroprevalence for Leptospira spp.was 39.4%.The most frequent serovars found were Pyrogenes,Canicola,Bratislava and Australis,with serological titers between 1:100 to 1:800.Clinical signs included fever,depression,decreased body condition,vomiting and hema‑turia.Signifcant laboratory fndings were anemia,leukocytosis,thrombocytopenia,increased liver enzymes,urea and creatinine,hyperbilirubinemia and hyperphosphatemia.All leptospira seronegative dogs were positive for one or both monitored homoparasites(i.e.,E.canis and A.phagocytophilum);only three leptospira seropositive dogs were positive for one or both hemoparasites.Findings also suggest that endemic hemoparasites of dogs should be moni‑tored in dogs with a kidney condition for a better clinical picture of the patients and therapeutic approach.

文摘Photocatalytic synthesis of hydrogen peroxide has gradually become a promising method for in-situ pro-duction of hydrogen peroxide,which relies on sustainable solar energy.However,the commonly used photocatalyst,i.e.,carbon nitride(CN),still suffers from the drawbacks of narrow light absorption range and fast charge recombination.Here,we report a facile method to introduce nitrogen defects into carbon nitride together with sodium ion.By adjusting the ratio of sodium dicyandiamide,the band gap of carbon nitride can be controlled,while the carrier separation and transfer ability of carbon nitride is improved.The modified CN with sodium doping and nitrogen defect(SD-CN)demonstrates outstanding H_(2)O_(2)pro-duction performance(H_(2)O_(2)yield rate of 297.2μmol L^(−1)h^(−1))under visible light irradiation,which is approximately 9.8 times higher than that of pristine CN.This work deepens the understanding of the coordinated effect of structural defect and element doping of carbon nitride on the photocatalytic H_(2)O_(2)production performance,and provides new insight into the design of photocatalytic system for efficient production of H_(2)O_(2).
文摘The Pennsylvanian unconformity,which is a detrital surface,separates the beds of the Permian-aged strata from the Lower Paleozoic in the Central Basin Platform.Seismic data interpretation indicates that the unconformity is an angular unconformity,overlying multiple normal faults,and accompanied with a thrust fault which maximizes the region's structural complexity.Additionally,the Pennsylvanian angular unconformity creates pinch-outs between the beds above and below.We computed the spectral decomposition and reflector convergence attributes and analyzed them to characterize the angular unconformity and faults.The spectral decomposition attribute divides the broadband seismic data into different spectral bands to resolve thin beds and show thickness variations.In contrast,the reflector convergence attribute highlights the location and direction of the pinch-outs as they dip south at angles between 2° and 6°.After reviewing findings from RGB blending of the spectrally decomposed frequencies along the Pennsylvanian unconformity,we observed channel-like features and multiple linear bands in addition to the faults and pinch-outs.It can be inferred that the identified linear bands could be the result of different lithologies associated with the tilting of the beds,and the faults may possibly influence hydrocarbon migration or act as a flow barrier to entrap hydrocarbon accumulation.The identification of this angular unconformity and the associated features in the study area are vital for the following reasons:1)the unconformity surface represents a natural stratigraphic boundary;2)the stratigraphic pinch-outs act as fluid flow connectivity boundaries;3)the areal extent of compartmentalized reservoirs'boundaries created by the angular unconformity are better defined;and 4)fault displacements are better understood when planning well locations as faults can be flow barriers,or permeability conduits,depending on facies heterogeneity and/or seal effectiveness of a fault,which can affect hydrocarbon production.The methodol

文摘Integrins are allosteric cell adhesion receptors that cycle from a low to a high affinity ligand binding state,a complex process of receptor activation that is of particular importance in blood cells such as platelets or leukocytes.Here we highlight recent progress in the understanding of the molecular pathways that regulate integrin activation in platelets and leukocytes,with a special focus on the structural changes in platelet integrinαIIbβ3 brought about by key intracellular proteins,namely talin and kindlins,that are of crucial importance in the regulation of integrin function.Evidence that the small GTPase Rap1 and its guanine exchange factor CalDAG-GEF1,together with RIAM,a Rap1GTP adaptor protein,promote the interaction of talin with the integrinβsubunit,has greatly contributed to fill the gap in our understanding of the signaling pathway from G-coupled agonist receptors and their phospholipase C-dependant second messengers,to integrin activation.Studies of patients with the rare blood cell disorder LAD-III have contributed to the identification of kindlins as new co-regulators of the talin-dependent integrin activation process in platelets and leukocytes,underlining the relevance for the in-depth investigation of patients with rare genetic blood cell disorders.

文摘Labeo victorianus(Boulenger,1901)is one of the endemic fishes in Lake Victoria Basin(LVB)but is now threatened by multiple stressors caused by human activities.We investigated spatial and temporal variability in food composition and condition of L.victorianus in influent rivers of Lake Victoria,Kenya.Sampling was done during the dry and wet seasons by electrofishing.Food composition analysis showed that L.victorianus is a benthophagus and omnivorous species whose diet is dominated by detritus,periphyton and insects.There were differences in food composition among rivers,with significant river X season interactions(PERMANOVA F=11.6,df=4,p=0.001),suggesting that the diet depended on prevailing environmental conditions.In turbid rivers,the diet was dominated by detritus while in less turbid rivers it was dominated by insects and periphyton.Sand and mud also formed a significant part of the diet,which was an indication of a limited occurrence of preferable food items.There were ontogenetic shifts in food composition(PERMANOVA F=4.6,df=3,p=0.001),but also with a spatial interaction(PERMANOVA F=5.6,df=7,p=0.001),further indicating the role of environmental conditions in determining the diet for different size classes.Interestingly,the fish condition did not differ among rivers.This study shows that turbidity and organic matter and nutrient loading determine the diet of L.victorianus in LVB rivers,and provides further justification for the maintenance of water quality as a conservation measure for threatened species.
文摘Dear Editor,Infertility affects about 15%of couples of childbearing age.i About half of these cases can be attributed predominantly to a male factor,such as a quantitative or qualitative impairment in spermatogenesis.The etiology of male infertility is related to anatomic,genetic,and environmental causes,being sometimes multifactorial and of unknown or idiopathic origins.At present,genetic screening for nonobstructive azoospermia(NOA)is generally limited to karyotyping for the identification of chromosome abnormalities such as 47,XXY,46,X,der(X)t(X;Y)(p22.3;p11.2),chromosome rearrangements,and Y chromosome microdeletions.Whole-genome analyses have shown that over 100 gene variants are associated with a male infertility phenotype,and the list continues to grow;hence,a large number of genes are involved in spermatogenesis.The continuing identification of genes responsible for male infertility in general and azoospermia,in particular,is a major challenge in both research and the diagnosis ofmale infertility.
文摘Treatment options for ichthyosis and ichthyosiform disorders are limited and often unsatisfactory.Twenty-four patients used combination creams of 2%cholesterol with 2%lovastatin,and 10%glycolic acid with 0.025% tretinoin and 2%ketoconazole daily.At one month(n=20),the average percent reduction in severity scores was 40%,and at three months(n=10),it was 60.3%.Side effects were mainly mild irritation.These findings suggest that these two combination creams could be beneficial in the treatment of ichthyosiform disorders.
