METHODS: One hundred and sixty nine patients with pyogenic liver abscess managed at Sher-i-Kashmir In- stitute of Medical Sciences, Srinagar, Kashmir (India) from July 2001 to August 2006 were studied to evaluate and define the optimum treatment. RESULTS: Mortality in the surgically treated group of patients was 9.4% (12/119), while those treated non-surgically had a fatality rate of 16.66% (7/42). Multiple liver abscesses treated surgically had a surprisingly low mortality of 30%. The biliary tract (64.97%) was the most common cause of liver abscess. Multiple abscesses, mixed organisms and abscess complications are all associated with a significantly increased mortality. However, the lethality of the primary disease process was the most important factor in determining survival.CONCLUSION: Transperitoneal surgical drainage and antibiotics are the mainstay of treatment. Percutaneous drainage is recommended for high risk patients only.展开更多

HPA is caused by invasion and migration of nematode,Ascaris lumbricoides,in to the biliary tract and pancreatic duct. Patients present with biliary colic, cholangitis, cholecystitis, hepatic abscesses and acute pancreatitis. Ascarides traverse the ducts repeatedly, get trapped and die, leading to formation of hepatolithiasis. HPA is ubiquitous in endemic regions and in Kashmir, one such region,HPA is the etiological factor for 36.7%,23%,14.5% and 12.5% of all biliary diseases, acute pancreatitis, liver abscesses and biliary lithiasis respectively. Ultrasonography is an excellent diagnostic tool in visualizing worms in gut lumen and ductal system. The rational treatment for HPA is to give appropriate treatment for clinical syndromes along with effective anthelmintic therapy. Endotherapy in HPA is indicated if patients continue to have symptoms on medical therapy or when worms do not move out of ductal lumen by 3 wk or die within the ducts. The worms can be removed from the ductal system in most of the patients and such patients get regression of symptoms of hepatobiliary and pancreatic disease.展开更多

These changes occur as a result of pressure on bile ducts from bridging tortuous paracholedochal, epicholedochal and cholecystic veins. Bile duct ischemia may occur due prolonged venous pressure effect or result from insufficient blood supply. In addition, encasement of ducts may occur due fibrotic cavernoma. Majority of patients are asymptomatic. Portal biliopathy is a progressive disease and patients who have long standing disease and more severe bile duct abnormalities present with recurrent episodes of biliary pain, cholangitis and cholestasis. Serum chemistry, ultrasound with color Doppler imaging, magnetic resonance imaging with magnetic resonance cholangiopancreatography and magnetic resonance portovenography are modalities of choice for evaluation of portal biliopathy. Endoscopic retrograde cholangiography being an invasive procedure is indicated for endotherapy only. Management of portal biliopathy is done in a stepwise manner. First, endotherapy is done for dilation of biliary strictures, placement of biliary stents to facilitate drainage and removal of bile duct calculi. Next portal venous pressure is reduced by formation of surgical porto-systemic shunt or transjugular intrahepatic portosystemic shunt. This causes significant resolution of biliary changes. Patients who persist with biliary symptoms and bile duct changes may benefit from surgical biliary drainage procedures(hepaticojejunostomy or choledechoduodenostomy).展开更多

But here the betul belt has been presented with a different qualification, which is graphite mineralization. The graphite mineralization is popularly known within the proterozoic sediments of Aravalli Supegroup of rocks in South Rajasthan and in its extension in the Jhabua and Alirajpur districts of Madhaya Pradesh. The graphite mineralization in Betul belt is confined to the supracrustals enclosed within the granite gneisses and has been brought on the national map after the recently explored blocks by geological survey of India, which were auctioned for the commercial exploitation. This paper briefly discuses the possible mechanism of formation and the nature of mineralization of graphite here in this belt, essentially based on the inferences drawn from petrography supported by scanning electron microscopic and XRD data. It is concluded that major proportion of the graphite mineralization here is accumulated due to progressive metamorphism of the organic matter deposited along with various sediments, while a considerable component has been contributed by the precipitation from super critical graphite rich fluids.展开更多

Mangroves of Indus delta (PQA) provide immense benefits, products and unrecognized regulatory services. Products of Direct Economic Value: Fish, Minerals and food. There are Products of Natural System (intangible) supplies of rich nutrients to support productivity. Coastal communities benefit in a multitude of ways from Mangrove ecosystems. Collectively, these benefits are known as ecosystem services. The ecological role of mangrove ecosystems in the Indus Deltaic area of Port Qasim Authority (PQA) is, economically and socially significant. The Mangroves are well known for their high biological productivity and their consequent importance provided to the coastal community in terms of services and products of direct and indirect value to the adjacent coastal waters. Mangrove Ecosystem services are regularly involved in the provisioning of food and services and the decomposition of organic wastes. They export organic matter, mainly in detritus form of leaf litter to the marine environment, thus providing a highly nutritious food source for themselves and for the Benthic and terrestrial animals found in the mangrove areas, as well as for those in neighboring estuarine and marine ecosystems. Apart from nutrient export, mangroves also contribute to offshore fisheries by acting as nurseries and shelters for many species of commercially important finfish and crustaceans. Degradation of Indus Deltaic mangrove in PQA would destabilize the economic potential and the livelihood of communities’ which include services and benefits offered by the mangrove ecosystem. Rehabilitation and conservation of mangroves ecosystem in PQA is essential for sustained biological productivity in the region.展开更多

Pullulan esterified with succinic anhydride using dimethylaminopyridine showed a fairly high degree of substitution value as confirmed by1 H NMR spectroscopy. Pullulan succinate(Pull-Suc) was converted into the sodium salt(Pull-Suc-Na). The effect of contact time(5–200 min) and p H(2–8) on Cd-uptake by the sorbent(Pull-Suc-Na) was investigated. The sorbent showed more than 90% Cd-removal in first 15 min from distilled water(DW) and GW solution, respectively. Comparison of Pull-Suc-Na with other polysaccharidal sorbents suggested its high efficiency(DW 476.2 mg/g and GW 454.5 mg/g) and selectivity for the removal of Cd by an ion exchange mechanism, which is further supported by the negative Gibbs free energy values calculated from Langmuir isotherms. A Langmuir isotherm kinetic model provided the best fit for the sorption of Cd using Pull-Suc-Na. The sorbent showed a negligible decrease in Cd-uptake over three regeneration cycles. The thermal stability testing of the sorbents indicated that Pull-Suc-Na(sorbent) is more stable than Pull-Suc.展开更多

The spectral data of the complexes are consistent with the coordination of the thiones to antimony(III). The crystal structure of one of them, {[Sb(Imt)2Cl2]2(u2-Imt)}Cl2 (1), was determined by X-ray crystallography, which shows that the complex is dinuclear consisting of two [Sb(Imt)2Cl2] units bridged by an Imt molecule. In 1, the antimony atom is bonded to two chlorine atoms, two sulfur atoms of coordinated Imt molecules and one sulfur atom of a bridging Imt molecule. The antimony environment can be considered to be distorted octahedral with one Cl- ion weakly bound to antimony.展开更多

The P-dopant P430 was sprayed on ZnO pellets and annealed at different temperatures from 500 to 1000 ℃ with a step of 100 ℃ for one hour using a programmable furnace. The concentration of P was controlled by varying the annealing temperature and the maximum solubility of P (3% At) was achieved at annealing 800 ℃ determined by energy dispersive X-ray diffraction (EDX) measurements. X-ray diffraction (XRD) confirmed the hexagonal structure of ZnO and showed that the growth direction was along the c-axis. We observed a maximum up shift in the (002) plane at an annealing temperature of 800 ℃, suggesting that P atoms replaced Zn atoms in the structure which results in the reduction of the lattice constant. Room temperature photoluminescence (PL) spectrum consists of a peak at 3.28 eV and related to band edge emission, but samples annealed at 800 and 900 ℃ have an additional donor acceptor pair peak at 3.2 eV. Hall effect measurements confirmed the p-type conductivity of the sample annealed at 800 ℃.展开更多

文摘Colorectal cancer(CRC)is a predominant life-threatening cancer, with liver and peritoneal metastases as the primary causes of death.Intestinal inflammation,a known CRC risk factor, nurtures a local inflammatory environment enriched with tumor cells, endothelial cells, immune cells,cancer-associated fibroblasts, immunosuppressive cells, and secretory growth factors. The complex interactions of aberrantly expressed cytokines, chemokines, growth factors, and matrix-remodeling enzymes promote CRC pathogenesis and evoke systemic responses that affect disease outcomes.Mounting evidence suggests that these cytokines and chemokines play a role in the progression of CRC through immunosuppression and modulation of the tumor microenvironment, which is partly achieved by the recruitment of immunosuppressive cells. These cells impart features such as cancer stem cell-like properties, drug resistance, invasion, and formation of the premetastatic niche in distant organs, promoting metastasis and aggressive CRC growth.A deeper understanding of the cytokineand chemokine-mediated signaling networks that link tumor progression and metastasis will provide insights into the mechanistic details of disease aggressiveness and facilitate the development of novel therapeutics for CRC.Here,we summarized the current knowledge of cytokine-and chemokine-mediated crosstalk in the inflammatory tumor microenvironment, which drives immunosuppression, resistance to therapeutics, and metastasis during CRC progression. We also outlined the potential of this crosstalk as a novel therapeutic target for CRC.The major cytokine/chemokine pathways involved in cancer immunotherapy are also discussed in this review.

文摘Extracellular vesicles(EVs)are nano-sized,membranous structures secreted into the extracellular space. They exhibit diverse sizes, contents, and surface markers and are ubiquitously released from cells under normal and pathological conditions.Human serum is a rich source of these EVs,though their isolation from serum proteins and non-EV lipid particles poses challenges. These vesicles transport various cellular components such as proteins,mRNAs,miRNAs,DNA,and lipids across distances, influencing numerous physiological and pathological events, including those within the tumor microenvironment(TME).Their pivotal roles in cellular communication make EVs promising candidates for therapeutic agents, drug delivery systems, and disease biomarkers.Especially in cancer diagnostics,EV detection can pave the way for early identification and offers potential as diagnostic biomarkers. Moreover, various EV subtypes are emerging as targeted drug delivery tools, highlighting their potential clinical significance. The need for non-invasive biomarkers to monitor biological processes for diagnostic and therapeutic purposes remains unfulfilled.Tapping into the unique composition of EVs could unlock advanced diagnostic and therapeutic avenues in the future. In this review,we discuss in detail the roles of EVs across various conditions, including cancers(encompassing head and neck, lung, gastric, breast, and hepatocellular carcinoma),neurodegenerative disorders, diabetes, viral infections, autoimmune and renal diseases, emphasizing the potential advancements in molecular diagnostics and drug delivery.

文摘Hepatobiliary and pancreatic ascariasis(HPA) was described as a clinical entity from Kashmir,India in 1985. The worms can be removed from the ductal system in most of the patients and such patients get regression of symptoms of hepatobiliary and pancreatic disease.

文摘Portal biliopathy refers to cholangiographic abnormalities which occur in patients with portal cavernoma. Patients who persist with biliary symptoms and bile duct changes may benefit from surgical biliary drainage procedures(hepaticojejunostomy or choledechoduodenostomy).

文摘We examined the transverse momentum(pT)spectra of various identified particles, encompassing both light-flavored and strange hadrons(π++π−,K++K−,p+p¯,ϕ,K0s,Λ+Λ¯,Ξ−+Ξ¯+,andΩ−+Ω¯+),across different multiplicity classes in proton-proton collisions(p-p)at a center-of-mass energy of s√=7 TeV.Utilizing the Tsallis and Hagedorn models, parameters relevant to the bulk properties of nuclear matter were extracted. Both models exhibit good agreement with experimental data.In our analyses,we observed a consistent decrease in the effective temperature(T)for the Tsallis model and the kinetic or thermal freeze-out temperature(T0)for the Hagedorn model,as we transitioned from higher multiplicity(class-I)to lower multiplicity(class-X).This trend is attributed to the diminished energy transfer in higher multiplicity classes.Additionally,we observed that the transverse flow velocity(βT)experiences a decline from class-I to class-X.The normalization constant, which represents the multiplicity of produced particles, was observed to decrease as we moved toward higher multiplicity classes. While the effective and kinetic freeze-out temperatures,as well as the transverse flow velocity, show a mild dependency on multiplicity for lighter particles, this dependency becomes more pronounced for heavier particles. The multiplicity parameter for heavier particles was observed to be smaller than that of lighter particles, indicating a greater abundance of lighter hadrons compared to heavier ones.Various particle species were observed to undergo decoupling from the fireball at distinct temperatures:lighter particles exhibit lower temperatures, while heavier ones show higher temperatures, thereby supporting the concept of multiple freeze-out scenarios. Moreover,we identified a positive correlation between the kinetic freeze-out temperature and transverse flow velocity,a scenario where particles experience stronger collective motion at a higher freeze-out temperature. The reason for this positive correlation is that,as the multiplicity i

文摘The transverse momentum distributions of charged hadrons produced in proton-proton collisions at center-of-mass energies(√s)of 0.9 TeV and 2.36 TeV,as measured by the CMS detector at the Large Hadron Collider(LHC),have been analyzed within various pseudorapidity classes utilizing the thermodynamically consistent Tsallis distribution. The fitting procedure resulted in the key parameters, namely, effective temperature(T),non-extensivity parameter(q),and kinetic freezeout volume(V).Additionally, the mean transverse momentum(<pT>)and initial temperature(T_(i))of the particle source are determined through the fit function and string percolation method, respectively. An alternative method is employed to calculate the kinetic freezeout temperature(T_(0))and transverse flow velocity(β_(T))from T.Furthermore, thermodynamic quantities at the freezeout, including energy density(ε),particle density(n),entropy density(s),pressure(P),and squared speed of sound(C_(s)^(2)),are computed using the extracted T and q.It is also observed that, with a decrease in pseudorapidity, all thermodynamic quantities except V and q increase. This trend is attributed to greater energy transfer along the mid pseudorapidity.q increases towards higher values of pseudorapidity, indicating that particles close to the beam axis are far from equilibrium.Meanwhile,V remains nearly independent of pseudorapidity. The excitation function of these parameters(q)shows a direct(inverse)correlation with collision energy. The ε,n,s,and P show a strong dependence on collision energies at low pseudorapidities.Explicit verification of the thermodynamic inequality ε≥3P suggests the formation of a highly dense droplet-like Quark-Gluon Plasma(QGP).Additionally, the inequality T_(i)>T>T_(0)is explicitly confirmed, aligning with the evolution of the produced fireball.

文摘In this study,a comprehensive analysis of jets and underlying events as a function of charged particle multiplicity in proton-proton(pp)collisions at a center-of-mass energy of √s=7 TeV is conducted.Various Monte Carlo(MC)event generators, including Pythia8.308,EPOS 1.99,EPOSLHC,EPOS4_(Hydro),and EPOS4_(noHydro),are employed to predict particle production. The predictions from these models are compared with experimental data from the CMS collaboration. The charged particles are categorized into those associated with underlying events and those linked to jets, and the analysis is restricted to charged particles with|η|<2.4 and p_T>0.25 GeV/c.By comparing the MC predictions with CMS data,we find that EPOS4_(Hydro),EPOSLHC,and Pythia8 consistently reproduce the experimental results for all charged particles, underlying events, intrajets, and leading charged particles. For charged jet rates with p_T^(ch.jet)>5 GeV/c,EPOS4_(Hydro)and Pythia8 perform exceptionally well.In the case of charged jet rates with p_T^(ch.jet)→30 GeV/c,EPOSLHC reproduces satisfactorily good results, whereas EPOS4 Hydro exhibits good agreement with the data at higher charged particle multiplicities compared to the other models. This can be attributed to the conversion of energy into flow when"Hydro=on"leading to an increase in multiplicity. The EPOSLHC model describes the data better owing to the new collective flow effects, correlated flow treatment, and parameterization compared to EPOS 1.99.However,the examination of the jet p_T spectrum and normalized charged p_T density reveals that EPOS4_(Hydro),EPOS4_(noHydro),and EPOSLHC exhibit good agreement with the experimental results, whereas Pythia8 and EPOS 1.99 do not perform as well owing to the lack of correlated flow treatment.
文摘AIM: To review the clinical presentation and surgical management of complicated hydatid cysts of the liver and to assess whether conservative surgery is adequate in the management of complicated hydatid cysts of liver.METHODS:The study was carried out at Sher-i-Kashmir Institute of Medical Science,Srinagar,Kashmir,India.Sixty nine patients with hydatid disease of the liver were surgically managed from April 2004 to October 2005 with a follow up period of three years. It included 27 men and 42 women with a median age of 35 years. An abdominal ultrasound, computed tomography and serology established diagnosis.Patients with jaundice and high suspicion of intrabiliary rupture were subjected to preoperative endoscopic retrograde cholangiography.Cysts with infection, rupture into the biliary tract and peritoneal cavity were categorized as complicated cysts.Eighteen patients(26%)had complicated cysts and formed the basis for this study.RESULTS:Common complications were infection(14%),intrabiliary rupture(9%)and intraperitoneal rupture(3%).All the patients with infected cysts presented with pain and fever. All the patients with intrabiliary rupture had jaundice, while only four with intrabiliary rupture had pain and only two had fever.Surgical procedures performed in complicated cysts were:infection-omentoplasty in three and external drainage in seven;intrabiliary rupture-omentoplasty in two and internal drainage in four patients. Two patients with intraperitoneal rupture underwent external drainage. There was no mortality. The postoperative morbidity was 50%in complicated cysts and 16%in uncomplicated cysts.CONCLUSION:Complicated hydatid cyst of the liver can be successfully managed surgically with good long term results.

文摘Fluid and effective fracture identification in reservoirs is a crucial part of reservoir prediction. The frequency-dependent AVO inversion algorithms have proven to be effective for identifying fluid through its dispersion property. However, the conventional frequency-dependent AVO inversion algorithms based on Smith&Gidlow and Aki&Richards approximations do not consider the acquisition azimuth of seismic data and neglect the effect of seismic anisotropic dispersion in the actual medium. The aligned fractures in the subsurface medium induce anisotropy. The seismic anisotropy should be considered while accounting for the seismic dispersion properties through fluid-saturated fractured reservoirs.Anisotropy in such reservoirs is frequency-related due to wave-induced fluid-flow(WIFF)between interconnected fractures and pores. It can be used to identify fluid and effective fractures(fluid-saturated)by using azimuthal seismic data via anisotropic dispersion properties. In this paper, based on Rüger’s equation,we derived an analytical expression in the frequency domain for the frequencydependent AVOAz inversion in terms of fracture orientation, dispersion gradient of isotropic background rock, anisotropic dispersion gradient, and the dispersion at a normal incident angle. The frequency-dependent AVOAz equation utilizes azimuthal seismic data and considers the effect of both isotropic and anisotropic dispersion.Reassigned Gabor Transform(RGT)is used to achieve highresolution frequency division data.We then propose the frequency-dependent AVOAz inversion method to identify fluid and characterize effective fractures in fractured porous reservoirs.Through application to high-qualified seismic data of dolomite and carbonate reservoirs, the results show that the method is useful for identifying fluid and effective fractures in fluid-saturated fractured rocks.
文摘Betul belt is known for its volcanic massive sulphide type of mineralization in the geological circles of India. It is concluded that major proportion of the graphite mineralization here is accumulated due to progressive metamorphism of the organic matter deposited along with various sediments, while a considerable component has been contributed by the precipitation from super critical graphite rich fluids.
文摘Mangroves of the Indus Delta are the world’s fifth largest assemblage, and the largest in the arid zone. Rehabilitation and conservation of mangroves ecosystem in PQA is essential for sustained biological productivity in the region.

文摘Helicobacter pylori infection has been significantly linked to Peptic Ulcer Disease and Gastric Cancer.Metabolomic fingerprinting may offer a principal way of early diagnosis and to understand the molecular mechanism of H.pylori-induced pathogenicity. The rationale of the study is to explore the underlying distinct metabolic mechanisms of H.pylori-induced PUD and to identify potential biomarkers for disease diagnosis and associated risks using Gas chromatography/mass spectrometry.GC/MS-based analytical method was used to compare metabolic profiles of healthy controls(N=20)and peptic ulcer patients(N=45).Acquired metabolomic data were analyzed by constructing a diagnostic model using principal component analysis and a non-parametric two-tailed paired Wilcoxon analysis to identify disease-specific metabolic biomarkers.A total of 75 low-molecular-weight endogenous metabolites were detected during comparative metabolomic analysis of PUD vs.healthy gut tissues, among which 16 metabolites are being proposed to be diagnostic markers of Human PUD.Perturbations related to amino acids, carbohydrates, fatty acids, organic acids, and sterol metabolism were significantly revealed during this differential metabolomic profiling. Results convincingly suggest that metabolic profiles can contribute immensely in early diagnosis of the disease and understanding molecular mechanisms of disease progression for predicting novel drug targets for prophylactic and anaphylactic measures.

文摘BACKGROUND Sinapic acid(SA)has been shown to have various pharmacological properties such as antioxidant,antifibrotic,anti-inflammatory,and anticancer activities. Its mechanism of action is dependent upon its ability to curb free radical production and protect against oxidative stress-induced tissue injuries.AIM To study the hepatoprotective effects of SA against lipopolysaccharide(LPS)/Dgalactosamine(D-GalN)-induced acute liver failure(ALF)in rats.METHODS Experimental ALF was induced with an intraperitoneal(i.p.)administration of 8μg LPS and 800 mg/kg D-GalN in normal saline.SA was administered orally once daily starting 7 d before LPS/D-GalN treatment.RESULTS Data showed that SA ameliorates acute liver dysfunction, decreases serum levels of alanine transaminase(ALT),and aspartate aminotransferase(AST),as well as malondialdehyde(MDA)and NO levels in ALF model rats. However, pretreatment with SA(20 mg/kg and 40 mg/kg)reduced nuclear factor kappalight-chain-enhancer of activated B cells(NF-κB)activation and levels of inflammatory cytokines(tumor necrosis factor-αand interleukin 6).Also,SA increased the activity of the nuclear factor erythroid-related factor 2/heme oxygenase-1(Nrf2/HO-1)signaling pathway.CONCLUSION In conclusion,SA offers significant protection against LPS/D-GalN-induced ALF in rats by upregulating Nrf2/HO-1 and downregulating NF-κB.
文摘We study the time varying co-movement patterns of the crypto-currency prices with the help of wavelet-based methods;employing daily bilateral exchange rate of four major crypto-currencies namely Bitcoin,Ethereum,Lite and Dashcoin.First,we identify Bitcoin as potential market leader using Wavelet multiple correlation and Cross correlation.Further,Wavelet Local Multiple Correlation for the given cryptocurrency prices are estimated across different time-scales.From the results,it is found that that the correlation follows an aperiodic cyclical nature, and the crypto-currency prices are driven by Bitcoin price movements.Based on the results obtained,we suggest that constructing a portfolio based on crypto-currencies may be risky at this point of time as the other crypto-currency prices are mainly driven by Bitcoin prices, and any shocks in the latter is immediately transformed to the former.

文摘Chemically modified pullulan was evaluated for its sorption efficiency and selectivity to remove cadmium(Cd) from spiked high-hardness groundwater(GW). The thermal stability testing of the sorbents indicated that Pull-Suc-Na(sorbent) is more stable than Pull-Suc.
文摘Antimony(III) complexes of thioamides [thioamides=thiourea (Tu), N,N'dimethylthiourea (Dmtu), tetramethylthiourea (Tmtu), imidazolidine-2-thione (Imt) and diazinane-2-thione (Diaz)] with the general formulae, Sb(thione)nCl3 (n= 1, 2, 2.5, 3) were prepared and characterized by elemental analysis, IR and NMR (1H, 13C) spectroscopic methods. The antimony environment can be considered to be distorted octahedral with one Cl- ion weakly bound to antimony.

文摘We have demonstrated the effect of annealing temperature on the diffusion density of phosphors in zinc oxide. Hall effect measurements confirmed the p-type conductivity of the sample annealed at 800 ℃.
