To improve the controllability and utilization of distributed energy resources(DERs),distribution-level electricity markets based on consumers’bidding and offers have been proposed.However,the transaction costs will dramatically increase with the rapid development of DERs.Therefore,in this paper,we develop an energy sharing scheme that allows users to share DERs with neighbors,and design a novel incentive mechanism for benefit allocation without users’bidding on electricity prices.In the energy sharing scheme,an aggregator organizes a number of electricity users,and trades with the connected power grid.The aggregator is aimed at minimizing the total costs by matching the surplus energy from DERs and electrical loads.A novel index,termed as sharing contribution rate(SCR),is presented to evaluate different users’contributions to the energy sharing.Then,based on users’SCRs,an efficient benefit allocation mechanism is implemented to determine the aggregator’s payment to users that incentivize their participation in energy sharing.To avoid users’bidding,we propose a decentralized framework for the energy sharing and incentive mechanism.Case studies based on real-world datasets demonstrate that the aggregator and users can benefit from the energy sharing scheme,and the incentive mechanism allocates the benefits according to users’contributions. 展开更多

With increasing incidence of diabetes, use of diabetes specific nutrition supplements (DSNS) is common for better management of the disease. To study effect of 12-week DSNS supplementation on glycemic markers, anthropometry, lipid profile, SCFAs, and gut microbiome in individuals with diabetes. Markers studied were glycemic [Fasting Blood Glucose (FBG), Post Prandial Glucose (PPG), HbA1c, Incremental Area under curve (iAUC), Mean Amplitude of Glycemic Excursions (MAGE), Time in/above Range (TIR/TAR)], anthropometry [weight, Body Mass Index (BMI), waist circumference (WC)], lipid profile, diet and gut health [plasma short chain fatty acids (SCFAs)]. N = 210 adults were randomized to receive either DSNS with standard care (DSNS + SC;n = 105) or standard care alone (SC alone;n = 105). After 12 weeks, significant differences between DSNS + SC versus SC alone was observed in FBG [−3 ± 6 vs 14 ± 6 mg/dl;p = 0.03], PPG [−35 ± 9 vs −3 ± 9 mg/dl;p = 0.01], weight [−0.6 ± 0.1 vs 0.2 ± 0.1 kg;p = 0.0001], BMI [−0.3 ± 0.1 vs 0.1 ± 0.1 kg/m2;p = 0.0001] and WC [−0.3 ± 0.2 vs 0.2 ± 0.2 cm;p = 0.01]. HbA1C and low-density lipoprotein (LDL) were significantly reduced in DSNS + SC [−0.2 ± 0.9;p = 0.04 and −5 mg/dl;p = 0.03] respectively with no change in control. Continuous Glucose Monitoring (CGM) reported significant differences between DSNS + SC versus SC alone for mean glucose [−12 ± 65 vs 28 ± 93 mg/dl;p < 0.01], TAR 180 [−9 ± 42 vs 7 ± 45 mg/dl;p = 0.04], TAR 250 [−3 ± 27 vs 9 ± 38 mg/dl;p = 0.05], iAUC [−192 (1.1) vs −48 (1.1) mg/dl;p = 0.03]. MAGE was significantly reduced for both DSNS + SC (−19 ± 67;p < 0.001) and SC alone (−8 ± 70;p = 0.04), with reduction being more pronounced for DSNS + SC. DSNS + SC reported a decrease in carbohydrate energy % [−9.4 (−11.3, −7.6) %;p < 0.0001] and amount [−47.4 (−67.1, −27.7) g;p < 0.0001], increased dietary fiber [9.5 (7.2, 11.8) g;p < 0.0001] and protein energy % [0.9 (0.5, 1.3) %;p < 0.0001] versus SC alone. Along with a comparison of pairwise entanglement and monogamy properties, we explore the geometric information contained in them by constructing their canonical steering ellipsoids. Hepatocellular carcinoma(HCC)is the most common form of liver cancer worldwide.It is caused by a variety of risk factors,most common ones being infection with hepatitis viruses,alcohol,and obesity.HCC often develops in the background of underlying cirrhosis,and even though a number of interventional treatment methods are currently in use,recurrence is fairly common among patients who have had a resection.Therefore,whole liver transplantation remains the most practical treatment option for HCC.Due to the growing incidence of HCC,intense research efforts are being made to understand cellular and molecular mechanisms of the disease so that novel therapeutic strategies can be developed to combat liver cancer.In recent years,it has become clear that innate immunity plays a critical role in the development of a number of liver diseases,including HCC.In particular,the activation of Toll-like receptor signaling results in the generation of immune responses that often results in the production of proinflammatory cytokines and chemokines,and could cause acute inflammation in the liver.In this review,the current knowledge on the role of innate immune responses in the development and progression of HCC is examined,and emerging therapeutic strategies based on molecular mechanisms of HCC are discussed. 展开更多

BACKGROUND:Only 105 cases of neuroendocrine tumor(NET)of the ampulla of Vater have been described, mostly as single case reports.The incidence of NET is rising.The changes in incidence may result from changes in detection.This study was to determine the relative incidence and clinicopathological characteristics of high- grade neuroendocrine carcinoma(small cell carcinoma and large cell carcinoma)of the ampulla of Vater at a single institution. METHODS:Sections from paraffin blocks of tumors of the ampulla of Vater taken from 45 patients who underwent Whipple’s procedure and 6 patients who underwent palliative bypass between September 2003 and January 2007 were subjected to immunohistochemical analysis.The clinical and pathological data from 5 patients diagnosed with NET of the ampulla of Vater were analyzed. RESULTS:The patients were 3 men and 2 women,ranging in age from 39 to 47 years(mean 44 years).Operative procedures included Whipple’s procedure in 4 patients and palliative bypass in 1 patient.Histopathological examination revealed large-cell neuroendocrine carcinoma in 2 patients,small cell carcinoma in 2,and carcinoid in 1.Three patients with high-grade neuroendocrine carcinoma who had undergone Whipple’s procedure died at postoperatively 7,11,and 13 months.The patient who had undergone palliative triple bypass died 3 months after surgery.CONCLUSIONS:The relative incidence of high-grade neuroendocrine carcinomas of the ampulla of Vater is higher than that generally expected.The tumors behave aggressively and have a dismal prognosis despite aggressive treatment. 展开更多

A quantitative cross-sectional survey design was used. The self-administered questionnaire included 18 items 5-point Likert scale to identify the challenges experienced by male nursing students in their maternity clinical practice, and 14 items to assess comfort level in performing procedures in maternity clinical areas. Participants were 93 male nursing students who had completed their maternity clinical posting in different maternity clinical areas as a part of their nursing program, at the College of Nursing, Kuwait. Results showed that male nursing students are not very keen on undergoing maternity clinical training (Chi-square analysis χ<sup>2</sup> = 96.939;P χ<sup>2</sup> = 108.638;P χ<sup>2</sup> = 5.297;P = 0.021). In conclusion, this study showed that being treated differently and being refused to provide care to clients were the main challenges faced by male nursing students during their maternity practice. The participants had a strong belief that they will not be accepted by maternity clients because of the gender difference and cultural influence. There is a need to design strategies in both academic and clinical settings to overcome barriers to male nursing students’ engagement in the various maternity care areas. Our findings also indicate the need to embrace gender diversity in maternity clinical practice and call for professionalism, gender and cultural awareness creation, and advocacy. 展开更多

BACKGROUND:?The incidence of hepaticojejunostomy stricture is 4%-10% in experienced centres. Many access loops have been designed and used to facilitate endoscopic intervention for this complication of hepaticojejunostomy. In this study, we investigated the effectiveness and safety of gastric access loop. METHODS:A retrospective analysis was conducted on 13 patients who had undergone hepaticojejunostomy with gastric access loop between June 1999 and September 2003. Eleven patients were followed up for a mean period of 51 months (range 20-81 months). Two patients were lost to follow up. RESULTS:?On follow-up, 8 patients had patent jejuno- gastrostomy (end to side anastomosis between Roux loop of jejunum and stomach) and hepaticojejunostomy. Three patients developed stricture of jejunogastrostomy at 41 months, 63 months and 81 months of follow-up. Among these 3 patients, one also had hepaticojejunostomy stricture. In the patient with hepaticojejunostomy stricture, dilatation of jejunogastrostomy stricture was attempted but failed. None of the patients had any evidence of bile gastritis/cholangitis. There was no procedure related morbidity/mortality. CONCLUSIONS:Further studies involving large numbers of patients are required before wide application of gastric access loop in hepaticojejunostomy though it is a safer option than percutaneous transhepatic manipulations or revision surgery. 展开更多

Automated segmentation of brain tumors using Magnetic Resonance Imaging(MRI)data is critical in the analysis and monitoring of disease development.As a result,gliomas are aggressive and diverse tumors that may be split into intra-tumoral groups by using effective and accurate segmentation methods.It is intended to extract characteristics from an image using the Gray Level Co-occurrence(GLC)matrix feature extraction method described in the proposed work.Using Convolutional Neural Networks(CNNs),which are commonly used in biomedical image segmentation,CNNs have significantly improved the precision of the state-of-the-art segmentation of a brain tumor.Using two segmentation networks,a U-Net and a 3D CNN,we present a major yet easy combinative technique that results in improved and more precise estimates.The U-Net and 3D CNN are used together in this study to get better and more accurate estimates of what is going on.Using the dataset,two models were developed and assessed to provide segmentation maps that differed fundamentally in terms of the segmented tumour sub-region.Then,the estimates was made by two separate models that were put together to produce the final prediction.In comparison to current state-of-the-art designs,the precision(percentage)was 98.35,98.5,and 99.4 on the validation set for tumor core,enhanced tumor,and whole tumor,respectively. 展开更多

Nowadays,the COVID-19 virus disease is spreading rampantly.There are some testing tools and kits available for diagnosing the virus,but it is in a lim-ited count.To diagnose the presence of disease from radiological images,auto-mated COVID-19 diagnosis techniques are needed.The enhancement of AI(Artificial Intelligence)has been focused in previous research,which uses X-ray images for detecting COVID-19.The most common symptoms of COVID-19 are fever,dry cough and sore throat.These symptoms may lead to an increase in the rigorous type of pneumonia with a severe barrier.Since medical imaging is not suggested recently in Canada for critical COVID-19 diagnosis,computer-aided systems are implemented for the early identification of COVID-19,which aids in noticing the disease progression and thus decreases the death rate.Here,a deep learning-based automated method for the extraction of features and classi-fication is enhanced for the detection of COVID-19 from the images of computer tomography(CT).The suggested method functions on the basis of three main pro-cesses:data preprocessing,the extraction of features and classification.This approach integrates the union of deep features with the help of Inception 14 and VGG-16 models.At last,a classifier of Multi-scale Improved ResNet(MSI-ResNet)is developed to detect and classify the CT images into unique labels of class.With the support of available open-source COVID-CT datasets that consists of 760 CT pictures,the investigational validation of the suggested method is estimated.The experimental results reveal that the proposed approach offers greater performance with high specificity,accuracy and sensitivity. 展开更多

Introduction:Anagen effluvium (AE) is caused by impaired mitotic activity of matrix cells in the hair follicles.It presents with sudden loss of hair and myelosuppression.The diagnosis is based on the clinical history,trichoscopy,and trichogram.Case presentation:A 20-year-old woman presented with sudden loss of hair following treatment with oral azathioprine.Evaluation of the TPMT were within normal limits both in genotype and phenotype levels.Hair pull test was positive.Trichoscopy revealed yellow dots and multiple black dots,empty yellow dots,dark lines,upright regrowing hairs,and circle hairs.Azathioprine was stopped,and minoxidil 5% and oral multivitamin were started.Posttreatment trichoscopic analysis revealed terminal hairs and upright regrowing hairs.Discussion:AE is characterized by abrupt loss of anagen hairs due to impaired metabolic activity.Trichoscopy in AE shows coudability hairs,black dots,exclamation mark hairs,multiple broken shafts,and regrowing hairs.Empty yellow dots,yellow dots with black dots,pigtail hairs,and dark lines were additional features in this study.Conclusion:Trichoscopy assists in the accurate diagnosis of AE.Adverse effects of azathioprine can be observed even with normal TPMT levels.Weekly monitoring is therefore of prime importance. 展开更多

Multicarrier Waveform(MCW)has several advantages and plays a very important role in cellular systems.Fifth generation(5G)MCW such as Non-Orthogonal Multiple Access(NOMA)and Filter Bank Multicarrier(FBMC)are thought to be important in 5G implementation.High Peak to Average Power Ratio(PAPR)is seen as a serious concern in MCW since it reduces the efficiency of amplifier use in the user devices.The paper presents a novel Divergence Selective Mapping(DSLM)and Divergence Partial Transmission Sequence(D-PTS)for 5G waveforms.It is seen that the proposed D-SLM and PTS lower PAPR with low computational complexity.The work highlighted a combination of multi-data block partial transmit schemes along with tone reservation.In this,an overlapping factor is used to determine the number of data blocks for every group.Here,considering only those data blocks that have minimum signal power,the use of DSLM and DPTS are required to eliminate the segment’s peaks.Simulation results reveal that the suggested hybrid technique proves to be better than the conventional PTS scheme.Furthermore,the power saving performance of FBMC and NOMA is compared with the Orthogonal Frequency Division Multiplexing(OFDM)waveform. 展开更多

Li7P3S11solid electrolytes with high lithium-ion conductivity are promising candidates for use in all-solidstate lithium batteries.However,this electrolyte’s poor interfacial compatibility with lithium electrodes causes unstable cyclability.In this study,in order to address this problem,(100-x)Li7P3S11-xLi2OHBr(x=0,2,5,10,20,30,40,and 50)electrolytes are prepared by a high energy ball-milling technique and heat-treatment process.The resulting(100-x)Li7P3S11-xLi2OHBr(x=2,5,10,20,30,40,and 50)electrolytes provide improved electrochemical performance with good cycling stability and a wide electrochemical window of up to 10 V(vs.Li/Li+).Moreover,these electrolytes have high ionic conductivity of 10-4–10-5S/cm at room temperature.Particularly,the 90Li7P3S11-10Li2OHBr electrolyte displays the highest conductivity of 4.4×10-4S/cm at room temperature as well as improved cyclability.Moreover,90Li7P3S11-10Li2OHBr shows decreased interfacial resistance between the solid electrolyte and cathode electrode,which was revealed by Electrochemical Impedance Spectroscopy(EIS)analysis.The initial discharge capacity of 90Li7P3S11-10Li2OHBr was found to be 135 m Ah/g when used in a In|solid electrolyte|Li(Ni0.6Co0.2Mn0.2)O2 all-solid-state lithium battery(ASSLB).Thus,we can conclude the addition of Li2OHBr into the Li7P3S11results in enhanced electrochemical properties. 展开更多

Aileron reversal and flutter related problems are due in part to the elasticity of a typical airplane. Structural dynamics of an aircraft wing due to its aeroelastic nature are characterized by partial differential equations. Controller design for these systems is very complex as compared to lumped parameter systems defined by ordinary differential equations. In this paper, a stabilizing statefeedback controller design approach is presented for the heave dynamics of a wing-fuselage model. In this study, a continuous actuator in the spatial domain is assumed. A control methodology is developed by combining the technique of “proper orthogonal decomposition” and approximate dynamic programming. The proper orthogonal decomposition technique is used to obtain a low-order nonlinear lumped parameter model of the infinite dimensional system. Then a near optimal controller is designed using the single-network-adaptive-critic technique. Furthermore,to add robustness to the nominal single-network-adaptive-critic controller against matched uncertainties, an identifier based adaptive controller is proposed. Simulation results demonstrate the effectiveness of the single-network-adaptive-critic controller augmented with adaptive controller for infinite dimensional systems. 展开更多

The cast components further subjected to evaluation of physical properties and machining tests using two grades of coated inserts and PCD inserts. BACKGROUND: Infected pancreatic necrosis is associated with high morbidity and mortality and is mandatory for surgical or radiological intervention. A selected group of patients with CT evidence of infected pancreatic necrosis and a comparatively lower APACHE score may be clinically stable throughout the course of their illness. METHODS: Case records of 52 patients with severe acute pancreatitis admitted from October 2000 to September 2005 were retrospectively analysed to assess the feasibility of conservative management of infected pancreatic necrosis. CT evidence of retroperitoneal air pockets, deteriorated clinical condition, sepsis and positive blood culture were used to diagnose infected pancreatic necrosis. RESULTS: In the 52 male patients reviewed, 24 patients had infected pancreatic necrosis. Eighteen patients who had progressively deteriorated clinical conditions required surgical intervention; five patients of whom (27.8%) died. Six patients with transient end organ dysfunction and stable clinical conditions were treated with prolonged administration of antibiotics and ICU support. All these patients recovered and discharged from the hospital, and no symptoms or readmission happened during follow-up of 6-44 months. CONCLUSIONS: Selected patients with infected pancreatic necrosis who are clinically stable with transient end organ dysfunction can be treated conservatively with a favourable outcome. Necrosectomy associated with high morbidityand mortality in these patients can be avoided. The need for intervention should be individualized and based on clinical conditions of the patients. 展开更多

AIM:To identify the genes induced and regulated by the MYC protein in generating tumors from liver stem cells.METHODS:In this study,we have used an immortal porcine liver stem cell line,PICM-19,to study the role of c-MYC in hepatocarcinogenesis.PICM-19 cells were converted into cancer cells(PICM-19-CSCs)by overexpressing human MYC.To identify MYC-driven differential gene expression,transcriptome sequencing was carried out by RNA sequencing,and genes identified by this method were validated using real-time PCR.In vivo tumorigenicity studies were then conducted by injecting PICM-19-CSCs into the flanks of immunodeficient mice.RESULTS:Our results showed that MYC-overexpressing PICM-19 stem cells formed tumors in immunodeficient mice demonstrating that a single oncogene was sufficient to convert them into cancer cells(PICM-19-CSCs).By using comparative bioinformatics analyses,we have determined that>1000 genes were differentially expressed between PICM-19 and PICM-19-CSCs.Gene ontology analysis further showed that the MYCinduced,altered gene expression was primarily associated with various cellular processes,such as metabolism,cell adhesion,growth and proliferation,cell cycle,inflammation and tumorigenesis.Interestingly,six genes expressed by PICM-19 cells(CDO1,C22orf39,DKK2,ENPEP,GPX6,SRPX2)were completely silenced after MYC-induction in PICM-19-CSCs,suggesting that the absence of these genes may be critical for inducingtumorigenesis.CONCLUSION:MYC-driven genes may serve as promising candidates for the development of hepatocellular carcinoma therapeutics that would not have deleterious effects on other cell types in the liver. 展开更多

The aim of this work is to evaluate the performance of three different linear dimensionality reduction techniques (DR) techniques namely principal component analysis (PCA), multi dimensional scaling (MDS) and linear discriminant analysis (LDA) on classification of cardiac arrhythmias using probabilistic neural network classifier (PNN). The design phase of classification model comprises of the following stages: preprocessing of the cardiac signal by eliminating detail coefficients that contain noise, feature extraction through daubechies wavelet transform, dimensionality reduction through linear DR techniques specified, and arrhythmia classification using PNN. Linear dimensionality reduction techniques have simple geometric representations and simple computational properties. Entire MIT-BIH arrhythmia database is used for experimentation. The experimental results demonstrates that combination of PNN classifier (spread parameter, σ = 0.08) and PCA DR technique exhibits highest sensitivity and F score of 78.84% and 78.82% respectively with a minimum of 8 dimensions. 展开更多
