A total of 70 PE patients and 70 controls were included in this study. All men were heterosexual, had no other disorders and were either married or in a stable relationship. PE was defined as ejaculation that occurred within 1 min of vaginal intromission. Genomic DNA from patients and controls was analyzed using polymerase chain reaction, and allelic variations of the promoter region of the serotonin transporter gene (5-HTTLPR) were determined. The 5-HTTLPR (serotonin transporter promoter gene) genotypes in PE patients vs. controls were distributed as follows: L/L 16% vs. 17%, L/S 30% vs. 53% and S/S 54% vs. 28%. We examined the haplotype analysis for three polymorphisms of the 5-HTTLPR gene: LL, LS and SS. The appropriateness of the allele frequencies in the 5-HTTLPR gene was analyzed by the Hardy-Weinberg equilibrium using the Z-test. The short (S) allele of the 5-HTTLPR gene was significantly more frequent in PE patients than in controls (P 〈 0.05). We suggest that the 5-HTTLPR gene plays a role in the pathophysiology of all primary PE cases. Further studies are needed to evaluate the relationship between 5-HTTLPR gene polymorphism and patient subgroup (such as primary and secondary PE) responses to selective serotonin reuptake inhibitors as well as ethnic differences.展开更多

METHODS: Fifty-five H pylori positive and 48 H pylori negative patients were enrolled. The homeostasis model assessment (HOMA) was used to assess insulin resistance. Serum total antioxidant capacity (TAC), total oxidant status (TOS) and oxidative stress index (OSI) were determined in all subjects. RESULTS: The total antioxidant capacity was significantly lower in H pylori positive group than in H pylori negative group (1.36 ± 0.33 and 1.70 ± 0.50, respectively; P < 0.001), while the total oxidant status and oxidative stress index were significantly higher in H pylori positive group than in H pylori negative group (6.79 ± 3.40 and 5.08 ± 0.95, and 5.42 ± 3.40 and 3.10 ± 0.92, respectively; P < 0.001). Insulin resistance was significantly higher in H pylori positive group than in H pylori negative group (6.92 ± 3.86 and 3.61 ± 1.67, res- pectively; P < 0.001). Insulin resistance was found to be significantly correlated with total antioxidant capacity (r = -0.251, P < 0.05), total oxidant status (r = 0.365, P < 0.05), and oxidative stress index (r = 0.267, P < 0.05). CONCLUSION: Insulin resistance seems to be associated with increased oxidative stress in H pylori infection. Further studies are needed to clarify the mechanisms underlying this association and elucidate the effectof adding antioxidant vitamins to H pylori eradication therapy on insulin resistance during H pylori infection.展开更多

The mean age at presentation was 63.7 y(range 22-87 y). Partial lamellar scleroconjunctivectomy(PLSC) was performed in 105(93.7%) cases and enucleation was performed in 7(6.3%) cases due to bulbus invasion as the first step treatment. Treatments used in addition to PLSC included cryotherapy in 78 eyes(74.3%), alcohol epitheliectomy in 57 eyes(54.3%) for presence of corneal involvement, and amniotic membrane transplantation in 17 eyes(16.2%) for ocular surface reconstruction. Topical mitomycin C was used in 10 patients(9.5%) and strontium-90(Str-90) treatment in 4(3.8%) patients because surgical margins were tumor positive at the histopathological examination. Postoperative histopathologic diagnoses were squamous cell carcinoma(52 cases), carcinoma in situ(44 cases), moderate conjunctival intraepithelial neoplasia(11 cases), and mild conjunctiva intraepithelial neoplasia(5 cases). At a mean follow-up of 20.1 mo, tumor recurrence was observed in 21(18.8%) cases. The rate of recurrence was found to be lower in cases that underwent supplemental cryotherapy compared to those that did not(P<0.001). There was no metastasis in any case. CONCLUSION: In our series, the recurrence rate is 18.8% and overall globe salvage rate is 90.2% for OSSN at relatively short-term follow-up.展开更多

Walnut biological and nutritional value is also enhanced by its valuable protein and rich in nutrient composition such as vitamins and minerals. The most important characteristic of walnut oil is the abundance of polyunsaturated fatty acids, which makes it a unique food because of high amount of linoleic acid. Due to having valuable protein, vitamins and minerals it enhances biological and nourishment value, also. Recent epidemiological studies showed that consumption of walnut reduce cardiovascular diseases due to the rich in antioxidant properties, valuable fatty acids and tochopherols contents. In Turkey, walnut production and consumption increases year by year. The kernel of walnut genotypes shows variability in terms of their fat, fatty acid and tocopherol profiles. In this paper, it was aimed to characterize 10 walnut (Juglans regia L.) cultivars (Bilecik, Chandler, Hartley, Howard, Maras 12, Maras 18, Midland, Pedro, Sen and Serr) based on their fatty acid profiles using GC (Gas Chromatography), tocopherol and its isomers by HPLC (High Performance Liquid Chromatography) and total phenol content with spectrometric methods. Among the walnut cultivars “Hartley” was the highest linoleic acid (64.56%) and “Howard” was the α-linolenic acid 13.26 (%). The highest values of α (38.76 μg/g), β + γ (312.19 μg/g) and δ-tocopherol (40.77 μg/g) and total phenol (349 mg GAE/100 g ext) content were detected in “Sen” cultivar. Obtained results might be significant for further breeding programme to improve rich in especially γ-tocopherol linoleic acids and total phenolic compounds.展开更多

Design and Methods: A total of 82 patients admitted to our clinic were divided into 2 groups according to whether they are having an attack or not. Peripheral blood samples were taken from all patients and their native thiol, disulphide and total thiol concentrations were measured by a newly developed automated method. In addition, their disulphide/native thiol, disulphide/total thiol and native thiol/total thiol ratios were calculated. Results: The average disulphide values, disulphide/native thiol and disulphide/total thiol ratios of the patients with MS who were having an attack were found to be significantly higher compared to those in remission (p = 0.001, p = 0.001, p = 0.001, respectively). Conclusion: Our results have shown that thiol-disulphide balance was shifted to the oxidative area during the relapse. These homeostasis parameters, which can be easily measured by this newly developed method at low costs, may be important in terms of showing the disease activity and presenting the underlying mechanisms of the disease.展开更多

In this work, spectral responsivity measurements of a mono-crystalline, a poly-crystalline, a CIGS thin film and a bifacial module were measured using xenon-based flash type solar simulator system and a set of band pass filters. For the comprehensive characterization of parameters that may influence the spectral responsivity measurements, initially the simulator system was characterized both optically and thermally according to the IEC60904-9 and IEC60891 standard requirements. The optical characterizations in terms of spectral match, spatial non-uniformity and temporal instability indicate that the measured results (~3.0%, ~0.30% and ~0.20%) according to the IEC 60904-9 standard’s classification requirements correspond to A+A+A+ classes. Moreover, thermal characterizations in terms of the temperature uniformity show that over the 2 × 2 m area temperature uniformity of simulator system’s light distribution (1ºC) is almost two times better than the IEC 60891 standard requirements (±2ºC). Next, PV modules were electrically stabilized according to the IEC 61215-2 standard requirement’s (stability test) to reduce the fluctuations in their electrical performance parameters. Then, using the band pass filters, temperature controlled xenon-based solar simulator system and a reference PV module of the spectral responsivity of PV modules were measured from 400 nm to 1100 nm with 50 nm steps with relative uncertainty of 10-3 level.展开更多

In this work, in order to precisely determine the effects of temperature on the electrical parameters of a monocrystalline PV module, the temperature controlled, xenon light based solar simulator system with irradiance attenuating masks was used. This solar simulator, according to the IEC 60904-9 standard in terms of spectral match, spatial non-uniformity and temporal instability has A+A+A+ classes which are two times better than the standard requirements for a solar simulator to be used in PV module measurements. Moreover, the thermal chamber used in this work is a closed type chamber with fast opening door for not allowing the distortion of temperature uniformity over the surface of PV modules under test. Within about 2 m × 2 m area within 15°C to 75°C temperature interval, the temperature uniformity obtained for this system is less than 1.0°C which is almost two times better than the IEC 60891 standard requirements (±2.0°C). The temperature and irradiance dependent measurements of the electrical performance parameters of a mono-crystalline PV module at various irradiance levels and the evaluation of its temperature coefficients [α (% °C-1), β (% °C-1) and δ (% °C-1)] were done by implementing the interpolation method described in IEC 60891 standard.展开更多

The exit temperature of extrusion profile increases depending on heat generated by friction and forming. This situation can cause to surface defects such as hot cracks and grain thickening after extrusion process. Cooling of the mold has a critical role in terms of dissipating heat close to the forming zone. Local internal cooling experiments were performed in the hot aluminium extrusion mold within the scope of this study. It has been obtained that exit temperature of the aluminium extrusion profile can be significantly reduced in experimental results. Also it was found that the reduction of press time (s) and increase of press speed (mm/s). This paper focuses the effect of nitrogen gas cooling on extrusion mold for process efficiency. The extrusion parameters were performed for comparative analysis. Its obtained that process ensures efficiency.展开更多

文摘We evaluated the genotypes of the serotonin transporter gene (5-HTT) in patients with premature ejaculation (PE) to determine the role of genetic factors in the etiopathogenesis of PE and possibly to identify the patient subgroups. Further studies are needed to evaluate the relationship between 5-HTTLPR gene polymorphism and patient subgroup (such as primary and secondary PE) responses to selective serotonin reuptake inhibitors as well as ethnic differences.

文摘AIM:To determine the frequency of patients’vision survival and prognostic factors and evaluate clinical features in rhino-orbital mucormycosis.METHODS:Forty-three eyes of 43 patients followed up with orbital mucormycosis infections were included in the study.Demographic characteristics of the patients, symptoms at admission, ophthalmologic and non-ophthalmologic examination findings, clinical findings during follow-up,medical and surgical procedures, and complications were recorded.Patient survival was determined by assessing the incidence of mortality, and vision survival was defined as achieving a final visual acuity of at least light perception.RESULTS:Twenty-seven(62.8%)patients were male, and 16(37.2%)were female. When the underlying disease status of the patients was examined,it was observed that all patients had an underlying disease and diabetes constituted the majority(65.2%).Periorbital swelling(69.8%)and ophthalmoplegia(53.5%)were the most common symptoms and findings at the admission of patients with mucormycosis infection. The disease resulted in death in 22(51.2%)patients.The presence of fever and shorter duration of antifungal therapy were associated with lower patient survival.Exenteration surgery was not found to be associated with the survival of the patients.Frozen eye, loss of pupillary light reflex, and development of central retinal artery occlusion were associated with lower vision survival.CONCLUSION:This study presents one of the most extensive patient series in the literature on rhino-orbital mucormycosis.Knowing the patients’symptoms at the time of admission and the clinical findings during the infection process will increase awareness about the disease.
文摘BACKGROUND:As advocated in advanced trauma life support and prehospital trauma life support protocols, cervical immobilization is applied until cervical spine injury is excluded. This study aimed to show the difference in optic nerve sheath diameter(ONSD)between patients with and without a cervical collar using computed tomography(CT).METHODS:This was a single-center,retrospective study examining trauma patients who presented to the emergency department between January 1,2021,and December 31,2021.The ONSD on brain CT of the trauma patients was measured and analyzed to determine whether there was a difference between the ONSD with and without the cervical collar.RESULTS:The study population consisted of 169 patients. On CT imaging of patients with(n=66)and without(n=103)cervical collars, the mean ONSD in the axial plane were 5.43±0.50 mm and 5.04±0.46 mm respectively for the right eye and 5.50±0.52 mm and 5.11±0.46 mm respectively for the left eye.The results revealed an association between the presence of a cervical collar and the mean ONSD,which was statistically significant(P<0.001)for both the right and left eyes.CONCLUSION:A cervical collar may be associated with increased ONSD.The effect of this increase in the ONSD on clinical outcomes needs to be investigated, and the actual need for cervical collar in the emergency department should be evaluated on a case-by-case basis.
文摘Heat and mass transfer of a circular-shaped porous moist object inside a two-dimensional triangle cavity is investigated by using finite element method. The porous object is considered to be a moist food sample, located in the middle of the cavity with inlet and outlet ports with different configurations of inlet/outlet ports.Convective drying performance is numerically assessed for different values of Reynolds numbers(between 50 and 250),dry air inlet temperature(between 40 and 80℃)and different locations of the port.It is observed that changing the port locations has significant impacts on the flow recirculaitons inside the triangular chamber while convective drying performance is highly affected. The moisture content reduces with longer time and for higher Reynolds number(Re)values.Case P4 where inlet and outlet ports are in the middle of the walls provides the most effective configuration in terms of convective drying performance while the worst case is seen for P1 case where inlet and outlet are closer to the corners of the chamber. There is a 192% difference between the moisture reduction of these two cases at Re=250,T=80℃ and t=120 min.
文摘Gastrointestinal stromal tumors are the most common mesenchymal tumors of the gastrointestinal tract. They originate from the interstitial cells of Cajal and are usually found in extrahepatic gastrointestinal sites. However,a small subset are derived from the liver and are known as primary hepatic gastrointestinal stromal tumors(PHGIST).They have a poor prognosis and are historically difficult to diagnose. Our objective was to review and update the latest evidence-based knowledge concerning PHGIST,with a focus on epidemiology, etiology, pathophysiology, clinical presentation, histopathology, and treatment. These tumors are usually found incidentally, occur sporadically, and are associated with mutations of KIT and PDGFRA genes.PHGIST is a diagnosis of exclusion,as it has the same molecular, immunochemistry and histological appearance as gastrointestinal stromal tumors(GIST).Thus, imaging, such as positron emission tomography-computed tomography(PET-CT)must be used to rule out metastatic GIST before a diagnosis can be made. However, with mutation analysis and pharmacological advances, tyrosine kinase inhibitors are typically pursued with or without surgical intervention.Other potential treatments include transcatheter arterial chemoembolization and tumor ablation. However, these are typically considered palliative options. As there are only a limited number of publications regarding PHGIST,data concerning morbidity and mortality are not yet available.Immunohistopathology can help develop screening guidelines and evaluating resistance to treatment.

文摘BACKGROUND The role of the Notch pathway in carcinogenesis and tumor progression has been demonstrated in many organs, including the colon.Accordingly, studies aimed at developing therapies targeting this pathway in various cancers require the identification of several factors that may play a role in regulating Notch-1 expression.Although Numb,Itch, and seven in absentia homolog-1(Siah-1)have been shown to contribute to the regulation of Notch signaling, their role in colorectal carcinogenesis and tumor progression has not been fully elucidated to date.AIM To evaluate Numb,Itch, and Siah-1 expression in colorectal tumors to clarify their relationship with Notch-1 expression and their role in carcinogenesis and tumor behavior.METHODS Expression of Notch-1,Numb,Itch,and Siah-1 was investigated in 50 colorectal carcinomas,30 adenomas, and 20 healthy colonic tissues by immunohistochemistry and quantitative real-time polymerase chain reaction(PCR)analyses.RESULTS In contrast to Notch-1,which is expressed at higher levels in tumor tissues and adenomas, expression of Numb,Itch, and Siah-1 was stronger and more frequent in normal mucosa(P<0.01).There was a positive correlation between Notch-1 expression and high histological grade, the presence of lymph node metastasis, and advanced-stage tumors, whereas expression of Numb,Itch, and Siah-1 was absent or reduced in tumors with these clinicopathological parameters(P<0.05).In survival analysis, expression of Notch was related to poor prognosis but that of Numb,Itch, and Siah-1 correlated with improved survival(P<0.05).Multivariate analysis revealed Notch-1 expression and loss of Numb expression to be independent prognostic parameters together with lymph node metastasis(P<0.05).CONCLUSION Our findings support the role of Notch-1 in colorectal carcinoma and indicate that loss of Numb,Itch, and Siah-1 expression is associated with carcinogenesis. Our data also suggest that these three proteins might be involved in the Notch-1 pathway during colorectal carcinoma(CRC)progression and might play
文摘The aim of this study is to have an analysis of the plot and the characters in the novel,Agnes Grey,in terms of the perspectives of education and culture.Anne Brontë’s first novel focuses on the life of Agnes, who is a young governess living in the Victorian era.In the novel, the narrative shows a period of time in English literature, when industrialization has a significant impact on the aspects such as social life and education,in a way in which women experienced many kinds of struggles for holding on to their lives and reaching their dreams.Agnes Grey, the main character of the novel, came from a middle-class family. She lived to tell the adventures of starting her career as a governess, and the difficulties she experienced. It didn’t take long for Agnes, who started her first experience with the Bloomfield family,to face the rich upper-class culture and see the difficulties in educating children from another social class. The Murray family, with whom she stayed for a longer time, was the place where Agnes gained expression that extended into the adulthood.Agnes’teaching shows not only a kind of class struggle, but also a balance between the dialectical poles of the ideal and the reality.Agnes returned,in her adulthood,to her family with her teaching experiences.By keeping a concept of love in her mind,Agnes would be able to have the sense of reality in which culture and education came to offer.
文摘AIM: To evaluate the risk factors leading to recurrence in patients with ocular surface squamous neoplasia(OSSN)METHODS: The records of 112 patients with OSSN who underwent treatment and follow-up between February 1999 and August 2018 were reviewed retrospectively.RESULTS: Totally 67 patients(59.8%) were male and 45 patients(40.2%) were female. CONCLUSION: In our series, the recurrence rate is 18.8% and overall globe salvage rate is 90.2% for OSSN at relatively short-term follow-up.
文摘AIM: To describe the clinical characteristics and late results of patients with retinopathy of prematurity(ROP)treated with "6h cryotherapy".·METHODS: Out of 1252 infants screened for ROP, 52 patients were treated with temporal 6h cryotherapy from1997 to 2005 were recalled to our clinic. Among these 23 patients were available and 46 eyes of 23 infants were included to evaluate for visual acuity, refractive error, ocular alignment, nystagmus, retinal examination(abnormal branching of retinal vessels, retinal thinning, latis degenerations, tortuosity of vessels, straightening of temporal vessels, narrowing of the angle of vessel in the juxtapapillary entrance, pigment changes, macular heterotopia), optic atrophy and optic disc cupping, axial length at birth and axial length at 1y.·RESULTS: The median age at examination was 7(5-18)y.In 32.6% of patients, the visual acuity was ≤20/200 and the mean best corrected visual acuity was 20/35 as measured with a Snellen chart. Mean spherical refractive error was-1.76±2.69 D. The degree of myopia at the last examination was found to be correlated with the elongation of the eye in the first year of life. Exotropia was present in 17.4%(n =8) of infants and esotropia in13%(n =6). The most common retinal abnormality was abnormal branching of retinal vessels(82.6%) followed by retinal thinning(52.2%).·CONCLUSION: The late clinical outcomes of infants with ROP treated in our clinic with cryotherapy seems to comparable with results of laser treatment.
文摘BACKGROUND Tongue abscess(TA)is a very rare clinical condition and its treatment is very important.Surgical drainage is at the forefront in the treatment. Our study includes patients with tongue and tongue base abscesses.AIM To discuss the clinical and laboratory findings of these patients emphasizing the underlying causes and treatment options with the largest patient series in the English literature.METHODS We included patients with isolated TA who applied to our clinic between January 1,2020 and January 1,2023.Those who lack the recorded data, those who are not between the ages of 18-66,those who have not undergone surgery-interventional procedure, and those who have infection and/or abscess in another place were excluded from the study.RESULTS There were two female(18%)and nine male(82%)patients in our series consisting of 11 patients.Their ages ranged from 18 to 66,and the mean±SD was 48.63±16.3.Considering the localization of the abscess, three anterior abscesses(27%),two lateral abscesses(18%),and six abscesses at the base of the tongue(54%)were detected.CONCLUSION Tongue abscesses can cause acute upper airway obstruction and respiratory collapse. It may be necessary to act quickly for the tracheotomy procedure and this procedure can usually be performed under local anesthesia as intubation cannot be achieved. When we encounter an abscess in an unexpected organ, difficulties may be encountered in the management of the patient.
文摘As a member of nuts, walnut is consumed from snacks to salads and desserts to entrees and an importantpart of human diet for centuries. Obtained results might be significant for further breeding programme to improve rich in especially γ-tocopherol linoleic acids and total phenolic compounds.
文摘Objective: The aim of this study was to evaluate thiol-disulphide homeostasis in patients with Multiple Sclerosis (MS) during attack and remission. These homeostasis parameters, which can be easily measured by this newly developed method at low costs, may be important in terms of showing the disease activity and presenting the underlying mechanisms of the disease.
文摘One of the most important parameter used for the evaluation of the energy rating of PV modules is, their spectral responsivities which are the measure of electrical performance parameters per incident solar radiation. Then, using the band pass filters, temperature controlled xenon-based solar simulator system and a reference PV module of the spectral responsivity of PV modules were measured from 400 nm to 1100 nm with 50 nm steps with relative uncertainty of 10-3 level.
文摘Photovoltaic (PV) modules performance testing and energy rating as described in IEC 61853-1 standard depend on electrical performance parameters (short-circuit current, open-circuit voltage, maximum-power) of PV modules as a function of temperature and irradiance. The temperature and irradiance dependent measurements of the electrical performance parameters of a mono-crystalline PV module at various irradiance levels and the evaluation of its temperature coefficients [α (% °C-1), β (% °C-1) and δ (% °C-1)] were done by implementing the interpolation method described in IEC 60891 standard.
文摘It is aimed to investigate the efficiency of nitrogen gas cooling which is used in aluminium extrusion mold in this study. Its obtained that process ensures efficiency.
