Total RNA was extracted from 23 HCC and 10 healthy liver tissues using mir Vana mi RNA Isolation Kit. microR NA-17-5p(miR-17-5p) expression was mimicked and antagonized in Hu H-7 cell lines using Hi Per Fect Transfection Reagent, then total RNA was extracted using Biozol reagent then reverse transcribed into cD NA followed by quantification of mi R-17-5p and IGFBP-3 expression using Taq Man real-time quantitative PCR. Luciferase reporter assay was performed to validate the binding of miR-17-5p to the 3'UTR of IGFBP-3. Free IGF-Ⅱ protein was measured in transfected Hu H-7 cells using IGF-Ⅱ ELISA kit. RESULTS: Bioinformatic analysis revealed IGFBP-3 as a potential target for miR-17-5p. Screening of miR-17-5p and IGFBP-3 revealed a moderate negative correlation in HCC patients, where mi R-17-5p was extensively underexpressed in HCC tissues(P = 0.0012), while IGFBP-3 showed significant upregulation in the same set of patients(P = 0.0041) compared to healthy donors. Forcing mi R-17-5p expression in Hu H-7 cell lines showed a significant downregulation of IGFBP-3 mR NA expression(P = 0.0267) and a significant increase in free IGF-Ⅱ protein(P = 0.0339) compared to mock untransfected cells using unpaired t-test. Luciferase assay validated IGFBP-3 as a direct target of mi R-17-5p; luciferase activity was inhibited by 27.5% in cells co-transfected with miR-17-5p mimics and the construct harboring the wild-type binding region 2 of IGFBP-3 compared to cells transfected with this construct alone(P = 0.0474).CONCLUSION: These data suggest that regulating IGF-Ⅱ bioavailability and hence HCC progression can be achieved through targeting IGFBP-3 via manipulating the expression of miR NAs.展开更多

An artificial neural network (ANN) simulation model is used to estimate the entropy per rapidity dS/dy at the considered energies. The simulation results are compared with equivalent experimental data, and a good agreement is achieved. A mathematical equation describing the experimental data is obtained. Extrapolation of the transverse momentum spectra at pT =0 is required to calculate dS/dy;thus, we use two different fitting functions, the Tsallis distribution and hadron resonance gas (HRG) model. The success of the ANN model in describing the experimental measurements leads to the prediction of several spectra values for the mentioned particles, which may lead to further predictions in the absence of experiments.展开更多

Objectives: Evaluation of diabetes mellitus and neuropathy effect on the urinary bladder dynamics in children and adolescents with Type 1 diabetes mellitus. Patients and Methods: The study included 80 children and adolescents with Type 1 diabetes for at least 5 years;60 patients were with manifestations of autonomic and/or peripheral neuropathy and 20 patients were free of either. We assessed both groups for presence of cystopathy by means of uroflowmetry and cystometry. Results: All patients with diabetic neuropathy had abnormal urodynamic test results of variable types and degrees with bladder hypercompliance as the most frequent abnormality. Other urodynamic abnormalities were found in both diabetic patients’ groups with no significant difference in frequency. Conclusions: Diabetic neuropathy might be strongly related to urodynamic abnormalities particularly the bladder hypercompliance. Some diabetic patients may have cystopathy in absence of evident neuropathy. This may be due to undetected neuropathy or diabetes induced myopathy of the detrusor muscle.展开更多

Initial diagnosis is accomplished most often by cross-sectional imaging. Endoscopic ultrasound with fine needle aspiration has become the preferred test to help distinguish pseudocyst from other cystic lesions of the pancreas. Most pseudocysts resolve spontaneously with supportive care. The size of the pseudocyst and the length of time the cyst has been present are poor predictors for the potential of pseudocyst resolution or complications, but in general, larger cysts are more likely to be symptomatic or cause complications. The main two indications for some type of invasive drainage procedure are persistent patient symptoms or the presence of complications (infection, gastric outlet or biliary obstruction, bleeding). Three different strategies for pancreatic pseudocysts drainage are available: endoscopic (transpapillary or transmural) drainage, percutaneous catheter drainage, or open surgery. To date, no prospective controlled studies have compared directly these approaches. As a result, the management varies based on local expertise, but in general, endoscopic drainage is becoming the preferred approach because it is less invasive than surgery, avoids the need for external drain, and has a high long-term success rate. A tailored therapeutic approach taking into consideration patient preferences and involving multidisciplinary team of therapeutic endoscopist, interventional radiologist and pancreatic surgeon should be considered in all cases.展开更多

Treating fulminant ARDS is currently relegated to supportive care measures only. Thus, the best treatment for ARDS may lie with preventing this syndrome from ever occurring. Clinical studies were examined to determine why ARDS has remained resistant to treatment over the past several decades. In addition, both basic science and clinical studies were examined to determine the impact that early, protective mechanical ventilation may have on preventing the development of ARDS in at-risk patients. Fulminant ARDS is highly resistant to both pharmacologic treatment and methods of mechanical ventilation. However, ARDS is a progressive disease with an early treatment window that can be exploited. In particular, protective mechanical ventilation initiated before the onset of lung injury can prevent the progression to ARDS. Airway pressure release ventilation(APRV) is a novel mechanical ventilation strategy for delivering a protective breath that has been shown to block progressive acute lung injury(ALI) and prevent ALI from progressing to ARDS. ARDS mortality currently remains as high as 45% in some studies. As ARDS is a progressive disease, the key to treatment lies with preventing the disease from ever occurring while it remains subclinical. Early protective mechanical ventilation with APRV appears to offer substantial benefit in this regard and may be the prophylactic treatment of choice for preventing ARDS.展开更多

Production plants may start on a small scale with small capital investment then increase productivity later when the economy permits without financial penalty. This is in contract to smelting operations which necessitates large scale production from the start with high capital investment that may not be available locally.展开更多

Nevertheless, ARDS mortality remains unacceptably high(> 40%). Indeed, once ARDS is established it becomes refractory to treatment, and therefore avoidance is key. However, preventive techniques and therapeutics to reduce the incidence of ARDS in patients at high-risk have not been validated clinically. This review discusses the current data suggesting that preemptive application of the properly adjusted mechanical breath can block progressive acute lung injury and significantly reduce the occurrence of ARDS.展开更多

Extrapolation of the transverse momentum spectra at pT =0 is required to calculate dS/dy;thus, we use two different fitting functions, the Tsallis distribution and hadron resonance gas (HRG) model. The success of the ANN model in describing the experimental measurements leads to the prediction of several spectra values for the mentioned particles, which may lead to further predictions in the absence of experiments.
文摘Recently,Multicore systems use Dynamic Voltage/Frequency Scaling(DV/FS)technology to allow the cores to operate with various voltage and/or frequencies than other cores to save power and enhance the performance. In this paper,an effective and reliable hybridmodel to reduce the energy and makespan in multicore systems is proposed. The proposed hybrid model enhances and integrates the greedy approach with dynamic programming to achieve optimal Voltage/Frequency(Vmin/F)levels.Then,the allocation process is applied based on the availableworkloads. The hybrid model consists of three stages. The first stage gets the optimum safe voltage while the second stage sets the level of energy efficiency, and finally, the third is the allocation stage.Experimental results on various benchmarks show that the proposed model can generate optimal solutions to save energy while minimizing the makespan penalty.Comparisons with other competitive algorithms show that the proposed model provides on average 48%improvements in energy-saving and achieves an 18%reduction in computation time while ensuring a high degree of system reliability.
文摘The impedance spectroscopy, electrical conductivity and electric modulus of bulk phenol red were measured,as a function of both frequency and temperature.Artificial neural networks(ANNs)were used for modeling its electrical properties. The two parts(real and imaginary)of its complex impedance(Z^*)were analyzed and the activation energy related to the electrical relaxation process was evaluated.Nyquist curves were plotted showing semicircles for the different temperatures. The AC electrical conductivity follows a power lawσac(ω)αω^η.The maximum barrier height Bm was derived for specific temperatures.A plausible mechanism for the AC conduction of bulk phenol red was deduced from the temperature reliance of the frequency exponent. The dielectric data was analyzed using electric modulus as a tool.In addition,ANNs were used to model the impedance parts and the total electrical conductivity.Numerous runs were tried,to obtain the best performance. The training and prediction results were compared to the equivalent experimental results, with a good match obtained. An equation describing the experimental results was obtained mathematically, based on the use of ANNs.The outputs demonstrated that ANNs are an admirable tool for modeling experimental results.
文摘Background: Ovarian sex cord-stromal tumors are an uncommon heterogeneous group of tumors with different biological behaviors and clinico-patho- logic aspects. Aim of the Work: This study will review the clinico-pathologic aspects of sex-cord stromal ovarian tumors at the National Cancer Institute (NCI), Cairo University, Egypt, as well as their management and follow-up regarding disease free survival and overall survival. Patients and Methods: This retrospective study was conducted at the National Cancer Institute Cairo University, Egypt on female patients with ovarian sex cord stromal tumors in the period from January 2008 to December 2012 with a follow-up period of 24 to 84 months. The age of the patients, different clinical presentations, radiological findings, associated uterine bleeding (need for endometrial biopsy), pre-operative CA125 levels, surgical management done, different histopathological types, different biological behaviors, presence of ascites (and its correlation with the histopathology), Adjuvant chemotherapy (according to biological behavior and pathological type), and follow-up of non-benign cases for up to 84 months will all be documented and studied. Results: The mean age at presentation was 47.34;abdominal pain and mass were the commonest presentations 54.5% and 53.2% respectively;the main radiologic findings were a pelvic mass +/- ascites which had no correlation to the pathological type (p = 0.075). 文摘Immunotherapy becomes a promising line of treatment for breast cancer(BC)however, its success rate is still limited. Methods:The study was designed to optimize the condition for producing an effective dendritic cell(DCs)based immunotherapy by using DCs and T lymphocytes together with tumor-infiltrating lymphocytes(TILs)and tumor-infiltrating DCs(TIDCs),treated with anti-PD1 and anti-CTLA4 monoclonal antibodies. This mixture of immune cells was co-cultured with autologous breast cancer cells(BCCs)isolated from 26 BC females. Results:There was a significant upregulation of CD86 and CD83 on DCs(p=0.001 and 0.017,respectively),similarly upregulation of CD8,CD4 and CD103 on T cells(p=0.031,0.027,and 0.011,respectively).While there was a significant downregulation of FOXP3 and combined CD25.CD8 expression on regulatory T cells(p=0.014 for both).Increased CD8/Foxp3 ratio(p<0.001)was also observed.CD133,CD34 and CD44 were downregulated on BCCs(p=0.01,0.021,and 0.015,respectively).There was a significant increase in interferon-γ(IFN-γ,p<0.001),lactate dehydrogenase(LDH,p=0.02),and a significant decrease in vascular endothelial growth factor(VEGF,p<0.001)protein levels.Gene expression of FOXP3 and Programmed cell death ligand 1(PDL-1)were downregulated in BCCs(p<0.001,for both),similarly cytotoxic T lymphocyte antigen-4(CTLA4,p=0.02),Programmed cell death 1(PD-1,p<0.001)and FOXP3(p<0.001)were significantly downregulated in T cells.Conclusion:Ex-vivo activation of immune cells(DCs,T cells,TIDCs,and TILs)with immune checkpoint inhibitors could produce a potent and effective BC immunotherapy. However, these data should be validated on an experimental animal model to be transferred to the clinical setting.
文摘Background: Urinary bladder dysfunction is a major complication of diabetes mellitus and its mechanism has been attributed to autonomic and/or peripheral neuropathy. This may be due to undetected neuropathy or diabetes induced myopathy of the detrusor muscle.
文摘Pancreatic pseudocysts are complications of acute or chronic pancreatitis. A tailored therapeutic approach taking into consideration patient preferences and involving multidisciplinary team of therapeutic endoscopist, interventional radiologist and pancreatic surgeon should be considered in all cases.
文摘Mortality from acute respiratory distress syndrome(ARDS) remains unacceptable, approaching 45% in certain high-risk patient populations. Early protective mechanical ventilation with APRV appears to offer substantial benefit in this regard and may be the prophylactic treatment of choice for preventing ARDS.
文摘This study aimed to investigate the potential cardioprotective effects of moderate and high-intensity aerobic interval training(MIIT and HIIT)preconditioning. The focus was on histological changes,pro-oxidant-antioxidant balance, autophagy initiation, and apoptosis in myocardial tissue incited by isoproterenol-induced pathological cardiac remodeling(ISO-induced PCR).Male Wistar rats were randomly divided into control(n=6),ISO(n=8),MIIT(n=4),HIIT(n=4),MIIT+ISO(n=8),and HIIT+ISO(n=8)groups.The MIIT and HIIT protocols were administered for 10 weeks, followed by the induction of cardiac remodeling using subcutaneous injection of ISO(100 mg/kg for two consecutive days).Alterations in heart rate(HR),mean arterial pressure(MAP),rate pressure product(RPP),myocardial oxygen consumption(MVO_(2)),cardiac hypertrophy, histopathological changes,prooxidant-antioxidant balance, autophagy biomarkers(Beclin-1,Atg7,p62,LC3Ⅰ/Ⅱ),and apoptotic cell distribution were measured. The findings revealed that the MIIT+ISO and HIIT+ISO groups demonstrated diminished myocardial damage, hemorrhage, immune cell infiltration, edema, necrosis, and apoptosis compared to ISO induced rats.MIIT and HIIT preconditioning mitigated HR,enhanced MAP,and preserved MVO_(2)and RPP.The pro-oxidant-antioxidant balance was sustained in both MIIT+ISO and HIIT+ISO groups, with MIIT primarily inhibiting pro-apoptotic autophagy progression through maintaining pro-oxidant-antioxidant balance, and HIIT promoting pro-survival autophagy. The results demonstrated the beneficial effects of both MIIT and HIIT as AITs preconditioning in ameliorating ISO-induced PCR by improving exercise capacity, hemodynamic parameters, and histopathological changes. Some of these protective effects can be attributed to the modulation of cardiac apoptosis, autophagy, and oxidative stress.

文摘In a reserved forest parcel in a virgin eastern Hyrcanian mixed beech forest,80 ha was surveyed to determine the pit and mound topography, canopy gaps and dead trees. The aim was to investigate the spatial patterns and correlation of pit and mound features with canopy gaps.Seventy-five canopy gaps and 61 pit and mound features were identified. The univariate first order nearest neighbor(R_(CE))and bivariate second order test(Ripley’s K)statistic were applied.R_(CE) statistics highlighted a general aggregation pattern for canopy gaps and pits and mounds, while pits and mounds alone were more clumped.Distances between canopy gaps were 130 m average, whereas distances between pit and mound features and dead trees were 60 and 78 m,respectively.Spatial positive correlation of canopy gaps with pits and mounds were observed with all distances. The result of spatial correlations between canopy gaps with pits and mounds confirmed that windthrows cause micro successions in fallen tree ecosystem-scale correlated with gap phase dynamics in the forest community-scale.
文摘Hydrometallurgical technology offers a unique possibility for developing countries to exploit their mineral resources locally instead of shipping them as concentrates. Production plants may start on a small scale with small capital investment then increase productivity later when the economy permits without financial penalty. This is in contract to smelting operations which necessitates large scale production from the start with high capital investment that may not be available locally.
文摘The standard treatment for acute respiratory distress syndrome(ARDS) is supportive in the form of low tidal volume ventilation applied after significant lung injury has already developed. This review discusses the current data suggesting that preemptive application of the properly adjusted mechanical breath can block progressive acute lung injury and significantly reduce the occurrence of ARDS.
文摘A new series of quinoxalinophenazine derivatives were synthesized in good yields by the reaction of 2,3-dibromonaphthalene-1,4-dione with different aryl-1,2-diamines in DMF as solvent at 120-130℃ or under reflux conditions. 3,12-dimethylbenzo[a]quinoxalino[2,3-c] phenazine with bidentate character reacts with mercury(II) bromide to give suitable crystals.
