METHODS: The medical records of 60 con...AIM: To investigate the indication, feasibility, safety, and clinical utility of endoscopic submucosal dissection (ESD) in the management of various gastrointestinal pathologies. METHODS: The medical records of 60 consecutive patients (34 female, 26 male) who underwent ESD at the gastroenterology department of Kocaeli University from 2006-2010 were examined. Patients selected for ESDhad premalignant lesions or non-invasive early cancers of the gastrointestinal tract and had endoscopic and histological diagnoses. Early cancers were considered to be confined to the submucosa, with no lymph node involvement by means of computed tomography and endosonography. RESULTS: Sixty ESD procedures were performed. The indications were epithelial lesions (n = 39) (33/39 adenoma with high grade dysplasia, 6/39 adenoma with low grade dysplasia), neuroendocrine tumor (n = 7), cancer (n = 7) (5/7 early colorectal cancer, 2/7 early gastric cancer), granular cell tumor (n = 3), gastrointestinal stromal tumor (n = 2), and leiomyoma (n = 2). En bloc and piecemeal resection rates were 91.6% (55/60) and 8.3% (5/60), respectively. Complete and incomplete resection rates were 96.6% (58/60) and 3.3% (2/60), respectively. Complications were major bleeding [n = 3 (5%)] and perforations [n = 5 (8.3%)] (4 colon, 1 stomach). Two patients with colonic perforations and two patients with submucosal lymphatic and microvasculature invasion (1 gastric carcinoid tumor, 1 colonic adenocarcinoma) were referred to surgery. During a mean follow-up of 12 mo, 1 patient with adenoma with high grade dysplasia underwent a second ESD procedure to resect a local recurrence. CONCLUSION: ESD is a feasible and safe method for treatment of premalignant lesions and early malignant gastrointestinal epithelial and subepithelial lesions. Successful en bloc and complete resection of lesions yield high cure rates with low recurrence.展开更多

Diagnosis of this syndrome, which can be summarized as the small intestine being surrounded by a fibrous capsule not containing the mesothelium, is difficult in the preoperative period. A 47-year-old male patient was referred to the emergency department with complaints of abdominal pain, nausea, and vomiting for two days. The abdominal computed tomography examination detected dilated small intestinal loops containing air-fluid levels clustered in the left upper quadrant of the abdomen and surrounded by a thick, saclike, contrast-enhanced membrane. During exploratory surgery, a capsular structure was identified in the upper left quadrant with a regular surface that was solid-fibrous in nature. Ab-dominal cocoon syndrome is a rarely seen condition, for which the preoperative diagnosis is difficult. The combination of physical examination and radiological signs, and the knowledge of "recurrent characteristics of the complaints" that can be learned by a careful history, may be helpful in diagnosis.展开更多
BACKGROUND Nosocomial infections with carbapenem-resistant Acinetobacter baumanniicalcoaceticus complex(ABC)strains are great problem for intensive care units.ABC strains can develop resistance to all the antibiotics ...BACKGROUND Nosocomial infections with carbapenem-resistant Acinetobacter baumanniicalcoaceticus complex(ABC)strains are great problem for intensive care units.ABC strains can develop resistance to all the antibiotics available.Carbapenem resistance is common and colistin resistance is rare in our country.Knowing the risk factors for colistin resistance is important since colistin seems to be the only remaining therapeutic option for the patients with pneumonia due to extensively drug resistant ABC for our country.AIM To investigate the comparison of clinical responses and outcomes between pneumonia patients with colistin-susceptible and-resistant Acinetobacter sp.Strains.METHODS During the study period,108 patients with pneumonia due to colistin-susceptible strains and 16 patients with colistin-resistant strains were included retrospectively.Continuous variables were compared with the Mann-Whitney U test,and categorical variables were compared using Pearson’s chi-square test or Fisher’s Exact chi-square test for two groups.A binary logistic regression model was developed to identify the potential independent factors associated with colistin resistance in patients with colistin-resistant strains.RESULTS High Acute Physiology and Chronic Health Evaluation II scores(OR=1.9,95%CI:1.4-2.7;P<0.001)and prior receipt of teicoplanin(OR=8.1,95%CI:1.0-63.3;P=0.045)were found to be independent risk factors for infection with colistin-resistant Acinetobacter sp.Different combinations of antibiotics including colistin,meropenem,ampicillin/sulbactam,amikacin and trimethoprim/sulfamethoxazole were used for the treatment of patients with colistin-resistant strains.Although the median duration of microbiological cure(P<0.001)was longer in the colistin-resistant group,clinical(P=0.703),laboratory(P=0.277),radiological(P=0.551),microbiological response(P=1.000)and infection related mortality rates(P=0.603)did not differ between the two groups.Among the patients with infections due to colistin-resistant strains,seven were treated展开更多

Patients with BSS are predisposed to multiple skin appendage tumors such as cylindroma, trichoepithelioma, and spiradenoma. FC, however, is characterized by cylindromas and MFT by trichoepitheliomas as the only tumor type. These disorders have recently been associated with mutations in the CYLD gene. In this report, we describe three families with BSS, one with FC, and two with MFT phenotypes associated with novel and recurrent mutations in CYLD. Background:The effects of Helicobacter pylori(H.pylori)infection on growth are a controversial issue.We investigated the effects of long-term H.pylori infection on height and weight in children.Methods:A total of 200 ...Background:The effects of Helicobacter pylori(H.pylori)infection on growth are a controversial issue.We investigated the effects of long-term H.pylori infection on height and weight in children.Methods:A total of 200 children of 7-18 years old suffering from dyspeptic complaints were classified into two groups:H.pylori positive and negative groups,respectively.Whether the infection was impoved was followed up while performing urea breath test,and according to exposure time to the infection,the children were further divided into group 1(≤1.5 months),group 2(>1.5-≤6 months)and group 3(>6 months).Antropometric measurements were obtained and repeated every six months.Results:Mean growth velocity scores in the H.pylori positive and negative groups were 0.49±3.85[95%confidence interval(CI):-0.21-1.18]and 1.98±4.42(95%CI:1-2.96),respectively.The difference between both groups was statistically significant(P=0.012).Mean growth velocity scores in groups 1,2 and 3 were 0.96±3.84,0.16±4.51 and-0.85±3.09,respectively.Mean growth velocity scores of group 3 were significantly lower than those of groups 0 and 1(P=0.005 and P=0.041).The mean weight scores in group 3 were similar to those in group 2,but the scores in group 3 were signifi cantly lower than those in group 1(-1.75±1.05,-1.21±1.37 and-0.88±1.49,respectively).Conclusion:As the duration of exposure is prolonged in children with H.pylori infection,the negative effect of the infection on both height and weight is evident.展开更多

For this purpose, Monte Carlo Simulations are used to include the variability of both incident ground motion and soil parameters in the response spectra by mean of an appropriate coherency loss function and a site-dependent transfer function, respectively. The approach is built on the assumption of vertical propagation of SH type waves in soil strata with uncertain parameters. The response spectra are obtained by numerical integration of the governing equation of a single-degree-of-freedom (SDOF) system under non-stationary site-dependent and spatially varying ground motion accelerations simulated with non-uniform spectral densities and coherency loss functions. Numerical examples showed that randomness of soil properties signifi cantly aff ects the amplitudes of the response spectra, indicating that as the heterogeneity induced by the randomness of the parameters of the medium increases, the spectral ordinates attenuate.展开更多

CENELEC 50128 strongly recommends the utilization of finite state machines during system modelling stage and formal proof methods during the verifi- cation and testing stages of control algorithms. Due to the high importance of interlocking table at the design state of a sig- nalization system, the modelling and verification of inter- locking tables are examined in this work. For this purpose, abstract state machines are used as a modelling tool. The developed models have been performed in a generalized structure such that the model control can be done automatically for the interlocking systems. In this study, NuSMV is used at the verification state. Also, the consistency of the developed models has been supervised through fault injection. While the location of ELT can vary, it is usually localized on the gallbladder wall or in close proximity. ELT is associated with various complications, a major complication being extrahepatic hepatocellular carcinoma. A 59-year-old female underwent elective surgery for chronic cholecystitis with stones. During laparoscopic exploration, a 2-cm-diameter ELT was detected in the anterior gallbladder wall and a laparoscopic cholecystectomy was performed. The case is presented due to the rare nature of ELT and as a reminder of ELT-related complications.展开更多
Nano-sized hydroxyapatite(HA)particles were synthesized by sol-gel through water and ethanol based mediums of phosphoric acid(H3PO4)and calcium hydroxide(Ca(OH)2)at pH=11 for different calcination time(1 h,2 h,4 h).Th...Nano-sized hydroxyapatite(HA)particles were synthesized by sol-gel through water and ethanol based mediums of phosphoric acid(H3PO4)and calcium hydroxide(Ca(OH)2)at pH=11 for different calcination time(1 h,2 h,4 h).The effects of calcination time and solution on the crystallinity,morphology and impurity phases of the HA nanoparticles were examined via Fourier Transform Infrared(FTIR),Scanning Electron Microscopy(SEM),Energy Dispersive X-ray Spectroscopy(EDS)and X-ray Diffraction(XRD).It was found that crystallite size and the fraction crystallinity of the synthesized samples increased with calcination time.According to solution medium,only CaO as impurity was appeared in the water-based solvent,CaO and Ca(OH)2 impurities were appeared in the ethanol-based solvent.The lowest crystallinity was 0.92 and the highest crystallinity was 1.73 respectively,depending on the process parameters.The Ca/P atomic ratio closest to the bone was found as 1.5178.As a result,the employed water-based sol-gel processes for 1 h calcination time was determined as the optimum for the formation of nano-sized HA powders using calcium hydroxide and phosphoric acid.展开更多
Objective:To investigate which cytokines are produced after acute infection of mice with Toxoplasma gondii(T.Gondii) RH strain.Methods:Mus domesticus domesticus mice in infected group were inoculated with with highly ...Objective:To investigate which cytokines are produced after acute infection of mice with Toxoplasma gondii(T.Gondii) RH strain.Methods:Mus domesticus domesticus mice in infected group were inoculated with with highly virulent T.Gondii RH strain by intraperitoneally.Serum samples were obtained from infected and non-infected mice for cytokine levels for ELISA assay.Results:The concentrations of tumor necrosis factor-α,interferon-γ,interleukin(IL)- 10 and IL-12 in the cardiac blood sample of the infected mice were significantly higher than those in uninfected controls(P【0.05).The levels of transforming growth factor-1βdecreased in mice infected with T.gondii compared to those of the controls,the decrease was statistically significant(P【0.05).No significant difference was observed in levels of IL-4 between infected and healty control groups(P】0.05).Conclusions:According to our findings,immune response into T helper type 1 was predominant during acute T.gondii infection.Further characterization and purification of Toxoplasma molecule(s) implicated in the regulation of cytokines could lead to the development of new drug prospects to control Toxoplasma infection.展开更多

The comorbidity rate of groups was determined by using structured clinical interview for DSM-Ⅳ(SCID-Ⅰ). A sociodemographic data form including the story of previous psychiatric treatment, structured clinical interview for DSM-Ⅳ(SCID-Ⅰ), Beck Anxiety Inventory, Beck Depression Inventory(BDI) and Barratt Impulsiveness Scale-11 were applied to both of the groups.RESULTS In regard to 241 obese individuals included in the study, total score and score of attention subscale for BED(+) group were significantly high(P < 0.05). In addition, suicide attempt, story of psychiatric consultation, and score for BDI were again significantly high in the BED(+) group(P < 0.05).CONCLUSION In assessment of obese individuals, assessment of associated psychopathology such as impulsive characteristics and suicide attempt in addition to disrupted eating behaviors will allow to have a more extensive view.展开更多

With its approx. 39 Mt reserves (2.40% Cu), it represents the largest Cu deposit and the largest mining operation in the country (1.5 Mt ore/year). The thickness of the adjacent rocks is composed of olivine-pyroxenite basalts pillow lava, which is spilite, interchangeable ore lenses of chalcopyrite and pyrite is about 170 m and reaches a depth of 350 m. The mid-Eocene aged porphyritic, strongly altered spilites are locally interspersed with diabase and covered by conglomerates. The ores appear massive, stock work and disseminated. Main ore minerals are idiomorphic pyrite, cataclastic chalcopyrite and fine-grained magnetite. The geochemical composition of the Cu ores of the Siirt-Madenköy deposit shows in places high levels of Cu, Fe and S, as important trace elements, As, Ba, Co and Ti are listed. In relation to Clarke values, Se, Bi, Cu, Mo and Co are strongly enriched, while Na, K and Ca as well as their coherent trace elements Rb, Sr and Cd are depleted due to hydrothermal alteration. The elemental distribution is characterized by log-normal distribution, proportionality effect, high Cu/Ni ratio and significantly positive correlation between the element pairs MgO-Ni, Cr-Ni and Co/FeO-Co. The dependence of Cu and SO3 contents and Cu/FeO, SO3/FeO ratios are to be interpreted as an indication of the common origin of Cu, Fe and S. In general, Cu, Zn, Pb and S content decrease with depth, whereas those of Fe3O4 increase. The variograms of the ore distributions are characterized by hole effect, trend and zonal anisotropy, which reflect alternation of ores with host rocks and changes in elemental contents. The Siirt Madenköy deposit is attributable to Cu and Zn ratios of the Cu class of ophiolitic massive sulfide deposits. Due to the very high Cu/Pb and Cu/Zn ratios, it can be described as an analogous deposit of the mid oceanic ridge, for example comparable to ores of Galapagos Ridge. 文摘Celiac axis compression syndrome, one of the reasons of mesenteric ischemia, is an extremely rare etiology of abdominal pain. Primary pathological mechanism is the external compression of the celiac trunk by median arcuate ligament. The diagnosis of this condition is usually difficult and depends on angiographic findings and computerized tomography (CT) evaluations. Here we report a celiac axis compression syndrome case, presenting with chronic abdominal pain and weight loss, which was successfully treated by surgery.
文摘Background:The effects of Helicobacter pylori(H.pylori)infection on growth are a controversial issue.We investigated the effects of long-term H.pylori infection on height and weight in children.Methods:A total of 200 children of 7-18 years old suffering from dyspeptic complaints were classified into two groups:H.pylori positive and negative groups,respectively.Whether the infection was impoved was followed up while performing urea breath test,and according to exposure time to the infection,the children were further divided into group 1(≤1.5 months),group 2(>1.5-≤6 months)and group 3(>6 months).Antropometric measurements were obtained and repeated every six months.Results:Mean growth velocity scores in the H.pylori positive and negative groups were 0.49±3.85[95%confidence interval(CI):-0.21-1.18]and 1.98±4.42(95%CI:1-2.96),respectively.The difference between both groups was statistically significant(P=0.012).Mean growth velocity scores in groups 1,2 and 3 were 0.96±3.84,0.16±4.51 and-0.85±3.09,respectively.Mean growth velocity scores of group 3 were significantly lower than those of groups 0 and 1(P=0.005 and P=0.041).The mean weight scores in group 3 were similar to those in group 2,but the scores in group 3 were signifi cantly lower than those in group 1(-1.75±1.05,-1.21±1.37 and-0.88±1.49,respectively).Conclusion:As the duration of exposure is prolonged in children with H.pylori infection,the negative effect of the infection on both height and weight is evident.
文摘The goal of this study is to provide a stochastic method to investigate the eff ects of the randomness of soil properties due to their natural spatial variability on the response spectra spatial variation at sites with varying conditions. For this purpose, Monte Carlo Simulations are used to include the variability of both incident ground motion and soil parameters in the response spectra by mean of an appropriate coherency loss function and a site-dependent transfer function, respectively. The approach is built on the assumption of vertical propagation of SH type waves in soil strata with uncertain parameters. The response spectra are obtained by numerical integration of the governing equation of a single-degree-of-freedom (SDOF) system under non-stationary site-dependent and spatially varying ground motion accelerations simulated with non-uniform spectral densities and coherency loss functions. Numerical examples showed that randomness of soil properties signifi cantly aff ects the amplitudes of the response spectra, indicating that as the heterogeneity induced by the randomness of the parameters of the medium increases, the spectral ordinates attenuate.
文摘Railway transportation system is a critical sector where design methods and techniques are defined by international standards in order to reduce possible risks to an acceptable minimum level. CENELEC 50128 strongly recommends the utilization of finite state machines during system modelling stage and formal proof methods during the verifi- cation and testing stages of control algorithms. Due to the high importance of interlocking table at the design state of a sig- nalization system, the modelling and verification of inter- locking tables are examined in this work. For this purpose, abstract state machines are used as a modelling tool. The developed models have been performed in a generalized structure such that the model control can be done automatically for the interlocking systems. In this study, NuSMV is used at the verification state. Also, the consistency of the developed models has been supervised through fault injection. The developed models and software components are applied on a real railway station operated by Metro Istanbul Co.
文摘Ectopic liver tissue(ELT) is a rare condition, which is usually not diagnosed preoperatively, but coincidentally during abdominal surgery. While the location of ELT can vary, it is usually localized on the gallbladder wall or in close proximity. ELT is associated with various complications, a major complication being extrahepatic hepatocellular carcinoma. A 59-year-old female underwent elective surgery for chronic cholecystitis with stones. During laparoscopic exploration, a 2-cm-diameter ELT was detected in the anterior gallbladder wall and a laparoscopic cholecystectomy was performed. The case is presented due to the rare nature of ELT and as a reminder of ELT-related complications.
文摘Nano-sized hydroxyapatite(HA)particles were synthesized by sol-gel through water and ethanol based mediums of phosphoric acid(H3PO4)and calcium hydroxide(Ca(OH)2)at pH=11 for different calcination time(1 h,2 h,4 h).The effects of calcination time and solution on the crystallinity,morphology and impurity phases of the HA nanoparticles were examined via Fourier Transform Infrared(FTIR),Scanning Electron Microscopy(SEM),Energy Dispersive X-ray Spectroscopy(EDS)and X-ray Diffraction(XRD).It was found that crystallite size and the fraction crystallinity of the synthesized samples increased with calcination time.According to solution medium,only CaO as impurity was appeared in the water-based solvent,CaO and Ca(OH)2 impurities were appeared in the ethanol-based solvent.The lowest crystallinity was 0.92 and the highest crystallinity was 1.73 respectively,depending on the process parameters.The Ca/P atomic ratio closest to the bone was found as 1.5178.As a result,the employed water-based sol-gel processes for 1 h calcination time was determined as the optimum for the formation of nano-sized HA powders using calcium hydroxide and phosphoric acid.
文摘Objective:To investigate which cytokines are produced after acute infection of mice with Toxoplasma gondii(T.Gondii) RH strain.Methods:Mus domesticus domesticus mice in infected group were inoculated with with highly virulent T.Gondii RH strain by intraperitoneally.Serum samples were obtained from infected and non-infected mice for cytokine levels for ELISA assay.Results:The concentrations of tumor necrosis factor-α,interferon-γ,interleukin(IL)- 10 and IL-12 in the cardiac blood sample of the infected mice were significantly higher than those in uninfected controls(P【0.05).The levels of transforming growth factor-1βdecreased in mice infected with T.gondii compared to those of the controls,the decrease was statistically significant(P【0.05).No significant difference was observed in levels of IL-4 between infected and healty control groups(P】0.05).Conclusions:According to our findings,immune response into T helper type 1 was predominant during acute T.gondii infection.Further characterization and purification of Toxoplasma molecule(s) implicated in the regulation of cytokines could lead to the development of new drug prospects to control Toxoplasma infection.
文摘Anophthalmia is a condition of the absence of an eye and the presence of a small eye within the orbit.It is associated with many known syndromes.Clinical findings,as well as imaging modalities and genetic analysis,are important in making the diagnosis.Imaging modalities are crucial scanning methods.Cryptophthalmos,cyclopia,synophthalmia and congenital cystic eye should be considered in differential diagnoses.We report two clinical anophthalmic siblings,emphasizing the importance of neuroradiological and orbital imaging findings in distinguishing true congenital anophthalmia from clinical anophthalmia.
文摘AIM To investigate the levels of impulsiveness, and the relationship between the binge eating disorder(BED) and the levels of impulsiveness in obese individuals.METHODS Two hundred and forty-one obese patients who were included in the study and candidate for bariatric surgery(weight loss surgery) were clinically interviewed to identify the BED group, and patients were divided into two groups: Those with BED and those without BED. The comorbidity rate of groups was determined by using structured clinical interview for DSM-Ⅳ(SCID-Ⅰ). A sociodemographic data form including the story of previous psychiatric treatment, structured clinical interview for DSM-Ⅳ(SCID-Ⅰ), Beck Anxiety Inventory, Beck Depression Inventory(BDI) and Barratt Impulsiveness Scale-11 were applied to both of the groups.RESULTS In regard to 241 obese individuals included in the study, total score and score of attention subscale for BED(+) group were significantly high(P < 0.05). In addition, suicide attempt, story of psychiatric consultation, and score for BDI were again significantly high in the BED(+) group(P < 0.05).CONCLUSION In assessment of obese individuals, assessment of associated psychopathology such as impulsive characteristics and suicide attempt in addition to disrupted eating behaviors will allow to have a more extensive view.
文摘The Siirt Madenköy massive sulfide ore deposit has been in operation since 2005. With its approx. 39 Mt reserves (2.40% Cu), it represents the largest Cu deposit and the largest mining operation in the country (1.5 Mt ore/year). The thickness of the adjacent rocks is composed of olivine-pyroxenite basalts pillow lava, which is spilite, interchangeable ore lenses of chalcopyrite and pyrite is about 170 m and reaches a depth of 350 m. The mid-Eocene aged porphyritic, strongly altered spilites are locally interspersed with diabase and covered by conglomerates. The ores appear massive, stock work and disseminated. Main ore minerals are idiomorphic pyrite, cataclastic chalcopyrite and fine-grained magnetite. The geochemical composition of the Cu ores of the Siirt-Madenköy deposit shows in places high levels of Cu, Fe and S, as important trace elements, As, Ba, Co and Ti are listed. In relation to Clarke values, Se, Bi, Cu, Mo and Co are strongly enriched, while Na, K and Ca as well as their coherent trace elements Rb, Sr and Cd are depleted due to hydrothermal alteration. The elemental distribution is characterized by log-normal distribution, proportionality effect, high Cu/Ni ratio and significantly positive correlation between the element pairs MgO-Ni, Cr-Ni and Co/FeO-Co. The dependence of Cu and SO3 contents and Cu/FeO, SO3/FeO ratios are to be interpreted as an indication of the common origin of Cu, Fe and S. In general, Cu, Zn, Pb and S content decrease with depth, whereas those of Fe3O4 increase. The variograms of the ore distributions are characterized by hole effect, trend and zonal anisotropy, which reflect alternation of ores with host rocks and changes in elemental contents. The Siirt Madenköy deposit is attributable to Cu and Zn ratios of the Cu class of ophiolitic massive sulfide deposits. Due to the very high Cu/Pb and Cu/Zn ratios, it can be described as an analogous deposit of the mid oceanic ridge, for example comparable to ores of Galapagos Ridge. The Siirt Madenköy deposit is co
文摘We have read with great interest an article by Kocaoglu et al published in May issue of the World Journal of Pediatrics.[1]A total of 243 children aged between 8-18 years were examined based on growth determinants and existence of Helicobacter pylori(H.pylori)colonization in Turkey.The conclusion of this research is that H.pylori colonization affected children growth;and the longer duration of infection,the worse effect on growth.
