Acute variceal hemorrhage,a life-threatening condition that requires a multidisciplinary approach for effective therapy,is defined as visible bleeding from an esophageal or gastric varix at the time of endoscopy,the presence of large esophageal varices with recent stigmata of bleeding,or fresh blood visible in the stomach with no other source of bleeding identified.Transfusion of blood products,pharmacological treatments and early endoscopic therapy are often effective;however,if primary hemostasis cannot be obtained or if uncontrollable early rebleeding occurs,transjugular intrahepatic portosystemic shunt(TIPS)is recommended as rescue treatment.The TIPS represents a major advance in the treatment of complications of portal hypertension.Acute variceal hemorrhage that is poorly controlled with endoscopic therapy is generally well controlled with TIPS,which has a 90%to 100%success rate.However,TIPS is associated with a mortality of 30%to 50%in such a setting.Emergency TIPS should be considered early in patients with refractory variceal bleeding once medical treatment and endoscopic sclerotherapy failure,before the clinical condition worsens.Furthermore,admission to specialized centers is mandatory in such a setting and regional protocols are essential to be organized effectively.This review article discusses initial management and then focuses on the specific role of TIPS as a primary therapy to control acute variceal hemorrhage,particularly as a rescue therapy following failure of endoscopic approaches. 展开更多

The Jiangmen Underground Neutrino Observatory(JUNO)is a large liquid scintillator detector designed to explore many topics in fundamental physics.In this study,the potential of searching for proton decay in the p→νK^(+)mode with JUNO is investigated.The kaon and its decay particles feature a clear three-fold coincidence signature that results in a high efficiency for identification.Moreover,the excellent energy resolution of JUNO permits suppression of the sizable background caused by other delayed signals.Based on these advantages,the detection efficiency for the proton decay via p→νK^(+)is 36.9%±4.9%with a background level of 0.2±0.05(syst)±0.2(stat)events after 10 years of data collection.The estimated sensitivity based on 200 kton-years of exposure is 9.6×1033 years,which is competitive with the current best limits on the proton lifetime in this channel and complements the use of different detection technologies. 展开更多

Sporadic venous malformations are genetic conditions primarily caused by somatic gain-of-function mutation of PIK3CA or TEK,an endothelial transmembrane receptor signaling through PIK3CA.Venous malformations are associated with pain,bleedings,thrombosis,pulmonary embolism,esthetic deformities and,in severe cases,life-threatening situations.No authorized medical treatment exists for patients with venous malformations.Here,we created a genetic mouse model of PIK3CA-related capillary venous malformations that replicates patient phenotypes.We showed that these malformations only partially signal through AKT proteins.We compared the efficacy of different drugs,including rapamycin,a mTORC1 inhibitor,miransertib,an AKT inhibitor and alpelisib,a PI3Kαinhibitor at improving the lesions seen in the mouse model.We demonstrated the effectiveness of alpelisib in preventing vascular malformations’occurrence,improving the already established ones,and prolonging survival.Considering these findings,we were authorized to treat 25 patients with alpelisib,including 7 children displaying PIK3CA(n=16)or TEK(n=9)-related capillary venous malformations resistant to usual therapies including sirolimus,debulking surgical procedures or percutaneous sclerotherapies.We assessed the volume of vascular malformations using magnetic resonance imaging(MRI)for each patient.Alpelisib demonstrated improvement in all 25 patients.Vascular malformations previously considered intractable were reduced and clinical symptoms were attenuated.MRI showed a decrease of 33.4%and 27.8%in the median volume of PIK3CA and TEK malformations respectively,over 6 months on alpelisib.In conclusion,this study supports PI3Kαinhibition as a promising therapeutic strategy in patients with PIK3CA or TEK-related capillary venous malformations. 展开更多

Research into environmentally friendly strategies for hydrogen transfer reduction is increasing, along with the need for more elaborate heterocyclic platforms. Within this context, we develop a new approach for substituted dihydrobenzo[c]carbazoles and indoles. These compounds were synthesized through an iron-catalyzed hydrogen transfer reduction of nitroarenes, followed by intramolecular cyclization. This transformation involves using a Kn?lker-type catalyst, Cs_(2)CO_(3) as the base, and benzyl alcohol as the nonexpensive and low volatile hydrogen donor. We synthesize 30 examples of aza-heterocycles with moderate to excellent yields by applying this strategy. Additionally, DFT calculations demonstrated that the pathway reaction could follow an anionic mechanism. 展开更多

The short term(hourly scale)variability of heterotrophic prokaryote(HP)vertical distribution and respiratory activity,was investigated in the north-western(NW)Mediterranean Sea.HP vertical distribution was determined on board by flow cytometry analysis of seawater samples collected by series of CTD casts.Cell counts and viability were determined for all samples.HP respiratory rates were determined later in the laboratory from filtered seawater samples(23 dm^(3))from 300-1150-m depth.The average cell viability was 94.8%±2.2%(n=240).There was no accumulation of dead cells,due to quick decay of damaged cells.In the epipelagic layer,three HP groups were distinguished,two(HNA1,HNA2)who se cells exhibited a high nucleic acid content and one(LNA)with low nucleic acid content cells.HNA2 was most populated at 50 m but not detected at 90 m and below,presumably aerobic anoxygenic photoheterotrophic bacteria(AAPs).The variability in HP abundance was mainly confined in the upper 80 m.A few secondary peaks of HP abundance were observed(80-150 m)in connection with abundance troughs in the surface layer.HP cells were continuously present in a wide layer around 500 m(mean 191×10^(3)cells/cm^(3)).Below this layer,HP abundance randomly exhibited peaks,coupled to respiratory rate peaks.The HP abundance and variability in the water column was suppressed during a strong wind event.The observed sporadic variability was tentatively interpreted through a pulsed carbon-export mechanism induced by the microorganism production of dissolved poly saccharide s,followed by flocculation and rapid sinking.This mechanism would thus contribute to(ⅰ)preventing organic matter accumulation in the epipelagic layer,(ⅱ)seeding the water column with live HP cells,and(ⅲ)supplying the aphotic water column with fre sh and labile organic matter.This important vertical flux mechanism needs further observations and modelling. 展开更多

Factors released by surrounding cells such as cancer-associated mesenchymal stromal cells(CA-MSCs)are involved in tumor progression and chemoresistance.In this study,we characterize the mechanisms by which naYve mesenchymal stromal cells(MSCs)can acquire a CA-MSCs phenotype.Ovarian tumor cells trigger the transformation of MSCs to CA-MSCs by expressing pro-tumoral genes implicated in the chemoresistance of cancer cells,resulting in the secretion of high levels of CXC chemokine receptors 1 and 2(CXCR1/2)ligands such as chemokine(C-X-C motif)ligand 1(CXCL1),CXCL2,and interleukin 8(IL-8).CXCR1/2 ligands can also inhibit the immune response against ovarian tumor cells.Indeed,through their released factors,CA-MSCs promote the differentiation of monocytes towards M2 macrophages,which favors tumor progression.When CXCR1/2 receptors are inhibited,these CA-MSC-activated macrophages lose their M2 properties and acquire an anti-tumoral phenotype.Both ex vivo and in vivo,we used a CXCR1/2 inhibitor to sensitize ovarian tumor cells to carboplatin and circumvent the pro-tumoral effects of CA-MSCs.Since high concentrations of CXCR1/2 ligands in patients*blood are associated with chemoresistance,CXCR1/2 inhibition could be a potential therapeutic strategy to revert carboplatin resistance. 展开更多

JUNO is a multi-purpose neutrino observatory under construction in the south of China.This publication presents new sensitivity estimates for the measurement of the △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,which is one of the primary physics goals of the experiment.The sensitivities are obtained using the best knowledge available to date on the location and overburden of the experimental site,the nuclear reactors in the surrounding area and beyond,the detector response uncertainties,and the reactor antineutrino spectral shape constraints expected from the TAO satellite detector.It is found that the △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data collection.In the same period,the △m_(31)^(2) parameter will be determined to about 0.2%precision for each mass ordering hypothesis.The new precision represents approximately an order of magnitude improvement over existing constraints for these three parameters. 展开更多

Early recognition and treatment are essentia... <strong>Background:</strong> Puerperal sepsis (PS) is one of the major causes of maternal death, contributing to 26,000 deaths per year in developing countries. Early recognition and treatment are essential to managing PS, but numerous social, cultural and technical barriers prevent or delay access to care and necessary medical attention. Through this qualitative study, we identified barriers to care seeking for puerperal sepsis among recently delivered women in Matiari, Pakistan. <strong>Methods: </strong>We conducted 20 in-depth interviews among recently delivered women with and without sepsis and their family members in September 2012. Key informant interviews were conducted with 14 healthcare providers and traditional birth attendants. The themes used for content analysis were knowledge of danger signs, factors affecting care seeking and local treatment practices for postpartum sepsis. <strong>Results: </strong>A total of 34 interviews were conducted. Recently delivered women, their family members and traditional birth attendants were unaware of the word PS or the local translated term for PS. However, they were familiar with most of the individual symptoms associated with PS. Healthcare providers were aware of the condition and the associated symptoms. The healthcare providers’ understanding of the seriousness of PS was directly proportional their age and clinical experience. The most common barriers to care seeking was the division of labor within the household, obtaining permission from the primary decision maker, access to transportation, lack of financial resources and support from family members. <strong>Conclusion:</strong> To improve maternal care seeking behaviors for PS, interventions focusing on increasing knowledge of PS, addressing gender inequality, implementing an affordable community transport service and enhancing TBA’s knowledge and skills to manage PS need to be implemented. 展开更多

The Belgian disaster victim identification(DVI)team is involved in many investigations in our country.Indeed,this specialized team of the federal police oversees searching for and investigating criminally buried dead bodies,identification of unknown putrefied corpses,and more.The Belgian DVI team also assists with the identification of victims of mass disasters(natural,accidental,and mass murders).In this article,we consider the contributions of different teams(forensic pathology,anthropology,and odontology,federal police,and crime scene investigation)both on the scene of the attack at the Brussels National Airport(Zaventem)and in the laboratory work(autopsies,sample studies). 展开更多

Magnetotactic bacteria are a diverse group of motile prokaryotes that are ubiquitous in aquatic habitats and cosmopolitan in distribution. In this study, we collected magnetotactic bacteria from the Mediterranean Sea. A remarkable diversity of morphotypes was observed, including multicellular types that seemed to differ from those previously found in North and South America. Another interesting organism was one with magnetosomes arranged in a six-stranded bundle which occupied one third of the cell width. The magnetosome bundle was evident even under optic microscopy. These cells were connected together and swam as a linear entire unit. Magnetosomes did not always align up to form a straight linear chain. A chain composed of rectangle magnetosomes bent at a posi- tion with an oval crystal. High resolution transmission electron microscopy analysis of the crystal at the pivotal position suggested uncompleted formation of the crystal. This is the first report of Mediterranean magnetotactic bacteria, which should be useful for studies of biogeochemical cycling and geohistory of the Mediterranean Sea. 展开更多

The risk of both severe and frequent injury associated with active pursuits in mountain areas is acknowledged. This phenomenon is all the most relevant to investigate in France that there is a growing enthusiasm for outdoor sports and recreation. This paper focuses on the social-demographic and sporting profiles of accident victims. Our intent is to better understand who is specifically at risk, while practicing mountain recreation, in the French mountains. Such identification doubtlessly constitutes an important asset in order to enhance prevention. Three critical activities are notably tackled, among some others: hiking, a widespread activity that exposes many practitioners to hazardous settings; mountaineering and ski touring, supposedly entailing the most severe injuries.In order to align the information collected, then provide estimations based on the reasoned crossing of these secondary data, we consulted and summarised all the databases that we knew of on the French territory: rescue team intervention reports, aggregated statistics from the sports ministry, sporting federation accident declarations, avalanche experts statistics, mountain guides union accident/incident reports, healthcare facilities admissions, etc. Only the sources consideredto be the most reliable and representative were used, owing to the number of cases processed, the exhaustive nature of data collection, or the level of information detail available. Our method also included consultation of all the parties playing an active role in prevention, training and mountain rescue, thus ensuring a participative, co-constructive approach to the study with the people that have the best knowledge of victim profiles. Around fifty such parties were interviewed on the methods implemented, the benefits and limits of current data, and the main expectations in terms of accidentology progress, thus further enhancing the credibility of the study proposed in this paper. Although the central neck dissection (CND) is recommended in differentiated thyroid carcinoma, the indication for lateral neck dissection (LND) remains controversial. Design: Retrospective study. Methods and Main Outcome Measures: Pathological analysis of systematic ipsilateral central neck dissection (CND) and LND performed with total thyroidectomy in differentiated thyroid carcinoma was retrospectively reviewed according to “side” and to “patient”. Results: A total of 56 sides (46 patients) were suitable for analysis. Analysis by “side” revealed that CND and LND dissection samples were both negative in 15 cases, both positive in 32, CND was positive and LND was negative for 8 cases and CND was negative and LND was positive in 1 case. The combined presence of positive LND and positive CND was therefore observed in 32/40 “sides” and 26/46 “patients”. Analysis by “side” of the impact of the treatment decision to perform ipsilateral LND only in patients with positive CND and vice versa demonstrated a sensitivity, specificity, and accuracy of 97%, 65%, and 84%, respectively. Conclusions: In most cases, the presence of positive LND was associated with positive ipsilateral CND. The very low prevalence of positive LND in patients with negative CND may justify LND as a second step procedure only in patients with positive CND, except in the case of documented lateral neck metastasis. 展开更多

PURPOSE: A tension-free anastomosis in a restorative proc- tocolectomy requires sufficient length of small-bowel mesentery. To ensure adequate length, it has been proposed that the superior mesenteric artery be divided and the right colon marginal vascular arcade be preserved. This study was designed to evaluate the influence of mesenteric lengthening techniques on the need for a stoma and on early outcomes after restorative proctocolectomy. METHODS: Records of patients who had a restorative proctocolectomy between January 1998 and October 2003 were reviewed. Patient and disease characteristics, operative techniques and findings, the need for a stoma, and postoperative complications were recorded. RESULTS: In one patient a restorative proctocolectomy was not possible. The remaining 220 patients were divided into two groups: Group A (inflammatory bowel disease; n = 123) and Group B (noninflammatory bowel disease; n = 97). Sixty-nine patients (31.4 percent) had major comorbidities. A length- ening technique was performed in 120 patients (54.5 percent) by dividing the ileocecal artery (n = 37) or the superior mesenteric artery (n = 88); 5 patients had only the marginal vascular arcade preserved. An ileostomy was not required in 116 patients (52.7 percent). In multivariate analysis, in Group B the only surgical variable influencing the need for an ileostomy was preservation of the marginal vascular arcade (50 vs. 14.7 percent; P < 0.0005). Complications occurred in 41 patients (18.6 percent), more frequently for those in Group A and for patients receiving steroids (23.6 vs. 12.4 percent, P = 0.012; 10.4 vs. 6.8 percent, P = 0.0172). CONCLUSIONS: The use of mesentery lengthening techniques allows a restorative proctocolectomy to be performed in almost all patients without increasing morbidity and may reduce the number of covering stomas. Because division of the ileocecal and/or superior mesenteric arteries may be required, preservation of the marginal vascular arcade is essential whenever possible. 展开更多

The Jiangmen Underground Neutrino Observatory(JUNO)features a 20 kt multi-purpose underground liquid scintillator sphere as its main detector.Some of JUNO's features make it an excellent location for^8B solar neutrino measurements,such as its low-energy threshold,high energy resolution compared with water Cherenkov detectors,and much larger target mass compared with previous liquid scintillator detectors.In this paper,we present a comprehensive assessment of JUNO's potential for detecting^8B solar neutrinos via the neutrino-electron elastic scattering process.A reduced 2 MeV threshold for the recoil electron energy is found to be achievable,assuming that the intrinsic radioactive background^(238)U and^(232)Th in the liquid scintillator can be controlled to 10^(-17)g/g.With ten years of data acquisition,approximately 60,000 signal and 30,000 background events are expected.This large sample will enable an examination of the distortion of the recoil electron spectrum that is dominated by the neutrino flavor transformation in the dense solar matter,which will shed new light on the inconsistency between the measured electron spectra and the predictions of the standard three-flavor neutrino oscillation framework.IfDelta m^(2)_(21)=4.8times10^(-5);(7.5times10^(-5))eV^(2),JUNO can provide evidence of neutrino oscillation in the Earth at approximately the 3sigma(2sigma)level by measuring the non-zero signal rate variation with respect to the solar zenith angle.Moreover,JUNO can simultaneously measureDelta m^2_(21)using^8B solar neutrinos to a precision of 20% or better,depending on the central value,and to sub-percent precision using reactor antineutrinos.A comparison of these two measurements from the same detector will help understand the current mild inconsistency between the value of Delta m^2_(21)reported by solar neutrino experiments and the KamLAND experiment. 展开更多

The influence of acetate on U(Ⅵ) sorption on silica from aqueous solutions was studied at p H 2–7 by complementary experimental methods of macroscopic measurement, spectroscopic investigation and thermodynamic calculation. Sorption percentage of U(Ⅵ) in the absence and in the presence of different acetate concentrations was determined by batch sorption procedures. Attenuated total reflection Fourier transform infra-red spectroscopy(ATR-FTIR) was used to elucidate the mechanisms of uranyl sorption on silica in the presence of acetate, by investigating, in-situ, the changes in the U(Ⅵ) sorption on silica surface in presence of ligand: the absorption bands of carboxylate ν(COO) in the range 1300–1700 cm-1and those around 850–950 cm-1of uranyl species νas(UO2) are followed. The decrease of U(Ⅵ) sorption with increasing acetate concentration was observed. Sorption of U(Ⅵ)-acetate(U-Ac) species on silica surface was demonstrated for the first time.ATR-FTIR investigations clearly evidenced the absorption bands characteristic of sorbed U-Ac complexes both from the acetate and uranyl spectral regions those the wavenumbers correspond to sorbed species. No sequence effect of acetate and uranyl on the sorption on silica in ternary systems U(Ⅵ)-acetate-silica was observed from the sorption data. From the ATR-FTIR investigations, we can induce that the sorption of U-Ac leads to chemical equilibria and makes possible the modeling of sorption isotherms by surface complexation models. The related thermodynamic constants were modeled using CCM surface complexation modeling. 展开更多

Fresh water is one of the most significant resources for human activities and survival,and irrigation is among the most important uses of water.The sustainibility and performance of irrigation canals can be greatly affected by sediment transport and deposition.In our previous works,we proposed a Lattice Boltzmann model for simulating a free surface flow in an irrigation canal,as an alternative to more traditional models mainly based on shallow water equations.Here we introduce the sedimentation phenomenon into our model by adding a new algorithm,based on the earlier work by B.Chopard,A.Dupuis and A.Masselot[9,11,12,27].Transport,erosion,deposition and toppling of sediments are taken into account and enable the global sedimentation algorithm to simulate different transport modes such as bed load and suspended load.In the present work,we study both the behaviour of a sediment deposit located at an underflow submerged gate(depending on the gate opening and the flow discharge)and the influence of the presence of such a deposit on the flow.Both numerical and experimental validations have been performed.The experiments were realized on the micro-canal of the LCIS laboratory at Valence,France.The comparisons between simulations and experiments give good qualitative agreement. 展开更多
