It is estimated that over 35% of tumors are diagnosed at locally advanced disease, stage ⅠB2-ⅡB with an estimated 5-year overall survival of 60%. During the last decades, the initial treatment for these women has been debated and largely varies through different countries. Thus, radical concurrent chemoradiation is the standard of care in United Sated and Canada, and neoadjuvant chemotherapy followed by radical surgery is the first line of treatment in some institutions of Europe, Asia and Latin America. Until today, there is no evidence of which strategy is better over the other. This article describe the evidence as well as the advantages and disadvantages of the main strategies of treatment for women affected by uterine cervical cancer stage ⅠB2-ⅡB.展开更多

Over the last decades significant advancements in the field have led to standard treatment options. These clinical recommendations have evolved mostly as a result of rando.mized controlled trials and consensus conferences among experts where existing evidence has been reviewed and future goals for research and practice guidelines have been pro- posed. Management of varices/variceal hemorrhage is based on the clinical stage of portal hypertension. No specific treatment has shown to prevent the formation of varices. Prevention of first variceal hemorrhage depends on the size/characteristics of varices. In patients with small varices and high risk of bleeding, nonselective β-blockers are recommended, while patients with medium/large varices can be treated with either β-blockers or esophageal band ligation. Standard ofcare for acute variceal hemorrhage consists of vasoacrive drugs, endoscopic band ligation and antibiotics prophylaxis. Transjugular intrahepatic portosystemic shunt (TIPS) is reserved for those who fail standard of care or for patients who are likely to fail ("early TIPS"). Prevention of recurrent variceal hemorrhage consists of the combination of β-blockers and endoscopic band ligation.展开更多

METHODS: We carried out a cross-sectional study in a check-up unit in a university hospital in Mexi...AIM: To establish an association between the presence of metabolic syndrome and the development of gallstone disease. METHODS: We carried out a cross-sectional study in a check-up unit in a university hospital in Mexico City. We enrolled 245 subjects, comprising 65 subjects with gallstones (36 women, 29 men) and 180 controls (79 women and 101 men without gallstones). Body mass index, waist circumference, blood pressure, plasma insulin, and serum lipids and lipoproteins levels were measured. Insulin resistance was calculated by homeostasis model assessment. Unconditional logistic regression analysis (univariate and multivariate) was used to calculate the risk of gallstone disease associated with the presence of at least three of the criteria (Adult Treatment Panel Ⅲ). Analyses were adjusted for age and sex. RESULTS: Among 245 subjects, metabolic syndrome was present in 40% of gallstone disease subjects, compared with 17.2% of the controls, adjusted by age and gender (odds ratio (OR) = 2.79; 95%CI, 1.46-5.33; P= 0.002), a dose-dependent effect was observed with each component of metabolic syndrome (OR=2.36, 95%CI, 0.72-7.71; P= 0.16 with one component and OR = 5.54, 95%CI, 1.35-22.74; P = 0.02 with four components of metabolic syndrome). Homeostasis model assessment was significantly associated with gallstone disease (adjusted OR = 2.25; 95%CI, 1.08-4.69; P= 0.03). CONCLUSION: We conclude that as for cardiovascular disease and diabetes mellitus, gallstone disease appears to be strongly associated with metabolic syndrome.展开更多

It provokes chronic gastric inflammation that leads to the development of serious gastric diseases such as peptic ulcers, gastric cancer and Mucosa associated lymphoid tissue lymphoma. It is known that these ailments can be avoided if the infection by the bacteria can be prevented or eradicated. Currently, numerous antibiotic-based therapies are available. However, these therapies have several inherent problems, including the appearance of resistance to the antibiotics used and associated adverse effects, the risk of re-infection and the high cost of antibiotic therapy. The delay in developing a vaccine to prevent or eradicate the infection has furthered research into new therapeutic approaches. This review summarises the most relevant recent studies on vaccine development and new treatments using natural resources such as plants, probiotics and nutraceuticals. In addition, novel alternatives based on microorganisms, peptides, polysaccharides, and intragastric violet light irradiation are presented. Alternative therapies have not been effective in eradicating the bacteria but have been shown to maintain low bacterial levels. Nevertheless, some of them are useful in preventing the adverse effects of antibiotics, modulating the immune response, gastroprotection, and the general promotion of health. Therefore, those agents can be used as adjuvants of allopathic anti-H. pylori eradication therapy.展开更多

Two hundred and eighty-seven subjects were included: 97 cases with gallstones and 190 controls. Body mass index (BMI), fasting plasma leptin, insulin, serum lipid, and lipoprotein levels were measured. Insulin resistance was calculated by homeostasis model assessment (HOMA-IR). Unconditional logistic regression analysis (univariate and multivariate)stratified by BMI was used to calculate the risk of GD.RESULTS: The multivariate conditional regression analysis revealed a model for those patients with BMI <30. The selected variables in the model were HOMA-IR index with OR = 1.31, P= 0.02 and leptin higher than median with OR = 2.11, P= 0.05. In the stratum of BMI ≥30, we did not find a useful model.CONCLUSION: We concluded that insulin resistance and the development of GD appears to be associated with serum leptin levels in subjects with overweight, but not in obese subjects with similar metabolic profiles.展开更多
Intraductal papillary mucinous neoplasm(IPMN) of the pancreas is a noninvasive epithelial neoplasm of mucinproducing cells arising in the main duct(MD) and/or branch ducts(BD) of the pancreas.Involved ducts are dilate...Intraductal papillary mucinous neoplasm(IPMN) of the pancreas is a noninvasive epithelial neoplasm of mucinproducing cells arising in the main duct(MD) and/or branch ducts(BD) of the pancreas.Involved ducts are dilated and filled with neoplastic papillae and mucus in variable intensity.IPMN lacks ovarian-type stroma,unlike mucinous cystic neoplasm,and is defined as a grossly visible entity(≥ 5 mm),unlike pancreatic intraepithelial neoplasm.With the use of high-resolution imaging techniques,very small IPMNs are increasingly being identified.Most IPMNs are solitary and located in the pancreatic head,although 20%-40% are multifocal.Macroscopic classification in MD type,BD type and mixed or combined type reflects biological differences with important prognostic and preoperative clinical management implications.Based on cytoarchitectural atypia,IPMN is classified into low-grade,intermediategrade and high-grade dysplasia.Based on histological features and mucin(MUC) immunophenotype,IPMNs are classified into gastric,intestinal,pancreatobiliary and oncocytic types.These different phenotypes can be observed together,with the IPMN classified according to the predominant type.Two pathways have been suggested:gastric phenotype corresponds to less aggressive uncommitted cells(MUC1-,MUC2-,MUC5 AC +,MUC6 +) with the capacity to evolve to intestinal phenotype(intestinal pathway)(MUC1-,MUC2 +,MUC5 AC +,MUC6- or weak +) or pancreatobiliary /oncocytic phenotypes(pyloropancreatic pathway)(MUC1 +,MUC 2-,MUC5 AC +,MUC 6 +) becoming more aggressive.Prognosis of IPMN is excellent but critically worsens when invasive carcinoma arises(about 40% of IPMNs),except in some cases of minimal invasion.The clinical challenge is to establish which IPMNs should be removed because of their higher risk of developing invasive cancer.Once resected,they must be extensively sampled or,much better,submitted in its entirety for microscopic study to completely rule out associated invasive carcinoma.展开更多
To evaluate the effects of dietary protein levels and probiotic supplementation on microbial intestinal fermentation, diarrhea incidence, and performance in weaned piglets, 162 piglets were randomly assigned to three ...To evaluate the effects of dietary protein levels and probiotic supplementation on microbial intestinal fermentation, diarrhea incidence, and performance in weaned piglets, 162 piglets were randomly assigned to three treatments: high-protein diet (20%), with antibiotics (HPa);high-protein diet (20%), without antibiotics (HP);and low-protein diet, without antibiotics but with probiotics (LPpb). Piglets and feed were weighed weekly to calculate the average daily gain (ADG), average daily feed intake (ADFI), and gain: feed ratio (G:F). Four piglets per treatment were killed on day 21 postweaning to collect ileal and colon digesta for measurement of short-chain fatty acids (SCFAs), branched-chain fatty acids (BCFAs), lactic acid (LA), and ammonia (AM). In the digesta collected from the ileum and colon, piglets fed the HPa diet had the lowest concentration of volatile fatty acids. Additionally, the HP diet produced the highest concentration of BCFAs, while the LPpb diet produced moreacetic, propionic, and butyric acids than the HPa and HP diets. Piglets fed the HP diet had higher incidence and severity of diarrhea than piglets fed LPpb and HPa diets, and similar values were observed between these two groups. The second week postweaning was the most critical for diarrhea measurements;during the second week, animals had higher incidence and severity of diarrhea. Piglets fed the HPa and LPpb diets had similar ADGs, while those fed the HP diet had the poorest ADG. Similar results were observed with ADFI and G:F. A low-protein diet supplemented with probiotics changed the fermentation profile, reducing toxic metabolites, promoting gut health, decreasing the incidence and severity of postweaning diarrhea, and improving the performance of piglets.展开更多

D. rotundus is the main vector and transmitter of the rabies virus, which affects human beings as well as several livestock species so the study of this bat species is of high importance within the fields of animal agriculture and public health. The present study describes and compares the histologic characteristics of the urinary system of two hematophagous bat species. A total of 5 bats from each species were captured in the municipalities of Progreso de Obregón, Hidalgo (D. rotundus), and Huayacocotla, Veracruz (D. ecaudata). Organs belonging to the urinary system were extracted: kidneys, ureters, urinary bladder, and urethra;samples were fixed using 10% formalin and processed by the paraffin embedding technique, obtaining sections of 5 µm thickness, which in turn were stained using hematoxylin-eosin (H-E) and Gomori trichrome (GT) stains. From the obtained histologic preparations, a descriptive and comparative analysis of the structural organography of the urinary system of both species was made, and no noteworthy histological differences between samples were noted. The present research is intended to provide a framework for future studies of these species’ currently understudied microscopic anatomy.展开更多

The work was carried out at the FMVZ-BUAP Gen...The purpose of the study was to evaluate the sperm viability of semen infected with PRRSV viral particles, observing the effect of the Virus on the motility of boar sperm. The work was carried out at the FMVZ-BUAP Genetics and Reproduction Laboratory. 5 stallions were used. Each sample contained 1 × 10<sup>6</sup> sperm, the PRRS virus strain was ATCC-VR-2332 (0, 10<sup>2</sup>, 10<sup>4</sup> and 10<sup>6</sup> copies of RNA/mL in triplicate), it was observed daily at the CASA;Hamilton Thorne<sup>®</sup>. Cells with MT (P < 0.05) on days 1, 3, 5, 7 and 10 of evaluation with 201 ± 7.3, 167 ± 10.1, 165 ± 14.6, 134 ± 8.2 and 120 ± 8.8, respectively. The % MP between control and virus concentrations (P ≥ 0.05). The LCV on day 1 and 7 PI at 10X<sup>2</sup> and 10X<sup>6</sup> (P < 0.05) vs control. In the Correlation Matrix, where it is observed that there is a correlation between VSL and VAP, VSL and VCL, VCL and ALH, VAP with ALH. There is a correlation of VSL and ALH, STR and ALH. In this study there were (P ≤ 0.01) in the VCL, in the concentrations (10<sup>2</sup>) 162.81 ± 10.65 and (10<sup>6</sup>) 177.12 ± 5.77 vs 193.04 ± 4.62 of control. This indicates that altering these parameters would be related to fertility and the PRRS virus affects the LCV. Regarding the VSL, it was observed that the sperm infected with viruses 10<sup>2</sup>, 10<sup>4</sup> and 10<sup>6</sup> of 48.00 ± 3.38, 49.88 ± 1.83 and 50.55 ± 2.24 Vs. 56.66 ± 1.68 of control respectively, the control would have greater possibilities of fertilizing the oocyte. In this study, it was found (P ≤ 0.01) in the VAP with 102 of 77.26 ± 5.16, 10<sup>4</sup> with 83.35 ± 2.41 and 10<sup>6</sup> with 81.29 ± 3.14 vs the control with 90.56 ± 2.07. Regarding the ALH there is (P < 0.05) a 10<sup>4</sup> with 8.70 ± .26 and 10<sup>6</sup> with 9.64 ± 0.23 vs control 8.50 ± 0.27. Weaning is a challenging stage of pig farming.Animals undergo environmental,social and dietary changes leading to weaning stress syndrome.In order to compensate for the detrimental effects of weaning stress,antibiotic...Weaning is a challenging stage of pig farming.Animals undergo environmental,social and dietary changes leading to weaning stress syndrome.In order to compensate for the detrimental effects of weaning stress,antibiotics and natural extracts are used as feed additives,sometimes without fully understanding the interactions between them or even with low concentrations of mycotoxins that are frequently present in feed.The aim of this study was to evaluate the effect of fosfomycin(FOS),Cynara scolymus extract(CSE),deoxynivalenol(DON) and their combined administration on intestinal health of weaned piglets.The experiment was designed as a 2×2×2 factorial arrangement with 3 factors(FOS,CSE and DON treatments),2 levels each(presence and absence) and 3 repeats.Weaned piglets(n=24)were randomly divided in groups to receive the different treatments,namely DON administered in diet(50 μg/kg BW),FOS administered into the drinking water(30 mg/kg BW),CSE administered in diet(15 mg/kg BW) and all their combinations.After 15 d,the animals were euthanized and gastrointestinal tract samples were immediately taken to evaluate gastrointestinal pH,Enterobacteriaceae to lactic acid bacteria(E:L) ratio,volatile fatty acid(VFA) concentrations,disaccharidase(lactase,sucrase and maltase)activity,histology(intestinal absorptive area [IAA] and goblet cells count) and mucus ability to adhere pathogenic Escherichia coli.From our results,FOS and CSE treatments,individually or combined,produced a lower E:L ratio,an enhanced production of butyrate,increased disaccharidase activity(particularly maltase),and a greater IAA and goblet cells count along with an increase in pathogenic bacteria adherence to intestinal mucus.Deoxynivalenol did not show interactions with the other factors and its administration produced decreases on VFA,disaccharidase activity and goblet cells count.In conclusion,weaning piglets receiving diets containing FOS,CSE or both exhibited evident beneficial intestinal effects compared to animals receiving diets free from these c展开更多
Objective: To evaluate the cytotoxic effects of Spirulina platensis extracts on acute leukemia Kasumi-1 and chronic leukemia K-562 cancer cell lines.Methods: Various concentrations of Spirulina platensis extracts(0.25...Objective: To evaluate the cytotoxic effects of Spirulina platensis extracts on acute leukemia Kasumi-1 and chronic leukemia K-562 cancer cell lines.Methods: Various concentrations of Spirulina platensis extracts(0.25–50.00 mg/m L)obtained with different solvents were used to treat cell lines for 72 h. For cytotoxic effect studies, cell viability test with trypan blue solution, MTT assay and microscopic cytomorphological assessment were done.Results: Spirulina extract obtained with 70% ethanol showed significant cytotoxicity in K562 and Kasumi-1 cell lines. With trypan blue solution, IC_(50) values were found to be4.64 mg/m L for K-562 and 3.68 mg/m L for Kusumi-1 cell lines. Spirulina aqueous extract also showed cytotoxicity with trypan blue method, at a slightly higher dose; where IC_(50) values were 12.68 mg/m L for K-562 and 2.13 mg/m L for Kusumi-1 cell lines. The IC_(50) values were found 0.40 mg/m L for K-562 and 0.31 mg/m L for Kusumi-1 cell lines for the 70% ethanol extract according to the MTT assay. Spirulina extract obtained with water also showed cytotoxicity but the dose was a little higher where IC_(50) values were15.77 mg/m L for K-562 and 9.44 mg/m L for Kusumi-1 cell lines. The effect of cytotoxicity with ethanol extract is quite comparable with that observed for cyclophosphamide, which is a chemical used as anticancer agent.Conclusions: The cytotoxicity exhibited by Spirulina extract to cancer cell lines might be due to the presence of phytopigments(carotenoids, chlorophyll, phycocyanin) as well as polysaccharides that were reported previously as constituents of the extract. So crude extracts of Spirulina can be used as a source to develop anticancer drugs.展开更多
Objective:To assess the role of polymerase chain reaction(PCR)in serum sauples,in the diagnosis of osteoarticular tuberculosis(OTB)in a setting where only clinical and imaging diagnoses determine the treatment.Methods...Objective:To assess the role of polymerase chain reaction(PCR)in serum sauples,in the diagnosis of osteoarticular tuberculosis(OTB)in a setting where only clinical and imaging diagnoses determine the treatment.Methods:A total of 44 consecutive serum specimens were collected from clinically suspected OTB patients,based on clinical and radiological[X-ray or magnetic resonance imagng/computecl tomography]features.They were scrcened by in-house nested PCR.In addition,a few specimens were examined by Gram stain,acid-fast bacilli stain,histand routine bacterial culture.A total of 39 specimens were collected from patients suffering from other bone diseases of nontuberculous origin and included as negative controls.Results:of the 44 clinically suspected OTB patients,in-house nested PCR was positive in 40(91%)cases;PCR was negative in 38(97%)negative controls.Sensitivity and specificity of our in—house nested PCR was 90.3%and 97.4%,respectively.The PCR report was available within 48 h.It was possible to standardize serum PCR technique and in positive cases,a good n was observed in terms of an adequate treatment response.Conclusions:Nested PCR in serum samples is a rapid,highly sensitive and specific modality for OTB detection,PCR should be performed in addition to clinical evaluation,imaging studies,acidfast bacilli staining,culture and histopathology diagnosis,if possible.展开更多

They have an important eco- logical role, especially by modifying physical and chemical properties of soils. Biological crusts may also modify seed germination and seedling establishment. The effects vary widely according to the type of crust and the vas- cular plant species. The objective of this study was to determine the effect of moss-dominated biological soil crusts on the emergence, biomass and survival of Poa ligularis Nees ex Steud. under different irrigation regimes. We col- lected seeds of P. ligularis and biological soil crusts composed of two species of mosses: Syntrichia princeps (De Not.) Mitt and Ceratodon purpureus (Hedw.) Brid. from an area in the Monte of Argentina. The result showed that seedling emergence of P. ligularis was higher in treatments with bare soil than in soil covered by crusts, and also in those with watering to field capacity. Mean emergence time was higher in treatments with bare soil and watering to field capacity. Seedling biomass also showed significant differences between treatments. These results suggest that biological soil crusts dominated by mosses do not promote P. ligularis emergence, although they would not affect its survival.展开更多
The intersection between metabolic-associated steatotic liver disease(MASLD)and chronic hepatitis B virus(HBV)infection is an emerging area of research with significant implications for public health and clinical prac...The intersection between metabolic-associated steatotic liver disease(MASLD)and chronic hepatitis B virus(HBV)infection is an emerging area of research with significant implications for public health and clinical practice.Wang et al’s study highlights the complexities of managing patients with concurrent MASLD and HBV.The findings revealed that patients with concurrent MASLD-HBV exhibited more severe liver inflammation and fibrosis,whereas those with HBV alone pre-sented a better lipid profile.The growing recognition of metabolic dysfunction in liver disease,reflected in the shift from nonalcoholic liver disease to MASLD,demands updates to clinical guidelines,particularly for patients with dual etio-logies.Understanding the biological interactions between MASLD and HBV could lead to novel therapeutic approaches,emphasizing the need for personalized treatment strategies.The coexistence of MASLD and HBV presents therapeutic challenges,particularly in managing advanced fibrosis and cirrhosis,which are more likely in these patients.The aim of this editorial is to analyze the interaction between MASLD and HBV,highlight the pathophysiological mechanisms that exacerbate liver disease when both conditions coexist,and discuss the clinical implications of the findings of Wang et al.展开更多

Objectives of the research were: 1) Iden...Re-vegetation plays a fundamental role for erosion control and plant recovery in lands affected by gully erosion. Bioengineered practices facilitate the gullies rehabilitation. Objectives of the research were: 1) Identify taxonomically the pioneer vegetation on each gully section; 2) Characterize vegetation distribution preferences and 3) Assess structural/functional traits to recognize erosion control key species. Bioengineering was applied in a watershed belonging to Sierra Madre del Sur, at Oaxaca, Mexico, on eight gullies, with local support and minimal investment. "La Mixteca" is a poor ecological and socio-economic region, comparable to other regions of the world. The Initial Floristic Composition(IFC) inventory is the baseline of the successional process. The transect method was used to determine the colonization of species. Cover abundance of registered species was estimated using the semi-quantitative scale of Braun-Blanquet. This procedure was repeated in five different positions(floor, hillslopes and tops), in the cross section of the gully. Throughcorrespondence analysis and clustering, the distribution of species was analyzed. Adequate responses were obtained in soil retention(quantity) and plant cover(existence and diversity); as measurable indicators of the bioengeneering works efficiency. Occupation of soil by native species from the Tropical Deciduous Forest was favored using live barriers. We detected species guilds with spatial distribution preferences in the gullies cross section. Plant cover characterization includes: native colonizer species, herbaceous, shrubby and trees of the forest community bordering the gully area, with cover abundance and structural/functional traits, useful to protect degraded areas. This spatial occupation process of plants responds to a secondary succession in gullies, where the proposed IFC model is correctly represented through bioengineering. AIM To characterize the prescribing patterns for hydrocortisone for patients with septic shock and perform an exploratory analysis in order to identify the variables associated with better outcomes.METHODS This prospe...AIM To characterize the prescribing patterns for hydrocortisone for patients with septic shock and perform an exploratory analysis in order to identify the variables associated with better outcomes.METHODS This prospective cohort study included 59 patients with septic shock who received stress-dose hydrocortisone.It was performed at 2 critical care units in academic hospitals from June 1st, 2015, to July 31 st, 2016. Demographic data, comorbidities, medical management details, adverse effects related to corticosteroids, and outcomes were collected after the critical care physician indicated initiation of hydrocortisone. Univariate comparison between continuous and bolus administration of hydrocortisone was performed, including multivariate analysis, as well as Kaplan-Meier analysis to compare the proportion of shock reversal at 7 d after presentation. Receiver operating characteristic(ROC) curves determined the best cut-off criteria for initiation of hydrocortisone associated with the highest probability of shock reversal. We addressed the effects of the taper strategy for discontinuation of hydrocortisone, noting risk of shock relapse and adverse effects.RESULTS All-cause 30-d mortality was 42%. Hydrocortisone was administered as a continuous infusion in 54.2% of patients; time to reversal of shock was 49 h longer in patients who were given a bolus administration [59 h(range, 47.5-90.5) vs 108 h(range, 63.2-189); P = 0.001]. The maximal dose of norepinephrine after initiation of hydrocortisone was lower in patients on continuous infusion [0.19 μg/kg per minute(range, 0.11-0.28 μg)] compared with patients who were given bolus [0.34 μg/kg per minute(range, 0.16-0.49); P = 0.004]. Kaplan-Meier analysis revealed a higher proportion of shock reversal at 7 d in patients with continuous infusion compared to those given bolus(83% vs 63%; P = 0.004). Although antivirals are important tools to control severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection,effective vaccines are essential to control the current coronavirus disease 2019(COVID-19)pandemi...Although antivirals are important tools to control severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection,effective vaccines are essential to control the current coronavirus disease 2019(COVID-19)pandemic.Plant-derived virus-like particle(VLP)vaccine candidates have previously demonstrated immunogenicity and efficacy against influenza.Here,we report the immunogenicity and protection induced in rhesus macaques by intramuscular injections of a VLP bearing a SARS-CoV-2 spike protein(CoVLP)vaccine candidate formulated with or without Adjuvant System 03(AS03)or cytidine-phospho-guanosine(CpG)1018.Although a single dose of the unadjuvanted CoVLP vaccine candidate stimulated humoral and cell-mediated immune responses,booster immunization(at 28 days after priming)and adjuvant administration significantly improved both responses,with higher immunogenicity and protection provided by the AS03-adjuvanted CoVLP.Fifteen micrograms of CoVLP adjuvanted with AS03 induced a polyfunctional interleukin-2(IL-2)-driven response and IL-4 expression in CD4 T cells.Animals were challenged by multiple routes(i.e.,intratracheal,intranasal,and ocular)with a total viral dose of 106 plaque-forming units of SARS-CoV-2.Lower viral replication in nasal swabs and bronchoalveolar lavage fluid(BALF)as well as fewer SARS-CoV-2-infected cells and immune cell infiltrates in the lungs concomitant with reduced levels of proinflammatory cytokines and chemotactic factors in the BALF were observed in animals immunized with the CoVLP adjuvanted with AS03.No clinical,pathologic,or virologic evidence of vaccineassociated enhanced disease was observed in vaccinated animals.The CoVLP adjuvanted with AS03 was therefore selected for vaccine development and clinical trials.展开更多
文摘Uterine cervical cancer is the second most common gynecological malignancy. It is estimated that over 35% of tumors are diagnosed at locally advanced disease, stage ⅠB2-ⅡB with an estimated 5-year overall survival of 60%. During the last decades, the initial treatment for these women has been debated and largely varies through different countries. Thus, radical concurrent chemoradiation is the standard of care in United Sated and Canada, and neoadjuvant chemotherapy followed by radical surgery is the first line of treatment in some institutions of Europe, Asia and Latin America. Until today, there is no evidence of which strategy is better over the other. This article describe the evidence as well as the advantages and disadvantages of the main strategies of treatment for women affected by uterine cervical cancer stage ⅠB2-ⅡB.
文摘Portal hypertension is the main complication of cirrhosis and is defined as an hepatic venous pressure gradient (HVPG) of more than 5 mmHg. Clinically significant portal hypertension is defined as HVPG of 10 mmHg or more. Development of gastroesophageal varices and variceal hemorrhage are the most direct consequence of portal hypertension. Over the last decades significant advancements in the field have led to standard treatment options. These clinical recommendations have evolved mostly as a result of rando.mized controlled trials and consensus conferences among experts where existing evidence has been reviewed and future goals for research and practice guidelines have been pro- posed. Management of varices/variceal hemorrhage is based on the clinical stage of portal hypertension. No specific treatment has shown to prevent the formation of varices. Prevention of first variceal hemorrhage depends on the size/characteristics of varices. In patients with small varices and high risk of bleeding, nonselective β-blockers are recommended, while patients with medium/large varices can be treated with either β-blockers or esophageal band ligation. Standard ofcare for acute variceal hemorrhage consists of vasoacrive drugs, endoscopic band ligation and antibiotics prophylaxis. Transjugular intrahepatic portosystemic shunt (TIPS) is reserved for those who fail standard of care or for patients who are likely to fail ("early TIPS"). Prevention of recurrent variceal hemorrhage consists of the combination of β-blockers and endoscopic band ligation.
文摘AIM: To establish an association between the presence of metabolic syndrome and the development of gallstone disease. METHODS: We carried out a cross-sectional study in a check-up unit in a university hospital in Mexico City. We enrolled 245 subjects, comprising 65 subjects with gallstones (36 women, 29 men) and 180 controls (79 women and 101 men without gallstones). Body mass index, waist circumference, blood pressure, plasma insulin, and serum lipids and lipoproteins levels were measured. Insulin resistance was calculated by homeostasis model assessment. Unconditional logistic regression analysis (univariate and multivariate) was used to calculate the risk of gallstone disease associated with the presence of at least three of the criteria (Adult Treatment Panel Ⅲ). Analyses were adjusted for age and sex. RESULTS: Among 245 subjects, metabolic syndrome was present in 40% of gallstone disease subjects, compared with 17.2% of the controls, adjusted by age and gender (odds ratio (OR) = 2.79; 95%CI, 1.46-5.33; P= 0.002), a dose-dependent effect was observed with each component of metabolic syndrome (OR=2.36, 95%CI, 0.72-7.71; P= 0.16 with one component and OR = 5.54, 95%CI, 1.35-22.74; P = 0.02 with four components of metabolic syndrome). Homeostasis model assessment was significantly associated with gallstone disease (adjusted OR = 2.25; 95%CI, 1.08-4.69; P= 0.03). CONCLUSION: We conclude that as for cardiovascular disease and diabetes mellitus, gallstone disease appears to be strongly associated with metabolic syndrome.

文摘Helicobacter pylori(H. pylori) is a successful pathogen that can persist in the stomach of an infected person for their entire life. It provokes chronic gastric inflammation that leads to the development of serious gastric diseases such as peptic ulcers, gastric cancer and Mucosa associated lymphoid tissue lymphoma. It is known that these ailments can be avoided if the infection by the bacteria can be prevented or eradicated. Currently, numerous antibiotic-based therapies are available. However, these therapies have several inherent problems, including the appearance of resistance to the antibiotics used and associated adverse effects, the risk of re-infection and the high cost of antibiotic therapy. The delay in developing a vaccine to prevent or eradicate the infection has furthered research into new therapeutic approaches. This review summarises the most relevant recent studies on vaccine development and new treatments using natural resources such as plants, probiotics and nutraceuticals. In addition, novel alternatives based on microorganisms, peptides, polysaccharides, and intragastric violet light irradiation are presented. Alternative therapies have not been effective in eradicating the bacteria but have been shown to maintain low bacterial levels. Nevertheless, some of them are useful in preventing the adverse effects of antibiotics, modulating the immune response, gastroprotection, and the general promotion of health. Therefore, those agents can be used as adjuvants of allopathic anti-H. pylori eradication therapy.

文摘AIM: To establish an association between the serum leptin levels and the development of gallstone disease (GD).METHODS: We carried out a non-matched case-controlled study in a university hospital in Mexico City. Two hundred and eighty-seven subjects were included: 97 cases with gallstones and 190 controls. Body mass index (BMI), fasting plasma leptin, insulin, serum lipid, and lipoprotein levels were measured. Insulin resistance was calculated by homeostasis model assessment (HOMA-IR). Unconditional logistic regression analysis (univariate and multivariate)stratified by BMI was used to calculate the risk of GD.RESULTS: The multivariate conditional regression analysis revealed a model for those patients with BMI <30. The selected variables in the model were HOMA-IR index with OR = 1.31, P= 0.02 and leptin higher than median with OR = 2.11, P= 0.05. In the stratum of BMI ≥30, we did not find a useful model.CONCLUSION: We concluded that insulin resistance and the development of GD appears to be associated with serum leptin levels in subjects with overweight, but not in obese subjects with similar metabolic profiles.
文摘Intraductal papillary mucinous neoplasm(IPMN) of the pancreas is a noninvasive epithelial neoplasm of mucinproducing cells arising in the main duct(MD) and/or branch ducts(BD) of the pancreas.Involved ducts are dilated and filled with neoplastic papillae and mucus in variable intensity.IPMN lacks ovarian-type stroma,unlike mucinous cystic neoplasm,and is defined as a grossly visible entity(≥ 5 mm),unlike pancreatic intraepithelial neoplasm.With the use of high-resolution imaging techniques,very small IPMNs are increasingly being identified.Most IPMNs are solitary and located in the pancreatic head,although 20%-40% are multifocal.Macroscopic classification in MD type,BD type and mixed or combined type reflects biological differences with important prognostic and preoperative clinical management implications.Based on cytoarchitectural atypia,IPMN is classified into low-grade,intermediategrade and high-grade dysplasia.Based on histological features and mucin(MUC) immunophenotype,IPMNs are classified into gastric,intestinal,pancreatobiliary and oncocytic types.These different phenotypes can be observed together,with the IPMN classified according to the predominant type.Two pathways have been suggested:gastric phenotype corresponds to less aggressive uncommitted cells(MUC1-,MUC2-,MUC5 AC +,MUC6 +) with the capacity to evolve to intestinal phenotype(intestinal pathway)(MUC1-,MUC2 +,MUC5 AC +,MUC6- or weak +) or pancreatobiliary /oncocytic phenotypes(pyloropancreatic pathway)(MUC1 +,MUC 2-,MUC5 AC +,MUC 6 +) becoming more aggressive.Prognosis of IPMN is excellent but critically worsens when invasive carcinoma arises(about 40% of IPMNs),except in some cases of minimal invasion.The clinical challenge is to establish which IPMNs should be removed because of their higher risk of developing invasive cancer.Once resected,they must be extensively sampled or,much better,submitted in its entirety for microscopic study to completely rule out associated invasive carcinoma.
文摘To evaluate the effects of dietary protein levels and probiotic supplementation on microbial intestinal fermentation, diarrhea incidence, and performance in weaned piglets, 162 piglets were randomly assigned to three treatments: high-protein diet (20%), with antibiotics (HPa);high-protein diet (20%), without antibiotics (HP);and low-protein diet, without antibiotics but with probiotics (LPpb). Piglets and feed were weighed weekly to calculate the average daily gain (ADG), average daily feed intake (ADFI), and gain: feed ratio (G:F). Four piglets per treatment were killed on day 21 postweaning to collect ileal and colon digesta for measurement of short-chain fatty acids (SCFAs), branched-chain fatty acids (BCFAs), lactic acid (LA), and ammonia (AM). In the digesta collected from the ileum and colon, piglets fed the HPa diet had the lowest concentration of volatile fatty acids. Additionally, the HP diet produced the highest concentration of BCFAs, while the LPpb diet produced moreacetic, propionic, and butyric acids than the HPa and HP diets. Piglets fed the HP diet had higher incidence and severity of diarrhea than piglets fed LPpb and HPa diets, and similar values were observed between these two groups. The second week postweaning was the most critical for diarrhea measurements;during the second week, animals had higher incidence and severity of diarrhea. Piglets fed the HPa and LPpb diets had similar ADGs, while those fed the HP diet had the poorest ADG. Similar results were observed with ADFI and G:F. A low-protein diet supplemented with probiotics changed the fermentation profile, reducing toxic metabolites, promoting gut health, decreasing the incidence and severity of postweaning diarrhea, and improving the performance of piglets.
文摘Desmodus rotundus and Diphylla ecaudata, both of which are mammals of the order Chiroptera, Desmodontidae family, their diet consisting exclusively of blood. D. rotundus is the main vector and transmitter of the rabies virus, which affects human beings as well as several livestock species so the study of this bat species is of high importance within the fields of animal agriculture and public health. The present study describes and compares the histologic characteristics of the urinary system of two hematophagous bat species. A total of 5 bats from each species were captured in the municipalities of Progreso de Obregón, Hidalgo (D. rotundus), and Huayacocotla, Veracruz (D. ecaudata). Organs belonging to the urinary system were extracted: kidneys, ureters, urinary bladder, and urethra;samples were fixed using 10% formalin and processed by the paraffin embedding technique, obtaining sections of 5 µm thickness, which in turn were stained using hematoxylin-eosin (H-E) and Gomori trichrome (GT) stains. From the obtained histologic preparations, a descriptive and comparative analysis of the structural organography of the urinary system of both species was made, and no noteworthy histological differences between samples were noted. The present research is intended to provide a framework for future studies of these species’ currently understudied microscopic anatomy.
文摘The broke of a tailings dam from Vale S.A. destroyed a gigantic area killing 270 persons in Brumadinho/Brazil. Organizing activities after a mass disaster is a complex process that requires the involvement of many people and resources in the laboratory. To DNA identification of the victims, a daunting work had to be made by DNA laboratory staff in a collaborative effort with many partners. Efforts to implement good practice guidelines in Disaster Victim Identification (DVI) have revealed several important aspects that need to change in the forensic DNA laboratory. This article highlights the challenges of implementing DVI best practice guidelines in resource-poor settings, but with professionals from different sectors engaged in the same goal of briefly identifying victims and helping families and society.
文摘The purpose of the study was to evaluate the sperm viability of semen infected with PRRSV viral particles, observing the effect of the Virus on the motility of boar sperm. The work was carried out at the FMVZ-BUAP Genetics and Reproduction Laboratory. 5 stallions were used. Each sample contained 1 × 10<sup>6</sup> sperm, the PRRS virus strain was ATCC-VR-2332 (0, 10<sup>2</sup>, 10<sup>4</sup> and 10<sup>6</sup> copies of RNA/mL in triplicate), it was observed daily at the CASA;Hamilton Thorne<sup>®</sup>. Cells with MT (P < 0.05) on days 1, 3, 5, 7 and 10 of evaluation with 201 ± 7.3, 167 ± 10.1, 165 ± 14.6, 134 ± 8.2 and 120 ± 8.8, respectively. The % MP between control and virus concentrations (P ≥ 0.05). The LCV on day 1 and 7 PI at 10X<sup>2</sup> and 10X<sup>6</sup> (P < 0.05) vs control. In the Correlation Matrix, where it is observed that there is a correlation between VSL and VAP, VSL and VCL, VCL and ALH, VAP with ALH. There is a correlation of VSL and ALH, STR and ALH. In this study there were (P ≤ 0.01) in the VCL, in the concentrations (10<sup>2</sup>) 162.81 ± 10.65 and (10<sup>6</sup>) 177.12 ± 5.77 vs 193.04 ± 4.62 of control. This indicates that altering these parameters would be related to fertility and the PRRS virus affects the LCV. Regarding the VSL, it was observed that the sperm infected with viruses 10<sup>2</sup>, 10<sup>4</sup> and 10<sup>6</sup> of 48.00 ± 3.38, 49.88 ± 1.83 and 50.55 ± 2.24 Vs. 56.66 ± 1.68 of control respectively, the control would have greater possibilities of fertilizing the oocyte. In this study, it was found (P ≤ 0.01) in the VAP with 102 of 77.26 ± 5.16, 10<sup>4</sup> with 83.35 ± 2.41 and 10<sup>6</sup> with 81.29 ± 3.14 vs the control with 90.56 ± 2.07. Regarding the ALH there is (P < 0.05) a 10<sup>4</sup> with 8.70 ± .26 and 10<sup>6</sup> with 9.64 ± 0.23 vs control 8.50 ± 0.27. 文摘Weaning is a challenging stage of pig farming.Animals undergo environmental,social and dietary changes leading to weaning stress syndrome.In order to compensate for the detrimental effects of weaning stress,antibiotics and natural extracts are used as feed additives,sometimes without fully understanding the interactions between them or even with low concentrations of mycotoxins that are frequently present in feed.The aim of this study was to evaluate the effect of fosfomycin(FOS),Cynara scolymus extract(CSE),deoxynivalenol(DON) and their combined administration on intestinal health of weaned piglets.The experiment was designed as a 2×2×2 factorial arrangement with 3 factors(FOS,CSE and DON treatments),2 levels each(presence and absence) and 3 repeats.Weaned piglets(n=24)were randomly divided in groups to receive the different treatments,namely DON administered in diet(50 μg/kg BW),FOS administered into the drinking water(30 mg/kg BW),CSE administered in diet(15 mg/kg BW) and all their combinations.After 15 d,the animals were euthanized and gastrointestinal tract samples were immediately taken to evaluate gastrointestinal pH,Enterobacteriaceae to lactic acid bacteria(E:L) ratio,volatile fatty acid(VFA) concentrations,disaccharidase(lactase,sucrase and maltase)activity,histology(intestinal absorptive area [IAA] and goblet cells count) and mucus ability to adhere pathogenic Escherichia coli.From our results,FOS and CSE treatments,individually or combined,produced a lower E:L ratio,an enhanced production of butyrate,increased disaccharidase activity(particularly maltase),and a greater IAA and goblet cells count along with an increase in pathogenic bacteria adherence to intestinal mucus.Deoxynivalenol did not show interactions with the other factors and its administration produced decreases on VFA,disaccharidase activity and goblet cells count.In conclusion,weaning piglets receiving diets containing FOS,CSE or both exhibited evident beneficial intestinal effects compared to animals receiving diets free from these c

文摘Objective: To evaluate the cytotoxic effects of Spirulina platensis extracts on acute leukemia Kasumi-1 and chronic leukemia K-562 cancer cell lines.Methods: Various concentrations of Spirulina platensis extracts(0.25–50.00 mg/m L)obtained with different solvents were used to treat cell lines for 72 h. For cytotoxic effect studies, cell viability test with trypan blue solution, MTT assay and microscopic cytomorphological assessment were done.Results: Spirulina extract obtained with 70% ethanol showed significant cytotoxicity in K562 and Kasumi-1 cell lines. With trypan blue solution, IC_(50) values were found to be4.64 mg/m L for K-562 and 3.68 mg/m L for Kusumi-1 cell lines. Spirulina aqueous extract also showed cytotoxicity with trypan blue method, at a slightly higher dose; where IC_(50) values were 12.68 mg/m L for K-562 and 2.13 mg/m L for Kusumi-1 cell lines. The IC_(50) values were found 0.40 mg/m L for K-562 and 0.31 mg/m L for Kusumi-1 cell lines for the 70% ethanol extract according to the MTT assay. Spirulina extract obtained with water also showed cytotoxicity but the dose was a little higher where IC_(50) values were15.77 mg/m L for K-562 and 9.44 mg/m L for Kusumi-1 cell lines. The effect of cytotoxicity with ethanol extract is quite comparable with that observed for cyclophosphamide, which is a chemical used as anticancer agent.Conclusions: The cytotoxicity exhibited by Spirulina extract to cancer cell lines might be due to the presence of phytopigments(carotenoids, chlorophyll, phycocyanin) as well as polysaccharides that were reported previously as constituents of the extract. So crude extracts of Spirulina can be used as a source to develop anticancer drugs.
文摘Objective:To assess the role of polymerase chain reaction(PCR)in serum sauples,in the diagnosis of osteoarticular tuberculosis(OTB)in a setting where only clinical and imaging diagnoses determine the treatment.Methods:A total of 44 consecutive serum specimens were collected from clinically suspected OTB patients,based on clinical and radiological[X-ray or magnetic resonance imagng/computecl tomography]features.They were scrcened by in-house nested PCR.In addition,a few specimens were examined by Gram stain,acid-fast bacilli stain,histand routine bacterial culture.A total of 39 specimens were collected from patients suffering from other bone diseases of nontuberculous origin and included as negative controls.Results:of the 44 clinically suspected OTB patients,in-house nested PCR was positive in 40(91%)cases;PCR was negative in 38(97%)negative controls.Sensitivity and specificity of our in—house nested PCR was 90.3%and 97.4%,respectively.The PCR report was available within 48 h.It was possible to standardize serum PCR technique and in positive cases,a good n was observed in terms of an adequate treatment response.Conclusions:Nested PCR in serum samples is a rapid,highly sensitive and specific modality for OTB detection,PCR should be performed in addition to clinical evaluation,imaging studies,acidfast bacilli staining,culture and histopathology diagnosis,if possible.

文摘Biological soil crusts are widely distributed in arid and semiarid regions. They have an important eco- logical role, especially by modifying physical and chemical properties of soils. Biological crusts may also modify seed germination and seedling establishment. The effects vary widely according to the type of crust and the vas- cular plant species. The objective of this study was to determine the effect of moss-dominated biological soil crusts on the emergence, biomass and survival of Poa ligularis Nees ex Steud. under different irrigation regimes. We col- lected seeds of P. ligularis and biological soil crusts composed of two species of mosses: Syntrichia princeps (De Not.) Mitt and Ceratodon purpureus (Hedw.) Brid. from an area in the Monte of Argentina. The result showed that seedling emergence of P. ligularis was higher in treatments with bare soil than in soil covered by crusts, and also in those with watering to field capacity. Mean emergence time was higher in treatments with bare soil and watering to field capacity. Seedling biomass also showed significant differences between treatments. These results suggest that biological soil crusts dominated by mosses do not promote P. ligularis emergence, although they would not affect its survival.
文摘The intersection between metabolic-associated steatotic liver disease(MASLD)and chronic hepatitis B virus(HBV)infection is an emerging area of research with significant implications for public health and clinical practice.Wang et al’s study highlights the complexities of managing patients with concurrent MASLD and HBV.The findings revealed that patients with concurrent MASLD-HBV exhibited more severe liver inflammation and fibrosis,whereas those with HBV alone pre-sented a better lipid profile.The growing recognition of metabolic dysfunction in liver disease,reflected in the shift from nonalcoholic liver disease to MASLD,demands updates to clinical guidelines,particularly for patients with dual etio-logies.Understanding the biological interactions between MASLD and HBV could lead to novel therapeutic approaches,emphasizing the need for personalized treatment strategies.The coexistence of MASLD and HBV presents therapeutic challenges,particularly in managing advanced fibrosis and cirrhosis,which are more likely in these patients.The aim of this editorial is to analyze the interaction between MASLD and HBV,highlight the pathophysiological mechanisms that exacerbate liver disease when both conditions coexist,and discuss the clinical implications of the findings of Wang et al.

文摘Re-vegetation plays a fundamental role for erosion control and plant recovery in lands affected by gully erosion. Bioengineered practices facilitate the gullies rehabilitation. Objectives of the research were: 1) Identify taxonomically the pioneer vegetation on each gully section; 2) Characterize vegetation distribution preferences and 3) Assess structural/functional traits to recognize erosion control key species. Bioengineering was applied in a watershed belonging to Sierra Madre del Sur, at Oaxaca, Mexico, on eight gullies, with local support and minimal investment. "La Mixteca" is a poor ecological and socio-economic region, comparable to other regions of the world. The Initial Floristic Composition(IFC) inventory is the baseline of the successional process. The transect method was used to determine the colonization of species. Cover abundance of registered species was estimated using the semi-quantitative scale of Braun-Blanquet. This procedure was repeated in five different positions(floor, hillslopes and tops), in the cross section of the gully. Throughcorrespondence analysis and clustering, the distribution of species was analyzed. Adequate responses were obtained in soil retention(quantity) and plant cover(existence and diversity); as measurable indicators of the bioengeneering works efficiency. Occupation of soil by native species from the Tropical Deciduous Forest was favored using live barriers. We detected species guilds with spatial distribution preferences in the gullies cross section. Plant cover characterization includes: native colonizer species, herbaceous, shrubby and trees of the forest community bordering the gully area, with cover abundance and structural/functional traits, useful to protect degraded areas. This spatial occupation process of plants responds to a secondary succession in gullies, where the proposed IFC model is correctly represented through bioengineering. 文摘AIM To characterize the prescribing patterns for hydrocortisone for patients with septic shock and perform an exploratory analysis in order to identify the variables associated with better outcomes.METHODS This prospective cohort study included 59 patients with septic shock who received stress-dose hydrocortisone.It was performed at 2 critical care units in academic hospitals from June 1st, 2015, to July 31 st, 2016. Demographic data, comorbidities, medical management details, adverse effects related to corticosteroids, and outcomes were collected after the critical care physician indicated initiation of hydrocortisone. Univariate comparison between continuous and bolus administration of hydrocortisone was performed, including multivariate analysis, as well as Kaplan-Meier analysis to compare the proportion of shock reversal at 7 d after presentation. Receiver operating characteristic(ROC) curves determined the best cut-off criteria for initiation of hydrocortisone associated with the highest probability of shock reversal. We addressed the effects of the taper strategy for discontinuation of hydrocortisone, noting risk of shock relapse and adverse effects.RESULTS All-cause 30-d mortality was 42%. Hydrocortisone was administered as a continuous infusion in 54.2% of patients; time to reversal of shock was 49 h longer in patients who were given a bolus administration [59 h(range, 47.5-90.5) vs 108 h(range, 63.2-189); P = 0.001]. The maximal dose of norepinephrine after initiation of hydrocortisone was lower in patients on continuous infusion [0.19 μg/kg per minute(range, 0.11-0.28 μg)] compared with patients who were given bolus [0.34 μg/kg per minute(range, 0.16-0.49); P = 0.004]. Kaplan-Meier analysis revealed a higher proportion of shock reversal at 7 d in patients with continuous infusion compared to those given bolus(83% vs 63%; P = 0.004). 文摘Although antivirals are important tools to control severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection,effective vaccines are essential to control the current coronavirus disease 2019(COVID-19)pandemic.Plant-derived virus-like particle(VLP)vaccine candidates have previously demonstrated immunogenicity and efficacy against influenza.Here,we report the immunogenicity and protection induced in rhesus macaques by intramuscular injections of a VLP bearing a SARS-CoV-2 spike protein(CoVLP)vaccine candidate formulated with or without Adjuvant System 03(AS03)or cytidine-phospho-guanosine(CpG)1018.Although a single dose of the unadjuvanted CoVLP vaccine candidate stimulated humoral and cell-mediated immune responses,booster immunization(at 28 days after priming)and adjuvant administration significantly improved both responses,with higher immunogenicity and protection provided by the AS03-adjuvanted CoVLP.Fifteen micrograms of CoVLP adjuvanted with AS03 induced a polyfunctional interleukin-2(IL-2)-driven response and IL-4 expression in CD4 T cells.Animals were challenged by multiple routes(i.e.,intratracheal,intranasal,and ocular)with a total viral dose of 106 plaque-forming units of SARS-CoV-2.Lower viral replication in nasal swabs and bronchoalveolar lavage fluid(BALF)as well as fewer SARS-CoV-2-infected cells and immune cell infiltrates in the lungs concomitant with reduced levels of proinflammatory cytokines and chemotactic factors in the BALF were observed in animals immunized with the CoVLP adjuvanted with AS03.No clinical,pathologic,or virologic evidence of vaccineassociated enhanced disease was observed in vaccinated animals.The CoVLP adjuvanted with AS03 was therefore selected for vaccine development and clinical trials.
