The sonomorphological findings were grouped according to a five-part classification scheme.RESULTS: Application of the new classification resulted in the following distribution of sonomorphological patterns among the patients examined: hailstorm(54.1%); pseudocystic(13.5%); ossification(13.0%); hemangioma-like(8.1%); and metastasis-like(6.5%). Only 4.9% of lesions could not be assigned to a sonomorphological pattern.CONCLUSION: The sonomorphological classification proposed in the present study facilitates the diagnosis,interpretation and comparison of hepatic alveolar echinococcosis in routine practice and in the context of scientific studies.展开更多

METHODS: One hundred patient...AIM: To establish the extent to which contrast enhancement with SonoVue in combination with quantitative evaluation of contrast-medium dynamics facilitates the detection of hepatic tumors. METHODS: One hundred patients with histologically confirmed malignant or benign hepatic tumor (maximum size 5 cm) were analyzed. Contrast-enhanced ultrasound (bolus injection 2.5 mL SonoVue) was carried out with intermittent breath-holding technique using a multifrequency transducer (2.5-4 MHz). Native vascularization was analyzed with power Doppler. The contrast-enhanced dynamic ultrasound investigation was carried out with contrast harmonic imaging in true detection mode during the arterial,portal venous and late phases. Mechanical index was set at 0.15. Perfusion analysis was performed by post-processing of the raw data time intensity curve (TIC) analysis. The cut-off of the gray value differences between tumor and normal liver tissue was established using Receiver Operating Characteristic (ROC) analysis 64-line multi-slice computed tomography served as reference method in all cases. Magnetic resonance tomography was used additionally in 19 cases. RESULTS: One hundred patients with 59 malignant (43 colon,5 breast,2 endocrine metastases,7 hepatocellular carcinomas and 2 kidney cancers) and 41 benign (15 hemangiomas,7 focal nodular hyperplasias,5 complicated cysts,2 abscesses and 12 circumscribed fatty changes) tumors were included. The late venous phase proved to be the most sensitive for classification of the tumor type. Fifty-eight of the 59 malignant tumors were classified as true positive,and one as false negative. This resulted in a sensitivity of 98.3%. Of the 41 benign tumors,37 were classified as true negative and 4 as false negative,which corresponds to a specificity of 90.2%. Altogether,95.0% of the diagnoses were classified as correct on the basis of the histological classification. No investigator-dependency (P = 0.23) was noted. However, reporting o...Inflammatory myofibroblastic tumors (IMTs) or inflammatory pseudotumors (IPs) have been extensively discussed in the literature. They are usually found in the lung and upper respiratory tract. However, reporting of cases involving the biliopancreatic region has increased over recent years. Immunohistochemical study of these lesions limited to the pancreatic head or distal bile duct seems to be compatible with those observed in a new entity called autoimmune pancreatitis, but usually intense fibrotic reaction (zonation) predominates producing a mass. When this condition is limited to the pancreatic head, the common bile duct might be involved by the inflammatory process and jaundice may occur often resembling adenocarcinoma of the pancreas. We have previously reported a case of IMT arising from the bile duct associated with autoimmune pancreatitis which is an extremely rare entity. Four years after Kaush-Whipple resection, radiological examination on routine follow-up revealed a tumor mass, suggesting local recurrence. Ultrasound-guided FNA confirmed our suspicious diagnosis. This present case, as others, suggests that persistent follow-up is necessary in order to prevent irreversible liver damage at this specific location.展开更多

Objectives: The aim of this study was to systematically review the relationship between tooth loss/number of teeth and hypertension. Materials and Methods: This review was conducted based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) Guidelines. We searched all the relevant studies in international databases of PubMed and Google Scholar search engines from January 2000 to December 2023. Results: Fourteen studies (11 cross sectionals, 2 descriptives and 1 Cohort) met the inclusion criteria for this review. Most cross-sectional studies showed that participants with more tooth loss exhibited a greater proportion of hypertension and higher systolic blood pressure than those with less tooth loss. Cohort study showed that the loss of 8 or more teeth was associated with a higher risk of developing hypertension. Descriptive studies reported that missing teeth were marginally significantly associated with hypertension. Conclusion: This study has revealed a significantly higher SBP in patients with fewer remaining teeth or more tooth loss. In addition, those with more tooth loss showed a higher risk of hypertension than those with less tooth loss.展开更多

Expert systems are being utilized increasingly in medical fields for the purposes of assisting diagnosis and treatment planning in Dentistry. Existing systems used few symptoms for dental diagnosis. In Dentistry, few symptoms are not enough for diagnosis. In this research, a conditional probability model (Bayes rule) was developed with increased number of symptoms associated with a disease for diagnosis. A test set of recurrent cases was then used to test the diagnostic capacity of the system. The generated diagnosis matched that of the human experts. The system was also tested for its capacity to handle uncommon dental diseases and the system portrayed useful potential. Method: The study used the Subjective Mathematical Bayes Model (SBM) approach and employed Logistic Regression Mathematical Model (LMR) techniques. The external validation of the subjective mathematical Bayes model (MSB) concerns the real cases of 625 patients who developed alveolar osteitis (OA). We propose strategies for reproducibility and reporting standards, outlining an updated WAMBS (when to Worry and how to Avoid the Misuse of Bayesian Statistics) checklist. Finally, we outline the impact of Bayesian analysis Logistic Regression Mathematical Model (LMR) techniques and on artificial intelligence, a major goal in the next decade. Results: The internal validation had identified seven (7) etiological factors of OA, which will be compared to the cases of MRL, for the external validation which retained six (6) etiological factors of OA. The experts in the internal validation of the MSB had generated 40 cases of OA and a COP of (0.5), which will be compared to the MRL that collected 625 real cases of OA to produce a Cop of (0.6) in the external validation, which discriminates between healthy patients (Se) and sick patients (Sp). Materials and...The aim of this study was to determine the relationship between inter canine width (ICW) and inter alar width (IAW), inter commissural width (ICoW), and inter canthal distance (ICaD) in Bantu population. Materials and Methods: It was a pilot study included all participants who were aged at least 18 years, without a history of orthodontics treatment from Prosthodontics Department, Faculty of Dental Medicine, University of Kinshasa. The Ethics committee of the National Center of Research for Dental Sciences in DR Congo approved the study protocol (CNRSB 1504.218). The participants medical records were obtained from the interview and dental examination. Nature of the study was explained with participant information sheet, and an informed consent of all participants was obtained. The inclusion criteria were no missing maxillary and mandibular teeth, no diastema, and no anterior restorations, 18 years old before enrollment in the study. The exclusion criteria were inability to meet the above requirements, pregnancy, and refusal to participate in the study. The inter-canine width (ICW), inter alar width (IAW), inter canthal distance (ICaD) and inter-commissural width (ICoW) of each participant were measured with a digital Vernier caliper (Mitutoyo, UK Ltd.,) (0.01 mm) three times and the average value was recorded. The inter canthal distance (ICaD) was measured without applying pressure by bringing the recording parts of the caliper just in contact with the medial angle. The inter alar width (IAW) was marked with a fine marked pencil on the widest outer surface of the alae of the nose on either side or width. Those two points were measured without applying pressure by bringing the recording parts of the caliper just in contact with the outer surface. The participant was told to stop breathing shortly to avoid any change in shape of the nose. The inter-commissural width (ICoW) was determined by measuring the maxillary lip vermilion from commissure to commissure without the application of pressure on the tissue in the re展开更多

Unfortunately, while the majority of GC patient...Gastric cancer(GC) is a global health problem and a major cause of cancer-related death with high recurrence rates ranging from 25% to 40% for GC patients staging Ⅱ-Ⅳ. Unfortunately, while the majority of GC patients usually present with advanced tumor stage; there is still limited evidence-based therapeutic options. Current approach to GC management consists mainly of; endoscopy followed by, gastrectomy and chemotherapy or chemo-radiotherapy. Recent studies in GC have confirmed that it is a heterogeneous disease. Many molecular characterization studies have been performed in GC. Recent discoveries of the molecular pathways underlying the disease have opened the door to more personalized treatment and better predictable outcome. The identification of molecular markers is a useful tool for clinical managementin GC patients, assisting in diagnosis, evaluation of response to treatment and development of novel therapeutic modalities. While chemotherapeutic agents have certain physiological effects on the tumor cells, the prediction of the response is different from one type of tumor to the other. The specificity of molecular biomarkers is a principal feature driving their application in anticancer therapies. Here we are trying to focus on the role of molecular pathways of GC and well-established molecular markers that can guide the therapeutic management.展开更多

The aim of this study was to assess the prevalence of SP and to compare its ...Purpose: Severe periodontitis (SP) is the sixth most prevalent disease world-wide. Prevalence and incidence are poorly reported in DR Congo. The aim of this study was to assess the prevalence of SP and to compare its profile with that of mild to moderate periodontitis (MMP). Materials and Methods: Four hundred and four patients from four hospitals of Kinshasa City Capital participated in this study. Probing pocket depth (PPD) at six sites per tooth, bleeding on probing (BoP) and plaque index (PIl) were the main clinical parameters assessed. Results: Out of 404 patients examined in this study, 92 (22.8%) had severe periodontitis and 276 (68.3%) had mild to moderate periodontitis. Among patients with SP, 48 (52%) were female, 44 (48%) were male and 63% of them had a PPD of 6 mm. SP was significantly different from MMP according to patients age, 47.3 ± 18.6 versus 32.87 ± 14.40 years old, p < 0.001;plaque index, 1.153 ± 0.39 versus 0.90 ± 0.39, p < 0.001;BoP, 26.65 versus 17.04, p < 0.001. Conclusion: The Frequency of SP is relatively high in patients attending dental services in Kinshasa City and SP is significantly different from MMP.展开更多

The current distributions and radiation patterns of some NMHAs are calculated.A comparison is made between results obtained using this helical segment algorithm and a linear segment algorithm, and the results of the two algorithms agree fairly well. When calculating the impedance matrix [Z], all the elements of the matrix can be obtained by only calculating a few elements with the application of the symmetric and periodic characteristics of the NMHA.Therefore, the CPU time and the memory storage are significantly reduced, with the accuracy and speed enhanced.展开更多
Phase-field modeling is an effective but computationally expensive method for capturing the mesoscale morphological and microstructure evolution in materials.Hence,fast and generalizable surrogate models are needed to...Phase-field modeling is an effective but computationally expensive method for capturing the mesoscale morphological and microstructure evolution in materials.Hence,fast and generalizable surrogate models are needed to alleviate the cost of computationally taxing processes such as in optimization and design of materials.The intrinsic discontinuous nature of the physical phenomena incurred by the presence of sharp phase boundaries makes the training of the surrogate model cumbersome.We develop a framework that integrates a convolutional autoencoder architecture with a deep neural operator(DeepONet)to learn the dynamic evolution of a two-phase mixture and accelerate time-to-solution in predicting the microstructure evolution.We utilize the convolutional autoencoder to provide a compact representation of the microstructure data in a low-dimensional latent space.After DeepONet is trained in the latent space,it can be used to replace the high-fidelity phase-field numerical solver in interpolation tasks or to accelerate the numerical solver in extrapolation tasks.展开更多
AIM To evaluate the prognostic value of the tumor-stroma ratio(TSR) in rectal cancer.METHODS TSR was determined on hematoxylin and eosin stained histological sections of 154 patients treated for rectal adenocarcinoma ...AIM To evaluate the prognostic value of the tumor-stroma ratio(TSR) in rectal cancer.METHODS TSR was determined on hematoxylin and eosin stained histological sections of 154 patients treated for rectal adenocarcinoma without prior neoadjuvant treatment in the period 1996-2006 by two observers to assessreproducibility. Patients were categorized into three categories: TSR-high [carcinoma percentage(CP) ≥ 70%], TSR-intermediate(CP 40%, 50% and 60%) and TSR-low(CP ≤ 30%). The relation between categorized TSR and survival was analyzed using Cox proportional hazards model.RESULTS Thirty-six(23.4%) patients were scored as TSR-low, 70(45.4%) as TSR-intermediate and 48(31.2%) as TSRhigh. TSR had a good interobserver agreement(κ = 0.724, concordance 82.5%). Overall survival(OS) and disease free survival(DFS) were significantly better for patients with a high TSR(P = 0.01 and P = 0.02, respectively). A similar association existed for disease specific survival(P = 0.06). In multivariate analysis, patients without lymph node metastasis and an intermediate TSR had a higher risk of dying from rectal cancer(HR = 5.27, 95%CI: 1.54-18.10), compared to lymph node metastasis negative patients with a high TSR. This group also had a worse DFS(HR = 6.41, 95%CI: 1.84-22.28). An identical association was seen for OS. These relations were not seen in lymph node metastasis positive patients. CONCLUSION The TSR has potential as a prognostic factor for survival in surgically treated rectal cancer patients, especially in lymph node negative cases.展开更多

Finally, we outline the impact of Bayesian analysis Logistic Regression Mathematical Model (LMR) techniques and on artificial intelligence, a major goal in the next decade. Results: The internal validation had identified seven (7) etiological factors of OA, which will be compared to the cases of MRL, for the external validation which retained six (6) etiological factors of OA. The experts in the internal validation of the MSB had generated 40 cases of OA and a COP of (0.5), which will be compared to the MRL that collected 625 real cases of OA to produce a Cop of (0.6) in the external validation, which discriminates between healthy patients (Se) and sick patients (Sp). Compared to real cases and the logistic regression model, the Bayesian model is efficient
文摘The aim of this study was to determine the relationship between inter canine width (ICW) and inter alar width (IAW), inter commissural width (ICoW), and inter canthal distance (ICaD) in Bantu population. Materials and Methods: It was a pilot study included all participants who were aged at least 18 years, without a history of orthodontics treatment from Prosthodontics Department, Faculty of Dental Medicine, University of Kinshasa. The Ethics committee of the National Center of Research for Dental Sciences in DR Congo approved the study protocol (CNRSB 1504.218). The participants medical records were obtained from the interview and dental examination. Nature of the study was explained with participant information sheet, and an informed consent of all participants was obtained. The inclusion criteria were no missing maxillary and mandibular teeth, no diastema, and no anterior restorations, 18 years old before enrollment in the study. The exclusion criteria were inability to meet the above requirements, pregnancy, and refusal to participate in the study. The inter-canine width (ICW), inter alar width (IAW), inter canthal distance (ICaD) and inter-commissural width (ICoW) of each participant were measured with a digital Vernier caliper (Mitutoyo, UK Ltd.,) (0.01 mm) three times and the average value was recorded. The inter canthal distance (ICaD) was measured without applying pressure by bringing the recording parts of the caliper just in contact with the medial angle. The inter alar width (IAW) was marked with a fine marked pencil on the widest outer surface of the alae of the nose on either side or width. Those two points were measured without applying pressure by bringing the recording parts of the caliper just in contact with the outer surface. The participant was told to stop breathing shortly to avoid any change in shape of the nose. 文摘该随机临床试验的主要目的是研究慢性腹壁疼痛病人接受超声引导下腹横肌平面(transversus abdominis plane,TAP)阻滞与超声引导下激痛点注射(trigger point injections,TPI)治疗,术后3个月随访时疼痛数字评分法(NRS)评分情况。主要结局评估方法是使用意向性分析(ITT)方法,比较TAP组和TPI组在随访第3个月时NRS评分的差异。共纳入60例病人,以1:1的比例随机分入超声引导下TAP阻滞组(n=30)或超声引导下TPI组(n=30)。TAP组和TPI组的平均基线疼痛评分分别为5.5和4.7。在随访第3个月的ITT分析中,组间疼痛评分差异为1.7(95%CI,0.3-3.0),TPI组效果更好。在后续按方案分析中,组间疼痛评分差异为1.8(95%CI,0.4-3.2),TPI组效果更好。在ITT和按方案分析中,组间疼痛评分差异达到了中等效应量。该研究表明与接受TAP阻滞的慢性腹壁疼痛病人相比,接受TPI的病人在随访第3个月时的疼痛评分显著降低。
文摘Gastric cancer(GC) is a global health problem and a major cause of cancer-related death with high recurrence rates ranging from 25% to 40% for GC patients staging Ⅱ-Ⅳ. Unfortunately, while the majority of GC patients usually present with advanced tumor stage; there is still limited evidence-based therapeutic options. Current approach to GC management consists mainly of; endoscopy followed by, gastrectomy and chemotherapy or chemo-radiotherapy. Recent studies in GC have confirmed that it is a heterogeneous disease. Many molecular characterization studies have been performed in GC. Recent discoveries of the molecular pathways underlying the disease have opened the door to more personalized treatment and better predictable outcome. The identification of molecular markers is a useful tool for clinical managementin GC patients, assisting in diagnosis, evaluation of response to treatment and development of novel therapeutic modalities. While chemotherapeutic agents have certain physiological effects on the tumor cells, the prediction of the response is different from one type of tumor to the other. The specificity of molecular biomarkers is a principal feature driving their application in anticancer therapies. Here we are trying to focus on the role of molecular pathways of GC and well-established molecular markers that can guide the therapeutic management.
文摘Purpose: Severe periodontitis (SP) is the sixth most prevalent disease world-wide. Prevalence and incidence are poorly reported in DR Congo. The aim of this study was to assess the prevalence of SP and to compare its profile with that of mild to moderate periodontitis (MMP). Materials and Methods: Four hundred and four patients from four hospitals of Kinshasa City Capital participated in this study. Probing pocket depth (PPD) at six sites per tooth, bleeding on probing (BoP) and plaque index (PIl) were the main clinical parameters assessed. Results: Out of 404 patients examined in this study, 92 (22.8%) had severe periodontitis and 276 (68.3%) had mild to moderate periodontitis. Among patients with SP, 48 (52%) were female, 44 (48%) were male and 63% of them had a PPD of 6 mm. SP was significantly different from MMP according to patients age, 47.3 ± 18.6 versus 32.87 ± 14.40 years old, p < 0.001;plaque index, 1.153 ± 0.39 versus 0.90 ± 0.39, p < 0.001;BoP, 26.65 versus 17.04, p < 0.001. Conclusion: The Frequency of SP is relatively high in patients attending dental services in Kinshasa City and SP is significantly different from MMP.
文摘In this letter, a new moment method using helical segments is presented to model Normal Mode Helical Antenna (NMHA). Using this method, the NMHA can be modeled by a few segments. The current distributions and radiation patterns of some NMHAs are calculated.A comparison is made between results obtained using this helical segment algorithm and a linear segment algorithm, and the results of the two algorithms agree fairly well. When calculating the impedance matrix [Z], all the elements of the matrix can be obtained by only calculating a few elements with the application of the symmetric and periodic characteristics of the NMHA.Therefore, the CPU time and the memory storage are significantly reduced, with the accuracy and speed enhanced.

文摘Phase-field modeling is an effective but computationally expensive method for capturing the mesoscale morphological and microstructure evolution in materials.Hence,fast and generalizable surrogate models are needed to alleviate the cost of computationally taxing processes such as in optimization and design of materials.The intrinsic discontinuous nature of the physical phenomena incurred by the presence of sharp phase boundaries makes the training of the surrogate model cumbersome.We develop a framework that integrates a convolutional autoencoder architecture with a deep neural operator(DeepONet)to learn the dynamic evolution of a two-phase mixture and accelerate time-to-solution in predicting the microstructure evolution.We utilize the convolutional autoencoder to provide a compact representation of the microstructure data in a low-dimensional latent space.After DeepONet is trained in the latent space,it can be used to replace the high-fidelity phase-field numerical solver in interpolation tasks or to accelerate the numerical solver in extrapolation tasks.
文摘AIM To evaluate the prognostic value of the tumor-stroma ratio(TSR) in rectal cancer.METHODS TSR was determined on hematoxylin and eosin stained histological sections of 154 patients treated for rectal adenocarcinoma without prior neoadjuvant treatment in the period 1996-2006 by two observers to assessreproducibility. Patients were categorized into three categories: TSR-high [carcinoma percentage(CP) ≥ 70%], TSR-intermediate(CP 40%, 50% and 60%) and TSR-low(CP ≤ 30%). The relation between categorized TSR and survival was analyzed using Cox proportional hazards model.RESULTS Thirty-six(23.4%) patients were scored as TSR-low, 70(45.4%) as TSR-intermediate and 48(31.2%) as TSRhigh. TSR had a good interobserver agreement(κ = 0.724, concordance 82.5%). Overall survival(OS) and disease free survival(DFS) were significantly better for patients with a high TSR(P = 0.01 and P = 0.02, respectively). A similar association existed for disease specific survival(P = 0.06). In multivariate analysis, patients without lymph node metastasis and an intermediate TSR had a higher risk of dying from rectal cancer(HR = 5.27, 95%CI: 1.54-18.10), compared to lymph node metastasis negative patients with a high TSR. This group also had a worse DFS(HR = 6.41, 95%CI: 1.84-22.28). An identical association was seen for OS. These relations were not seen in lymph node metastasis positive patients. CONCLUSION The TSR has potential as a prognostic factor for survival in surgically treated rectal cancer patients, especially in lymph node negative cases.
