2型糖尿病是一种以慢性高血糖为主要特征的代谢性疾病,其发病机制复杂.近年来的研究发现,糖脂代谢紊乱造成的慢性炎症可引起机体胰岛损伤,与2型糖尿病发生发展密切相关.但是,由代谢紊乱诱发的胰岛炎症如何发展并最终导致胰岛功能不可逆... 2型糖尿病是一种以慢性高血糖为主要特征的代谢性疾病,其发病机制复杂.近年来的研究发现,糖脂代谢紊乱造成的慢性炎症可引起机体胰岛损伤,与2型糖尿病发生发展密切相关.但是,由代谢紊乱诱发的胰岛炎症如何发展并最终导致胰岛功能不可逆性损伤的机制尚未完全阐明.本研究,首先选取自发性2型糖尿病GK大鼠和转基因动物IDOL小鼠,建立了胰岛炎症实验模型并对相关炎症因子变化进行检测,结果显示巨噬细胞与胰岛炎症进程相关.随后建立相应的定量数学模型描述胰岛β细胞和巨噬细胞相互作用,理论模型的结果表明,胰岛β细胞和巨噬细胞之间的正反馈回路可能导致IL-1β的激活过程存在不可逆双稳态和磁滞回线效应,并导致胰岛炎症的不可逆性发展.本研究提出了胰岛炎症过程中动力学机理,希望能够为2型糖尿病发病机制的深入研究和治疗药物的研发提供新的思路和有益的帮助. 展开更多

We report the results of a search for radio pulsars in five supernova remnants(SNRs)with the FAST telescope.The observations were made using the 19-beam receiver in“snapshot”mode.The integration time for each pointing was 10 min.We discovered a new pulsar,PSR J1845–0306,which has a spin period of 983.6 ms and a dispersion measure of 444.6±2.0 cm^(−3)·pc,in observations of SNR G29.6+0.1.To judge the association between the pulsar and the SNR,further verification is needed.We also re-detected some known pulsars in the data from SNRs G29.6+0.1 and G29.7–0.3.No pulsars were detected in the observations of the other three SNRs. 展开更多

目的探究不同剂量舒芬太尼用于小儿腹腔镜下疝内环口缝扎术的临床效果。方法选取2015年1月-2018年1月南京医科大学附属儿童医院收治的腹股沟斜疝患儿120例,均于腹腔镜下行疝内环口缝扎术。术前按照随机数字表法将其分为A、B和C组,每组4... 目的探究不同剂量舒芬太尼用于小儿腹腔镜下疝内环口缝扎术的临床效果。方法选取2015年1月-2018年1月南京医科大学附属儿童医院收治的腹股沟斜疝患儿120例,均于腹腔镜下行疝内环口缝扎术。术前按照随机数字表法将其分为A、B和C组,每组40例。3组患儿均采用静脉诱导,A、B、C组患儿依次给予舒芬太尼(0.3、0.4、0.5μg/kg)、咪哒唑仑(0.1 mg/kg)和丙泊酚(2.0 mg/kg)。术中采用顺苯磺阿曲库铵0.12 mg/(kg·h)和丙泊酚10 mg/(kg·h)维持,根据心率(HR)决定是否追加舒芬太尼。记录各组患儿麻醉诱导前(T0)、气管插管前(T1)、气管插管时(T2)、手术开始时(T3)、气腹建立时(T4)、气腹后5 min(T5)和气管拔管时(T6)的HR、收缩压(SBP)和血氧饱和度(SpO2)。比较3组麻醉诱导时间、手术时间、苏醒时间、舒芬太尼追加率、不良反应情况及术后Wong-Baker脸谱法镇痛评分和Ramsay镇静评分。结果A组麻醉诱导时间长于B和C两组(P<0.05)。3组术中不同时间点的HR、SBP和SpO2有差异(P<0.05)。3组术后不同时间点镇痛和镇静评分差异有统计学意义(P<0.05),A组镇痛评分高于B和C组,但镇静评分低于B和C组。结论对行腹腔镜下疝内环口缝扎术的患儿,诱导麻醉选用0.4μg/kg舒芬太尼可提供满意的麻醉效果,安全性高,值得推广。 展开更多

目的观察纳美芬对新生儿腹腔镜幽门环切术后苏醒的临床效果。方法择期气管插管吸入麻醉下腹腔镜幽门环切术新生儿60例,年龄15~28 d,美国麻醉医师协会(ASA)Ⅱ~Ⅲ级,随机分为纳美芬组(M组)和纳洛酮组(L组),每组30例。手术结束时M组静脉注... 目的观察纳美芬对新生儿腹腔镜幽门环切术后苏醒的临床效果。方法择期气管插管吸入麻醉下腹腔镜幽门环切术新生儿60例,年龄15~28 d,美国麻醉医师协会(ASA)Ⅱ~Ⅲ级,随机分为纳美芬组(M组)和纳洛酮组(L组),每组30例。手术结束时M组静脉注射纳美芬0.25μg/kg,L组静脉注射纳洛酮1.00μg/kg。并记录静脉注射纳美芬或纳洛酮前(T_0)、给药后10 min(T_1)、拔管后30 min(T_2)和拔管后2 h(T_3)的脉搏血氧饱和度(SpO_2)、平均动脉压(MAP)、心率(HR)和呼吸频率(RR);记录自主呼吸恢复时间、拔管时间、麻醉后监测治疗室(PACU)停留时间及手术结束后24 h内不良反应(如呼吸抑制、恶心呕吐、嗜睡和躁动等)。结果两组在T_0、T_1、T_2和T_3时点,SpO_2、MAP和HR比较均差异无统计学意义(P>0.05),两组患儿T_1、T_2和T_3时的RR,均较T_0时明显升高(P<0.05),且T_1时,M组RR明显快于L组(P<0.05)。M组患儿自主呼吸恢复时间、拔管时间和PACU停留时间差均明显短于L组(P<0.05)。术后不良反应差异无统计学意义。结论纳美芬在合理剂量范围内,有助于新生儿腹腔镜幽门环切术后早期苏醒。和纳络酮相比,可有效缩短拔管时间,促进早期恢复。 展开更多

目的观察舒芬太尼在儿童全麻肠镜检查中的临床效果和安全性。方法择期全麻下行肠镜检查患儿80例,年龄5~10岁,体重18~35 kg,随机分为舒芬太尼组(S组)和芬太尼组(F组),每组40例。S组静注舒芬太尼0.2μg/kg,F组静注芬太尼2.0μg/kg,随后两... 目的观察舒芬太尼在儿童全麻肠镜检查中的临床效果和安全性。方法择期全麻下行肠镜检查患儿80例,年龄5~10岁,体重18~35 kg,随机分为舒芬太尼组(S组)和芬太尼组(F组),每组40例。S组静注舒芬太尼0.2μg/kg,F组静注芬太尼2.0μg/kg,随后两组均静注丙泊酚2.50 mg/kg,置入双腔喉罩,丙泊酚6.00~8.00 mg/(kg·h)泵注复合七氟醚2%~3%维持麻醉。记录麻醉诱导前(T_0)、放置喉罩时(T_1)、肠镜至回盲瓣时(T_2)及手术结束时(T_3)等时点的心率(HR)、平均动脉压(MAP)、脉搏氧饱和度(Sp O_2)及呼吸频率(RR),同时记录苏醒时间、注射部位疼痛、术中体动、术中有无返流、术后舌后坠、恶心呕吐、咽喉部疼痛及躁动等情况。结果 F组在T1和T2时HR、MAP高于麻醉诱导前(P<0.05),而S组HR、MAP差异无统计学意义(P>0.05)。T_1时两组RR均较诱导前明显降低(P<0.05),组间比较差异无统计学意义(P>0.05)。T_1、T_2时S组HR和MAP明显低于F组(P<0.05)。S组注射疼、诱导及苏醒时间、术后躁动发生率均较F组低(P<0.05)。结论舒芬太尼用于静吸复合全麻双腔喉罩通气下行小儿肠镜检查安全有效,麻醉效果满意,术中呼吸循环更稳定,术后苏醒质量高。 展开更多

目的:通过网状Meta分析评价中医药不同干预措施治疗外伤性视神经病变的临床疗效。方法:计算机检索中文数据库(CNKI、VIP、WanFang、SinoMed)和英文数据库(PubMed、Embase、Cochrane Library)自建库至2020-03已发表的关于中医药治疗外伤... 目的:通过网状Meta分析评价中医药不同干预措施治疗外伤性视神经病变的临床疗效。方法:计算机检索中文数据库(CNKI、VIP、WanFang、SinoMed)和英文数据库(PubMed、Embase、Cochrane Library)自建库至2020-03已发表的关于中医药治疗外伤性视神经病变的随机对照临床试验(RCT)文献,采用Cochrane评价工具对纳入研究进行质量评价,运用Stata 14.2软件进行网状Meta分析。结果:纳入21项RCT研究,患者1297例,涉及7种干预措施,其中中药、中药+激素类药物、针灸+中药+营养神经类药物治疗后视力改善情况优于单纯使用营养神经类药物,中药+激素类药物、针灸+中药+营养神经类药物、针灸+中药+激素类药物治疗后视力改善情况优于单纯使用激素类药物(P<0.05),治疗有效率排序第一位的干预措施是针灸+中药+营养神经类药物。结论:中医药治疗外伤性视神经病变具有优势,其中针灸+中药+营养神经类药物成为最优选择的可能性最大。 展开更多

为了探究设置左转专用相位对称放行时,双流向互阻塞型左转短车道进口道最大通过量及车流互阻塞概率的变化规律,基于车流泊松到达特征,考虑相邻周期车辆排队影响,运用概率论及交通流理论建立左转短车道进口道最大通过量解析模型。通过实... 为了探究设置左转专用相位对称放行时,双流向互阻塞型左转短车道进口道最大通过量及车流互阻塞概率的变化规律,基于车流泊松到达特征,考虑相邻周期车辆排队影响,运用概率论及交通流理论建立左转短车道进口道最大通过量解析模型。通过实测数据及VISSIM仿真对模型结果进行非参数假设检验及误差分析,并对影响进口道最大通过量的关键因素(短车道容量、周期、绿信比、相序、交通流量)进行敏感性分析。结果表明:在5%置信水平下模型值与实测值无显著性差异,相对误差绝对值小于10%,模型拟合度较高;当进口道车流量小于500pcu·h^(-1)时,周期时长对进口道通过量无影响,否则进口道通过量随周期的增加先缓慢增加后逐渐减小,存在最佳周期,最佳周期随左转车比例或短车道容量的增加而增加,范围为[40s,80s];当进口道车流量大于700pcu·h^(-1)时,增加短车道容量可有效提升进口道最大通过量,与独立左转车道进口道相比,短车道容量越大,通过量折减越小;当周期小于60s或左转与相邻直行车流量小于350pcu·h^(-1)时,相序对进口道通过量影响较小,反之则影响较大;先放行饱和度较大的车流可提升进口道通过量;利用该方法以进口道通过量最大为目标获得的短车道容量,相比于AASHTO短车道容量经验值,在左转或直行车比例大于等于70%时能够达到更优的车流运行效益,满足实际运行需求。 展开更多

Platycodin D(PD), a triterpenoid saponin isolated from Platycodonis Radix, is a famous Chinese herbal medicine that has been shown to have anti-proliferative effects in several cancer cell lines. The aim of this study was to determine the changes in cellular proteins after the treatment of hepatocellular carcinoma HepG2 cells with PD using proteomics approaches. The cell viability was determined using the MTT assay. The proteome was analyzed by two-dimensional difference gel electrophoresis and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. Western blot analysis was used to confirm the expression of changed proteins. Our results showed that PD inhibited the proliferation of HepG2 cells in concentration- and time-dependent manners. Sixteen proteins were identified to be up-regulated in PD-treated HepG2 cells, including ATP5 H, OXCT1, KRT9, CCDC40, ERP29, RCN1, ZNF175, HNRNPH1, HSP27, PA2G4, PHB, BANF1, TPM3, ECH1, LGALS1, and MYL6. Three proteins(i.e., RPS12, EMG1, and KRT1) decreased in HepG2 cells after treatment with PD. The changes in HSP27 and PHB were further confirmed by Western blotting. In conclusion, our results shed new lights on the mechanisms of action for the anti-cancer activity of PD. 展开更多

BACKGROUND Light chain(AL)amyloidosis is a plasma cell dyscrasia characterized by the pathologic production and extracellular tissue deposition of fibrillar proteins derived from immunoglobulin AL fragments secreted by a clone of plasma cells,which leads to progressive dysfunction of the affected organs.The two most commonly affected organs are the heart and kidneys,and liver is rarely the dominant affected organ with only 3.9%of cases,making them prone to misdia-gnosis and missed diagnosis.CASE SUMMARY A 65-year-old woman was admitted with a 3-mo history of progressive jaundice and marked hepatomegaly.Initially,based on enhanced computed tomography scan and angiography,Budd-Chiari syndrome was considered and balloon dilatation of significant hepatic vein stenoses was performed.However,addi-tional diagnostic procedures,including liver biopsy and bone marrow-exami-nation,revealed immunoglobulin kapa AL amyloidosis with extensive liver involvement and hepatic vascular compression.The disease course was progre-ssive and fatal,and the patient eventually died 5 mo after initial presentation of symptoms.CONCLUSION AL amyloidosis with isolated liver involvement is very rare,and can be easily misdiagnosed as a vascular disease. 展开更多

BACKGROUND Primary hepatic angiosarcoma(PHA)is a rare malignancy with a poor prognosis.It is difficult to diagnose PHA because of the lack of specific symptoms or tumour markers,and it rapidly progresses and has a high mortality.To our knowledge,PHA has not been reported to mimic hepatic sinusoidal obstruction syndrome.Herein,we present a case of PHA manifesting as hepatic sinusoidal obstruction syndrome,diagnosed using transjugular liver biopsy,that resulted in the death of the patient.CASE SUMMARY A 71-year-old man was admitted with the primary complaint of abdominal distension,decreased appetite,fatigue in the previous month,and loss of 10 kg of weight in the past 2 years.Both the liver and spleen were enlarged,and the liver had a medium-hard texture on percussion.Laboratory examinations were performed,and abdominal plain computed tomography(CT)and contrastenhanced CT showed hepatomegaly and splenomegaly,as well as diffuse lowdensity shadows distributed in the liver and spleen.Contrast-enhanced CT revealed diffuse,hypodense,nodular or flake shadows in the liver and heterogeneous enhancement in the spleen.A transjugular liver biopsy was performed.Based on the pathology results,the patient was diagnosed with hepatic sinusoidal obstruction syndrome secondary to PHA.The patient’s status further deteriorated and he developed serious hepatic failure.The patient was discharged,and died 3 d later.CONCLUSION PHA is rare and has a poor prognosis;however,transjugular liver biopsy can be safely performed to aid in diagnosis. 展开更多

BACKGROUND Sinusoidal obstruction syndrome has been reported after oxaliplatin-based chemotherapy,but liver fibrosis and non-cirrhotic portal hypertension(NCPH)are rarely reported.CASE SUMMARY Here,we describe the case of a 64-year-old woman who developed isolated gastric variceal bleeding 16 mo after completing eight cycles of oxaliplatin combined with capecitabine chemotherapy after colon cancer resection.Surprisingly,splenomegaly and thrombocytopenia were not accompanied by variceal bleeding,which has been reported to have predictive value for gastric variceal formation.However,a liver biopsy showed fibrosis in the portal area,suggesting NCPH.The patient underwent endoscopic treatment and experienced no further symptoms.CONCLUSION It is necessary to guard against long-term complications after oxaliplatin-based chemotherapy.Sometimes splenic size and platelet level may not always accurately predict the occurrence of portal hypertension. 展开更多

A new version of LSSVM, SVD-LSSVM, to save time of selecting hyper parameters for LSSVM is proposed. SVD-LSSVM is trained through singular value decomposition (SVD) of kernel matrix. Cross validation time of selecting hyper parameters can be saved because a new hyper parameter, singular value contribution rate (SVCR), replaces the penalty factor of LSSVM. Several UCI benchmarking data and the Olive classification problem were used to test SVD-LSSVM. The result showed that SVD-LSSVM has good performance in classification and saves time for cross validation. 展开更多

BACKGROUND It now seems that all pulmonary hamartomas(PHs)are large cystic-solid lesions that are difficult to diagnose.However,few cases of large cystic-solid PHs have been reported.The present case report presents a large cystic-solid PH and provides a literature review of the imaging features,formation mechanism and histopathological basis of PHs.CASE SUMMARY A 53-year-old woman with no clinical symptoms underwent a chest computed tomography(CT)examination at our hospital.Nonenhanced CT images revealed a large,flat tumor with multiple air-containing cysts in the left thoracic cavity and a cystic part confined to the medial side of the tumor;the solid part of the tumor showed abundant fat and lamellar soft tissue components.Multiple small blood vessels were detected in the solid part of the tumor on contrast-enhanced CT images.Given the large size of the lesion,the patient elected to undergo surgery.Histological examination revealed PH.A detailed review of the patient’s CT imaging showed that the lesion had a small vascular pedicle to the left lower lobe,which was a clue to its lung tissue histological origin.According to immunohistochemical staining,the confined multiple air-containing cysts were caused by the entrapment of respiratory/alveolar epithelium.CONCLUSION This case shows the imaging manifestations of a large PH.Heightened awareness of its formation mechanism and histopathological basis may alert radiologists to consider this diagnosis in their daily workflow. 展开更多

The reaction of singlet oxygen with ethylene has been studied at the ab initio level with both HF/3-21G and HF/6-31G basis sets, fully optimizing the geometries of the critical points. The transition state leading to the intermediate peroxirane from the initial reactants is found,Iocated at 81.71 kJ/mol above the dissociation limit. The vibrational analysis is done with two basis sets. From the results it can be seen that the frequencies have not been made an improvement obviously with augmented 6-31G compared to 3-21G basis; it follows that main reason for too high HF/3-21G frequency could mainly be the vibrational anharmonicity. The eigenvector corresponding to the single imaginary vibrational frequency is dominated by the larger O-O distance. The finding of the transition state confirms that the peroxirane minimum can be reachable passing through a peroxirane-like saddle point. In addition , the mechanisms of the reaction forming oxirane are discussed as well. 展开更多

Exposure to fine ambient particulate matter(PM_(2.5)) is known to be associated with cardiovascular disease. To uncover the molecular mechanisms involved in cardiovascular toxicity of PM_(2.5), we investigated alterations in the protein profile of human umbilical vein endothelial cells(HUVECs) treated with PM_(2.5) using two-dimensional electrophoresis in conjunction with mass spectrometry(MS). A total of 31 protein spots were selected as differentially expressed proteins and identified by matrix-assisted laser desorption/ionization-time of flight(MALDI-TOF) MS. The results demonstrated that DNA damage and cell apoptosis are important factors contributing to PM_(2.5)-mediated toxicity in HUVECs. It is further proposed that PM_(2.5) can inhibit superoxide dismutase(SOD) activity and increase reactive oxygen species(ROS) and malonaldehyde(MDA) production in a concentration-dependent manner. Induction of apoptosis and DNA damage through oxidative stress pathways may be one of the key toxicological events occurring in HUVECs under PM_(2.5) stress. These results indicated that the toxic mechanisms of PM_(2.5) on cardiovascular disease are related to endothelial dysfunction. 展开更多
