We present a controlled fabrication of selective ultrathin metal-organic framework(MOF)nanosheets as preassembling platforms,yolk-shell structured with a few-layered N-doped carbon(NC)shell-encapsulated Ni_(0.85)Se core(denoted as Ni_(0.85)Se@NC)via an oriented phase modulation(OPM)strategy.The ultrathin nature of the MOF nanosheets gave rise to the modification of structure at the electronic level with abundant Se-vacancies and effective electronic coupling via an Ni-N_(x) coordination at the interface between the Ni0.85Se core and NC shell.The Ni0.85Se@NC obtained exhibited low overpotentials for both oxygen evolution reaction(OER;300 mV)and hydrogen evolution reaction(HER;157 mV)at 10 mA·cm^(−2) under an alkaline condition,outperforming their corresponding bulk MOF-derived counterparts.By exploiting Ni_(0.85)Se@NC as anode and cathode catalysts,a low cell voltage of 1.61 V was achieved by performing alkaline water electrolysis.Remarkably,it also reached a high activity in natural seawater(pH=7.98)and simulated seawater(pH=7.86)electrolytes,even surpassing integrated Pt/C-RuO_(2)/CC electrodes.Density functional theory(DFT)studies illustrated that abundant Se-vacancies effectively regulated the electronic structure of Ni_(0.85)Se@NC by accelerating electron transfer from Ni to N atoms at the interface,and thus,enabling the Ni_(0.85)Se@NC to attain a near-optimal electronic configuration that stimulated ideal adsorption-free energy toward key reaction intermediates. 展开更多

However, little is known about the unplanne... Background Unplanned extubation is associated with adverse outcomes in intensive care unit. The massive burn patient differs from other critically ill patients in many ways. However, little is known about the unplanned decannulation (UD) in Burn Intensive Care Unit. This paper describes the special features of the circumstances and outcome of UD of tracheotomy tube in massive burn patients. Methods A case series study was performed between January 1999 and December 2008 and UD of tracheotomy tube was analyzed retrospectively. A total of 21 patients with 29 UD events were identified. Demographic data, diagnosis, intervention, UD events and outcome of UD patients were collected. Differences in proportions were compared using the chi-square (X2) or Fisher's exact test. Results Patients with UD were often burned with head and neck (67%) and combined with inhalation injury (62%). The majority of them (76%) were transferred patients, occurred early (55%) and were accidental UD (79%). UD events tended to happen in day shift (90%) and to be associated with the medical procedure that was performing by caregivers at besides (79%). Loose of the stabilizing rope, medical procedure and tracheotomy malposition were the main causes of UD. Early UD and reintubation failure were associated with patients' death. Conclusions UD happened to massive burn patients can lead to patient death. Careful management of respiratory tract was essential for massive burn patients. 展开更多

目的探讨双蒂皮瓣在复杂软组织缺损创面修复中的应用价值。方法2014年1月至2018年12月,海军军医大学(第二军医大学)长海医院烧创伤外科对92例因肿瘤、创伤等因素导致软组织缺损的患者行双蒂皮瓣移植术,观察并记录病因、缺损部位、双蒂... 目的探讨双蒂皮瓣在复杂软组织缺损创面修复中的应用价值。方法2014年1月至2018年12月,海军军医大学(第二军医大学)长海医院烧创伤外科对92例因肿瘤、创伤等因素导致软组织缺损的患者行双蒂皮瓣移植术,观察并记录病因、缺损部位、双蒂皮瓣的长宽比和覆盖创面的面积、供瓣区的处置方式、血管穿支的保留情况、双蒂皮瓣的成活情况、供瓣区是否需要二次手术等。结果92例患者中,引起缺损的原因包括肿瘤切除术后49例、外伤36例(骨外露17例、内固定外露12例、肌腱外露7例)、其他原因7例(压疮5例、电烧伤2例)。软组织缺损部位依次为小腿(28例)、踝(16例)、大腿(14例)、躯干(12例)、上肢(11例)、臀部(6例)、足(5例)。双蒂皮瓣的长宽比为1.2~2.8,平均覆盖创面面积为(16.55±4.83)cm×(9.88±4.20)cm。供瓣区的处置方式包括直接缝合43例、延迟缝合26例、植皮23例。9例因皮瓣转移跨度、面积或长宽比较大,在分离时保留部分肌皮血管穿支。术后21 d时,89例皮瓣完全成活,3例皮瓣边缘少许坏死,祛除坏死部分后拉拢缝合。9例供瓣区存在残余创面,行二次植皮手术。结论双蒂皮瓣具有操作简便、存活可靠、创伤较少、部分供瓣区能直接或延迟缝合的优势,在肿瘤、创伤等复杂软组织缺损创面的修复中具有良好的应用价值。 展开更多
