In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named "2019 novel coronavirus(2019-nCoV)" by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. Hepatic artery infusion chemotherapy using mFOLFOX versus transarterial chemoembolization for massive unresectable hepatocellular carcinoma:a prospective non.randomized study 被引量:110

Background: Transarterial chemoembolization(TACE) is recommended as the standard care for unresectable hepatocellular carcinoma(HCC) at Barcelona Clinic Liver Cancer(BCLC) stage A-B. However, the efficacy of TACE on large(> 10 cm) stage A-B HCC is far from satisfactory, and it is proposed that hepatic artery infusion chemotherapy(HAIC)might be a better first-line treatment of this disease. Hence, we compared the safety and efficacy of HAIC with the modified FOLFOX(mFOLFOX) regimen and those ofTACE in patients with massive unresectable HCC.Methods: A prospective, non-randomized, phase II study was conducted on patients with massive unresectable HCC. The protocol involved HAIC with the mFOLFOX regimen(oxaliplatin, 85 mg/m^2 intra-arterial infusion; leucovorin,400 mg/m^2 intra-arterial infusion; and fluorouracil, 400 mg/m2 bolus infusion and 2400 mg/m^2 continuous infusion)every 3 weeks and TACE with 50 mg of epirubicin, 50 mg of lobaplatin, 6 mg of mitomycin, and lipiodol and polyvinyl alcohol particles. The tumor responses, time-to-progression(TTP), and safety were assessed.Results: A total of 79 patients were recruited for this study: 38 in the HAIC group and 41 in the TACE group. The HAIC group exhibited higher partial response and disease control rates than did the TACE group(52.6% vs. 9.8%, P < 0.001;83.8% vs. 52.5%, P = 0.004). The median TTPs for the HAIC and TACE groups were 5.87 and 3.6 months(hazard radio[HR] = 2.35,95% confidence interval [CI] = 1.16-4.76, P = 0.015). More patients in the HAIC group than in the TACE group underwent resection(10 vs. 3,P = 0.033). The proportions of grade 3-4 adverse events(AE) and serious adverse events(SAE) were lower in the HAIC group than in the TACE group(grade 3-4 AEs: 13 vs. 27, P = 0.007;SAEs: 6 vs. 15,p = 0.044). Major and profound changes have taken place in China over the past 30 years. Rapid socioeconomic progress has exerted a great impact on lifestyle, ranging from food, clothing, working and living conditions, and means of transportation to leisure activities and entertainment. At the same time, new health problems have emerged, and health services are facing new challenges. Presently, cardiovascular diseases (CVD) are among the top health problems of the Chinese people, and pose a serious challenge to all engaged in the prevention and control of these diseases. An epidemic of CVD in China is emerging as a result of lifestyle changes, urbanization and longevity. Both national policy decision-making and medical practice urgently need an authoritative report which comprehensively reflects the trends in the epidemic of CVD and current preventive measures. Since 2005, guided by the Bureau of Disease Prevention of the Ministry of Health of the People's Republic of China and the National Center for Cardiovascular Diseases of China, nationwide experts in the fields of epidemiology, clinical medicine and health economics in the realms of CVD, cerebrovascular disease, diabetes and chronic kidney disease, completed the Report on Cardiovascular Diseases in China every year. The report aims to provide a timely review of the trend of the epidemic and to assess the progress of prevention and control of CVD. In addition, as the report is authoritative, representative and readable, it will become an information platform in the CVD field and an important reference book for government, academic institutes, medical organizations and clinical physicians. This publication is expected to play a positive role in the prevention and control of CVD in China. BACKGROUND: Angiogenesis is known to be essential to the survival, growth, invasion, and metastasis of tumor cells. Vascular endothelial growth factor (VEGF) are an important angiogenic factor regulating tumor angiogenesis, but its significance and tumor pathologic features are un- clear in hepatocellular carcinoma (HCC). In the present study, we analyzed expression of tissue VEGF, alteration of microvascular density (MVD) in microvessel angiogenesis, development and metastasis of HCC, and level of serum VEGF in differential diagnosis of benign and malignant liv- er diseases. METHODS: Tumor specimens were prospectively collected from HCC patients undergoing resection. Total RNAs were extracted and the expression levels were detected from different parts of HCC tissues. The cellular distributions of VEGF and MVD of liver tumors and their paracancerous and distal cancerous tissues were investigated by streptavi- din peroxidase (S-P) immunohistochemistry, respectively. The VEGF levels of circulating blood and hepatoma tissues were measured by enzyme-linked immunosorbent assay. RESULTS: The incidence of VEGF expression was 63.9% in HCCs (23/36 cases), 78.3% in non-encapsulated HCCs (18/23), and 90.9% in HCCs with extrahepatic metastasis (10/11), respectively. The VEGF expression was tightly correlated with MVD (P <0.01). The MVD in HCC with metastasis, low differentiation or non-encapsulation was significantly higher than that in HCC with intact capsule, high differentiation, or no metastasis. No significant diffe- rence was found between VEGF, MVD, tumor size, and hepatitis virus infection. The level of total RNA in HCC tis- sues was significantly lower but the VEGF level significantly higher than those in paracancerous or distal cancerous ones (P<0.01). The abnormal expression levels of VEGF in sera of HCC patients were directly correlated with the me- tastasis and recurrence of tumors. Apart from composi... The 7xxx series alloys are heat treatable wrought aluminium alloys based on the Al-Zn-Mg(-Cu) system. They are widely used in high-performance structural aerospace and transportation applications. Apart from compositional, casting and thermo-mechanical processing effects, the balance of properties is also significantly influenced by the way in which the materials are heat-treated. This paper describes the effects of homogenisation, solution treatment, quenching and ageing treatments on the evolution of the microstructure and properties of some important medium to high-strength 7xxx alloys. With a focus on recent work at Monash University, where the whole processing route from homogenisation to final ageing has been studied for thick plate products, it is reported how microstructural features such as dispersoids, coarse constituent particles, fine-scale precipitates, grain structure and grain boundary characteristics can be controlled by heat treatment to achieve improved microstructure-property combinations. In particular, the paper presents methods for dissolving unwanted coarse constituent particles by controlled high- temperature treatments, quench sensitivity evaluations based on a systematic study of continuous cooling precipitation behaviour, and ageing investigations of one-, two- and three-step ageing treatments using experimental and modelling approaches, in each case, the effects on both the microstructure and the resulting properties are discussed. 展开更多

Widely distributed on the southern margin of the North China Craton,the Taihua complex extends roughly in an east-west direction and the relatively complete successions are found in Lushan County,Henan Province.Like many other Archean terranes,the Taihua Complex can be divided into two major lithological units along the Dangze River,namely gneisses series and supracrustal rocks.The former is located on the north side of the river and chiefly composed of TTG gneisses and amphibolites;the latter on the south side of the river is mainly supracrustal rock.Coupled with the previous studies,the results obtained by this study show that the Taihua complex was formed in a large time span from Neoarchean to Palaeoproterozoic.The TTG gneisses and amphibolites are dated at Neoarchean (2794–2752 Ma).The 2.9 Ga and 3.1 Ga zircons in amphibolites could be xenocrysts.In the North China Craton,the 2.8–2.7 Ga old rocks crop out in several areas e.g.western Shandong Province,Jiaodong Peninsula and Lushan area of Henan Province.In addition,2.8–2.7 Ga detrital zircons or xenocryst zircons have been recognized in Huai’an,Fuping,Wutai areas of North China Craton and also reported in the Early Paleozoic diamondiferous kimberlites in Mengyin and Fuxian.All these age results indicate that the 2.8–2.7 Ga rocks may have been developed much broader region than today’s outcrops.Zircon Hf and whole rock Nd isotopes show that the 2.8–2.7 Ga tectono-thermal event represents an important period of crustal growth with minor ancient crust reworked in the North China Craton.However the formation of supracrustal rocks is limited to 2.2–2.0 Ga in the Palaeoproterozoic time,not the Archaean,as previously believed.Combined with the chronological data of aluminium-rich metamorphic rocks (Khondalite series) on the southern margin of the North China Craton and adjacent areas,it is suggested that the above areas have widely developed Paleoproterozoic passive continental margin environment. 展开更多

AIM: To observe the inhibition of antisense oligonucleotides (asON) phosphorthioate to the tissue inhibitors metalloproteinase-1 (TIMP-1) gene and protein expression in the liver tissue of immunologically induced hepatic fibrosis rats. The possibility of reversing hepatic fibrosis through gene therapy was observed. METHODS: Human serum albumin (HSA) was used to attack rats, as hepatic fibrosis model, in which asONs were used to block the gene and protein expressing TIMP-1. According to the analysis of modulator, structure protein, coding series of TIMP-1 genome, we designed four different asONs. These asONs were injected into the hepatic fibrosis models through coccygeal vein. The results was observed by RT-PCR for measuring TIMP-1 mRNA expression, immunohistochemistry and in situ hybridization for collagen I, II, special staining of collagen fiber, and electron microscopic examination. RESULTS: Hepatic fibrosis could last within 363 days in our modified model. The expressing level of TIMP-1 was high during hepatic fibrosis process. It has been proved by the immunohistochemical and the electron microscopic examination that the asON phosphorthioate of TIMP-1 could exactly express in vivo. The effect of colchicine was demonstrated to inhibit the expressing level of mRNA and the content of collagen I, III in the liver of experimental hepatic fibrosis rats. However, the electron microscopy research and the pathologic grading of hepatic fibrosis showed that there was no significant difference between the treatment group and the model group (P】 0.05). CONCLUSION: The experimental rat model of hepatic fibrosis is one of the preferable models to estimate the curative effect of anti-hepatic fibrosis drugs. The asON phosphorthioate of TIMP-1 could block the gene and protein expression of TIMP-1 in the liver of experimental hepatic fibrosis rats at the mRNA level. It is possible to reverse hepatic fibrosis, and it is expected to study a new drug of antihepatic fibrosis on the genetic level. Colchicine has very limited th 展开更多

MYB-type transcription factors contain the conserved MYB DNA-binding domain of approximately 50 amino acids and are involved in the regulation of many aspects of plant growth, development, metabolism and stress responses. From soybean plants, we identified 156 GmMYB genes using our previously obtained 206 MYB unigenes, and 48 were found to have full-length open-reading frames. Expressions of all these identified genes were examined, and we found that expressions of 43 genes were changed upon treatment with ABA, salt, drought and/or cold stress. Three GmMYB genes, GmMYB76, GmMYB92 and GmMYB177, were chosen for further analysis. Using the yeast assay system, GmMYB76 and GmMYB92 were found to have transactivation activity and can form homodimers. GmMYB177 did not appear to have transactivation activity but can form heterodimers with GmMYB76. Yeast onehybrid assay revealed that all the three GmMYBs could bind to cis-elements TAT AAC GGT TTT TT and CCG GAA AAA AGG AT, but with different affinity, and GmMYB92 could also bind to TCT CAC CTA CC. The transgenic Arabidopsis plants overexpressing GmMYB 76 or GmMYB177 showed better performance than the GmMYB92-transgenic plants in salt and freezing tolerance. However, these transgenic plants exhibited reduced sensitivity to ABA treatment at germination stage in comparison with the wild-type plants. The three GmMYB genes differentially affected a subset of stress-responsive genes in addition to their regulation of a common subset of stress-responsive genes. These resuits indicate that the three GmMYB genes may play differential roles in stress tolerance, possibly through regulation of stress-responsive genes. 展开更多

A new vortex identification criterion called W-method is proposed based on the ideas that vorticity overtakes deformation in vortex.The comparison with other vortex identification methods like Q-criterion and λ_2-method is conducted and the advantages of the new method can be summarized as follows:(1) the method is able to capture vortex well and very easy to perform;(2) the physical meaning of W is clear while the interpretations of iso-surface values of Q and λ_2 chosen to visualize vortices are obscure;(3)being different from Q and λ_2 iso-surface visualization which requires wildly various thresholds to capture the vortex structure properly, W is pretty universal and does not need much adjustment in different cases and the iso-surfaces of W=0.52 can always capture the vortices properly in all the cases at different time steps, which we investigated;(4) both strong and weak vortices can be captured well simultaneously while improper Q and λ_2 threshold may lead to strong vortex capture while weak vortices are lost or weak vortices are captured but strong vortices are smeared;(5) W=0.52 is a quantity to approximately define the vortex boundary. Note that, to calculate W, the length and velocity must be used in the non-dimensional form. From our direct numerical simulation, it is found that the vorticity direction is very different from the vortex rotation direction in general 3-D vortical flow,the Helmholtz velocity decomposition is reviewed and vorticity is proposed to be further decomposed to vortical vorticity and non-vortical vorticity. 展开更多

Magnesite-bearing garnet Iherzolite from the Altyn Tagh, associated with garnet pyroxenite, and garnet-bearing felsic gneiss, crops out as lenses in Proterozoic gneiss about 100 km east of the Jianggelasayi River. The garnet Iherzolite, together with the eclogite in western Jianggelasayi, composes a high-pressure to ultrahigh-pressure metamorphic belt in the southern margin of the Altyn Tagh. Parageneses of minerals from magnesite-bearing garnet Iherzolite indicate that the rock evolved a multi-stage metamorphism. The peak-stage metamorphism produces an assemblage of Grt+Ol+Opx+Cpx±Mgs, in which A12O3 content of Opxis very low (0.30-0.66wt%). The calculated P-T condition of the peak stage is 3.8-5.lGPa and 880-970°C, some exsolution rods of clinopyroxene and rutile occur in the Grt and magnesite is rimed by dolomite and orthopyroxene, all implying that the peak stage was a UHP metamorphic process. Background and purpose Stroke is the leading cause of mortality and disability in China.Precise aetiological classification,imaging and biological markers may predict the prognosis of stroke.The Third China National Stroke Registry(CNSR-Ⅲ),a nationwide registry of ischaemic stroke or transient ischaemic attack(TIA)in China based on aetiology,imaging and biology markers,will be considered to clarify the pathogenesis and prognostic factors of ischaemic stroke.Methods Between August 2015 and March 2018,the CNSR-Ⅲrecruited consecutive patients with ischaemic stroke or TIA from 201 hospitals that cover 22 provinces and four municipalities in China.Clinical data were collected prospectively using an electronic data capture system by face-to-face interviews.Patients were followed for clinical outcomes at 3 months,6 months and 1-5 year annually.Brain imaging,including brain MRI and CT,were completed at baseline.Blood samples were collected and biomarkers were tested at baseline.Results A total of 15166 stroke patients were enrolled,among which 31.7%patients were women with the average age of 62.2±11.3 years.Ischaemic stroke was predominant(93.3%,n=14146)and 1020(6.7%)TIAs were enrolled.Conclusions CNSR-Ⅲis a large scale nationwide registry in China.Data from this prospective registry may provide opportunity to evaluate imaging and biomarker prognostic determinants of stroke. 展开更多

This study focuses on resolving moderate amounts of crustal motion at the continental scale based on a large volume of global positioning system(GPS) data during 1998e2014. A state-of-the-art GPS processing strategy was used to resolve position time series and velocities from carrier beat phases for all available data. Position time series were closely analyzed to estimate linear constant, coseismic displacements, postseismic motions, and other parameters. We present coseismic offsets inferred from the GPS data for the 2010 Yushu and 2014 Yutian earthquakes, and also illustrate transient postseismic motions following the 2001 Kokoxili, 2008 Wenchuan, and 2011 Tohoku-Oki earthquakes. Since not all GPS position time series dominated by postseismic motions can be modeled and corrected reasonably, we present contemporary horizontal velocities from 2009 to 2014 for campaign stations and from 1998 to 2014 for continuous stations, irrespective of postseismic deformations. Our study concludes that we need to accumulate observations over a greater duration and apply accurate postseismic modeling to correct for transient displacement in order to resolve reasonable interseismic velocity. 展开更多

OBJECTIVE To investigate the anti-pyroptotic effects of melatonin in atherosclerotic endothelium and to elucidate the potential mechanisms.METHODS ApoE-/-mice were randomly divid.ed into four groups(n=8):the normal-diet group(ND),the normal-diet group treated with melatonin(10 mg·kg^(-1))(ND+MLT),the high-fat-diet group(HFD),and the high-fat-diet group treated with melatonin(HFD+MLT).After 12 weeks,the expression levels of pyroptosis related genes including NLRP3,ASC,cleaved-caspase 1,GSDMD-N,IL-1β and IL-18 were examined in aortic endothelium by Western blotting,qRT-PCR and immunofluorescent staining.Besides,levels of MEG3 and miR-223 were also tested by qRT-PCR.The interaction between MEG3 and miR-223 was detected by luciferase assay.For in vitro study,human aortic endothelial cells(HAECs) were transiently transfected with miR-223 mimic,miR-223 inhibitor(AMO-223),MEG3-overexpressing plasmid or negative controls.After 6 h of transfection,the medium was replaced by fresh medium with or without ox-LDL(25 μg·mL-1) for 24 h and then treated with or without melatonin(10 μmol·L^(-1)) for 48 h.Cell pyroptosis was evaluated by Hoechst 33342/PI staining and differentially expressed pyroptosis related genes.RESULTS Melatonin markedly reduced the atherosclerotic plaque in aorta of ApoE-/-mice.Meanwhile,melatonin also attenuated the expression NLRP3,ASC,cleaved-caspase1,NF-κB/GSDMD,GSDMD-N termini,IL-1β,and IL-18 in aortic endo.thelium.Consistent anti-pyroptotic effects were also observed in ox-LDL-treated HAECs.We found that lncRNAMEG3 enhanced pyroptosis in HAECs.Moreover,MEG3 acted as an endogenous sponge by sequence complementarity to suppress the function of miR-223 and to increase NLRP3 expression and enhance endothelial cell pyroptosis.Furthermore,knockdown of miR-223 blocked the anti-pyroptotic actions of melatonin in ox-LDL-treated HAECs.CONCLUSION Melatonin prevents endothelial cell pyroptosis via MEG3/miR-223/NLRP3 axis in atherosclerosis and therefore melatonin replacement might be considered a new strat 展开更多

In situ U-Pb dating and Lu-Hf isotopic analysis were carried out for detrital zircons from quartzite in the Paleoproterozoic Songshan Group on the southern margin of the North China Craton (NCC). The re- sults provide further constraints on the crustal formation and evolution history of NCC. Four 207Pb/206Pb age populations were obtained from 99 analyses, with clusters at ~3.40 Ga, 2.77―2.80 Ga, ~2.50 Ga and 2.34 Ga, respectively. The 3.40 Ga old zircons have similar Hf isotopic compositions to those from Ar- chean rocks in the Jidong and Anshan areas of NCC. However, crustal remnants older than 3.6 Ga have been identified in the southern margin of NCC, the South China Craton, the northwestern part of the Qinling Orogen and its adjacent area. Thus, it is not easy to trace the source rock from which the 3.40 Ga detrital zircons were derived. It can be inferred that the crustal remnants older than 3.40 Ga might have been widely distributed in the North China Craton. The 2.77―2.80 Ga zircons make up a relatively small proportion and have the highest εHf (t) values (up to 6.1±1.6), consistent with the Hf isotopic composition of the depleted mantle at 2.83 Ga. Their single-stage Hf model age of 2.83 Ga is close to their crystallized age, suggesting that their source rocks were extracted from the contemporaneous depleted mantle. The ~2.50 Ga zircon grains constitute about 85% of the total grain population and their Hf isotopic compositions indicate major growth of juvenile crust at ~2.50 Ga but minor reworking of ancient crust. The youngest zircon dated in this study gave an U-Pb age of 2337±23 Ma, which can be considered the maximum depositional age of the formation of the Songshan Group. 展开更多

Microplastics are considered as the carrier to heavy metals in the environment.But the sorption ability of microplastics influenced by photo-aging is remaining unclear.In the present study,the sorption of two kinds of metal ions(Cu^2+ and Zn^2+)in the aqueous solution by both the virgin and aged microplastics was investigated.Polyethylene terephthalate(PET)debris,one of the typical kinds of microplastics was chosen in this study.Photo-aging of microplastics in environment was simulated using UV radiation in the laboratory.Date analysis indicated that the aged microplastics had higher adsorption capacity of heavy metals than original ones.This could be related to the increased surface area and oxygen containing function appeared in the surface of aged microplastics after UV radiation.When prolonging the time of radiation,the enhanced adsorption capacities of microplastics appeared for Cu^2+ and Zn^2+.These results showed a great interaction between the aging degree of plastics and sorption capacity to heavy metals.Meanwhile,external conditions including temperature and pH value were also showed great influence to the adsorption behavior. 展开更多

According to the traditional immunization procedure, after the first injection of the sample A (emulsion of aimed antigen and Freund's complete adjuvant) to immunize rabbit, successive injections of the sample B (emulsion of aimed antigen and Freund's incomplete adjuvant) were followed every 2-4 weeks. In general,high titer of the corresponding polyclonal antisera will be observed after 4-5 injections of sample B in 3-4months. This report presents a simply modified procedure that was able to stimulate the antisera formation in one month and achieve enough avidity to satisfy either Western blot or immunohistochemistry analysis.It just applied an additional injection of the sample A to the rabbit at the 3rd day after the primary immunization injection. You could gain the high titer of the antisera right after the first sample B injection in one month. This method has produced the desired results in three different recombinant antigens with different molecular weight (5.9 KD-55 KD) expressed from prokaryotic or eukaryotic cells. 展开更多
