摘要
目的探讨社区医生督导对严重精神障碍患者的影响。方法选取2017年迪庆州香格里拉市和文山州广南县的33个社区严重精神障碍患者1000例为研究对象,评估患者危险性并根据患者临床表现为依据,实施四级管理,进行督导。观察患者治疗前后管理分级、服药依从性、健康教育及肇事肇祸发生状况。结果管理后一级、二级、三级精神障碍患者低于管理前,四级精神障碍患者高于管理前(P<0.05)。管理前患者服药依从性为58.0%,管理后用药依从性为70.0%;管理前患者社区集体康复活动以及心理健康教育参与率为29.0%,管理后参与率为72.0%。管理后精神障碍患者的肇事肇祸率高于管理前(P<0.05)。结论将社区医生督导应用于严重精神障碍患者管理与治疗中,可改善患者的管理分级,促进患者服用药物依从性,并减少患者的不良反应,值得广泛应用和推广。
Objective To explore the influence of community doctor supervision on patients with severe mental disorders.Methods 1 000 patients with severe mental disorders in 33 communities in Shangri-la City of Diqing Prefecture and Guangnan County of Wenshan Prefecture in 2017 were selected as subjects.The risk of the patients was assessed and four levels of management were implemented according to the clinical manifestations of the patients.The management classification,medication compliance,health education and accident occurrence were observed before and after treatment.Results After management,the first,second and third grade mental disorder patients were lower than before,and the fourth grade mental disorder patients were higher than before(P<0.05).The compliance of patients before and after management was 58.0%,70.0%,29.0%and72.0%respectively.The accident rate of patients with mental disorders after management was higher than that before management(P<0.05).Conclusion The application of community doctor supervision in the management and treatment of patients with severe mental disorders can improve the management grade of patients,promote the compliance of patients to take drugs,and reduce the adverse reactions of patients,which is worthy of wide application and promotion.
