摘要
目的 比较膝关节半月板3度损伤缝合术和切除术的临床疗效。方法 选取2019年1月至2022年6月江西省贵溪市人民医院骨科收治的64例膝关节半月板3度损伤患者,按照随机数字表法分为试验组(32例)与对照组(32例)。对照组采用关节镜下半月板切除术,试验组采用关节镜下半月板缝合术,比较两组手术效果。结果试验组治疗总有效率高于对照组,差异有统计学意义(P<0.05),试验组手术时间短于对照组,术中出血量和术后引流量少于对照组,差异有统计学意义(P<0.05),试验组首次下床活动时间、拆线时间以及住院时间均短于对照组,差异有统计学意义(P<0.05),试验组骨性关节炎、下肢静脉血栓、关节腔积血等术后并发症发生率低于对照组,差异有统计学意义(P<0.05),试验组术后视觉模拟评分法(VAS)评分低于对照组,且膝关节量表(LKS)评分与膝关节活动度高于对照组,差异有统计学意义(P<0.05)。结论 和半月板切除术相比,半月板缝合术能够最大程度保护患者膝关节结构及其功能,减轻关节疼痛,并降低远期并发症的发生风险,促进术后恢复进程,临床疗效较好。
Objective To compare the clinical efficacy of suture and resection of knee meniscus injury in the third degree.Methods A total of 64 patients with knee meniscus third degree injury admitted by the Department of Orthopedics of Guixi People's Hospital in Jiangxi Province from January 2019 to June 2022 were selected and divided into the test group(32 cases)and the control group(32 cases)according to the random number table method.The control group was treated with arthroscopic meniscectomy,and the test group was treated with arthroscopic meniscus suture.The surgical effect of the two groups were compared.Results The total effective rate of the test group was higher than that of the control group,with a statistically significant differences(P<0.05).The operation time of the test group was shorter than that of the control group,and the intraoperative bleeding volume and postoperative drainage volume were less than those of the control group,with statistically significant difference(P<0.05).The time of getting out of bed for the first time,the time of removing stitches and the time of hospitalization in the test group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications such as osteoarthritis,lower limb vein thrombosis and joint cavity hematocele in the test group was lower than that in the control group,with a significant difference(P<0.05).The visual analogue scale(VAS)score in the test group was lower than that in the control group,and the Lysholm knee scale(LKS)score and knee joint range of motion in the test group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion Compared with meniscectomy,meniscus suture can protect the structure and function of the knee joint to the greatest extent,reduce joint pain,reduce the risk of long-term complications,promote postoperative recovery process,and has a good clinical effect.
