摘要
基于中国健康与养老追踪调查(CHARLS)2018年数据,运用有序多分类logistic回归和夏普利值分解模型分析老年人口日常生活自理能力的城乡差异及成因。研究结果表明,城乡老年人口在不同失能水平上存在差异,农村失能老年人口多处于中轻度失能水平,城镇失能老年人口中轻度水平较低但重度失能水平高。不同年龄段老年人口在不同失能水平上存在城乡差异,农村老年人失能年龄晚但发展水平快。已婚、有规律的锻炼和社交活动、良好的自评健康水平有利于降低老年人失能风险,高龄、独居和高血压、恶性肿瘤、肺部疾病、心脏病和中风等相关疾病是重要危险因素。应重点关注独居女性、低收入以及医疗保险类型为城镇职工医疗保险和患有精神疾病的特殊农村人群的健康权益,构建城镇重度失能健康养老服务体系,为城镇老年人口日常体检增加恶性肿瘤和风湿病筛查项目。
Based on the data of China Health and Retirement Longitudinal Survey(CHARLS)in 2018,the urban-rural differences and their reasons in self-care ability of seniors in daily life were analyzed by using ordered multi-classification Logistic regression and Shapley value decomposition model.It is indicated that there are differences between the urban and rural elderly in different levels of disability,the rural disabled elderly are mostly at the level of mild to moderate disability,and the level of moderate to mild disability in urban elderly is low but the level of severe disability is high.There are urban-rural differences in the different levels of disability among the elderly of different age groups.The elderly in rural have a late age of disability but a rapid development level.Married,regular exercise and social activities,and a good self-reported health level are conducive to reducing the risk of disability in the elderly.Old age,living alone and high blood pressure,malignant tumors,lung disease,heart disease,stroke and other related diseases are important risks factor.Based on these,it is proposed to focus on the health rights of rural special people,such as,women living alone,low-income,medical insurance for urban employees,and suffering from mental illness.To build an urban severely disabled health care service system,and increase screening items for malignant tumors and rheumatism in daily physical examinations for the urban elderly population.
