Testing and analysis on exposure dose of X-ray bone density scan patient

摘要 目的了解常规X射线骨密度扫描时受检患者体表关注部位的受照剂量水平,为X射线骨密度扫描的规范应用和人员防护提供科学依据。方法在仿真人体模型实验确认测试方案可行的基础上,于2017-2021年抽取在北京积水潭医院接受双能X射线骨密度测定的122名受检者作为测量对象,使用GE PRODIGY型双能X射线骨密度仪和GR-200A热释光探测器(LiF:Mg,Cu,P)对受检患者性腺相应体表部位的受照剂量进行测试和分析。结果常规正位腰椎+左髋骨密度扫描时,122例受检患者性腺相应体表部位的受照剂量为0.40~31.57μGy,左、右侧受照剂量平均值分别为(11.53±6.22)和(3.36±2.99)μGy;不同体质指数(body mass index,BMI)组女性受检患者左侧性腺体表部位受照剂量差异有统计学意义(P<0.01)。结论常规X射线骨密度扫描对受检患者产生的剂量低,健康影响小,但仍需严格实施正当化和最优化原则,规范操作流程,有效控制非必须的X射线骨密度检查。 Objective To understand the exposure dose level of the patient’s body surface during routine X-ray bone density scans,and provide scientific basis for the standardized application of X-ray bone density scans and personnel protection.Methods Based on the feasibility of the test plan confirmed by the simulated human body model experiment,122 subjects who underwent dualenergy X-ray bone density measurement in Beijing Jishuitan Hospital were selected as the measurement objects from 2017-2021.The GE PRIDIGY dual-energy X-ray bone densitometer and GR-200 A thermoluminescence detectors(LiF:Mg,Cu,P)were used to test and analyze the exposure dose of the corresponding body surface parts of the gonad of the patients.Results During the conventional orthostatic lumbar spine+left hip bone mineral density scan,the exposure dose of the corresponding body surface parts of the gonads of 122 patients were 0.40-31.57μGy,and the average exposure dose of the left and right sides were(11.53±6.22)and(3.36±2.99)μGy.There was a statistically significant difference in the exposure dose to the left gonadal surface of female patients in different BMI groups(P<0.01).Conclusion Conventional X-ray bone density scans produce low doses and have little health impact on the tested patients.However,it is still necessary to strictly implement the principles of justification and optimization,standardize the corresponding operating procedures,and effectively control non-essential X-ray bone density inspections.
