摘要
目的通过调查深圳市公立医院高值医用耗材的使用情况,研究和优化高值医用耗材的使用管理方法。方法采取数据采集、现场抽样等方式对全市56家公立医院的高值医用耗材进行专项调查。结果深圳市40家三级医院年业务收入合计437.33亿元,年高值医用耗材使用金额合计48.86亿元,18类重点治理高值医用耗材年使用金额合计14.40亿元;在高值医用耗材管理机构制度建设、产品的合法性、采购管理等方面,相关管理工作开展总体良好;高值医用耗材重点监控、超常预警和评价制度等仍需完善。结论控制高值医用耗材使用的重点在大型三级医院,应加强对此类医院骨科、心血管科、普外科等科室的监管,持续开展医院内、医院间的高值医用耗材使用管理调研;制定高值医用耗材遴选目录,开展使用权限和定额定量管理;选择部分专科使用特色明显、临床路径清晰、临床耗材使用选择分类和规格较明确的病种开展试点,研究、制定常见疾病高值医用耗材临床应用规范、使用原则及目录。
Objective The use and management method of high-value medical consumables were studied and optimized by investigating the use of high-value medical consumables in governmental hospitals in Shenzhen.Methods Data collection and field sampling were used to conduct a special survey on high-value medical consumables in 56 governmental hospitals in the city.Results The total annual business income of 40 tertiary hospitals was 43.733 billion yuan,the total annual use amount of high-value medical consumables was 4.886 billion yuan,and the total annual use amount of 18 key management high-value medical consumables was 1.440 billion yuan;in terms of system construction of high-value medical consumables management organization,product legitimacy,procurement management etc.,relevant management work was generally well carried out;the key monitoring,abnormal early warning and evaluation system of high-value medical consumables still need to be improved.Conclusions The focus of controlling the use of high-value medical consumables is in large tertiary hospitals.We should strengthen the supervision of orthopedics,cardiovascular department,general surgery and other departments in such hospitals,and continue to carry out the research on the use and management of high-value medical consumables in and between hospitals;formulate the selection catalogue of high-value medical consumables,and carry out the use authority and quota quantitative management;select the diseases with obvious use characteristics,clear clinical path and clear classification and specifications for the use of clinical consumables in some specialties to carry out the pilot,and study and formulate the clinical application specifications,use principles and catalogue of high-value medical consumables for common diseases.
