Effect of comprehensive rehabilitation therapy on upper limb spasticity in stroke patients

摘要 目的研究中枢与外周综合性康复治疗脑卒中后上肢肌张力增高患者的效果。方法纳入75例上肢肌张力增高的脑卒中患者,实验组38例,对照组37例,对照组采用醒脑开窍针刺法和任务导向性上肢功能训练,实验组在对照组的基础上增加深层肌肉刺激仪(DMS)治疗。治疗前及治疗6周后,采用上肢简化Fugl-Meyer(FMA-UE)、上肢动作研究量表(ARAT)和改良Ashworth量表(MAS)对比2组疗效。结果实验组患者治疗后的FMA-UE(38.9±8.8 vs 35.6±8.1)、ARAT(29.2±7.5 vs 26.2±5.2)和MAS(1.6±0.6 vs 2.0±0.5)评分均较对照组显著改善,差异均有统计学意义(P<0.05)。结论DMS配合醒脑开窍针刺法和任务导向性上肢功能训练可有效降低脑卒中后上肢肌张力,改善上肢功能,提高患者生活质量。 Objective To study the effect of central and peripheral comprehensive rehabilitation therapy on upper limb dysto⁃nia after stroke.Methods A total of 75 cases of stroke patients with increased upper limb muscle tension were divided into experi⁃mental group(38 cases)and control group(37 cases).The control group using the Xingnaokaiqiao acupuncture method and task-ori⁃ented training of upper limb function,while the experimental group was treated with deep muscle stimulator(DMS)on the basis of the control group.Before treatment and after 6 weeks of treatment,the efficacy of the two groups by the scores of the Fugl-Meyer(FMA-UE),upper extremity action research scale(ARAT)and modified Ashworth scale(MAS)were compared.Results The scores of FMA-UE(38.9±8.8 vs 35.6±8.1),ARAT(29.2±7.5 vs 26.2±5.2)and MAS(1.6±0.6 vs 2.0±0.5)in the experimental group af⁃ter treatment were improved compared with those in the control group(P<0.05).Conclusion Deep muscle stimulator combined with the Xingnaokaiqiao acupuncture method and task-oriented training can effectively reduce the educe upper limb muscle tension after stroke,then improve the upper limb function and the quality of life of patients.
