摘要
目的总结面部美容注射导致眼部血管阻塞的临床治疗方法及其效果。方法回顾性研究。收集郑州大学第五附属医院眼科2016年1月至2019年12月面部美容注射导致的眼部血管阻塞患者32例(32眼)的临床资料。按原注射材料的不同分为透明质酸组(24例)和自体脂肪组(8例)。总结其治疗方法及治疗效果。结果其中15例为眼部动脉部分或完全阻塞,14例为视网膜中央动脉或视网膜分支动脉阻塞,1例为视网膜中央动脉合并视网膜中央静脉阻塞,2例为睫状后动脉阻塞。发病后就诊时间为(11.44±12.60)h。就诊时视力为无光感~1.0,治疗后9例视力提高,23例视力无改善。自体脂肪注射组与透明质酸注射组相比,脑梗死的发生差异具有统计学意义(P<0.05)。结论面部美容注射后眼部阻塞的血管以眼动脉及视网膜中央动脉为主,自体脂肪注射者易发生脑梗死。
Objective To summarize the clinical treatment methods and efficacy of ocular vascular obstruction caused by facial cosmetic injection.Methods This was a retrospective study.The clinical data of 32 eyes of 32 cases with ocular vascular obstruction caused by facial cosmetic injections from Jan.2016 to Dec.2019 in the Fifth Affiliated Hospital of Zhengzhou University were collected.All cases were divided into 24 cases of the hyaluronic acid group and 8 cases of autologous fat group,based on the original injection materials.The treatment methods and efficacy were summarized.Results Among the 32 patients,partial or complete ophthalmic artery obstruction occurred in 15 cases,central retinal artery or branch retinal artery occlusion in 14 cases,central retinal artery with central retinal vein occlusion in 1 case,and posterior ciliary artery occlusion in 2 cases.The beginning to treatment time was(11.44±12.60)hours.At the time of seea-doctor,the visual acuity ranged from no light perception to 1.0.After treatment,the visual acuity improved in 9 cases,and no changed in 23 cases.Compared with the hyaluronic acid injection group,the occurrence of cerebral infarction in the autologous fat injection group was statistically significant(P<0.05).Conclusion After facial cosmetic injection,the main blood vessels blocked in the eye are the ophthalmic artery and central retinal artery.Cerebral infarction is likely to occur in autologous fat injections.
