摘要
目的:研究精子DNA损伤和顶体功能检测对体外受精-胚胎移植助孕结局的影响。方法:选取2019年1月-2021年1月接受体外受精-胚胎移植助孕的男性和女性患者各100例,均采用卵泡期长方案促排卵,按照精子DNA碎片指数(DFI)是否超过25%分为观察组(精子DNA损伤组)和对照组(未损伤组),各50例;收集男性精液标本,分别采用精子染色质扩散法(SCD)与固相BAPNA法检测精子DNA损伤程度与顶体酶活性。观察两组精子DFI、精子顶体酶活性、精子浓度、活力及正常形态精子率,两组受精率、可移植胚胎率、优胚率、妊娠率以及流产率,运用Spearman分析法分析两组男性患者精子DFI与顶体酶活性的关系。结果:观察组男性精子DFI高于对照组,精子顶体酶活性低于对照组,差异有统计学意义(P<0.05);观察组男性精子浓度、活力及正常形态精子率均低于对照组,差异有统计学意义(P<0.05);观察组受精率、可移植胚胎率、优胚率、妊娠率均低于对照组,流产率高于对照组,差异有统计学意义(P<0.05);Spearman关性分析结果显示,男性精子DFI与顶体酶活性之间呈明显的负相关性。结论:精子DNA损伤和顶体功能检测可对体外受精-胚胎移植助孕结局进行预测,精子DNA损伤会影响顶体功能及体外受精-胚胎移植助孕结局。
Objective:To study the effects of sperm DNA damage and acrosome function test on the outcome of assisted pregnancy by in vitro fertilization and embryo transfer(IVF-et).Methods:From January 2019 to January 2021,a total of 100 male and female patients who received in vitro fertilization and embryo transfer were divided into the observation group(sperm DNA damage group)and the control group(undamaged group)according to whether the sperm DNA fragment index(DFI)was more than 25%with 50 cases in each group.Male semen samples were collected and sperm chromatin diffusion(SCD)and solid phase BAPNA were used to detect sperm DNA damage and acrosin activity.The sperm DFI,sperm acrosin activity,sperm concentration,sperm motility and normal morphology sperm rate,fertilization rate,transplantable embryo rate,excellent embryo rate,pregnancy rate and abortion rate were observed in the two groups.Spearman analysis was used to analyze the relationship between sperm DFI and acrosin activity in the two groups of male patients.Results:The sperm DFI of the observation group was higher than that of the control group,the acrosin activity of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).The sperm concentration,motility and normal morphological sperm rate of the observation group were significantly lower than those of the control group,the difference was statistically significant(P<0.05).The fertilization rate,transplantable embryo rate,excellent embryo rate and pregnancy rate in the observation group were lower than those in the control group,and the abortion rate in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).Spearman correlation analysis showed that there was a significant negative correlation between male sperm DFI and acrosin activity.Conclusion:Sperm DNA damage and acrosome function detection can predict the outcome of in vitro fertilization-embryo transfer assisted pregnancy.Sp
