摘要
早发性卵巢功能不全(POI)是指女性在40岁以前出现卵巢功能减退,同时可能伴随一系列围绝经期症状的临床综合征,发病率约为1%~3.8%,现已呈逐年上升并日益年轻化趋势,对女性的生育健康及心理状态都造成了严重的负面影响。从现代医学来讲,其发病机制一般涉及遗传因素、免疫因素、酶学障碍及医源性因素等。中医对本病的论治以肾为根本,强调肾对女性生殖与月经的主导和决定性作用。中医治病强调整体观念,认为心、肝、脾、肾相互为病,气郁、瘀血、痰饮为其致病因素。目前国内外研究显示,大部分POI患者的病因仍不明确,导致POI发生的因素错综复杂,关于其具体作用机制还需进一步研究。现对POI中西医发病机制的研究进展进行综述,以期对POI有进一步的了解为对该病的预防与防治提供理论依据。
Premature ovarian insufficiency(POI)refers to the clinical syndrome of ovarian function decline before the age of 40,which may be accompanied by a series of perimenopausal symptoms.The incidence rate is about 1%-3.8%,and it has been increasing year and getting younger,which has caused serious negative effects on the reproductive health and psychological state of women.From the perspective of modern medicine,its pathogenesis generally involves genetic factors,immune factors,enzyme disorders and iatrogenic factors.The treatment of this disease in traditional Chinese medicine is based on kidney,emphasizing the dominant and decisive role of kidney in female reproduction and menstruation.Traditional Chinese medicine treatment emphasizes the overall concept,that the heart,liver,spleen and kidney are diseases,Qi stagnation,blood stasis,phlegm drinking as its pathogenic factors.Current studies at home and abroad have shown that the etiology of most POI patients is still unclear,and the factors leading to POI are complex.Further research is needed on the specific mechanism of POI.This paper reviews the progress of the pathogenesis of POI in traditional Chinese and Western medicine in order to further understand POI and provide theoretical basis for the prevention and treatment of POI.
