摘要
①目的探讨全身麻醉与腰-硬联合麻醉在老年骨科手术中患者围手术期的最佳护理方式。②方法选取2016年7月~2018年7月在我院接受老年骨科手术治疗的76例患者,按就诊顺序随机分为观察组与对照组,每组各38例。对照组采用全身麻醉的方法,观察组采用腰-硬联合麻醉的方法,术前、术中、术后对照组采用常规护理方式,观察组采用人性化护理方式。对比两组患者麻醉前、麻醉完成后30分钟的心率(HR)、收缩压(SBP)、舒张压(DBP)以及血氧饱和度(SpO2)等相关数据的变化情况、不良反应的发生率以及护理满意度。③结果观察组的不良反应发生率为7.89%,明显低于对照组(57.89%),差异具有统计学意义(P<0.05)。两组患者血流动力学指标在麻醉前差异均无统计学意义(P>0.05),麻醉后30分钟观察组各项指标均显著优于对照组(P<0.05)。观察组总护理满意度(94.74%)显著高于对照组(84.21%),差异具有统计学意义(P<0.05)。④结论采用人性化护理可使患者对护理工作的满意度提高,有效促进医患关系,且人性化护理对于老年股骨颈骨折患者在围手术期的心理安慰、手术后的疼痛感都有明显的减轻。
Objective To explore the perioperative nursing of patients undergoing general anesthesia combined with lumbar-hard anesthesia in elderly orthopedic surgery.Methods Seventy-six patients aged orthopedic surgery were selected on July 2016 to July 2018 in our hospital,according to the order of treatment were randomly divided into two groups,the observation group and the control group,38 cases of patients in each group.The control group with general anesthesia,the observation group were epidural anesthesia,preoperative and postoperative care control group with conventional ways,the observation group humane care approach.The changes of heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP)and oxygen saturation(SpO2),and the incidence of adverse reactions,and two groups of patients before and after anesthesia were compared.Results The incidence of adverse reactions in the observation(7.89%)was significantly lower than that in the control group(57.89%).There were significant difference(P<0.05).Two groups of hemodynamic parameters in patients before anesthesia were not statistically significant(P>0.05),30min after anesthesia indexes observation group were significantly better than the control group indexes(P<0.05).The total satisfaction of the observation group was 94.74%,which was significantly higher than the total satisfaction of the control group by 84.21%,the difference was statistically significant(P<0.05).Conclusion The use of humanized nursing can greatly improve the satisfaction of patients'nursing work and the professional quality of nurses,and effectively promote the relationship between doctors and patients.The humanized nursing care for the elderly patients with femoral neck fractures during psychological perioperative period and postoperative operation,the pain after operation is obviosly relieved.has a clear effect.
