摘要
目的探讨血清前白蛋白(PA)、国际标准比率(INR)及终末期肝病模型(MELD)评分与肝衰竭患者预后的关系。方法回顾性分析我院2017年1月至2018年1月住院的肝衰竭患者101例临床资料,将患者分为存活组(57例)及死亡组(44例)。研究对象入院后常规检测血清PA、INR,计算MELD评分。结果血清PA死亡组为(23.5±9.6)mg/L,显著低于存活组(38.8±12.8)mg/L,P<0.05。INR死亡组为3.7±2.0,显著高于存活组(2.3±0.5,P<0.05),MELD评分死亡组为30.5±6.2,显著高于存活组(23.5±2.5,P<0.05)。经Pearson相关性分析,血清前白蛋白、INR、MELD评分与预后(死亡)的相关值为-0.879,0.856,0.610。结论血清前白蛋白、INR、MELD评分可作为判断肝衰竭预后的指标,血清前白蛋白越低,INR、MELD评分越高,肝衰竭预后越差,反之则预后较好。
Objective To investigate the relationship between serum prealbumin(PA),international standard ratio(INR) and model of end-stage liver disease(MELD) score and prognosis in patients with Liver failure. Methods The clinical data of 101 patients with Liver failure admitted to our hospital from January 2017 to January 2018 were retrospectively analyzed. The patients were divided into survival group (57 cases) and death group (44 cases). Subjects were routinely tested for serum PA (normal values 250-400mg/l),INR and the MELD score was calculated. Results The serum PA death group was 23.5±9.6mg/l,which was significantly lower than the survival group (38.8±12.8mg/l,P<0.05). The INR death group was 3.7±2.0,which was significantly higher than the survival group(2.3±0.5,P<0.05),and the MELD score death group was 30.5±6.2,which was significantly higher than the survival group (23.5±2.5,P<0.05). According to Pearson correlation analysis,serum prealbumin was negatively correlated with prognosis(death),and INR and MELD scores were positively correlated with death. Conclusion Serum prealbumin,INR,and MELD scores can be used as indicators to determine the prognosis of liver failure. The lower the serum prealbumin,the higher the INR and MELD scores,the worse the prognosis of liver failure,and the better the prognosis.
