摘要
目的:探讨龈-牙性露龈笑患者行微创牙冠延长术后的美学疗效。方法:选取18例龈-牙性露龈笑患者共94颗患牙,行微创牙冠延长术;分析其临床疗效,并应用菌斑指数(plaque index,PLI)、牙松动度(tooth mobility,MOB)评价牙龈、牙周情况,以及微笑线水平评价患牙治疗前后的美观效果,并通过视觉模拟评分法评价患者治疗前后的满意度。结果:微创牙冠延长术术后1周PLI增高,但经口腔卫生指导后,术后8周、6个月PLI降低。术后1周MOB有加大趋势,于术后8周趋于稳定。患者的微笑线水平术后均达Ⅱ,Ⅲ类标准,患者对前牙牙龈的美学效果较为满意。结论:通过适应症的正确选择,手术的精细操作及正规的口腔卫生保健,微创牙冠延长术可获得满意临床疗效。
Objective: To evaluate the aesthetic effects of minimally invasive crown lengthening surgery on the pa- tients with bare - gummed smile. Methods: The patients who suffered from gummy smile and of course underwent a minimally invasive crown lengthening surgery were chosen. The plaque index(PLI) and the tooth mobility ( MOB )were used to evaluate the gingival and periodontal conditions. The aesthetic effects of the treated teeth were evaluated with the smile line level. Vis- ual analogue scale(VAS) was used to examine patientg satisfaction degree before and after treatment. Results:One week after minimally invasive crown lengthening, the PLI was increased. But after oral hygiene instruction, PLI were decreased at 8 weeks and 6 months after surgery, respectively. The tooth mobility had a growing trend at 1 week after surgery and stabilized at 8 weeks after operation. The patients were satisfied with the gingival aesthetic effect of the maxillary anterior teeth, with the patient's smile line reaching the class Ⅱ and class Ⅲ standards. Conclusion:Patients could attain satisfactory clinical effects using the minimally invasive crown lengthening surgery, combined with the correct selection of indications, delicate operation and regular oral health care.
